Abstract

Introduction: Mobile health (mHealth) applications have emerged as promising tools to support diabetes self-management, yet their overall effectiveness remains to be systematically evaluated. Objective: This review evaluates the effectiveness of mobile health (mHealth) apps in promoting diabetes self-management behaviors by synthesizing findings from high-quality systematic reviews published in the last five years. Method: A comprehensive search was conducted across Scopus, PubMed, Cochrane Library, EBSCO Host, and ProQuest. Six systematic reviews met the inclusion criteria, with a mean AMSTAR quality score of 10.33 out of 11. Reviews were analyzed for clinical outcomes, user engagement, and adherence. Result: Findings indicate that mHealth apps significantly improve glycemic control, evidenced by reductions in HbA1c levels. Additional benefits include increased user engagement, better adherence, behavioral improvements, and psychosocial outcomes such as reduced anxiety and improved quality of life. However, heterogeneity across studies, challenges related to digital literacy, and concerns about data privacy limit generalizability. Conclusion: mHealth apps show strong potential to enhance diabetes self-management through clinical and psychosocial benefits. Future work must address barriers such as digital literacy, data security, and variability in intervention quality to support sustained adoption and effectiveness.

Abstract

Introduction: This study aimed to evaluate the impact of a cadre-led intervention on physical health, cardiovascular knowledge, and mental well-being in rural Indonesian communities. Cardiovascular disease remains a leading cause of mortality in Indonesia, particularly in rural areas with limited healthcare access and low health literacy. With a growing burden of non-communicable diseases in low-resource settings, the objective was to assess a community-based approach addressing early detection and lifestyle modification gaps in the existing literature. Methods: This quasi-experimental one-group pre-post study was conducted over six weeks in a rural Indonesian setting. A total of 91 adult participants were enrolled, and data were collected through anthropometric measurements, biochemical tests, a 15-item cardiovascular knowledge questionnaire, and the Patient Health Questionnaire-9 (PHQ-9) for mental health screening. Ethical approval was obtained from the Medical and Health Research Ethics Committee of the Faculty of Medicine and Health Sciences, Universitas Muhammadiyah Yogyakarta, Indonesia. Results: The primary outcome of the study was the change in cardiovascular risk indicators and knowledge. Findings showed a significant reduction in BMI (mean change = -0.56 kg/m², 95% CI: -0.83 to -0.29, p < 0.001, Cohen’s d = 0.47) and waist circumference (mean change = -0.91 cm, 95% CI: -1.80 to -0.02, p = 0.044, Cohen’s d = 0.28), along with a significant improvement in cardiovascular knowledge scores (mean change = +9.00, 95% CI: +3.44 to +14.56, p = 0.002, Cohen’s d = 0.68). Additionally, PHQ-9 screening showed that 76.9% of participants reported minimal depressive symptoms at the time of assessment. Statistical analyses indicated the effectiveness of the intervention in improving physical and knowledge-related health outcomes. Conclusion: This study contributes to the understanding of health promotion in low-resource rural settings by demonstrating the effectiveness of cadre-led interventions for cardiovascular contexts and the feasibility of integrating mental health screening into community programs. Future studies should explore long-term sustainability and scalability, advancing knowledge in the field of international health.

Abstract

Telepharmacy, a critical subset of telemedicine, has gained prominence as a transformative innovation addressing challenges in pharmaceutical care by enabling the remote delivery of services, including medication dispensing, patient counseling, and therapeutic monitoring. This narrative review investigates the state of telepharmacy in Indonesia, with a focus on the knowledge, attitudes, and perceptions of young pharmacists in the Special Region of Yogyakarta. The findings reveal significant knowledge gaps concerning telepharmacy tools and regulatory frameworks, which adversely influence pharmacists’ attitudes and perceptions. Although young pharmacists demonstrate higher technological proficiency and generally positive attitudes toward telepharmacy, hesitancy persists due to insufficient training opportunities and ambiguities in existing regulations. Telepharmacy offers substantial opportunities to bridge geographical barriers and enhance healthcare access, particularly in underserved and rural regions. However, its implementation is hindered by critical barriers, including regulatory gaps, inadequate technical infrastructure, and limited awareness among stakeholders. Drawing on successful telepharmacy models from the United States, Canada, and Australia, this review underscores the need for targeted educational initiatives, robust infrastructural investments, and regulatory reform to support telepharmacy adoption. Addressing these challenges will position telepharmacy as a vital tool in transforming Indonesia’s healthcare system, fostering equitable access, and advancing pharmaceutical care nationwide.

Abstract

Diabetes mellitus (DM) is a chronic metabolic disorder characterized by persistent hyperglycaemia due to ineffective insulin use. In Indonesia, DM is a leading non-communicable disease. Medication adherence plays a critical role in managing DM, yet barriers such as long treatment regimens and limited patient-provider communication often hinder adherence. This study evaluated the effectiveness of a video-based digital intervention in improving medication adherence among DM patients at Public Health Center of Pasar Simpang in Kota Agung Timur, Tanggamus Regency. The study employed a cross-sectional design with 62 respondents divided into control and experimental groups. The intervention involved educational videos shared via WhatsApp. Adherence was assessed using the MMAS-8 (Morisky Medication Adherence Scale). Results revealed that adherence levels significantly improved in the experimental group post-intervention. The pre-test MMAS-8 mean score was 11.23±1.26, increasing to 13.97±1.33 post-intervention, with a mean difference of 2.74 (p = 0.000). In contrast, the control group showed no significant change, with pre- and post-test mean scores of 12.55±1.55 and 12.90±1.42, respectively (p = 0.315). Overall, 79% of respondents demonstrated good adherence, with significant improvements noted in the intervention group. These findings highlight the potential of video-based digital interventions in enhancing adherence among DM patients, providing valuable insights into diabetes care.

Abstract

Pharmaceutical services in Community Health Centers play an essential role in ensuring rational drug use and quality of health services. The government has set standards for pharmaceutical services through Ministerial Regulation No. 74 of 2016 and Ministerial Regulation No. 26 of 2020. However, implementation has not been optimal in several areas, especially with difficult access. The purpose of this study was to observe the implementation of pharmaceutical service standard policies in Community Health Centers in the peripheral area of Samarinda City, East Kalimantan. This study used a mixed method approach with data collection techniques through observation with questionnaire instruments, in-depth interviews, and document reviews. The results of the study showed that pharmaceutical preparation management activities had been carried out very well (94-100%) based on applicable standards. Meanwhile, the implementation of clinical pharmacy services showed variations in achievement among the health centres. Several activities such as prescription assessment and counselling have been optimal, but other indicators such as monitoring of drug side effects, evaluation of drug use, and inpatient visit activities have not been running optimally. In conclusion, the implementation of pharmaceutical services at the Health Centers in the peripheral area of Samarinda City has mostly been following standard policies, especially in the aspect of managing pharmaceutical preparations and disposable medical materials. However, strengthening of clinical pharmacy services is still needed to optimize the role of pharmacists in comprehensive pharmaceutical services.

Abstract

Electronic Medical Records (EMRs) are increasingly recognized as vital tools for enhancing the efficiency, accuracy, and quality of healthcare delivery. Despite regulatory mandates in Indonesia, the adoption of EMRs remains uneven, particularly in rural healthcare settings. This study applied the Unified Theory of Acceptance and apply of Technology (UTAUT) to investigate the behavioral intention of healthcare professionals working in private hospitals to use electronic medical records. A quantitative, cross-sectional design was implemented involving 90 participants selected through purposive sampling in an Indonesian hospital. The study's data were gathered between October 2024 and January 2025 using a validated 18-item UTAUT-based questionnaire. Data analysis was conducted with SPSS and SmartPLS software. Results indicated that all four UTAUT construct – Performance Expectancy (β = 0.200, p = 0.016), Effort Expectancy (β = 0.353, p < 0.001), Social Influence (β = 0.291, p < 0.001), and Facilitating Conditions (β = 0.262, p = 0.008 – had statistically significant positive effects on Behavioral Intention. The model demonstrated moderate explanatory power (R² = 0.655) and strong predictive relevance (Q² = 0.512). These results validate the UTAUT model's suitability in this context and provide practical insights for strengthening EMR implementation strategies. Future research should consider longitudinal approaches and multi-site comparisons to enhance generalizability and policy relevance.
