Abstract

The quality control of herbal medicine should be started from the determination of the active substance at harvest time. The Raman spectrometry has been used for this propose. The aim of this study is to determine the quantification of curcumin in turmeric rhizome (Curcuma longa Linn.) using Raman spectroscopy combined with multivariate analysis of PLS-R that are expected to provide reference method for quality control in turmeric rhizome, especially for raw materials of Herbal drugs. Parameters that can be used for analysis of curcumin levels on turmeric rhizome obtained using intensity of data Raman and the data obtained from the standard method will be processed with multivariate analysis methods PLS-R. The validation value of quantification result using Raman-PLSR is seen from R2 value of 0.957, RMSEC value of 0,199 and p-value of 0.00. The study showed the developed method could be implemented on to determine the prediction quantification of raw material herbal medicine.Keyword : Curcumin,  PLS-R, Turmeric, Raman Spectroscopy

Abstract

Pada era jaminan kesehatan nasional, salah satu target pelayanan kefarmasian residensial adalah pasien hipertensi yang masuk ke dalam kategori kelompok pasien rujuk balik. Penelitian ini bertujuan untuk mengetahui pengaruh pelayanan kefarmasian residensial yang dilakukan apoteker terhadap ketaatan dan luaran klinis pasien hipertensi tanpa penyakit penyerta yang menjalani rawat jalan di Rumah Sakit Umum Daerah Wangaya Kotamadya Denpasar Bali. Desain penelitian ini adalah eksperimental dengan one group pre-post test design. Sebanyak 13 pasien yang memenuhi kriteria inklusi diberikan pelayanan kefarmasian residensial selama 16 kali kunjungan dalam kurun waktu tiga bulan. Skor hasil pengukuran dianalisis menggunakan regresi linier dan Uji Wilcoxon. Pelayanan kefarmasian residensial mampumeningkatkan ketaatan pasien dalam penggunaan obat, pengaturan diet, dan aktivitas fisik dari tingkat baik menjadi tingkat ketaatan tinggi (p=0,001) serta dapat meningkatkan ketaatan pasien terhadap pembatasan konsumsi alkohol dan rokok dari tingkat baik menjadi sangat baik. Pelaksanaan pelayanan kefarmasian dapat memberikan pengaruh dalam perbaikan luaran klinis pasien berupa penurunan tekanan darah sistolik (p=0,000). Pelayanan kefarmasian residensial memiliki pengaruh terhadap peningkatan ketaatan pasien terhadap penggunaan obat, pelaksanaan diet, pelaksanaan aktivitas fisik, serta terhadap pembatasan rokok dan konsumsi alkohol sehingga dapat memperbaiki luaran klinis pasien hipertensi.Kata kunci: Hipertensi, ketaatan pasien, pelayanan kefarmasian residensial, tekanan darahImpact of Pharmaceutical Home Care on Compliances and Clinical Outcomes of Hypertensive PatientsIn the Indonesian health universal coverage system the hypertensive patients that grouped into the refer back patient treatment category, is one target of pharmaceutical home care. The aim of this study was to carried out the impact of pharmaceutical home care on patient compliances and clinical outcomes ofhypertension without compeling indication out-patient on Wangaya General Hospital in the municipality of Denpasar Bali. Design research is an experimental study with one group pre-post test design. The thirteen patients who met the inclusion criteria will be given pharmaceutical home care services for 16 visits over three months period. The complianced levels were scored and statistical analyzed using linear regression and wilcoxon test. The pharmaceutical home care visit could increase the patients adherence to antihypertensive drug administration, increasing diet compliance, and adherence of physical exercise from good adherence to excellent adherence (p value=0,001), Pharmaceutical home care visit could increase patient compliance to restrictions of smoking and alcohol consumption from good adherence to very good adherence. The decreasing of the patient’s systolic blood pressure correlated to the pharmacist home visit (p value=0,000). The pharmaceutical home care has influenced on health behavior of hypertensive patients and the patients concordance to take their medication and introduced better clinical outcome.Keywords: Blood pressure, hypertension, patient compliances, pharmaceutical home care

Abstract

ABSTRAK: Uji skrining dan konfirmasi kandungan morfin dan kodein dalam plasma telah dikembangkan menggunakan high-performance thin-layer chromatography (HPTLC)-Spektrofotodensitometri. Penetapan kadar morfin dan kodein pada plat HPTLC dideteksi pada panjang gelombang 212 nm. Ekstraksi morfin dan kodein dari plasma menggunakan campuran kloroform dan isopropanol, dengan menggunkan metanol atau isopropanol sebagai pengendap protein. Uji konfirmasi dengan teknik di atas belum memberikan hasil yang kurang tajam. Batas dekteksi morfin adalah 48,8 ng dan kodein 70,2 ng. Diperoleh variasi batas deteksi dan batas kuantisasi analit yang sangat besar antar plat HPTLC walaupun memberikan persamaan regrasi yang hampir sama. Kata Kunci: Uji skrining, Konfirmasi, morfin, kodein, HPTLC ABSTRACT: Screening and confirmation test for morphine and codeine by high-performance thin-layer chromatography-densitometry technique has been carried out. The determination test consist of limit detection (LOD) and limit quantification (LOQ) of the analyte using HPTLC scanned on wave length of 212 nm. The morphine and codeine were extracted by liquid-liquid extraction using mixture of chloroform and isopropanol from plasma. The screening and confirmation test based on corrected hRf – value and insitu UV-spectrum of analyte delivered a not convenient result. The LOD of morphine and codeine was 48,8 and 70,2 ng. The determination of opiate by HPTLC-densitometry was obtained a great range of LOD and LOQ between plat of HPTLC, although had a fast equal of linear regression-coefficient.

Abstract

ABSTRAK: Penelitian ini bertujuan untuk mengetahui perubahan pola spektrum Raman Efedrin Hidroklorida, Dekstrometorfan dan Tramadol Hidroklorida pada tablet obat (Decolsin®,Tramifen®) dan sampel simulasi. Tablet Obat serta sampel simulasi Efedrin Hidroklorida, Dekstrometorfan dan Tramdol Hidroklorida dengan konsentrasi 80%, 60%, 40% dan 20% dalam serbuk Amprotab diukur dengan spektroskopi Raman 1064 nm pada bilangan gelombang 200-2000 cm-1. Hasil yang didapat dianalisis dengan cross correlation function dan dilihat nilai koefisien korelasi (r). Pada masing-masing tablet Obat hanya Parasetamol yang dapat diidentifikasi karena nilai koefisien korelasi r > 0,95. Berdasarkan hasil sampel simulasi diketahui bahwa konsentrasi Efedrin Hidroklorida, Dekstrometorfan dan Tramadol Hidroklorida pada suatu campuran berpengaruh terhadap pola spektrum Raman yang dihasilkan. Pola puncak spektrum Raman untuk identifikasi Efedrin Hidroklorida yaitu pada bilangan gelombang 1305 cm-1 (C-N), 1380 cm-1 (C-CH3), 1662 cm-1 (C=C), 1600 cm-1 (C-OH), dan 315 cm-1 (C-C alifatik); Dekstrometorfan yaitu puncak pada bilangan gelombang 1040 cm-1 (cincin Aromatis I), 1495 cm-1 (cincin Aromatis II), 768 cm-1 (C-Cl), 1580 cm-1 (cincin Hetero), 1667 cm-1 (C=C ) dan 855 cm-1 (C-O-C); Tramadol Hidroklorida yaitu puncak pada bilangan gelombang 1006 cm-1 (cincin aromatis I), 1467 cm-1 (cincin aromatis II), 1552 cm-1 (C-OH), 1635 cm-1 (C=C), 285 cm-1 (C-C Alifatik), 1345 cm-1 (C-N) dan 855 cm-1 (C-O-C). ABSTRACT: The aims of this study were determined to change in the pattern of Raman spectrum of Ephedrine Hydrocloride, Dextrometorfan and Tramadol Hydrocloride on tablet (Decolsin®,Tramifen®) and simulation samples. Tablets and simulation samples of this active compound made in Amprotab with concentration 80%, 60%, 40% and 20% measured by 1064 nm Raman spectroscopy in wave numbers 200-2000 cm-1. The results can be analyzed by cross-correlation function and showed the value of the correlation coefficient r. On tablet only Paracetamol can be identified because it has coefficient correlation r > 0,95. Based on the results of the simulation sample is known that the concentration of active compound in a mixture affect the pattern of the Raman spectra. Abstract

Heroine is hydrolyzed spontaneously in water and plasma. This will influence the determination, especially on the drug profiling. Spectrophotodensitometry has been used to analyze drug profiling of illicit heroine. This article reports the AL-TLC separation of heroine, 6-monoacetylmorphine (6-MAM), morphine, acetyl codeine, and the heroine hydrolysis in water and plasma. Heroin, 6-MAM, morphine, and acetyl codeine can be well-separated by mobile phase of toluene:siclohexane:diethyl amine (75:15:10, v/v). The limit of detection was 165.16 ng/spot and the limit of quantification was 550.55 ng/spot. Heroine was hydrolyzed in water and plasma under first order reaction. The rate of reaction was 0.55 min-1 in water with the half time reaction of 12.47 minutes. On the other hand the hydrolysis rate in plasma was slower with the half time of 16 minutes. Abstract

Phenol compound in Piper betle leaves has several pharmacology activities such as antibacteria, antifungi and antioxidant. The pharmacology activities of a herbal drug are influenced by the phytochemistry content, so in order to do a quality determination that provides phytochemistry profile and consistent pharmacology activities, a standardization is required. Fingerprint is the main standard to perform quality control for herbal drug. TLC-spectrophotodensitometry was used as the method in order to provide fingerprint profile of phenol compound. In this experiment, Piper betle leaves samples were extracted by reflux method using ethanol 96% as the solvent. Identification of phenol compound was done using TLC-spectrophotodensitometry with Silica gel 60 F254 as the stationary phase, toluena: ethyl acetate (93:7 v/v) as the mobile phase, FeCl3 and Folin-Ciocalteau as the reagent. The fingerprint profile of phenol compund was shown in Rf value 0,19; 0,42; and 0,62. Positive results of phenol compound are black spot on FeCl3 colour test and dark blue spot  on Folin-Ciocalteau colour test. Maximum wavelength of phenol compound was 283 nm.

Abstract

The codeine has been used as a marker for illicit heroin uses, that for increasing differentiating illicit heroin uses, it needs the codeine pharmacokinetic data. Eleven rabbits have been injected 11.04 mg codeine (with body weight: ± 1.8 kg) via marginal vena. Blood has taken at every 5, 15, 30, 60, 90, and 120 minutes. Serum has stored at -20 0C before analyzed. Blood protein was predicated by isoprophanol. Codeine was extracted with chloroform-isopropanol (3:1) and buffer phosphate 0.2N pH 9.3 then separated by phase toluene-acetone-ethanol-ammonia (45:45:7:3) as mobile phase and aluminium sheet of silica thin layer chromatography Gel 60 F254 as the stationary phase. Codeine determined by spectrophotodensitometry at 215 nm. Limit of detection and limit of quantification of codeine was 184.43 ng and 563.62 ng per spot, respectively the percentage recovery of in vitro was 60.60%. Profile concentration of codeine after IV bolus showed decreasing concentration at the first terminal and increasing codeine concentration at the last terminal that indicated an enterohepatic cycle. Codeine pharmacokinetic followed the first compartment with the half time was 30.61 minutes and the distribution volume was 7760.50 mL. Abstract

The knowledge of farmers on pesticides and its association on health and the environment still remains an issue. Located in the rural area of Bali, the village of Songan, Kintamani, Bangli with its fertile soil accommodates many farm lands. The counseling and assistance included 20 farmers during their working hours to allow direct observation. The counseling and assistance targeted at issues regarding safe uses of pesticides and its association to health and the environment. The knowledge of participants was evaluated by pre- and post-test. Less than a half (40%) were able to identify an association between pesticide use and health, while almost all do not know the chronic effect of pesticides to health and/or the environment. Recommendations target health care facilitators are expected to continue counseling and assistance to the farmers and others in order to maintain a healthier and more sustainable future.

Abstract

Daun katuk (Sauropus androgynus (L.) Merr) dapat digunakan sebagai obat tradisonal sakit kerongkongan, meningkatkan produksi ASI, serta memiliki aktivitas sebagai antidislipidemia. Senyawa kimia yang terkandung dalam ekstrak etanol 90% yaitu alkaloid, saponin, flavonoid, tanin, dan terpenoid. Salah satu kandungan kimia yang paling banyak terkandung pada daun katuk adalah terpenoid. Tujuan dari penelitian ini yaitu untuk mendapatkan fraksi terpenoid dari ekstrak etanol 90% daun katuk. Pemisahan fraksi terpenoid dilakukan dalam beberapa tahap yaitu: ekstraksi, fraksinasi dengan kromatografi kolom lambat dengan pelarut campur kloroform:metanol (9:1-1:9 v/v), identifikasi kandungan kimia dengan KLT, skrining fitokimia. Hasil fraksinasi kromatografi kolom didapatkan 20 fraksi dengan hasil positif terpenoid sebanyak 5 fraksi dari fraksi nomor 13-17.
