Abstract

Background The 1st, 2nd and small 3rd degree hemorrhoids are non-operatively treated by using anti-inflammatory and plebotonic medicines, high fiber diet and changing lifestyle. Graptophyllum pictum (L.) Griff (GP) is potential to be a hemorrhoid medicine (anti-inflammatory and plebotonic medicine) since its anti-inflammatory effect may be related to antioxidant property. ObjectiveTo elaborate the anti-inflammatory and anti-oxidant effect of GP on experimental hemorrhoid in male Wistar rats. MethodsA Randomized Controlled Trial Post-test only design in hemorrhoid-like rats using croton oil induced into the anus for 30 seconds each day for 3 consecutive days. 14 Wistar rats were randomly allocated into 2 groups. Group 1 got normal saline solution, while group 2 treated with GPE 100mg/Kg bw for 5 consecutive days. On the 9th experimental day, the blood was drawn for Elisa examination of COX-2 and SOD levels. As COX-2 and SOD normally distributed, pool t-test and product moment correlation was used for statistical analysis. Results All of Wistar rats were still alive. COX-2 levels (ng/ml) in the GPE group was 266.28±158.47, was significantly lower compared to control (595.21±68.92) (p=0.000). Serum SOD levels (ng/ml) in the GPE was 77.00±11.0, and was significantly higher than the control 28.00±4.00 (p=0.02). Correlation between COX-2 and SOD serum level was significantly strong negative correlation (p=0,000 and r=-0.842) Conclusions GPE at a dose of 100 mg/kg bw has antioxidant and anti-inflammatory effects on artificial hemorrhoids through decreased COX-2 levels, and elevated SOD levels. Abstract

LATAR BELAKANG : Saat ini, berbagai tanaman asli Indonesia dikembangkan sebagai bahan perawatan luka untuk hasil penyembuhan yang lebih baik. Salah satunya, yaitu Moringa oleifera L., yang  merupakan tanaman asli Indonesia serta berpotensi dapat membantu penyembuhan luka  melalui regulasi jalur sitokin inflamasi, interleukin (IL)-1 dan IL-10. TUJUAN : Untuk mengetahui pengaruh pemberian ekstrak Moringa oleifera L. terhadap IL-1, IL-10, dan rasio IL-1: IL-10 di jaringan kulit, pada tikus dengan luka insisi. METODE : Penelitian ini menggunakan desain post test only controlled group dengan sampel berupa 15 tikus Wistar yang dilakukan luka insisi. Sampel dibagi menjadi 3 kelompok perlakuan: perawatan luka dengan framycetin sulphate (KP), krim ekstrak Moringa oleifera L. 15% (M), dan krim plasebo (KN). Pengukuran kadar IL-1, IL-10, dan rasio IL-1:  IL-10 dilakukan pada hari ke-10 perlakuan melalui pengambilan jaringan. HASIL :  Kadar IL-1 pada kelompok perlakuan (M) tidak berbeda bermakna dibanding KN dan KP (33,56±4,68 vs 40,92±27,32 vs 30,23±3,92 pg/mL, p=0,543). Kelompok KP memiliki IL-10 lebih tinggi dibandingkan kelompok M dan KN (113,95±9,38; 91,75±5,65 dan 94,93±2,39 pg/mL, p<0,001), dan tidak didapatkan perbedaan antara kelompok M dan KN (p=0,296). Rasio IL-1: IL-10 kelompok KP, M, dan KN tidak menunjukan perbedaan bermakna (0,27±0,05, 0,37±0,06 dan 0,44±0,30, p=0,249). KESIMPULAN: Pemberian ekstrak Moringa oleifera L. secara topikal tidak mempengaruhi kadar IL-1 dan rasio IL-1 : IL-10 di jaringan kulit tikus dengan luka insisi. Kadar IL-10 di jaringan kulit tidak berbeda dibanding kelompok kontrol negatif, serta lebih rendah dibanding kelompok framycetin sulphate. Abstract

Background: Diabetic ulcer is one of the most feared chronic infections due to Diabetes Mellitus because it can lead to amputation and death. Aim: To prove the effectiveness of sucralfate and platelet-rich plasma (PRP) combination for Vascular Endothelial Growth Factor (VEGF) expression in diabetic ulcer healing. Methods: This research is an experimental study of Phase I Clinical Trial with post-test only group design. There were 20 patients with diabetic ulcers divided into two groups, namely the treated group that was given sucralfate and PRP therapy and the control group was given standard therapy of normal saline drainage and gauze covered. Parameters were VEGF expression levels, wound area after being given therapy, and side effect from the treatment. Data on VEGF expression levels were obtained by means of examination with the Quantikine Human VEGF-ELISA Quantikine, R&D System, Inc, Minneapolis. The measurement of the wound area was assessed based on several criteria, namely grade 0 (no change), grade 1 (wound size reduced to less than of the previous wound), grade 2 (wound size was reduced to less than of the previous wound, but granulation was visible), and grade 3 (wound has closed completely). Result: In unpaired t-test, the mean VEGF expression was 98.18+10.96 in the treatment group and 66.69+23.79 in the control group which showed significant difference in VEGF expression levels (p = 0.003). In Mann-Whitney test, the mean wound area was 0.68+0.40 in the treatment group and 0.77+0.67 in the control group which showed that there was not any significant difference in wound area (p = 0.152). There were no side effects in both study group. Conclusions: The combination of sucralfate and PRP can increase VEGF levels significantly in diabetic ulcer patients but does not show a different effect in reducing wound area compared to standard treatment. The combination did not cause any side effects in the study subjects, as well as those using standard treatment.

Abstract

Background: Breast-related Symptom Questionnaire (BREAST-Q) has been globally used and validated to measure the outcomes among women undergoing various breast surgery. There was a lack of tools to evaluate patient’s quality of life and satisfaction after mastectomy in Indonesia. In addition, most patients in Indonesia may feel reluctant to express their honest opinions regarding the post-operative outcome openly. Prior to this issue, this study aims to translate and validate the BREAST-Q Mastectomy Module into Bahasa Indonesia, which can be applied to the Indonesian population. Method: Primarily based on the guidelines from the International Society for Pharmacoeconomics and Outcome Research (ISPOR), the process includes preparation, forward translation, back translation, panel discussion, cognitive debriefing interview with 57 respondents, statistic evaluation of validity and reliability, and final consensus. Results: Two translators worked on two different forward translation, while a third translator handled the back translation. The translations were then discussed and reviewed to reach an agreement on the finalized version. Statistic evaluation showed Pearson’s r value 0.26 in all items, with Cronbach’s alpha range 0.72–0.92 ( 0.70). Bland-Altman plots comparison showed that the direction of the mean difference between score 1 and score 2 is close to zero. At the same time, the limits of agreements exceed the minimally detectable change for both scales, indicating all items are valid, reliable, and reflect the concepts of the BREAST-Q Mastectomy Module. The final version of the Bahasa Indonesia Translation is then approved by the Q-Portfolio team.Conclusion: The translation of BREAST-Q questionnaire Mastectomy Module in Bahasa Indonesia is valid and reliable. It is a suitable instrument to assess the quality of life of patients who underwent mastectomy in the Indonesian population.

Abstract

To determine the effect of clove flower extract on the healing time of incision wounds, bacterial count and IL-10 levels in rats infected with MRSA.Method. Experimental study with a post-test-only control group design in rats that were given incisions on their backs. Random allocation was carried out to divide the Sprague Dawley rats into 6 groups (@5 rats). Healing time was assessed based on the length of days, the bacterial count was examined from the blood and then counted by culture using nutrient agar. IL-10 levels were examined from the venous blood of rats and then measured using an ELISA kit assessed by a certified microbiologist. Data were analyzed and processed using the One Way Anova - Post Hoc hypothesis test.Results. The bacterial count in the clove flower extract group was lower than the group without antibiotics and there was no difference compared to the group given antibiotics. IL-10 levels in the group given clove flower extract were lower when compared to the group given antibiotics. The wound healing time in the clove flower extract group was faster when compared to the group with or without antibiotics.Conclusion. Clove flower extract enhancewound healing in MRSA-infected rats.

Abstract

Introduction: Atherosclerosis is a cardiovascular disease which is the leading cause of 16% of the world's total deaths. The main comorbidities of atherosclerosis are obesity and diabetes mellitus. One of obesity surgery treatment that is currently very often used is the Sleeve Gastrectomy. Sleeve Gastrectomy is the most effective and significant long-term therapy for weight loss and preventing various comorbidities such as heart disease. This study analyze the effect of sleeve gastrectomy on the expression of atherosclerosis anti-inflammatory cytokines (TGF-β and IL-10) in the abdominal aorta on the process of atherosclerosis in obesity and diabetes mellitus subjects.Methods: An in vivo laboratory experimental study with post-test control group design on 15 male rats (Sprague Dawley) that was randomly divided into 3 groups: K1 (negative control), K2 (positive control with obesity and diabetes mellitus) and P1 (obese and DM rats treated with sleeve gastrectomy). After 10th day of the procedure, we analyze the body weight, fasting blood glucose, and gene expression of TGF-β and IL-10 using the PCR method. Statistical analysis was done using Anova, Kruskal Wallis and Post Hoc LSD.Results: There was significant decrease in body weight (p=0,000) and fasting blood sugar levels (p=0,000) in the P1 compared to the K2. There was significant increase of TGF-β (p = 0,005) and IL-10 (p = 0,002) gene expression in P1 compared to K1 and K2.Conclusion: SG decreased body weight and fasting blood sugar levels of obese and DM subjects. Also improved the expression of atherosclerosis anti-inflammatory cytokines (TGF-β and IL-10) in abdominal aorta.

Abstract

Background: Exercise generally improves the quality of life (QoL) in adult populations especially for patients with chronic health problems such as breast cancer. The purpose of this study was to investigate the effect of physical exercise/activity on the quality of life of breast cancer survivors. Subjects and Method: This was a systematic review study that used articles from online data­bases, such as  PubMed, Science Direct, ProQuest dan Medline, which was published from 2000 to 2021. The author used keywords of “physical exercise”, “physical activity”, “quality of life”, “breast cancer survivor”. The dependent variable was quality of life. The independent variable was physical exercise/ activity. As many as 5 articles were selected for this study from 837 articles located in South Korea, Iran, Malaysia, and China.Results: Moderate or high levels of physical activity had positive relationship on the quality of life related to overall physical health among breast cancer survivors.Conclusion: Moderate to high physical activity improve the quality of life among breast cancer survivors.Keywords: physical exercise, physical activity, quality of life, breast cancer survivor Correspondence: Arief Wahyudi Jadmiko. Abstract

Women with breast cancer who underwent mastectomy treatment procedures got psychological problems as the impacts on thoughts, emotions, behaviors and physical sensations. They are categorized as psychological distress which requires psychoeducational needs identification. This research aimed to identify the need for psychoeducation to reduce psychological distress in post-mastectomy breast cancer women. This research was an exploratory, descriptive qualitative study with a thematic analysis approach using Atlas-ti7 software. The target population of the study was women with post-mastectomy breast cancer as inpatients. The sampling technique used was purposive sampling. A total of 15 participants underwent interviews. This research found five themes as issues (psychological concerns), namely problems with physical sensations, the impact of thoughts, the impact of emotions, lack of information and psychological support. 5 psychoeducational needs were identified, namely management strategies for overcoming physical problems, overcoming problems of negative thought effects, managing negative emotions, explaining information regarding the benefits and impacts of mastectomy, and how to ask for help to get psychological support. This research recommended five ways or techniques of fulfilling psychoeducational needs to reduce psychological distress, namely explaining the benefits and impacts of mastectomy, early ambulation and range of motion (ROM) techniques, deep breathing techniques, muscle progressive relaxation techniques, self-talk techniques and psychological support.

Abstract

BACKGROUND: Sorafenib, the standard therapeutic agent for advanced hepatocellular carcinoma (HCC), may induce hepatic dysfunction, thereby necessitating adjunctive therapy to mitigate this adverse effect. While preliminary research has suggested that Soursop (Annona muricata) leaves exhibit anti-tumor and hepatoprotective properties, their efficacy in mitigating liver damage associated with sorafenib treatment remains unexplored. This study was conducted to assess the liver-protective effects of soursop leaf extract in Wistar rats receiving sorafenib for HCC treatment.METHODS: Ethanol extract of soursop leaves was prepared using the maceration method. Twenty-nine Wistar rats were divided into five groups: healthy control (HC) group, HCC groups receiving no treatment, sorafenib only, sorafenib + 50 mg/kgBW/day soursop extract, and sorafenib + 100 mg/kgBW/day soursop extract. All groups, except the HC group, were given Diethyl Nitrosamine (DEN) to cause HCC. Following a two-week treatment period, serum levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were assessed using colorimetric methods, while total bilirubin was assessed using diazo with sulphanilic acid method. From histopathological specimen, relative liver weight was measured and liver damage score was assessed using Hematoxylin and Eosin.RESULTS: Administration of sorafenib resulted in a reduction of AST, ALT, total bilirubin, relative liver weight, and liver damage scores. Furthermore, the combined administration of sorafenib with soursop leaf extract at dosages of 50 and 100 mg/kgBW/day led to a dose-dependent amelioration of these indicators. The most pronounced improvement was observed with the highest dose of soursop extract, which significantly reduced AST, ALT, total bilirubin, relative liver weight, and liver damage scores compared to the sorafenib-only group.CONCLUSION: Soursop leaf extract at 100 mg/kgBW/day effectively reduced AST, ALT, bilirubin levels, and liver damage score in sorafenib-treated Wistar rats with HCC, indicating its hepatoprotective effects. Abstract

BACKGROUND Breast imaging reporting and data system (BI-RADS) is a globally recognized method for categorizing breast lesions. However, it is dependent on subjective interpretation, which can result in variability between radiologists. Currently, no scoring system exists to assist in categorizing borderline findings as either benign or malignant. This study aimed to introduce a scoring system that classifies borderline breast lesion findings without altering the standard BI-RADS interpretation and minimizes inter-reader variability. METHODS This single-center retrospective cross-sectional study included 215 women who underwent breast ultrasound (US) and histopathology between January 2021 and December 2022, excluding those with non-neoplastic breast lesions. The index test was US BI-RADS features, and the reference standard was histopathology. Prevalence ratios (PRs) and probability scores were used to assess the risk contribution of individual US features. The diagnostic performance of the BI-RADS interpretation strategy and predictive scoring model was compared with that of the standard BI-RADS classification using SPSS software version 26. RESULTS Margin, orientation, and age had the highest PRs of malignancy, with odds ratios and 95% confidence intervals of 39.86 (13.19–120.47), 17.47 (2.42–125.72), and 9.74 (3.51–27.04), respectively. These three characteristics increased the probability of malignancy by 99.53%. A comparison of diagnostic tests between the modified BI-RADS interpretation strategy and the standard BI-RADS classification revealed improvements in specificity (94.0% versus 84.7%), positive predictive value (85.0% versus 70.5%), accuracy (89.3% versus 84.7%), and area under the curve (0.931 versus 0.873). CONCLUSIONS According to the interpretation strategy, margin, orientation, and age, this US scoring model appear to be promising tools for classifying borderline malignant masses.
