Abstract

Kepuasan pelanggan terhadap pelayanan kesehatan yang diterima dapat diwujudkan dengan melaksanakan pelayanan prima. Penelitian ini bertujuan mengetahui pengaruh pelayanan prima berbasis budaya terhadap tingkat kepuasan pasien di rumah sakit. Metode penelitian menggunakan quasi experiment dengan rancangan pre and post with control group design. Jumlah sampel adalah tiga puluh lima perawat dan seratus empat puluh pasien. Teknik pengambilan sampel untuk perawat menggunakan total sampling, sementara untuk pasien dilakukan dengan consecutive sampling. Hasil penelitian menunjukkan adanya pengaruh yang bermakna (p< 0,05) terhadap tingkat kepuasan pasien setelah perawat mendapatkan pelatihan pelayanan prima berbasis budaya pada sebelum dan sesudah di kelompok intervensi. Rekomendasi yang dapat diberikan adalah perlunya meningkatkan peran supervisi agar keberlangsungannya tetap terjaga. Selain itu, untuk penelitian berikut dapat juga dilakukan dengan model triangulasi. Abstract Nursing Care Prima Culture-Based Influence to Patient Satisfaction in Hospitals. This research aimed to determine the effect of service excellent based on culture to patient satisfaction level in installation of hospitalization Hospital Jayapura. This research is a quasi experiment with pre and post with control group design. The number of samples is someone 35 nurses and 140 patients. The results showed have significant effect (p< 0,05) on the level of patient satisfaction after nurses receive training excellent service based onculture pre and post intervention group. Recommendations can be given is the need to enhance the role of supervision in order to maintain continuity, for the following research maybe done by triangulation models. Keywords: culture, nursing, patient satisfaction, service excellence

Abstract

Luka dekubitus adalah suatu masalah endemic bagi populasi pasien yang dirawat di rumah atau rumah perawatan lainnya. Pasien-pasien tersebut memiliki resiko untuk mengalami terjadinya luka dekubitus selama perawatan. Insiden dan prevalensi terjadinya luka dekubitus pada populasi ini di Amerika Serikat cukup tinggi untuk mendapatkan perhatian dari kalangan tenaga kesehatan. Penelitian menunjukkan bahwa prevalensi luka dekubitus bervariasi, tetapi secara umum dilaporkan bahwa 5% sampai 11% terjadi di tatanan perawatan akut (“acute care”), 15% sampai 25% di tatanan perawatan jangka-panjang (”longterm care”), dan 7% sampai 12% di tatanan perawatan rumah (“home health care”).Berdasarkan panduan praktek klinik yang dikeluarkan oleh AHCPR, intervensi keperawatan yang digunkan untuk mencegah terjadinya luka dekubits terdiri dari tiga kategori yaitu: perawatan kulit dan penanganan dini, penggunaan berbagai matras atau alas, dan edukasi pasien.Tinjauan pustaka terpadu ini bertujuan untuk menggali hasil penelitian yang dilakukan berkaitan dengan intervensi keperawatan untuk mencegah terjadinya luka dekubitus pada populasi orang dewasa, dan hasilnya akan dibandingkan dengan panduan praktek klinik yang dikeluarkan oleh AHCPR.Dalam penelusuran kepustakaan dengan menggunakan fasilitas CINAHL pada tahun 1990-1995, didapatkan 12 artikel penelitian yang berkaitan dengan intervensi keperawatan untuk mencegah terjadinya luka dekubitus, yang terdiri dari 8 penelitian tentang penggunaan matras, 2 penelitian tentang pengaturan posisi baring, dan 2 penelitian tentang edukasi pasien.Penelitian dalam kurun waktu lima tahun terakhir terlihat lebih berfokus pada efek dari berbagai matras untuk mengurangi penekanan jaringan dan perkembangan luka dekubitus. Sedangkan penelitain tentang perawatan kulit dan posisi tubuh, dan edukasi pasien terbatas.Berdasarkan hasil penelitian yang telah dilakukan dalam kurun waktu lima tahun terakhir didapatkan bahwa 1) penggunaan matras yang mereduksi penekanan jaringan dapat menjadi tindakan yang efektif untuk mencegah terjadinya luka dekubitus, 2) intervensi dengan melakukan pengangkatan bagian tubuh tertentu sebagai tambahan jadual dari perubahan posisi yang rutin membantu dalam mencegah terjadinya luka dekubitus, 3) interval perubahan posisi setiap dua jam mungkin dapat merugikan integritas kulit pada populasilanjut usia, 4) perawata yang terlibat didalam edukasi pasien agar lebih menyadari bahwa tindakannya dalam upaya meningkatkan pengetahuan dan keterampilan pasien untuk mencegah terjadinya luka dekubitus akan sangat mempengaruhi sikap dan perilaku pasien dalam melakukan tindakan-tindakan untuk mencegah terjadinya luka dekubitus.Untuk peneliti selanjutnya direkomendasikan agar lebih memperhatikan area penelitin yang berhubungan dengan perawatan kulit, pengaturan posisi dan edukasi pasien karena kategori intervensi keperawatan ini masih sangat terbatas diteliti. Dan suatu penelitian replikasi perlu dilakukan untuk melihat efektifitas dari interval perubahan posisi baring yang secra tradisional dilakukan setiap dua jam.  Pressure ulcers are an endemic problem in hospitalization, institutionalized, and debilitated patient populations. Patients are at risk for the development of pressure ulcers. The incidence and prevalence of pressure ulcers among these populations in the United States are high enough to warrant concern of health care providers. Studies indicate that the prevalence of pressure ulcers varies, but it has been generally reported to be 5% to 11% in acute care settings, 15% to 25% in long term settings, and 7%-12% in home healthy settings.Based on the AHCPR’s clinical practice guideline, preventive nursing interventions for the development of pressure ulcers were categorized into skin care and early treatment, use of mechanical loading and support surface, and patient education.The purpose of this paper  is to explore research study has been conducted in area of nursing interventions to prevent the development of pressure ulcers among adult populations within the last five years, and to compare the result with the clinical practice guideline of predicting and prevention of pressure ulcers by AHCPR.In reviewing the literatures by using the CINAHL in 1990 through 1995, there were twelve research projects related to the subject has been found consisting of eight studies related to the use of various support surfaces, two studies related to body positions, and other two studies related to patient education.Literature within the last five years has appeared to focus on the effects of various support surface on pressure reduction and pressure sore development. Whereas, studies on skin care and body position and patient education have been limited.Based on the findings from the studies being conducted the last five years, the following can be concluded: 1) the utilization of pressure-reducing support surfaces could be an effective measure to prevent the development of pressure ulcers, 2) the small shift intervention in conjunction with routine turning schedules aids in the prevention of pressure ulcers, 3) the traditional two-hour turning intervention may be detrimental to the skin integrity of older adults, 4) nurses engaged most frequently in educational encounters with the client in the prevention of pressure ulcers; therefore, nurses need to remember that their actions can influence the attitudes of clients towards the acquisition of pressure ulcers prevention knowledge and skill.For the researcher it is recommended that the area of skin care interventions and patient education should be more considered, due to the limitations of studies have been conducted in these areas. A replication study to explore the effectiveness of specified turning intervals should be conducted. It is also recommended that a one and half hour turning policy be established with using a larger sample than a study conducted by Knox et al (1994).

Abstract

AbstrakPelayanan kesehatan yang berorientasi pada promosi atau peningkatan status kesehatan klien harus dikembangkan untuk menuju Indonesia Sehat 2010. Hal ini juga sejalan dengan perubahan paradigma di bidang kesehatan dari paradigma sakit kepada paradigm sehat. Salah satu upaya di bidang keperawatan adalah melalui perubahan orientasi dari pemberian asuhan keperawatan yang berorientasi kepada masalah menjadi pemberian asuhan yang berorientasi pada kemampuan dan kekuatan klien. Salah satu upaya itu ditunjukkan dengan perumusan atau penegakan diagnosa keperawatan sejahtera yang merupakan salah satu bentuk diagnosa keperawatan yang perlu dikembangkan. Penulisan diagnosa sejahtera ini dapat dilakukan di berbagai tatanan pelayanan kesehatan dan menuntut perubahan perilaku perawat dari yang berorientasi pada upaya penyembuhan kepada upaya peningkatan kesehatan dan kesejahteraan klien. AbstractHealth care services shoud be directed to health promotion measures in order to achieve the Healthy Indonesia 2010. This is in line with the changing of our health paradigm from illness to wellness. One of the nursing action is improving the orientation of nursing care from problem-oriented nursing care to client’s strength orientation. Developing wellness nursing diagnosis is believed as one of the nursing action to be improved in the future. Writing wellness diagnosis could be conduct in every health care facilities both clinical or community settings. It demanded the nurses to change their attitude in order to change their orientation to improve the client health and wellness.

Abstract

Seorang perawat dituntut untuk memiliki kompetensi kultural sehingga memiliki kepekaan terhadap kebutuhan budaya pasien. Artikel ini bertujuan untuk memberikan gambaran peningkatan kompetensi kultural perawat yang masih rendah melalui program Pelatihan Asuhan Keperawatan Peka Budaya pada Pasien dengan Gangguan Respirasi. Desain yang digunakan adalah quasi eksperimen dengan pre dan post test tanpa kelompok kontrol. Program pelatihan ini berlangsung selama 4 minggu melibatkan 93 orang perawat pelaksana dan manajer asuhan keperawatan di sebuah rumah sakit di Jakarta Timur yang dipilih secara purposif. Instrumen kompetensi kultural yang digunakan adalah instrumen yang dikembangkan oleh peneliti pada penelitian sebelumnya. Hasil pengukuran kompetensi kultural perawat sebelum dan setelah pelatihan menunjukkan perbedaan yang bermakna. Kompetensi kultural ditinjau dari aspek  pengetahuan budaya memiliki nilai p< 0,001 (α= 0,05), dari sikap budaya nilai p< 0,01 (α= 0,05),  dan dari aspek keterampilan budaya nilai p< 0,001 (α= 0,05). Kesimpulan yang dapat diambil adalah bahwa kegiatan Pelatihan Asuhan Keperawatan Peka Budaya efektif meningkatkan kompetensi kultural perawat dalam memberikan asuhan keperawatan. Rekomendasi untuk kegiatan selanjutnya adalah perlunya mensosialisasikan model Asuhan Keperawatan Peka Budaya yang digunakan dalam pelatihan ini kepada seluruh perawat agar dapat diterapkan kepada seluruh pasien dengan berbagai gangguan kesehatan. Kata kunci: kompetensi budaya, pelatihan perawat, asuhan keperawatan, peka budaya ABSTRACT Culturally-sensitive nursing care trainings effectively improve nurses’ cultural competency. Nurses are required to have cultural competency in order to provide culturally sensitive nursing care for their patients. The purpose of this study was to improve nurse cultural competences through A Training Program of Culturally Sensitive Nursing Care for Patients with Respiratory Health Problems. The study utilized quasi-experiments design without control. The four-week training program was organized involving 93 nurses at Persahabatan Hospital Jakarta who were selected using purposive sampling. The nurse cultural competency instruments modified by the author from her previous work was used. The results of this study showed that after the training program, the nurse cultural competency increased significantly (p< 0.001 for cultural knowledge, p= 0.003 for cultural attitude, and p< 0.001 for cultural skills). It was found that the training program was effective to increase every aspect of nurse cultural competency. In conclusion, the nursing care training program was effective to increase nurse’s cultural competency. Further programs are needed to improve the program outreach for all nurses and to be applied to the patients with a variety of health problems. Keywords: cultural competency, nurses training, culturally sensitive, nursing care

Abstract

A nursing audit is a professional evaluation of the quality of nursing services provided for patients by employing medical records or other supporting data conducted by nursing staff. This research aimed to investigate the root causes of the nursing audit performed by the quality care subcommittee in the hospital. To gain the data, the researchers employed secondary methods consisting of observation and interview. The collected data were analyzed by employing a systematic methodology to investigate the root causes by employing the Ishikawa diagram. The analysis results are presented by employing the 5-M method. The solution is proposed as a feedback to the hospital to develop a nursing audit. Meanwhile, the recommendation is rendered to the hospital to improve the quality of nursing care of the hospital particularly the nursing committee in performing nursing audits conducted by the quality care subcommittee by developing a documentation audit system. Abstract

Patient safety culture becomes the responsibility of every health care provider, but there are still barriers to reduce adverse events, and one of them is due to the barriers to incident reporting. This study obtained and used the databases from ProQuest, Science Direct, SAGE Publications, and EBSCOhost. The inclusion criteria were nurses and other health care providers as the sample population, barriers, factors, reporting, patient safety culture, adverse events, nurses' perception, implication, and experience. Four electronic databases were searched for data from 2016 to 2020 with English guidelines and full-text search assessed for the inclusion criteria. Fourteen articles fulfilled the inclusion criteria. The study found various barrier factors in incident reporting, including psychological problems and emotional reactions, such as shame, guilt, fatigue due to overwork, patient-nurse ratio, increased workload, insufficient time to respond the patients, lack of professionalism, medication errors, lack of resources and staff, and lack of feedback for error reporting. Personal and professional support for nurses or other health care providers is required to encourage error reporting without feeling guilty and ashamed to colleagues or feeling fearful of the supervisor in order to obtain more accurate data and improve the process that support patient safety and nurses’ self-awareness.

Abstract

Background: The increase in COVID-19 cases in Indonesia has resulted in changes in the hospital workflow, including the staffing process and scheduling, especially in the isolation units. Nurse managers are working hard in the scheduling system to ensure high-quality care is provided with the best human resources. Objective: This study aimed to explore the experiences of nurse managers in managing staff nurses’ work schedules during the COVID-19 pandemic. Methods: A qualitative descriptive design was used in this study. Eleven nurse managers from three COVID-19 referral hospitals were selected using purposive sampling. Data were collected using online semi-structured interviews. Thematic analysis was used for data analysis, and data were presented using a thematic tree. Consolidated criteria for reporting qualitative research (COREQ) checklist was used as a reporting guideline of the study. Results: Four themes were developed: (i) Nurse shortage, (ii) Strategically looking for ways to fulfill the workforce, (iii) Change of shift schedule, and (iv) Expecting guidance from superiors and compliance from staff. Conclusion: The lack of nurse staff is a problem during a pandemic. Thus, managing personnel effectively, mobilizing and rotating, and recruiting volunteers are strategies to fulfill the workforce during the pandemic. Using a sedentary shift pattern and sufficient holidays could prevent nurses from falling ill and increase compliance with scheduling. In addition, a staffing calculation formula is needed, and top nursing managers are suggested to provide guidance or direction to the head nurses to reduce confusion in managing the work schedule during the pandemic.

Abstract

Caring behavior is a fundamental aspect of nurses in performing nursing care including professional attitudes, knowledge and skills that have an impact on nursing care and services and patient safety. Objective study to identify the relationship of personality characteristics with nurses caring behavior. Using a cross sectional design with a sample of 124 nurses working in the Jakarta Government Hospital. The research instrument consisted of nurse characteristics, nurse caring behavior, Core Self Evaluation (CSE) personality model characteristics and Dominance, Influence, Steadiness and Compliance (DISC) personality type characteristics. Characteristics of the CSE personality models have a moderate relationship and positive direction (p = 0, 0001, r = 0.496) with caring behavior, and the greatest correlation is self-efficacy (p = 0.0001, r = 0.528), and the lowest correlation is emotional stability (p = 0.0006, r = 0.243). While the DISC personality type and nurse characteristics have no relationship (p> 0.05). CSE personality model characteristics are related to nurses caring behaviour. While DISC personality type characteristics and nurse characteristics do not have a relationship with nurse caring behaviour. The factor that most influences caring behaviour is personality self-efficacy.

Abstract

The training and development of nursing educators in management of pulmonary tuberculosis Background: The quality of education in patients is a key element in the quality of health promotion in hospitals. The phenomenon that occurs in the pulmonary clinic is the provision of very minimal education and there is no TB educator team.Purpose: To increase the competence of nurses as educators in the pulmonary clinic by carrying out TB educator training.Method: The action research, identification of problems with structured interview techniques, observation, and filling out questionnaires to the district head and 2 persons. Analysis of the problem is done with the fish bone method, solving the problem using the planing of action method.Results: As a condition of completeness of training instruments, curriculum structure and activity descriptions and modules containing training material have been made. From the results of this training a TB educator team will be formed at RS X.Conclusion: The manifestation of the responsibility of the nursing manager in improving the quality of nursing human resources in order to increase the competence of nurses as tb educators in health promotion and prevention of increasing numbers of patients with pulmonary TB in the hospital by compiling the curriculum structure and basic tb educator training modules.Keywords: The training; Development; Nursing educators; Management; Pulmonary tuberculosisPendahuluan: Kualitas edukasi pada pasien menjadi elemen kunci mutu promosi kesehatan di rumah sakit. Fenomena yang terjadi di poliklinik paru ialah pemberian edukasi yang sangat minim dan belum ada tim edukator TB.Tujuan: Untuk meningkatkan kompetensi perawat sebagai edukator di poliklinik paru dengan melaksanakan pelatihan edukator TB.Metode: Metode yang digunakan dengan penelitian tindakan, identifikasi masalah dengan teknik wawancara terstruktur, observasi, dan pengisian kuesioner kepada kabidyankep dan 2 orang kasie. Analisis masalah dilakukan dengan metode fish bone, pemecahan masalah menggunakan metode planing of action.Hasil: Sebagai syarat kelengkapan instrumen pelatihan telah dibuat struktur kurikulum dan deskripsi kegiatan serta modul yang berisi materi pelatihan. Dari hasil pelatihan ini akan dibentuk tim edukator TB RS X.Simpulan: Wujud tanggung jawab manajer keperawatan dalam peningkatan kualitas SDM keperawatan guna meningkatkan kompetensi perawat sebagai edukator TB dalam promosi kesehatan dan pencegahan peningkatan angka penderita TB Paru di rumah sakit yakni dengan menyusun struktur kurikulum dan modul pelatihan edukator TB dasar.

Abstract

Utilization of digital technologies as a strategy to optimizing management functions in nursingBackground: Nursing services need a good management of Nurse Managers to achieve a standardized service quality. Clinical supervision nursing has generally been implemented in hospitals, but its application has not been implemented optimally causing quality improvement not to run continuously.Purpose: To identify issues of implementation clinical supervision nursing and to develop problem-solving solutions.Method: The mini project, analysis of results and implementation gap with discussion based on literature review. Problems were analyzed using a fishbone diagram. Problem solving using Plan Do Study Action (PDSA) tools starts from Plan of Action (POA), implementation, evaluation, and follow up. The sample was taken from 5 the inpatient wards.Results: The problem identification found not optimal the quality  of nursing in the planning, monitoring and follow-up phase. Implementation of problem solving by workshop and socialization of guidance, procedure, dictionary and instrument of monitoring clinical supervision nursing with digital tecnology google form. The evaluation result shows that 100 % said the clinical form supervision of digital technology based on google form was very effective and efficientKeywords: Utilization; Digital technologies; Strategy; Optimizing; Management functions; NursingPendahuluan: Pelayanan keperawatan memerlukan pengelolaan yang baik dari Manajer Perawat untuk mencapai kualitas pelayanan keperawatan yang sesuai standar. Supervisi klinik keperawatan pada umumnya telah dilaksanakan oleh manajer keperawatan, namun penerapannya belum dilaksanakan secara optimal yang menyebabkan supervisi klinik keperawatan tidak berjalan secara berjenjang.Tujuan: Mengidentifikasi masalah pelaksanaan supervisi klinik keperawatan di ruang rawat serta mengembangkan solusi pemecahan masalah.Metode: Berupa mini project serta analisis hasil dan gap implementasi dengan pembahasan berdasarkan literature review. Masalah dianalisis menggunakan diagram fishbone. Penyelesaian masalah menggunakan tools Plan Do Study Action (PDSA) dimulai dari Plan of Action (POA), implementasi, evaluasi, dan tindak lanjut. Sampel diambil dari ruang rawat inap yang terdiri dari 5 Kepala Ruangan.Hasil: Ditemukan tidak optimalnya supervisi klinik keperawatan pada tahap perencanaan, pemantauan dan tindak lanjut. Implementasi pemecahan masalah berupa sosialisasi dan workshop supervisi klinik keperawatan berbasis teknologi digital google form. Hasil evaluasi menunjukkan bahwa  100 % mengatakan supervisi klinik keperawatan berbasis teknologi digital google form sangat efektif dan efesien.
