Abstract

Introduction: Neutropenic fever is commonly found in children with leukemia after chemotherapy. Fever occurs when children's immunity decreases after chemotherapy, and they are prone to infection. This study aims to describe the cultural care behaviors of multidisciplinary health workers in preventing neutropenic fever among hospitalized children with leukemia after chemotherapy. Methods: An ethnonursing study was conducted at a cancer referral hospital in Central Java, Indonesia. Data were collected through observation in two Pediatric ward and chemotherapy unit for three months and FGDs and in-depth interviews with health workers who provided care services to leukemia patients post-chemotherapy. Data were collected from 14 informants: six nurses, four doctors, two clinical pharmacists, and two nutritionists. This study employed four phases of Leininger analysis for qualitative data to develop its thematic structures. Results: The analysis of the entire data has revealed five themes: (1) performing antiseptic and aseptic techniques consistently, (2) nurse and dietician collaboration in handling eating difficulties post-chemotherapy, (3) collaborative interactions interdisciplinary in handling medication, (4) patient placement in an isolation room adjusted to patients' conditions, and (5) nurse's challenge to educate patients and their families. Conclusions: Health workers from multiple disciplines play essential roles in preventing neutropenic fever by involving patients, parents, and other family members. It is necessary for the nurses to collaborate with other healthcare professionals and educate patients, and families to participate in the care interventions for these patients in minimizing the occurrence of infection among leukemia patients.

Abstract

Kepemimpinan merupakan kompetensi esensial yang dibutuhkan dalam keperawatan. Pentingnya kepemimpinan bagi perawat sebagai pendidik guna mendukung optimalisasi pembelajaran klinik. Pembelajaran klinik dengan strategi kepemimpinan dapat mencetak perawat penerus yang kompeten di pelayanan kesehatan. Tujuan literature review ini adalah mengeksplorasi determinan kepemimpinan perawat sebagai pendidik di pelayanan kesehatan. Systematik review ini menggunakan diagram alur. Artikel bahasa inggris yang diterbitkan tahun 2019 sampai Maret 2023. Database yang digunakan; Pubmed, Embase, Scopus, Wiley Online Library. Peneliti memanfaatkan tool covidence untuk proses seleksi data sampai ekstraksi serta menggunakan instrumen The Joanna Briggs Institute Critical Appraisal Checklist untuk mengevaluasi artikel. Enam artikel telah diidentifikasi dan dianalisis. Artikel memiliki desain Cross Sectional dan kualitatif. Systematic review menghasilkan tiga tema analitis: (1) pengembangan diri, pembelajaran belajar berkelanjutan, (2) role model bagi perawat, (3) penghargaan diri bagi perawat pendidik. Disimpulkan bahwa perawat sebagai pendidik penting untuk pengembangan diri, menjadi role model dan menghargai diri sebagai pendidik. Hal tersebut berimplikasi pada staf perawat lain dalam menjalankan peran profesional. Rumah Sakit perlu mendukung perawat sebagai pendidik di pelayanan kesehatan dengan menerapkan konsep kepemimpinan dalam pembelajaran klinik.

Abstract

A defined career path for nurses is essential, as they constitute the largest segment of healthcare workers and hold strategic and leadership roles in determining the quality of patient care and overall hospital quality. This study aims to analyze the concept of professionalism in relation to the implementation of career paths for nurses within hospital settings. This concept analysis follows the steps outlined by Walker & Avant. A literature search was conducted using the PubMed, Scopus, ScienceDirect, and ProQuest databases to identify articles published between 2014 and 2024. After filtering, 9 articles were selected from various scientific disciplines such as nursing, medicine, education, and other health-related fields. This study identified three main attributes related to the clinical professional ladder: 1) integrity, 2) justice, and 3) beneficence. Antecedents include education, workshops/conferences, experience, and professional identity. Consequences include ethics, patient outcomes, nursing outcomes, future career opportunities, job satisfaction, retention, salary, and organizational commitment. This concept analysis enhances our understanding of the three attributes of the career ladder and professionalism (CLp) and provides insights into future development and uncertainties in nursing management. This concept is highly valuable for nurse management settings, particularly in enhancing professionalism through career path programs. Ultimately, this concept analysis approach encourages both new and experienced nurses to maintain professionalism in the delivery of nursing care.

Abstract

More than 80% of breast cancer patients in Indonesia who are diagnosed are at an advanced stage. Advanced cancer treatment is directed at palliative care from the beginning of diagnosis.  Patients undergoing palliative care must adjust to the changing needs that occur during palliative care in line with the decline in the condition of the disease and the impact of its treatment. It can affect changes in the patient's daily life patterns. This study was conducted to explore the unmet needs of breast cancer patients during palliative care in the context of the culture and healthcare system in Indonesia. This study utilized a descriptive qualitative phenomenological method. In-depth interviews with semi-structured interview guidelines were conducted on 14 breast cancer patients undergoing palliative care. The thematic analysis uses the Colaizzi approach. The results of this study obtained three specific themes of unmet needs in the study participants: 1) the need to achieve the 'New Normal' to return to activities with enthusiasm and hope, 2) the need for appreciation and recognition, and 3) Support to overcome the fear of death and the uncertainty of the future. Breast cancer patients want to return to their activities with enthusiasm and hope despite facing physical limitations and emotional challenges. In conclusion, achieving the 'New Normal' is an important aspect of this adjustment process, which involves physical, emotional, social, and spiritual support. This finding differs from previous research, where other publications did not explicitly mention the need to achieve the 'New Normal'. Therefore, the role of nurses in addressing unmet needs, both in the context of the 'New Normal' and more generally, is crucial in palliative care for breast cancer patients. This support enhances the patient's quality of life and gives greater meaning to the remainder of their lives.

Abstract

ABSTRACT Nurses' negative attitudes in reporting patient safety incidents has an impact on the low number of reported incidents (underreporting). Reporting incidents is beneficial and can be a lesson to prevent the recurrence of similar incidents so it’s improve patient safety. Self-efficacy is a factor that encourages nurses to have a positive attitude towards reporting incidents. Efficacy is an individual's belief in their ability to overcome a particular task. This research aims to identify the relationship between self-efficacy and nurses' attitudes in reporting patient safety incidents in hospitals. This cross-sectional study involved 117 nurses selected using probability sampling. The study used the Reporting of Clinical Adverse Events (RoCAES) questionnaire and the General Self Efficacy (GSE) questionnaire. The results shows that there is a strong relationship between nurse’s self-efficacy and nurses' attitudes in reporting patient safety incidents (p= 0.001). High self-efficacy improves nurses' attitudes towards reporting incidents. Recommendations for hospitals to establish clear procedures regarding the types of incidents that must be reported, create a climate of no blame and shame, and promote nurses' self-efficacy by regularly holding training and sharing knowledge and experience in reporting patient safety incidents. Keywords: Attitude; Nurse; Patient Safety Incident; Self-Efficacy ABSTRAK Masalah sikap perawat yang negatif dalam melaporkan insiden berdampak pada rendahnya angka insiden yang dilaporkan (underreporting). Pelaporan insiden bermanfaat sebagai pembelajaran untuk mencegah terulangnya kejadian serupa sehingga dapat meningkatkan keselamatan pasien. Efikasi diri merupakan faktor yang mendorong perawat untuk bersikap positif terhadap pelaporan insiden. Efikasi merupakan keyakinan individu terhadap kemampuan yang dimiliki dalam mengatasi suatu tugas tertentu. Penelitian ini bertujuan untuk mengidentifikasi hubungan efikasi diri dan sikap perawat dalam melaporkan insiden keselamatan pasien di rumah sakit. Penelitian cross-sectional ini melibatkan 117 perawat yang dipilih secara probability sampling. Penelitian menggunakan kuesioner Reporting of Clinical Adverse Events (RoCAES) dan kuesioner General Self Efficacy (GSE). Hasil didapatkan adanya hubungan yang kuat antara efikasi diri perawat dan sikap perawat dalam melaporkan insiden keselamatan pasien (p= 0,001). Efikasi diri yang tinggi meningkatkan sikap perawat terhadap pelaporan insiden. Rekomendasi bagi pihak rumah sakit untuk menetapkan prosedur yang jelas terkait jenis insiden yang harus dilaporkan, menciptakan iklim tidak menyalahkan dan memalukan, dan meningkatkan potensi efikasi diri perawat dengan rutin mengadakan pelatihan dan berbagi ilmu serta pengalaman dalam melaporkan insiden. Kata Kunci: Efikasi Diri; Insiden Keselamatan Pasien; Perawat; Sikap

Abstract

ABSTRACT Low birth weight infants (LBW) remain one of the health concerns that require severe care because of the short-term and long-term impacts of LBW on children's health, growth, and development. Problems with LBW frequently arise when infants return home with their families, as home care is inextricably linked with parenting based on family socio-cultural patterns. Research related to the culture of low-birth-weight infant care at home has not been widely conducted. The purpose of the study was to explore mothers' experiences in caring for Low-birth-weight infants according to West Javanese cultural values. This qualitative study was conducted using a descriptive phenomenological approach. Ten indigenous Sundanese mothers from West Java who had low birth weight babies were purposively selected for this study. Data was obtained through semi-structured interviews, and then analyzed thematically. Three main themes were generated: mothers' knowledge and perceptions related to LBW infants; breast feeding and complementary feeding; LBW care in relation to cultural aspects. The conclusion that can be drawn from this study is increasing mothers' knowledge and proficiency in providing culturally responsive care for low-birth-weight babies is crucial. Nurses must prepare women thoroughly through culturally sensitive education while they are still in the hospital so that they can care for their newborns when they arrive home. Keywords: Mothers, Caregiving, Experiences, Low-birth-weight, Cultural  ABSTRAK Bayi berat lahir rendah (BBLR) masih menjadi salah satu masalah kesehatan yang memerlukan perhatian khusus karena dampak jangka pendek dan jangka panjang dari BBLR terhadap kesehatan, pertumbuhan, dan perkembangan anak. Masalah BBLR sering kali muncul ketika bayi kembali ke rumah bersama keluarganya, karena perawatan di rumah terkait erat dengan pola pengasuhan yang didasarkan pada pola sosial budaya keluarga. Penelitian yang berkaitan dengan budaya perawatan bayi BBLR di rumah belum banyak dilakukan. Tujuan dari penelitian ini adalah untuk mengeksplorasi pengalaman ibu dalam merawat BBLR menurut nilai-nilai budaya Sunda, Jawa Barat. Penelitian kualitatif ini dilakukan dengan menggunakan pendekatan fenomenologi deskriptif. Sepuluh ibu asli Sunda dari Jawa Barat yang memiliki bayi dengan berat badan lahir rendah dipilih secara purposif untuk penelitian ini. Data diperoleh melalui wawancara semi-terstruktur, dan kemudian dianalisis secara tematik. Tiga tema utama yang dihasilkan: pengetahuan dan persepsi ibu terkait BBLR; pemberian ASI dan MP-ASI; perawatan BBLR dalam kaitannya dengan aspek budaya. Kesimpulan yang dapat diambil dari penelitian ini adalah meningkatkan pengetahuan dan kemahiran ibu dalam memberikan perawatan yang tanggap budaya untuk BBLR sangat penting. Perawat harus mempersiapkan ibu secara menyeluruh melalui pendidikan yang peka budaya ketika mereka masih di rumah sakit sehingga mereka dapat merawat bayi mereka yang baru lahir ketika mereka pulang rumah. Kata Kunci: Ibu, Pengasuhan, Pengalaman, Bayi Berat Lahir Rendah, Budaya

Abstract

Culturally congruent care practices are essential for providing holistic and meaningful care, particularly for multicultural populations and the end-of-life patients. However, national policies have not been fully implemented within hospital internal policies. This study aimed to develop a culturally congruent care practices guideline and a nursing assessment form for the end-of-life patient. A case study employing the Eight-Step Kotter Change Model was used to develop, implement, and evaluate the guideline and form at Hospital X Jakarta. Socialization was conducted with ICU nursing staff and nurse managers. Nineteen out of forty-one study participants were purposively selected to participate in the evaluation. The results demonstrated that the development of the guideline and form successfully improved nurses’ knowledge related to culturally sensitive nursing, with 80% of the evaluation questions showing increased scores. Subsequently, the guideline and form were integrated into hospital policies and electronic medical records to ensure sustainability. A systematic approach using Kotter’s Model effectively facilitated the development of culturally congruent care practices. Leadership support and ongoing competency development are necessary to sustain these changes and enhance the quality of care.

Abstract

Organizational culture is summary of shared values, beliefs, or perceptions held by employees in  organization or organizational unit and can influence staff attitudes and behaviors. This has impact the quality of life of nurses, including health, emotional exhaustion, job satisfaction, work stress. . The purpose this study to explore relationship between organizational culture and levels of stress and burnout among nurses in hospitals, provide more objective and comprehensive description of the existing evidence, identify knowledge gaps, and provide recommendations based on consistent findings.This research method is  detailed analysis using the PRISMA flow diagram, Literature search in several databases, namely Proquest, Science Direct, Taylor & Francis Online, PubMed, Sage, Whiley and Scopus which are published. Article quality assessment using the JBI Joanna Briggs Institute Critical Appraisal Checklist for Analytical Cross Sectional Studies. Eleven articles were identified and analyzed that met the inclusion criteria; articles have abstract and full text, search type in the database with journal articles, published in English, articles published in the last 10 years 2014 - 2024, articles discussing organizational culture; positive organizational culture consisting of; open and transparent communication, strong social support, good teamwork, trust and appreciation for nurses, safe and comfortable work environment, articles discussing stress levels and burnout of nurses in hospitals. Exclusion criteria: articles cannot be accessed as full text, books, are literature reviews, theses, books. The results of the synthesis show a positive organizational culture is less likely to indicate stress levels and burnout in nurses.

Abstract

The incidence of hypertension is likely to occur at an increasingly younger age. The incidence is mainly due to risk factors for unhealthy behavior and lifestyle in adolescents. The purpose of this study was to analyze behavioral and lifestyle risk factors that affect the incidence of hypertension in adolescents. This research design is a descriptive-analytic with the cross-sectional study. The subjects in this study were 407 respondents from 10 junior and senior high schools in Bima City. Independent variables were gender, age, some behaviors like smoking, alcoholic, drug abuse, exercise, and healthy diet, abdominal circumference, and body mass index. Whereas, the dependent variable covers the incidence of hypertension. Results: In this study, behavioral risk factors in adolescents that correlate with hypertension are body mass index, age, and smoking behavior. The omnibus test results p value = 0.018 (
