Abstract

Puerperium is a critical period in which mothers often experience physical and psychological problems, however it is the beginning of the breastfeeding process and determines the success of exclusive breastfeeding. The use of traditional herbs for postpartum mothers is part of Indonesian culture that is starting to be abandoned in urban area. This community service activity aims to increase the knowledge, attitudes and behaviour of the community regarding the importance of postpartum maternal health care and the use of traditional herbs with proper processing methods as a promotive and preventive treatment. This activity was carried out in Gubeng Surabaya District with three stages including preparation, implementation of activities and evaluation. In the preparation stage, a situation analysis was performed. Activities were conducted by delivering lectures, discussions, video demonstrations and training. The evaluation was carried out by obtaining feedback from the social and community empowerment section as well as the pre-test and post-test questionnaires filled out by the participants. Based on the evaluation, the community service can increase participants' knowledge about postpartum health care and lactation management, thereby increasing awareness to play an active role by disseminating information to the community about the importance of family and the neighbourhood support for postpartum and breastfeeding mothers. The participants 'knowledge about the use of traditional medicinal plants for the maintenance of postpartum health and successful breastfeeding also increased, accompanied by an increase in the participants' willingness to prepare traditional jamu from fresh herbal ingredients themselves. This community service needs to be carried out in a sustainable manner with innovative delivery methods.

Abstract

ABSTRAK Latar Belakang: Kebutuhan terapi hemodialisis (HD) masih terus berkembang. Di Indonesia, HD mewakili 82% dari pelayanan cuci darah di Puskesmas Indonesia. Namun, HD menyebabkan peningkatan penanda inflamasi serta perubahan profil lipid. Kombinasi vitamin E dan asam lemak omega-3 dapat membantu mengurangi indikator inflamasi dan meningkatkan profil lipid.Tujuan: Penelitian ini bertujuan untuk mengetahui pengaruh co-suplementasi vitamin E dan omega-3 terhadap profil lipid dan indikator inflamasi.Metode: Penelitian ini dilakukan di PubMed dan Google Scholar untuk mencari artikel-artikel secara sistematis, Kriteria inklusi yang digunakan adalah artikel bahasa Inggris dengan uji coba terkontrol acak (RCT) dan uji coba terkontrol (CT) yang menyelidiki efek vitamin E dan omega-3 pada profil lipid dan penanda inflamasi pasien hemodialisis. Semua artikel harus diterbitkan antara September 2013 - September 2022. AM dan AA melakukan seluruh proses penelitian sebagai reviewer dan seluruh ketidaksetujuan diselesaikan bersama NH sebagai penulis ketiga.Hasil: 1020 studi diidentifikasi dari database dan 12 artikel terpilih berdasarkan kriteria inklusi. Asam lemak omega-3 meningkatkan profil lipid, seperti kolesterol, LDL, HDL, dan trigliserida. Namun, studi ini hanya menemukan satu penelitian tentang efek vitamin E pada profil lipid. Asam lemak Omega-3 dan suplementasi vitamin E tidak memiliki efek signifikan pada penanda inflamasi, tetapi dampak suplementasi vitamin E pada tingkat IL-6 telah dilaporkan.Kesimpulan: Co-suplementasi Omega-3 dan vitamin E berpengaruh pada penurunan level indikator inflamasi dan profil lipid pasien hemodialisis. Kami merekomendasikan bahwa studi berikutnya harus berfokus pada penelitian uji klinis mengenai pengaruh suplementasi kombinasi ini terhadap indikator inflamasi dan profil lipid pasien hemodialisis. KATA KUNCI: hemodialisis; indikator inflamasi; omega-3; profil lipid; vitamin E ABSTRACTBackground: Hemodialysis therapy (HD) demands is still growing. In Indonesia, HD represents 82% of dialysis services in Indonesian health centers. However, HD causes an increase in inflammatory markers as well as lipid profile changes. The combination of vitamin E and omega-3 fatty acids can assist to decrease inflammatory indicators and enhance lipid profiles.Objectives: This study aims to investigate the effect of co-supplementation of vitamin E and omega-3 on lipid profiles and indicators of inflammation.Methods: This study was conducted on PubMed and Google Scholar to search articles systematically. The inclusion criteria used were English articles of randomized controlled trials (RCTs) and controlled trials (CT) that investigated the effects of vitamin E and omega-3 on lipid profiles and inflammatory markers of hemodialysis patients. All articles must be published between September 2013 - September 2022. AM and AA conducted the entire research process as reviewers and all disagreements were finalized with NH as a third author.Results: 1020 studies were identified from databases. Afterward, this study included 12 articles based on the inclusion criteria. Omega-3 fatty acids improve lipid profiles, such as cholesterol, LDL, HDL, and triglyceride. While we only found a single study about the effect of vitamin E on lipid profile. Furthermore, omega-3 fatty acids and vitamin E supplementation have no significant effect on inflammatory markers, but the impact of vitamin E supplementation on IL-6 levels was reported.Conclusions: Co-supplementation of omega-3 and vitamin E decrease levels of inflammatory indicators and lipid profiles in hemodialysis patients. We recommend that future studies should focus on clinical trials to investigate the effect of this co-supplementation on lipid profiles and inflammatory markers of hemodialysis patients. KEYWORD: hemodialysis; inflammatory markers; lipid profiles; omega-3, vitamin E

The administration of goat bile for 28 days had a toxic effect on the liver of the mice based on histopathological findings Abstract: Background: Bile consumption by Indonesians is believed to have therapeutic effects, especially goat bile. Goat bile is thought to contain harmful ingredients that can cause toxic effects on the liver. However, the 28-days oral toxicity study of goat bile has not been performed. Objective: To analyze the hepatotoxic effect of subchronic administration of goat bile on the liver of mice (Mus musculus). Material and Method: This was an experimental research with a post-test-only control group design. The samples used were 32 Balb/C mice (Mus musculus), which were grouped into 4 groups. The samples were administered with goat bile orally (3.2, 6.4, or 12.8 mL/kg/day) for 28 days. The liver was taken for histopathological examination and the hepatocytes injury score was performed. The scoring results were analyzed using Kruskal-Wallis, Mann-Whitney, and Spearman correlation tests (p<0.05). Result: Goat bile administration was associated with hepatocyte injury (p= 0.004). Groups with goat bile administration of 6.4 and 12.8 mL/kg/day had significant differences with the control group (p= .015 and .029 respectively) and the 3.2 mL/kg/day administered group (p= 0.006 and 0.009 respectively). Moreover, the increased administration of goat bile had a positive correlation with the level of hepatocyte injury (p= 0.004 and r_s= 0.504) Conclusion: Goat bile administration for 28 days had a significant toxic effect on the liver of mice at a dose of 6.4 mL/kg/day.

Abstract

Pemberdayaan kader kesehatan dalam program GERMAS melalui Posbindu di Desa Kentong, Kecamatan Glagah, Kabupaten Lamongan bertujuan untuk meningkatkan pengetahuan tentang pemanfaatan tanaman obat yang berkhasiat sebagai anti hipertensi. Tingginya prevalensi pasien hipertensi di daerah tersebut, diduga akibat dari kepatuhan berobat yang rendah, pengetahuan perilaku gaya hidup dan diet yang sehat masih belum optimal, serta minimnya informasi dan pengetahuan tentang pemanfaatan tanaman obat pada pasien hipertensi. Solusi yang ditawarkan untuk menyelesaikan permasalahan tersebut adalah memberikan penyuluhan tentang materi pentingnya mengelola hipertensi dengan baik, pengobatan rutin, perilaku gaya hidup serta diet sehat bagi pasien hipertensi,  sosialisasi tanaman obat berkhasiat sebagai antihipertensi dan demonstrasi pelatihan pembuatan sediaan dari tanaman obat. Metode yang digunakan melalui survei, kuesioner dan penyuluhan penyampaian materi, serta demonstrasi langsung di depan peserta. Ditemukan pada kuesioner pretes bahwa kader belum paham tentang ragam tanaman obat dan pengolahan yang tepat, setelah dilakukan penyuluhan dan demonstrasi terjadi peningkatan pengetahuan dengan para kader mampu menjawab pos tes dengan benar, peserta aktif berdiskusi secara oral, hal tersebut sebagai indikator bahwa peserta antusias dan dapat tercipta kader kesehatan yang mandiri.

Abstract

Introduction: The etiology of myoglobinuric acute kidney injury involves oxidative injury brought on by the Fenton reaction and myoglobin redox cycle. Renal tubules may be harmed, and lipid peroxidation compounds with vasoconstrictor characteristics may be produced. N-acetylcysteine (NAC) is an antioxidant shown to improve renal microcirculation and have protective effects in various models of renal damage. The aim of the study was to demonstrate the protective impact of NAC in glycerol-induced rats by measuring tissue malondialdehyde (MDA) level and renal function test (RFT), and to determine the correlation between the protective effect and NAC dose.Methods: This study measured tissue malondialdehyde (MDA) and renal function to examine any protective effect of NAC in a glycerol-induced rat model and to determine whether the effect was dose-related. Five groups of male Wistar rats were used: 1) saline control group, (2) glycerol (50%, 8mL/kg, i.m) plus saline i.v group, 3) glycerol plus NAC (100 mg/kg)-treated group, 4) glycerol plus NAC (200 mg/kg)-treated group, 5) glycerol plus NAC (400 mg/kg)-treated group. At 24 hrs, after glycerol injection, rats were sacrificed, cardiac blood was taken for renal function measurement, and renal tissues were removed for thiobarbituric acid MDA level assessment.Results: Our study revealed that glycerol administration significantly amplified renal tissue MDA, serum creatinine, and BUN (blood urea nitrogen) levels. However, NAC administration dampened the MDA increment and renal function deterioration (p<0.05). Moreover, tissue MDA, BUN, and serum creatinine levels were significantly correlated to NAC dose (r=0.485; r=0.491; rs=0.544, respectively; all p<0.05), indicating that NAC protection declines by dose increments.Conclusion: In this glycerol-induced acute kidney injury rat model, the administration of intravenous NAC 100 mg/kg reduced lipid peroxidation and improved renal function. Nevertheless, the protective effect was diminished in higher doses.

Abstract

Morbidity and mortality due to Covid-19 tend to be milder in adolescents. Nevertheless, their perception, attitude, and behavior towards Covid-19 deserve attention since they would also contribute to the success of Covid-19 control. Therefore, the aim of this study was to examine the perceived severity, susceptibility, attitudes, and behavior toward Covid-19 among high school students. This was a cross-sectional study conducted in five schools in Surabaya and Sidoarjo. Questionnaires were given online to teachers who then forwarded them to high school students. Data were taken from July to October 2021. Attitude and behavior were examined using the scoring previously tested. The data were then analyzed using Mann-Whitney U and Spearman's correlation tests. The significance level was set at p<0.05. More than 50% of students had good preventive behavior, the most frequent one was wearing masks (87.3%), followed by washing hands (73%). The behavior was correlated with their attitude (r= 0.285, p=0.000). Fifty-three percent of the respondents felt they would experience mild symptoms if they were infected with Covid-19. Most students perceived that they were susceptible to getting infected with Covid-19. Significant results were obtained using the Mann-Whitney test on gender toward attitudes and behavior (p<0.05). There were significant correlations between the mother's educational level (r=0.164, p=0.001) and the father's educational levels (r=0.120, p=0.013) with attitude toward Covid-19 control policies. Female students in Surabaya and Sidoarjo have better behavior in dealing with the Covid-19 pandemic. Parents' educational level plays a role in influencing the student's attitude toward Covid-19 control.

Abstract

The rapid spread of information and infodemic might result in public confusion and hinder the handling of the COVID-19 pandemic. Public comprehension of COVID-19 as part of health literacy is an important determinant to filter hoaxes from facts. Therefore, a scoring card called the Karlivid (the COVID-19 literacy and public vaccination scorecard) was developed to evaluate the individual’s comprehension level of COVID-19. A pilot study was conducted with this scoring card. The participants were recruited via consecutive random sampling by using emails from the researcher’s contact list (n=92). A total of 78.3% of the respondents were considered to have an adequate comprehension level. Approximately 77% of all respondents agreed that this card could help them know their comprehension level, 81.5% agreed that this card could improve their comprehension, 81.5% agreed that the items in this card could help them screen facts from hoaxes, and 81.5% agreed that the language used was easily understood by the laypersons. Therefore, the Karlivid is a valid and reliable scorecard that can be used to evaluate public comprehension of COVID-19. Most of the respondents also had a good level of comprehension of this assigned topic

Abstract

Background: Malnutrition is still common among the elderly, and meal provision has an important role in combating this issue. One of the meal provision programs is done by providing adequate quantity and quality of intake in nursing homes. Having less food waste can reflect to which extent intake and quality standards are applied in institutional food services. Objectives: This study aims to examine food waste as a factor in evaluating the adequacy of food intake and quality control in the food service management of nursing institutions. Methods: This study was conducted on 65 elderly at Surabaya Nursing Homes in 2023. Observations on the menu they had were carried out to determine the adequacy of their food intake. The Visual Comstock 7x hours method was used to assess the adequacy of food intake. Then, the food waste limit of 20% became a quality control in food service management. Results: The adequacy of nutritional intakes in the studied Surabaya nursing homes in terms of energy, carbohydrates, fiber, sodium, and potassium were 77.26%, 66.24%, 37.14%, 165.36%, and 27.17% of recommended daily allowance, respectively. This means that both the adequacy of the intake and the type of menu the elderly consumed are still below the standards. Food waste of more than 20% was identified from menus made of papaya, tongkol rica (spicy mackerel tuna), ote-ote (vegetable fritter), rice, green bean compote, sour vegetable soup, skipjack tuna balado (chili pepper skipjack tuna), and melon. Conclusions: The adequacy of food intake that the elderly consumed did not meet daily needs and recommendations. Apart from that, some menus still had >20% leftovers. The menu options need to be diversified to reach the recommended nutritional intake.

Abstract

Introduction: Type 2 diabetes mellitus is caused by decreased tissue sensitivity to insulin. The prevalence of diabetes in the world has almost doubled since 1980, from 4.7% to 8.5% in adult population. Early diagnosis and treatment aimed at normalizing glycemic control are very important. The objective of this study was to evaluate and compare glycemic control of metformin and glimepiride in monotherapy of type 2 diabetes mellitus patients at Islamic Jemursari Hospital Surabaya. Method: This was a retrospective observational study using secondary data (medical record), include glycemic control (RPG) before and two months after receiving therapy of outpatients’ type 2 diabetes mellitus with metformin or glimepiride therapy in 2018. 96 samples were found that fit the inclusion criteria. The data were analyzed by Mann-Whitney test. Result: Most patients were female, aged 50-69 years old, and dosage of metformin therapy 1500 mg/day or glimepiride therapy 2 mg/day. There was no significant difference (p greater than 0.05) of glycemic control (RPG) of metformin compared to glimepiride therapies in type 2 diabetes mellitus patients at Islamic Jemursari Hospital Surabaya in 2018. Conclusion: Metformin and glimepiride were not significantly different in glycemic control (RPG). There were patients with RPG greater than 200 mg/dl after two months of metformin or glimepiride therapy.

Abstract

Indonesia has a variety of plants with various health benefits. Communities in small linkages such as families, RT, and RW can utilize family medicinal plants (TOGA) to maintain stamina so that immunity remains optimal. Educational efforts to utilize TOGA simplisia were carried out in the form of community service in Sumorame, Candi District, Sidoarjo Regency. A total of 38 participants attended the training. The methods used in this activity include counseling and workshops. The knowledge domain of traditional medicine increased by 47.6 points higher than the pretest. A total of 26 participants (68.4%) stated that they often used TOGA. The reasons for using TOGA include natural remedies (n = 22; 57.9%), maintaining stamina and health (n = 12; 31.6%), and first aid when sick before seeing a doctor (n = 4; 10.5%). During the training, participants mentioned 2 to 6 parts of TOGA with their properties. Participants received information about TOGA and its utilization from social media (15%), and mass media (10-14%). The information sources from academics are still low (3%). The community has the potential to use TOGA which can be considered to maintain health in the long term. They already know its utilization from generations and mass media information. Continuous education by academics and health workers is needed for public health and to drive the economy.
