Abstract

Latar Belakang: Pandemi COVID-19 dapat berdampak pada status kesehatan wanita hamil serta pada pelayanan kesehatan. Selama pandemi COVID-19, terjadi penurunan kualitas pelayanan kesehatan ibu. Peningkatan kualitas layanan kesehatan dapat dilakukan dengan mempertimbangkan perspektif pengguna layanan untuk meningkatkan kompetensipenyedia layanan, serta mengetahui harapan dari pengguna layanan kesehatan terhadap pelayanan kesehatan yang diberikan.Tujuan: melihat apakah pelayanan kesehatan ibu dengan berdasarkan pedoman dan standar sudah sesuai dengankebutuhan ibu untuk pelayanan kesehatan maternal. Metode: Penelitian ini merupakan penelitian kualitatif dengan pendekatan fenomenologi untuk mengeksplorasi bagaimana pengalaman ibu hamil yang pernah terkonfirmasi COVID-19 dalam pemanfaatan layanan kesehatannya. Penelitian ini akan dilaksanakan di Kabupaten Sleman, Daerah IstimewaYogyakarta setelah mendapatkan ethical clearance dari Komisi Etik. Hasil: Pandemi COVID-19 keseluruhan berdampak secara biologis, psikologis, maupun sosial terhadap wanita hamil. Sebagian besar pelayanan kesehatan maternal di masa pandemi belum berjalan secara optimal baik dari segi pelayanan yang berfokus pada pasien, waktu tunggu, serta belum optimalnya pengadaan pelayanan psikologis kesehatan untuk ibu hamil dengan COVID-19 maupun untuk ibu hamil dalam kondisi normal. Harapan ibu hamil untuk pelayanan kesehatan yaitu adanya peningkatan fasilitas kesehatan untuk ibu hamil dengan COVID-19, adanya penguatan pengawasan, pemantauan, dan peningkatan rasa empati petugas kesehatan, serta adanya layanan psikologis untuk ibu hamil secara umum. Kesimpulan: Selama pandemi COVID-19, layanan kesehatan dapat memenuhi kebutuhan perawatan secara biologis namun belum mampu untuk memenuhi kebutuhan perawatan psikologis dan sosial. Diperlukan peningkatan respon dan empati dari petugas kesehatan dan peningkatan pelayanan kesehatan yang lebih berfokus pada kebutuhan menyeluruh wanita hamil. Abstract

Support from spouse and hospital-based self help group members for hemodialysis patients in public hospitalPurpose: This study aims to document examples of social support given by hemodialysis friends and self-help group to people undergoing hemodialysis in the hospital.Method: This is a qualitative research with phenomenology research design. Subject was selected using purposive sampling based on criteria inclusion and maximum variation. Data were collected by in-depth interview, consisted of 11 informants (6 male and 5 female) with length of hemodialysis experience between 6 months to 2 years. In addition, family were also invited for in-depth interview. The validity of data were assessed through triangulation and peer debriefing.Results: This study documented examples of friendliness among people who undergoing hemodialysis in the hospital. Occasionally they schedule community meetings or just a joint meal between patients and families after the hemodialysis therapy schedule takes place. At such meetings, they can exchange experiences and transfer the information from the old members to the new one. The group involved activities outside the hospital, the group involved new members to work together at local and national events in support of chronic kidney diseases patients.Conclusion: Our findings show that depressed patient who undergoing the hemodialysis gained new perception of their diseases and got a support to continue their treatment through self-help group. This research emphasize the role of hospitals as entry points for sufferers to find groups of fellow sufferers whose social support programs.

Abstract

Penetration of the internet and social media in Indonesia is increasing, with the most significant percentage of its users coming from the youth. The internet and social media can be a medium for promoting and selling products to youth. This study aimed to identify and describe videos potentially suggested when users enter the keyword "rokok" in a search on YouTube. We hypothesized that tobacco-related videos on YouTube could contain materials that either refer to (1) tobacco advertising, promotion, and sponsorship (TAPS); or (2) Non-TAPS categories. Both types of materials could have different engagement from the viewers. A YouTube search was performed in October 2022, and we only consider the videos published during June 2022. We assigned our team to classify the video content to the TAPS and Non-TAPS categories based on its title and description. We analyzed the number of views, likes, and comments of each video descriptively. We obtained 210 videos and excluded 24 videos unrelated to the study objectives in terms of the context or the language used. Most videos are related to TAPS (84%), and very few contain Non-TAPS (16%), in particular anti-smoking, including Public Service Advertisements (3%). Non-TAPS-related videos were mainly published by News Channels and the Government Public Relations Office Channels (57%), while individuals, including MSMEs and retailers, seemingly published most TAPS-related videos (78%). In addition, TAPS-related videos tend to have a higher number of likes and comments from viewers. YouTube is a potential medium to attract viewers and promote health, but TAPS-related videos are much easier to find. Public health agencies should establish a more significant presence on YouTube to reach more people and engage in broader partnerships to promote health educational material.

Abstract

Production of ‘Jamu Jogorogo’ herbal medicine product by the Group of Women Farmers (KWT Singosaren), in Kab. Bantul, Yogyakarta is stagnant after receiving a Dana Keistimewaan Provinsi (Yogyakarta Province’s Special Fund), and has passed a halal certificate. Based on the Ishikawa fishbone analysis, basic knowledge of legal aspects, quality, and marketing strategy. This study was designed to evaluate the effectiveness of an educational strategy applied to the members of KWT Singosaren. This strategy was implemented through a ‘one-day training’ that began with a Pretest, Post-test I, and Post-test II. A total of 80 members of KWT Singosaren were involved in this training, and 59 met the eligible criteria. A Pretest was conducted before the training to get the initial. After the training, participants worked on the Post-test I. Before leaving the training location, all participants were given a pocketbook containing training material for self-learning. Two weeks (14 days) later, they were asked to take the Post-test II, so the participant’s final knowledge could be investigated. As a result, it was found that there is a rising knowledge of 113.56 % and 71.96 % regarding the understanding of ‘quality’ and ‘legality’, respectively. Interestingly, the marketing strategy (product appeal) and sustainable production knowledge rose by 15.25%. Even so, after Post-test II an increase in scores still occurred. This indicates that participants successfully conducted the self-learning process. Post actualization period, we found that the SPP-IRT permits (legal permits for food production for the Home Industry) of 3 (three) herbal products were successfully issued. Keywords: Empowerment, women, effectiveness, knowledge, training.

Abstract

Background: The World Health Organization (WHO) encourages the integration of traditional medicine (TM) into healthcare services, recognizing its cultural importance in countries like Indonesia. However, this integration needs improvement to ensure quality and accessibility. Previous research has primarily focused on policy implementation or health outcomes with limited exploration of the acceptability of TM clinics in Primary Health Centers (PHCs) from the perspectives of stakeholders using the Theoretical Framework of Acceptability (TFA). Operational and ethical challenges of this integration remain underexplored.Purpose: The purpose of this study was to explore the acceptability of traditional medicine clinics in PHC settings using the TFA, focusing on the perspectives of healthcare workers, traditional medicine practitioners, and patients.Methods: An exploratory qualitative study was conducted in five PHCs in Boyolali, Indonesia, with 25 participants, including healthcare workers, traditional medicine practitioners, and patients. Participants were purposely selected based on the following inclusion criteria: (1) healthcare workers providing services in traditional medicine clinics at PHCs, (2) traditional medicine practitioners operating within the clinic area, and (3) patients who had used the clinic’s services at least once. Data were collected through in-depth interviews and analyzed using a thematic approach.Results: Thematic analysis identified four key themes: (1) Perceived Benefits and Trust in TM Clinics, with participants reporting high satisfaction and trust in the services, (2) Readiness and Competency of Healthcare Workers, emphasizing confidence, ongoing training, and collaboration with traditional medicine practitioners, (3) Barriers to Acceptability and Implementation, including heavy workloads, limited resources, and a lack of dedicated personnel, and (4) Alignment with Professional and Cultural Values, focusing on the importance of cultural integration and ethical considerations for the sustainability of TM clinics.Conclusion: Traditional medicine clinics in PHCs are generally well accepted, but their sustainability depends on addressing resource constraints and enhancing staff training. Strategic investment and policy support are crucial for overcoming operational and ethical challenges and ensuring the successful integration of public health services.

Abstract

Purpose: This study aims to determine the implementation of health promotion, including input, process, output, and outcome components in specific and sensitive nutrition interventions for handling stunting in the working area of Bati Health Center. Methods: This qualitative research uses the descriptive method with a purposive sampling technique so that the number of informants must meet the information adequacy requirements. Data analysis was carried out in a qualitative descriptive setting using a case study approach, which focused intensively on a particular object and studied it as a case. Case study data can be obtained from all parties concerned; in other words, data is collected from various sources. Results: In the input component, supporting health services and health promotion facilities, namely road access/transportation and electricity, still need to be improved. Another obstacle is the delay in providing incentives for health promotion human resources, which is related to delays in village funds. Conclusion: Cross-sectors need to support the implementation of health promotion in villages, including specific and sensitive nutrition interventions, to optimize the reduction of stunting rates in the working area of the Bati Health Center.

Abstract

Purpose: This research aims to analyze medical waste management practices and factors influencing medical waste management in Jember Regency hospitals. Methods: This study employed a qualitative research design with a case study approach. Research data was obtained from the results of in-depth interviews and observations. The determination of research informants was conducted using purposive sampling, involving 16 informants. Data analysis used causal loop diagram (CLD) analysis and the importance-performance analysis (IPA) method. Results: Medical waste management activities in Jember Lung Hospital and Jember Klinik Hospital were generally good. However, some activities could have been more suitable, including sorting, storage, in-situ transportation, and temporary storage. The two hospitals already had incinerators, but the incinerators were damaged due to a lack of maintenance. Therefore, the final medical waste management activities of the two hospitals were carried out with a licensed third party, PT. Sagraha, PT. Wastec International and PT. Universal Eco Pacific. The causal loop diagram (CLD) analysis reveals that the budget availability factor plays a crucial role in supporting effective medical waste management activities, particularly in terms of facility and infrastructure availability, human resource availability, and SOP implementation. Additionally, the quality of human resources and hospital accreditation has a positive influence on suitable medical waste management activities. The importance-performance analysis (IPA) method identified the main priority scale that requires improvement, specifically the hospital staff's discipline. Conclusion: Medical waste management activities in the Jember Lung Hospital and Jember Klinik Hospital, in general, have followed the RI Minister of Health Decree Number 1204/Menkes/SK/X/2004 and PerMenLHK No. 56 of 2015. However, segregation activities were still not optimal, and medical waste containers were not equipped with an infectious symbol. Therefore, efforts to increase periodic monitoring and evaluation of officer performance can improve officer discipline through SOP.

Abstract

Purpose: This study aimed to analyze activities related to using plastic waste in ecobricks for elderly school students. Methods: This type of research is qualitative and has a case study design. The sampling technique used is purposive sampling. The data analysis used is thematic research. Results: The description of implementing plastic waste into eco-bricks, as well as the preparation, implementation, monitoring, and evaluation stages. The results of this eco-brick-making activity are assessed from the quantity, three or four eco-bricks a month; the quality results are the weight of the eco-bricks according to the standard, and they are made in the form of tables, chairs, and decorations, so that they are more valuable. Environmental changes have occurred since the existence of eco-bricks; no plastic waste is thrown carelessly, and no plastic waste is burned. Conclusions: Elderly school students have carried out eco-bricks-making activities so that they impact environmental health for the better.

Abstract

Objective: To describe the expectation of PLHIV and key populations regarding HIV/AIDS services, especially on pandemic COVID-19. Method: This study conducted a pilot questionnaire survey in Puskesmas Tegalrejo and Mantrijeron, Yogyakarta; and Puskesmas Cilacap Selatan 2, Puskesmas Kesugihan 2, in Cilacap Region, Central Java. The participants were 62 PLWHA and 60 key populations (transgender, male sex workers, IDUs, and sex workers). This paper uses the data from an open-ended item in the questionnaire on respondents' expectation of HIV services in the puskesmas. Result: Both PLHIV and key populations expected equal service, friendly and respectful services without differentiating from other patients. Health workers would be better to know about the diversity of gender and sexual orientation and educate about HIV/AIDS transmission to reduce the fear and stigmatization of the community. In addition, the rotation issues of health workers should not change the quality standards of HIV/aids services. PLHIVs expected that health workers do not disclose their status to others (which should have become the standard of health professionals’ practice) and provide complete support and counseling related to HIV/AIDS with explaining details to the patients. ARV stock is expected to get a one-month stock supply, and queuing for taking drugs. Furthermore, key populations expect flexible time on VCT tests, especially in pandemic COVID-19. Conclusion: The availability and access of puskesmas services for PLHIVs and key populations was significant progress in health care systems. Notes from users emphasize the need for management support in building inclusive services, where health and administrative workers and other puskesmas treated everyone in need equitably.

Abstract

Objective: To describe the challenges and facilitating factors in handling domestic medical waste during the COVID-19 pandemic based on our community empowerment program in Dusun Ngrombo I, Karangmojo Village, Gunungkidul District, DIY. Content: During the Covid-19 pandemic, medical waste increased in the community, especially for families who were self-isolating at home. In our preliminary study, the community processes waste by burning, burying and throwing it into the environment. Expired medicines are even used as a mixture of plant fertilizers and animal feed. Public literacy about medical waste management is still limited. The local government has not provided medical waste facilities in public spaces. Waste management is not optimal due to the lack of coordination between the relevant government agencies. Furthermore, there has been limited effort from the local primary health care service (puskesmas) to solve medical waste problems. Limited human resources, budgets and different perspectives on regulations cause the puskesmas to resist in responding to these problems. Medical Waste Bank Program is a community waste collection system that manages household medical waste through local waste bank activities. Generally, the Waste Banks (Bank Sampah) in Indonesia only process domestic waste, this program innovates by providing community medical waste services that are needed, especially during this pandemic. Waste bank staff will process the waste that has been collected in the waste bank, then handed over to a third party (private company) through the puskesmas. This can help the puskesmas to reach medical waste in their working areas and also to prevent the transmission of infectious diseases in the community. Conclusion: The waste bank can become a movement to manage community medical waste. Each sector needs to work together in promoting health, providing adequate waste facilities, increasing human resource capacity, as well as monitoring and assisting in managing domestic medical waste.
