Abstract

Interaksi antara obat modern dan obat tradisional merupakan masalah yang perlu dicermati, tidak terkecuali dalam pengobatan diabetes mellitus. Nattokinaseadalah produk nutraceutical yang dapat digunakan bersamaan dengan obat antidiabetika oral sehingga memungkinkan terjadinya interaksi. Penelitian ini bertujuan untuk mengetahui pengaruh Nattokinase terhadap daya kerja MetforminHCl dalam menurunkan kadar glukosa darah tikus putih jantan (Rattus norvegicus) galur Wistar. Penelitian ini menggunakan metode uji toleransi glukosa oral dengan pembebanan glukosa dosis 4,5 g/KgBB. Hewan uji yang digunakan yaitu tikus putihjantan galur Wistar umur 2-3 bulan dengan berat badan 150-200 gram, sebanyak 20 ekor yang dibagi menjadi 4 kelompok, masing-masing kelompok terdiri dari 5 ekortikus. Kelompok I sebagai kelompok kontrol negatif diberi tween 80 - span 80, kelompok II sebagai kelompok kontrol positif diberi metformin HCl dosis 45 mg/kg BB, kelompokIII diberi nattokinase dosis 300 mg/kgBB, kelompok IV diberi nattokinase 300 mg/kgBB dan 1 jam kemudian diberi metformin HCl. 30 menit berikutnya semua kelompokperlakuan diberi glukosa. Saat perlakuan dianggap sebagai waktu ke-0. Pemberian glukosa dianggap sebagai waktu ke-30. Pengambilan darah melalui ekor pada menit ke- 0, 30, 60, 120, dan 180. Kadar glukosa darah diukur dengan alat Easy Touch. Efek penurunan kadar glukosa darah ditunjukkan dengan menghitung nilai LDDK0-180 (Luas Daerah Di bawah Kurva menit ke-0 sampai menit ke-180 dari grafik waktu vskadar glukosa darah). Data yang didapat diuji statistik dengan Kruskal-Wallis dan Mann Whitney dengan taraf kepercayaan 95%. Hasil penelitian menunjukan bahwa pemberian Metformin HCl 1 jam setelah pemberian Nattokinase dapat menurunkandaya kerja Metformin HCl sebesar 52,22%.

Abstract

Obat tradisional di Indonesia yang merupakan warisan budaya dan telah menjadi bagian integral dari kehidupan bangsa Indonesia, diinginkan untuk dapat dipakai dalam sistem pelayanan kesehatan formal. Sebagaimana syarat peredaran obat pada umumnya, obat tradisional seharusnya memenuhi persyaratan kualitas, aman, dan berkhasiat. Penelitian ini bertujuan untuk menganalisis kadar SGPT-SGOT dari pemberian suspensi ekstrak etanol buah pare (Momordica charantia L.) yang diberikan secara peroral. Penelitian dilakukan menggunakan tikus jantan putih galur Wistar berjumlah 36 ekor, dibagi menjadi 6 kelompok. Setiap kelompok terdiri dari 6 ekor tikus jantan putih. Kelompok I (kontrol) diberi larutan CMC 0,5% 2,5 ml/200 g BB. Kelompok II diberi sediaan suspensi ekstrak etanol buah pare dosis 0,5 g/kg BB, kemudian berturut-turut kelompok III dengan dosis sediaan uji 1 g/kg BB, kelompok IV diberi sediaan uji dosis 2 g/kg BB, kelompok V diberi sediaan uji dosis 4 g/kg BB, dan kelompok VI diberi sediaan uji dosis tertinggi 8 g/kg BB. Hasil uji ketoksikan akut pada tikus putih jantan galur Wistar menunjukkan bahwa dari pemberian peroral sediaan suspensi ekstrak etanol buah pare (Momordica charantia L.) dosis tunggal pada tikus jantan putih galur Wistar mulai dari dosis 0,5 g/kg BB sampai dengan dosis 8 g/kg BB tidak berpengaruh signifikan terhadap nilai SGPT-SGOT.

Abstract

The traditional medicine in Indonesia which is the cultural heritage and hasbecome an integral part of the life of the nation Indonesia, want to be employed in theformal health care system. As the circulation of drugs in general terms, traditionalmedicine should fulfill the requirements of quality, safe, and efficacious. This studyaims to analyze the levels of SGPT, SGOT of suspension of the ethanol extract of bittermelon fruit (Momordica charantia L.) by orally. The study was conducted using a whitemale Wistar rats amounted to 36 animals, divided into 6 groups. Each group consistedof 6 male white rats. Group I (control) were given 0.5% CMC solution of 2.5 ml/200 gBW. Group II was given a suspension dosage doses of bitter melon fruit extract ethanol0.5 g/kg, then consecutive group III with the test preparation doses of 1 g/kg, the testpreparation group IV were given a dose of 2 g/kg, group V was given dosage test doseof 4 g/kg, and group VI were given the highest dose of the test preparation 8 g/kg. Theresults of acute toxicity tests on white male Wistar rats showed that administration ofethanol extract of peroral dosage suspension pare (Momordica charantia L.) singledose on white male Wistar rats from the dose of 0.5 g/kg until a dose of 8 g/kg did notsignificantly influence the value of SGPT, SGOT.

Abstract

Interaction of modern drug and traditional drug is an issue that needs to beexamined, not least in the treatment of diabetes mellitus. Nattokinase is a nutraceuticalproduct that can be used in conjunction with antidiabetic oral drugs that allows theinteraction. This study aims to determine the effect of Nattokinase against their powerMetformin HCl in decreasing of blood glucose levels male white rats (Rattusnorvegicus) Wistar. This study uses an oral glucose tolerance test with glucose loadingdose of 4.5 g/kg. Animals test used were white male Wistar rats aged 2-3 monthsweighing 150-200 g, 20 rats were divided into 4 groups, each group consisted of 5 rats.Group I as a negative control group was given tween 80 - span 80, group II as a positivecontrol group was given metformin HCl doses of 45 mg/kg, group III was givenNattokinase dose 300 mg/kgBB, group IV given nattokinase 300 mg/kgBB 1 hour thenwere given metformin HCl. 30 minutes later all treatment of groups were given glucose.When treatment is considered as a time at-0. The given of glucose is considered as thetime at-30. Blood sampling via the tail at 0, 30, 60, 120, and 180. Blood glucose levelswas measured with the Easy Touch. The effect of decreasing blood glucose levelsindicated by the value of LDDK0-180 (Regional Area Under the Curve minute 0 tominute 180 of the graph time vs blood glucose levels). The data can be testedstatistically by Kruskal-Wallis and Mann Whitney with a level of 95%. The resultsshowed that administration of Metformin HCl after 1 hour administration Nattokinasecan be decreased 52.22% of Metformin HCl.

Abstract

Interaksi obat merupakan masalah yang perlu dicermati. Interaksi yang terjadi kemungkinan dapat menyebabkan perubahan efek farmakologi suatu obat. Interaksi yang terjadi bisa menguntungkan atau merugikan. Nattokinase merupakan produk nutraceutical yang mempunyai efek khusus melancarkan aliran darah. Pada penderita diabetes mellitus, tingginya kadar glukosa darah mengakibatkan viskositas darah menjadi meningkat dan hal ini akan sangat beresiko terhadap laju alir darah. Penelitian ini bertujuan untuk mengetahui pengaruh pemberian nattokinase terhadap daya kerja metformin HCl dalam menurunkan kadar glukosa darah tikus putih jantan (Rattus norvegicus) galur Wistar. Penelitian ini menggunakan metode uji toleransi glukosa oral dengan pembebanan glukosa dosis 4,5 g/KgBB. Hewan uji yang digunakan yaitu tikus putih jantan galur Wistar umur 2-3 bulan dengan berat badan 180-200 gram, sebanyak 24 ekor yang dibagi menjadi 4 kelompok, masing-masing kelompok terdiri dari 6 ekor tikus. Kelompok I sebagai kelompok kontrol negatif diberi aquadest, kelompok II sebagai kelompok kontrol positif diberi metformin HCl dosis 45 mg/kg BB, kelompok III diberi nattokinase dosis 22,5 mg/kg BB , kelompok IV diberi metformin HCl dosis 45 mg/KgBB dengan selang waktu 2 jam setelah pemberian nattokinase dosis 22,5 mg/kg BB secara peroral, 30 menit kemudian semua kelompok perlakuan diberi glukosa. Saat pemberian glukosa dianggap sebagai waktu ke-0. Pengambilan darah melalui sinus orbitalis mata pada menit ke (-60), (-30), 0, 60, 120, dan 180. Kadar glukosa darah diukur dengan alat Blood Glucose Test Meter GlucoDr. Efek penurunan kadar glukosa darah ditunjukkan dengan menghitung nilai LDDK0-300 (Luas Daerah Di bawah Kurva menit ke-0 sampai menit ke-300 dari grafik waktu vs kadar glukosa darah). Data yang didapat diuji statistik dengan uji Levene, uji Kolmogorof-Smirnov, dan uji lanjut dengan taraf kepercayaan 95%. Hasil penelitian menunjukkan bahwa pemberian metformin HCl 2 jam setelah pemberian nattokinase dapat menurunkan daya kerja metformin HCl sebesar 11,67 %.Kata kunci: diabetes mellitus, efek hipoglikemia, interaksi obat, metformin HCl, nattokinaseABSTRACTInteraction of modern drug and traditional drug is an issue that needs to be examined. Abstract

Disease Modifying Osteoarthritis Drugs (DMOADs) merupakan agen yang dapat mencegah, menghambat perkembangan atau mengembalikan perubahan morfologi pada penderita osteoarthritis. Salah satu yang termasuk DMOADs ini adalah diacerein. Diacerein menstimulasi terbentuknya Tissue Inhibitor Metalloproteinase-1 (TIMP-1) pada percobaan hewan. Tujuan penelitian ini adalah mengetahui pengaruh penggunaan diacerein pada ekspresi genTIMP-1 pada sel primer jaringan sinovial pasien osteoartritis. Pada penelitian ini dilakukan penambahan diacerein pada sel primer jaringan sinovial grade 4. Ekspresi gen dilihat dengan menggunakan alat Real Time PCR dari sampel yang telah diinkubasi dalam kurun waktu 24 jam dan 48 jam, kemudian dibandingkan dengan kontrol menggunakan metode ΔCT dengan GAPDH sebagai housekeeping gene. Dari hasil pengukuran ekspresi gen sampel 24 jam dan 48 jam didapatkan nilai berturut-turut 0,0179 dan 0,7423 dengan nilai kontrol 0,0005. Berdasarkan hasil penelitian, diketahui bahwa pada inkubasi diacerein selama 24 jam mampu meningkatkan ekspresi gen TIMP-1 dengan peningkatan sebesar 5,87 kali dibandingkan kelompok kontrol, sedangkan pada inkubasi selama 48 jam, diacerein mampu meningkatkan ekspresi gen TIMP-1 dengan peningkatan sebesar 1,53 kali jika dibandingkan dengan kelompok kontrol. Pada penelitian ini dapat disimpulkan bahwa overekspresi gen TIMP-1 berkaitan dengan pemberian diacerein pada osteoartritis.

Abstract

IL-1 is one of the cytokines involved in joint diseases such as osteoarthritis. IL-1 plays a role in maintaining the balance of proteolytic proteins:MMPs and TIMPs inhibitors. Increased expression and uncontrolled IL-1 activity tend to increase the role of MMPs in degrading proteoglycans and joint tissue collagen. This study aims to reveal the interaction model of one of the osteoarthritis drugs, namely diacerein. A drug belongs to a group of disease-modifying osteoarthritis drugs (DMOADs) to suppressthe development of the disease rate, improving the structure and function of the cartilage and surrounding tissue. "In silico" digital test uses the technique of "molecular docking: used as a method of the approach using the MOE 2007.09 software application. The test material was in the form of a diacerein 3D structure and five control ligands, while the IL-1β / IL-1RI receptor template was downloaded from pdb.org (PDB ID: 1ITB). The ligand pharmacokinetic profile will also be displayed obtained through the ADMETSAR server. The docking results showed diacerein had the lowest docking score of -12.3285 kcal/mol with the strongest affinity, the best pharmacokinetic profile but more toxic. This study proves that the mechanism of diacerein inhibition of IL-1β / IL-1RI receptors is similar to dexamethasone, prednisolone,and minocycline.

Abstract

The genes involved in cartilage degradation in the pathogenesis of osteoarthritis ( OA) are the Nuclear Factor Kappa Beta (NFĸβ) gene, which plays a vital role in modulating cartilage damage involving activation of pro-inflammatory cytokines, including prostaglandin E2 (PGE2). This pure experimental research aimed at analyzing the relationship between the expression of NFĸβ genes after administration of mesenchymal stem cells in osteoarthritis synoviocytes in vitro from Wharton's Jelly (WJ-MSCs). The samples were isolated from synovial tissues of patients with osteoarthritis who underwent Total Knee Replacement (TKR) surgery and then divided into six treatment groups, each with four replicates. Groups I and II were the synoviocyte-control classes of 24h and 48h (synoviocytes, incubated for 24 and 48 hours respectively). Groups III and IV were control groups of 24h and 48h WJ-MSCs (mesenchymal stem cells derived from the Wharton's Jelly, incubated for 24 and 48 hours respectively). Finally, Groups V and VI were the 24-hour and 48-hour co-cultural treatment groups (synoviocyte co-culture and WJ-MSCs, Incubated, for 24 and 48 hours respectively). Using qPCR, PGE2 gene and NF NFκβ gene expressions were observed in each population. Tests showed 0.61 and 0.61, respectively, after the administration of WJ-MSCs, the expressions of the NFκβ-p65 and PGE2 genes in the 24h classes. In the 48h group, meanwhile, both numbers fell to 0.15 and 0.47, respectively. These indicate a strong correlation between the NFκβ-p65 gene and PGE2 gene expressions in OA synovial cells after the administration of MSC-WJ (r = 0.903).
