Abstract

Background: Tuberculosis-diabetes mellitus (TB-DM) is a co-epidemic condition representing a double burden for high TB burden countries like Indonesia. Private primary care (PPC) in Indonesia was newly reintroduced to the TB program. Aims: This study explores the challenges PPC healthcare workers (HCWs) face in implementing TB-DM bi-directional screening and strategies to improve it. Methods: An explorative qualitative method was used by conducting two-round focus group discussions (FGDs) with HCWs (n=15) from five private clinics in Yogyakarta Special Region Province Indonesia and triangulated with 10 DM patients and the TB program manager from the provincial health office. The data were audio recorded, transcribed verbatim, and subjected to thematic analysis. Results: HCWs face complex barriers in TB-DM co-screening implementation from patients, HCWs, and health system aspects. An integrated system to remind, support decision-making, report, reduce documentation duplication, monitor, and evaluate co-screening implementation emerged as a potential strategy, although some challenges to deploy. Conclusion: Albeit HIS is promising in enhancing integrated TB-DM screening and management, a comprehensive approach and multisector roles are still needed to overcome impediments to TB-DM bidirectional screening in PPCs. Abstract

Pasien AMI merasakan nyeri yang sangat hebat. Nyeri yang diakibatkan oleh pasien AMI menyebabkan ketidaknyamanan dan penurunan fungsi tubuh yang berdampak berupa gejala fisik dan psikoemosional, yang dapat mempengaruhi kualitas hidup. Penelitian ini bertujuan untuk menganalisis pengaruh rehabilitasi jantung fase 1 Terhadap kenyamanan pasien acute myocardial infarction di Ruang IPI RS Baptis Kediri. Penelitian ini menggunakan metode quasy – experiment dengan post test design. Sampel penelitian diambil dengan mengunakan teknik quota sampling dengan jumlah sampel 40 yang dibagi menjadi 2 kelompok intervensi dan kelompok kontrol. Kelompok Intervensi dilakukan rehabilitasi jantung fase 1 dan kelompok kontrol mendapatkan pelayanan kesehatan sesuai prosedur yang ada di rumah sakit. Pengumpulan data dengan kuesioner SGCQ yang sebelumnya telah diuji validitas dan reliabilitasnya. Analisis data penelitian ini menggunakan uji statistik Independent Samples T-Test untuk mengetahui pengaruh kenyamanan pasien acute myocardial infarction. Hasil penelitian untuk kenyamanan pada kelompok intervensi mean 94.95 dan pada kelompok kontrol 78.20 Uji beda kenyamanan pada kelompok intervensi dan kontrol didapatkan hasil nilai t -4.36 dengan p<0.001. Kesimpulan Ada Perbedaan yang signifikan kenyamanan pada kelompok intervensi dan kelompok kontrol setelah dilakukan rehabilitasi jantung fase 1.

Abstract

Most falling occurs in the elderly because of the decrease of balance and muscle strength. Physical training for elderly needs to conduct to enhance physical fitness of the elderly and it can reduce the risk of falls by increasing muscle strength by increasing body balance in the elderly. The aim of this research is to find out the effectiveness of physical exercise to improve balance in the elderly. Method to collect database is conducted in Google Scholar and Pubmed to find literature. Literature used was literature that required the inclusion criteria published in the last three years, then the result is chose and the review is conducted. Result: there are 6 literature to be reviewed. Based on literature review, it is found that respondents are 65-91 years old. Most respondents are elderly women.

Abstract

A category of people who are susceptible to COVID-19 infection is pregnant women. The safety of the mother and fetus can be determined in large part by how prepared pregnant women are to give birth. Pregnant women who are mentally and physically prepared for childbirth can have a healthy delivery that includes the removal of the baby, placenta, and membranes. The purpose of this study is to investigate how prepared expectant mothers felt for childbirth during the COVID-19 epidemic. This study employs a qualitative methodology and phenomenology. In this study, 11 informants participated. Face-to-face interviews and semi-structured video were employed in the data collection process. According to the study's findings, there were five key elements in pregnant women's preparedness for childbirth during the COVID-19 pandemic: 1) worry about COVID-19 exposure, 2) keeping healthy throughout the COVID-19 pandemic, 3) adhering to our ancestors' culture, 4) preparing for a safe and comfortable delivery, and 5) receiving the best medical facility services. Pregnant women must maintain their health and carefully arrange their deliveries. Health facilities are crucial in assisting expectant mothers with labor preparation, particularly during the pandemic. During the COVID-19 pandemic, telehealth may be employed as a substitute method of reaching pregnant women.

Abstract

 Iron deficiency anemia is a significant global public health concern that disproportionately affects young children and pregnant women. One effective method for monitoring medication in pregnant women is through mobile applications. This study aims to investigatepregnant women’s experience with iron supplementation adherence management using amobile application. A qualitative study with a phenomenology approach was conductedamong 20 pregnant women and midwives. The qualitative method was used to explore participants’ experience using a maternal application called “MySmart Bumil” to improvepregnant women’s adherence to iron supplements. The study identified four themes: the factors of non-compliance, the motivation for taking iron supplements, and the benefits andthe weaknesses of using the application. Mobile app reminders can help pregnant womencheck medication frequency, remind them to take iron tablets, and build more consistentmedication habits. However, some pregnant women found that the reminder was ineffective. Pregnant women thought that the notifications should have alarms to be more effective. The mobile application has the potential to assist health providers in effectively monitoring the health state of pregnancy and improving adherence to iron supplementation

Abstract

Background: A Sharia Hospital in Central Java of Indonesia had previously used a balanced scorecard (BSC) to assess hospital performance. However, BSC has not been integrated into the hospital's strategic management process. Purpose: This study aims to develop a Sharia Hospital’s strategic plan by adopting a BSC integrated into Islamic values. Design/methodology/approach: A qualitative study was conducted involving a Strengths, Weaknesses, Opportunities, and Threats (SWOT) analysis with 10 respondents and focus group discussions with 11 strategic planning teams.Findings/Result: The results indicate that the hospital has completed several stages of developing a strategic plan, including defining the hospital's vision and mission, conducting a SWOT analysis, and formulating strategic objectives. Moreover, BSC has been implemented into the hospital's strategic planning process, particularly in formulating strategic objectives aligned with Islamic values. This reveals that the BSC has been integrated into the hospital’s strategic management process.Conclusion: The result of this study provides a reference for a Sharia Hospital to adopt BSC aligned with Islamic values in the preparation of a hospital strategic plan. By integrating Islamic values into BSC, the hospital can formulate and implement strategies and evaluate the hospital's performance more comprehensively and measurable, covering financial and non-financial performance while promoting Islamic values.Originality/value (State of the art): Developing a strategic plan using the BSC is commonly conducted, but integrating Islamic values into the BSC perspective is still relatively uncommon. Hence, this study contributes significantly to hospital strategic management practices, particularly in Sharia hospitals. Abstract

Background: In the era of the covid-19 pandemic, health facilities have challenges in optimizing health services, especially continuity of care. The results of the patient visit survey decreased by 50%. Health facilities must be able to make changes to access to care to be more flexible and effective. Objective: This community service aims to identify the readiness of health facilities for changes in health services with telemedicine. Method: This community service method uses an action research method which is divided into three stages, including the first stage identifying readiness for changes in health services with telemedicine; the second stage is conducting training on the implementation of telemedicine; the third stage evaluates the application of telemedicine-based on the number of patients who use it three months (January-March 2022) after the training. This service instrument uses a google form with 35 statement items given to the four primary clinics (10 respondents)—quantitative data analysis with frequency distribution. Findings: The results of this community service indicate that the readiness of health facilities to implement telemedicine reaches a score of 3.08 (ready) with four supporting factors including staff situation (2.74), change target attributes (2.80), management support (2.99), and attributes change (3.02). The readiness of health facilities to implement telemedicine after training has a positive impact on increasing the number of telemedicine patients by 260%. Conclusion: Changes in health service methods require complex readiness both from the management side as supporting facilities and increasing staff skills and the ability and willingness of staff as health service providers.

Abstract

Patient safety is an important factor in improving health services quality. Hospital accreditation aims to ensure patient safety by ensuring hospitals comply with established standards. Despite achieving Paripurna accreditation, hospitals still receive recommendations (accreditation surveyor findings) that must be followed up. The study aimed to evaluate whether there are differences in the fulfillment of Patient Safety Goals (SKP) standards in Paripurna-accredited private and government hospitals. This study is useful for improving service quality, especially patient safety aspects. The study result are expected to support the development of effective policies, hospital guidance, and  priority strategies establishment in improving hospital quality. The study was conducted in February 2024 using descriptive and comparative quantitative methods. The research sample was 126 Paripurna-accredited hospitals in East Java. Data analysis used Tableau and the Mann-Whitney U statistical test. The results showed that both private and government hospitals had the most accreditation surveyor findings in standard SKP6 (reducing the risk of patient injury due to falls in outpatient unit), 33.91% and 25.64%, respectively. Government hospitals had the same number of accreditation surveyor findings in standards SKP6 and SKP2 (effective communication) at 25.64%. The p-value of 0.015 indicates that there was a statistically significant difference in the fulfillment of SKP standards between private and government hospitals. The study concluded that the SKP needs to be improved by planning strategic improvements to the assessment elements that received accreditation surveyor findings. Private hospitals need to take strategic steps through strict supervision of health workers' compliance with procedures and standards

Abstract

Perawat yang bekerja di instalasi bedah sentral kerap mengalami beban kerja yang tinggi termasuk fisik dan emosional serta paparan radiasi berbahaya yang dapat menyebabkan stress kerja dan kejenuhan. Neuro-Linguistic Programming (NLP) adalah pendekatan yang berfokus pada mengubah pola pikir dan perilaku untuk meningkatkan kemampuan coping stress dengan memodifikasi respons individu ketika stress. Penelitian ini bertujuan untuk menganalisis pengaruh Neuro-Linguistic Programming (NLP) terhadap resiliensi dan stres perawat instalasi bedah sentral di Rumah Sakit Umum Diponegoro Dua Satu Klaten. Penelitian ini menggunakan metode kualitatif dengan jenis penelitian tindakan partisipan (action research). Penelitian ini melibatkan kolaborasi antara peneliti, instruktur NLP, dan 32 perawat yang dilaksanakan dalam dua siklus, dengan setiap siklus mencakup perencanaan, tindakan, observasi, dan refleksi. 6  perawat melalui kuesioner, wawancara, dan observasi, kemudian dianalisis untuk mengevaluasi efektivitas pelatihan NLP. Hasil penelitian ini bahwa NLP dapat meningkatkan resiliensi perawat dengan mengembangkan strategi coping yang efektif, keterampilan komunikasi, dan manajemen stress. Teknik reframing dan visualisasi positif membantu perawat melihat tantangan sebagai peluang, sementara teknik pacing dan leading meningkatkan hubungan dengan rekan kerja dan pasien.  Implikasi dari penelitian ini menunjukkan bahwa penerapan NLP dapat meningkatkan kepuasan kerja dan kualitas pelayanan sekaligus menurunkan tingkat turn-over perawat. Dengan demikian, pelatihan NLP diharapkan menjadi investasi strategis untuk kesejahteraan mental perawat dan meningkatkan efisiensi layanan kesehatan di lingkungan yang penuh tekanan seperti Instalasi Bedah Pusat

Abstract

Perawat merupakan pekerjaan dengan tingkat stres tinggi, apabila beban tidak secara proporsional sesuai dengan kemampuan fisik, keahlian dan ketersediaan waktu maka dapat menjadi sumber stress yang dapat menyebabkan penurunan yang signifikan pada Kesehatan fisik dan mental perawat hingga berdampak negatif kepada kesejahteraan perawat. Penelitian ini bertujuan untuk menganalisis efektivitas Neuro-Linguistic Programming (NLP) terhadap pengelolaan stres dan pengaruhnya pada peningkatan kinerja perawat. Metode penelitian yang digunakan adalah quasi eksperimen dengan one pretest-postest group design untuk penilaian stress (skor ENSS) pada kedua kelompok dan one pretest -postest group design untuk penilaian kinerja perawat pada kelompok intervensi. Sampel dipilih dengan teknik purposive sampling, didapatkan tiga puluh perawat bangsal RSU Diponegoro Dua Satu Klaten yang kemudian dibagi menjadi kelompok perlakuan dan kontrol. Variabel independent penelitian berupa pelatihan NLP. Variabel dependen berupa stress dan kinerja perawat. Instrumen penelitian menggunakan formulir penelitian kinerja pegawai dan Expanded Nursing Stress Scale (ENSS). Intervensi pelatihan NLP selama 1 minggu. Analisis data skor kinerja perawat dengan Paired T-test sedangkan skor ENSS dengan dependent t test yang dilanjutkan dengan independent t-test. Hasil penelitian menunjukkan terdapat perbedaan peningkatan yang signifikan rerata skor kinerja perawat pada kelompok perlakuan sebesar 3.8 (p<0.05). Terdapat perbedaan penurunan rerata yang signifikan skor ENSS pada kelompok perlakuan sebesar -20.66 (p<0.05). Tidak ada perbedaan yang signifikan skor ENSS pada kedua kelompok setelah diberikan intervensi NLP (p>0.05). Implikasi penelitian bahwa pelatihan Neuro-Linguistic Programming (NLP) dapat menurunkan stress (penurunan skor ENSS) pada perawat bangsal dengan kelompok perlakuan secara signifikan dan meningkatkan kinerja perawat bangsal pada kelompok kontrol secara signifikan.
