Abstract

Latar Belakang: Pandemi Covid-19 membuat masyarakat menjalankan pola hidup baru dengan menerapkan protokol kesehatan dengan membersihkan tangan menggunakan hand sanitizer. Hand sanitizer diformulasikan untuk dapat menghambat dan membunuh mikroorganisme dengan cara mendenaturasi lipid, namun efek dari cara kerjanya ini dapat meningkatkan risiko terjadinya dermatitis tangan. Objektif: Penelitian ini bertujuan untuk mengetahui hubungan pola penggunaan hand sanitizer dengan kejadian dermatitis tangan. Metode: Penelitian ini merupakan penelitian analitik observasional dengan metode cross-sectional study dengan teknik simple random sampling pada mahasiswa Program Studi Kedokteran Fakultas Kedokteran Universitas Andalas Angkatan 2021 dengan jumlah sampel sebanyak 80 orang. Data responden diperoleh dari kuesioner dan dianalisis menggunakan uji chi square. Hasil: Hasil penelitian ini didapatkan sebagian besar mahasiswa menggunakan hand sanitizer lebih dari 6 bulan (82,5%) dengan frekuensi 1 sampai 5 kali dalam sehari (86,3%). Lebih dari setengah mahasiswa menggosok tangan kurang dari 10 detik setiap menggunakan hand sanitizer (75%), dan tidak menggunakan pelembab (57,5%). Sebagian besar mahasiswa tidak mengalami dermatitis tangan (77,5%) dan tidak memiliki riwayat atopi (80%). Tidak terdapat hubungan yang signifikan antara durasi penggunaan, frekuensi penggunaan, durasi scrubbing penggunaan hand sanitizer dan penggunaan pelembab dengan kejadian dermatitis tangan dengan nilai p=0.599 untuk durasi penggunaan, p=0.086 untuk frekuensi penggunaan, p=1.00 untuk durasi scrubbing, dan p=0.316 untuk penggunaan pelembab. Terdapat hubungan antara riwayat atopi dengan kejadian dermatitis tangan dengan nilai p=0,000. Simpulan: Tidak terdapat hubungan yang signifikan antara pola penggunaan hand sanitizer dengan kejadian dermatitis tangan khususnya durasi penggunaan, frekuensi, dan durasi scrubbing dan penggunaan pelembab, namun terdapat hubungan antara riwayat atopi dengan kejadian dermatitis tangan. Abstract

Background: Coronavirus Disease 2019 (COVID-19) is a respiratory system disease transmitted to humans and has infected humans in almost every country. Plasma cytokine levels, such as C-reactive protein (CRP), are elevated inflammatory markers in most COVID-19 patients and are routinely examined, especially in the intensive care Unit (ICU). Elevated CRP levels are associated with higher rates of severity and death. Objective: This study aimed to determine the correlation between CRP levels and outcomes in COVID-19 patients at COVID ICU RSUP. Dr. M. Djamil Padang Period July – December 2021. Method: This research was conducted using by analytic observational method with a retrospective cohort design. Secondary data was collected from the medical records of COVID-19 patients treated in the ICU COVID RSUP. Dr. M. Djamil Padang from July – December 2021, using total sampling as a technique, 107 samples met the inclusion criteria. Data analysis was carried out using univariate and bivariate methods. Result: The result shows more male patients (51.4%) than women (48.6%). From the age range, most age was found in the range > 59 years (54.2%). The most common type of comorbid disease was diabetes (32.7%). The highest CRP level at the initial ICU admission was >160 mg/L (70.1%), and the highest ferritin level was >434 ng/mL (86.0%). The most common patient outcome was death (65.4%). There is a relationship between CRP levels (p = 0.029) and the outcome of COVID-19 patients in the COVID ICU. Conclusion: This study concludes that there is a correlation between C-Reactive Protein (CRP) levels and Outcomes in COVID-19 patients at COVID ICU RSUP. Dr. M. Abstract

Numerous studies have been carried out to ascertain the detrimental impact of air pollution on allergy conditions such as asthma, allergic rhinitis, and atopic dermatitis. Substandard indoor air quality exacerbates atopic dermatitis, which can be attributed to several reasons. Proper air circulation is crucial in households, especially in areas with high foot activity, since it can lead to elevated amounts of air contaminants entering the space. Indoor air pollutants, such as house dust mites, volatile chemical compounds, particle matter, and indoor mold, can aggravate atopic dermatitis. An air purifier effectively enhances the quality of indoor air. This gadget offers significant advantages for those with atopic dermatitis by effectively mitigating the frequency and intensity of disease relapses.

Abstract

Background: Vitiligo, a common depigmenting skin disorder, has an estimated prevalence of 0.5–2% of the population worldwide. The COVID-19 pandemic lockdown measures significantly affected the readiness of vitiligo patients to contact dermatologists. The aim of this study is to describe vitiligo patient visits during a pandemic situation. Methods: A retrospective descriptive study, using secondary data obtained from Dr. M. Djamil General Hospital Padang medical records during the period of 2019–2021. Results: During the pandemic (2020-2021), a total of 7 in 2020 and 19 in 2021 new vitiligo patient have been consulted. The ratio of male/female for two years was 16/10. The age of group 26-24 years had more frequently visited. A total 6 patients had segmental vitiligo and 16 patients had non segmental vitiligo. A total 4 patient had unstable vitiligo. During the pandemic, there were various vitiligo therapy such as oral and topical corticosteroid, topical immunomodulator, phototherapy, and punch grafting surgery. Compare to before the pandemic situation in 2019, a total 25 new vitiligo patient have been consulted in our outpatient clinic. Compared to 2019, the number of visits decrease significantly 72% in 2020 and 24% in 2021. Conclusion: The pandemic COVID-19 had an unprecedented impact on number of vitiligo patient visits to the tertiary hospitals. This condition may affect visits and patient compliance. There were significant reductions in the number of outpatient visits due to pandemic COVID-19.

Abstract

Background: Familial hypercholesterolemia (FH) is a rare inherited disorder characterized by high cholesterol levels, primarily high levels of low-density lipoprotein cholesterol (LDL-C). This condition can lead to premature cardiovascular disease. A less recognized but significant manifestation of FH is the development of multiple tuberous xanthomas, which are exogenous masses or fatty deposits on the skin. Case presentation: This report describes the case of a 4-year-old female child who presented to the Dermatology, Venereology, and Aesthetics Department of Dr. M. Djamil General Hospital Padang with multiple yellowish, painless, and non-itchy lumps on her body. These lumps were located on her knuckles, hands, elbows, buttocks, knees, back of knees, ankles, and feet. The lumps had been progressively increasing in size over the past month. Her medical history revealed that she first developed a yellowish lump on her right ankle two years prior, which gradually spread to other areas of her body. A dermoscopic examination showed a yellow-brown structureless area, and laboratory tests confirmed hypercholesterolemia with elevated LDL levels. Her triglycerides were normal, and both of her parents also had hypercholesterolemia. A histopathology examination confirmed the diagnosis of xanthoma. The patient was diagnosed with tuberous xanthomas and treated with simvastatin 1x10 mg, along with surgical excision of the xanthomas and flap reconstruction under general anesthesia. Conclusion: This case highlights the importance of a multidisciplinary approach in managing multiple tuberous xanthomas in a child with FH. While surgical excision can effectively remove the lesions, ongoing lipid-lowering therapy is crucial to prevent recurrence and reduce cardiovascular risks associated with FH. Collaboration among dermatologists, pediatric cardiologists, and pediatric nutritionists is vital to ensure comprehensive care and enhance the patient's overall quality of life.

Abstract

Background: Urticaria pigmentosa (UP) is the most common form of cutaneous mastocytosis in children, accounting for 70-90% of cases. UP typically presents in infancy or early childhood with characteristic skin lesions that vary in appearance. The hallmark of UP is the presence of reddish-brown macules or papules that urticate or blister upon rubbing, a phenomenon known as Darier's sign. Histopathological examination of a skin biopsy is confirmatory, demonstrating an increased number of mast cells in the dermis. Case presentation: We report a case of early-onset UP in a 7-month-old infant boy who presented with multiple hyperpigmented macules all over his body. The lesions were pruritic and had progressively increased in size and number over the past two weeks. Dermatological examination revealed multiple erythematous macules on the face, hyperpigmented macules on the chest, abdomen, and extremities, hyperpigmented plaques on the legs, and a nodule on the back. The lesions exhibited a positive Darier's sign, characterized by swelling and redness upon rubbing. Systemic examination was unremarkable, with no evidence of hepatomegaly, splenomegaly, or lymphadenopathy. Dermoscopy of the lesions revealed a central pigment network and a light brownish peripheral structure, suggestive of UP. Histopathological examination of a skin biopsy confirmed the diagnosis of UP, demonstrating an increased number of mast cells in the dermis. The patient was treated with oral antihistamines (cetirizine) and topical corticosteroids (hydrocortisone) for four weeks, resulting in significant regression of the lesions. Conclusion: This case highlights the importance of recognizing the clinical features of UP in infants. Early diagnosis and appropriate management can help alleviate symptoms and improve the quality of life for affected children.

Abstract

Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis that presents with rapidly progressive, painful ulcers and may be diagnostically challenging because it mimics infection, vasculitis, and other ulcerative dermatoses. This report describes an 88-year-old Minang woman who presented with multiple enlarging, painful ulcers on both lower legs and thighs over six months, with significant functional limitation requiring wheelchair use. Dermatological examination showed multiple ulcers with granulation to necrotic bases, irregular margins, and undermined edges, with surrounding erythema and hyperpigmentation. Laboratory evaluation demonstrated mild anaemia with otherwise unremarkable results, while Gram staining showed no bacteria. Dermoscopy revealed irregular ulcer borders with yellow-brown crusting on an erythematous background. Histopathology supported ulcerative PG by demonstrating epidermal hyperplasia with acantholysis, inflammatory cell infiltration including polymorphonuclear leukocytes and lymphocytes, mixed dermal infiltrate, and evidence of vascular destruction. The patient was treated with systemic methylprednisolone alongside topical wound care (saline compresses, topical antibiotics, and topical corticosteroids), resulting in reduced pain and lesion improvement at two-week follow-up with no new ulcers. This case highlights the importance of recognising characteristic clinical features, excluding infection, and using histopathology to support diagnosis, particularly in elderly patients where disease burden and complications may be greater and management must prioritise gentle wound care and avoidance of trauma.

Abstract

Pendahuluan: Subcorneal pustular dermatosis (SPD), atau penyakit Sneddon–Wilkinson, merupakan dermatosis neutrofilik kronis yang jarang, ditandai dengan pustul superfisial berulang. SPD semakin banyak dilaporkan berhubungan dengan penyakit sistemik, terutama gangguan metabolik seperti diabetes melitus tipe 2 dan dislipidemia, yang dapat berkontribusi terhadap onset dan kekambuhan penyakit. Laporan kasus: Seorang perempuan berusia 36 tahun yang datang dengan keluhan bercak kemerahan berulang disertai pustul superfisial flaksid berwarna kekuningan yang gatal pada daerah perut dan punggung bawah selama dua bulan terakhir. Lesi awalnya membaik dengan kortikosteroid topikal tetapi kambuh kembali dua minggu kemudian dengan cakupan yang lebih luas. Pasien tidak mengeluhkan demam maupun riwayat konsumsi obat sebelumnya. Pasien memiliki penyakit dasar diabetes melitus tipe 2 dan dislipidemia, keduanya dalam kondisi tidak terkontrol dengan baik. Pemeriksaan klinis menunjukkan plak eritematosa multipel dengan pustul superfisial, krusta, erosi, serta makula hiperpigmentasi pada perut dan punggung bawah. Pemeriksaan dermoskopi menunjukkan multiple yellow-white globule di atas dasar eritematosa yang dikelilingi sisik halus. Evaluasi psikologis menunjukkan tingkat kecemasan sedang dan dampak sedang terhadap kualitas hidup.  Kesimpulan: Kasus ini menekankan pentingnya mengevaluasi dan menangani komorbiditas sistemik, karena disfungsi metabolik dapat berkontribusi terhadap inflamasi persisten, kekambuhan berulang, dan respons terapi yang kurang optimal pada Subcorneal Pustular Dermatosis.
