Abstract

Non-communicable diseases (NCDs) are a growing global health problem and a leading cause of death. Health cadres play an important role in the detection and management of NCDs through health monitoring and community education. This community service aims to describe the role of health cadres in early detection of NCDs in the working area of Puskesmas Malang District. The training consisted of three main sessions, namely material on anthropometric measurements, the use of tensimeters and glucometers, and training in filling out NCD questionnaires commonly used in community health monitoring. This activity not only focuses on theory but also provides opportunities for participants to practice directly after the presentation session. Although most cadres have attended training, the application of skills in the use of medical devices and education related to NCDs is still limited. Continuous training and education is needed so that health cadres can optimise early detection and management of NCDs in the community.

Abstract

Emergency Installation (IGD) is the main point of entry for patients to the hospital.  Prolonged IGD waiting time. may lead to crowding that results in patients’ complaints, patient safety incident, and decrease in the quality of service. Waiting time starts from triage to when the patient is transferred to the hospital room with the standard waiting time set < 6 jam. The research aimed at identifying what factors dominantly affected prolonged IGD waiting time in Hospital X. The descriptive research, conducted in a private hospital in Malang Regency, used Time and Motion Study. The sample was collected using simple random sampling technique and included 50 IGD patients that met Emergency Safety index (ESI) 1, 2, dan 3 category.  The result revealed that the average waiting time of ESI 1, ESI 2 and ESI 3 patients was 5 hours. There were 12 patients (24%) with waiting time longer than 6 hours. Partial referral for CT scan, delayed patient transfer due to the wait time for result of consultation with attending physician, stabilization of patients, and unavailable hospital rooms were identified as contributing factors to prolonged IGD waiting time.

Abstract

Hospitals as healthcare providers depend on the claims process to maintain financial stability, especially in the National Health Insurance (JKN) program managed by BPJS. Inefficient claims management can cause delays in fund disbursement and increase pending claims. This descriptive qualitative study identifies waste in the Hemodialysis (HD) service claim process at RSU UMM using the Lean Management approach. The results of the analysis showed that administrative defects were the biggest waste with 22 occurrences, caused by paper-based manual systems, such as duplication or loss of files. In addition, waiting time waste was found 14 times, as well as overprocessing and motion 3 times each. To improve efficiency, digitization of the claims system is recommended to reduce administrative errors and speed up the disbursement process

Abstract

Patient-Centered Care (PCC) merupakan pendekatan penting dalam meningkatkan kualitas layanan, keterlibatan pasien, dan hasil kesehatan dengan menempatkan kebutuhan, preferensi, serta nilai pasien sebagai pusat proses pelayanan. Meskipun manfaat PCC telah diakui secara luas, implementasinya terutama di negara berkembang masih menghadapi berbagai tantangan, seperti kurangnya pemahaman tenaga kesehatan, keterbatasan waktu, beban kerja tinggi, dan minimnya dukungan organisasi. Artikel ini bertujuan melakukan scoping review terhadap faktor-faktor yang memengaruhi implementasi PCC. Pencarian artikel dilakukan secara sistematis melalui database PubMed, ProQuest, dan Google Scholar dengan kata kunci: “patient-centered care, factors influencing PCC, barriers to PCC.” Kriteria inklusi meliputi studi primer, tinjauan sistematis, atau studi kelayakan yang diterbitkan dalam Bahasa Indonesia, Inggris, atau Korea pada tahun 2019–2024. Dari 1.450 artikel yang diidentifikasi, sebanyak 16 artikel memenuhi kriteria dan dianalisis lebih lanjut. Hasil kajian menunjukkan bahwa faktor internal yang memengaruhi implementasi PCC mencakup empati, kompetensi literasi kesehatan, kelelahan kerja, kompetensi keselamatan pasien, pelatihan humanistik, stres kerja, kualitas hidup, profesionalisme perawat, dan komunikasi efektif. Faktor eksternal meliputi lingkungan kerja yang mendukung, budaya organisasi berbasis PCC, infrastruktur fasilitas, dukungan manajemen, aksesibilitas, dan persepsi pasien. Kesimpulan: PCC merupakan pendekatan strategis yang memberikan manfaat besar terhadap hasil kesehatan pasien dan efisiensi layanan. Namun, implementasi yang berhasil membutuhkan dukungan kebijakan, pelatihan, dan perubahan sistemik dalam organisasi pelayanan kesehatan.

Abstract

Objectives: Cardiac arrest can be experienced by anyone and evenincidentcardiac arrestdo not recognizeplace andtime.This article aims to maximize cardiopulmonary resuscitation training strategies with a focused review of articles on CPR training strategies for lay people. Methods:In this Scooping Reviewan international journal search was conducted on the topic by browsing the online databases of ProQuest, EBSCO, and ScienceDirect, using the keywords “strategy” AND “education” AND “improvement” AND “ability” AND “citizen” OR “lay person” OR “person layman” AND “bystander” AND “CPR” OR “cardiopulmonary rescucitation”. Literature was selected based on inclusion criteria, i.e. according to topic, full text articles, English, quantitative methods, experiments, CPR educational themes, full text, open access and articles published in the last 10 years (2013-2023), while the sample exclusion criteria were research thattherein isinvasive actions and there areparticipants with a background in nursing or other health professions. The stages of searching and selecting journal articles are adapted from the PRISMA flowchart Results:At initial disbursement, 858 articles were obtained with 7 studies that met the inclusion criteria. Articles were obtained using a pre-post intervention design (1 article), a prospective cohort intervention (3 articles), a cross-sectional experimental study (1 article), a retrospective design and qualitative methods (2 articles). Based on the articles obtained, there are 3 articles on educational strategies to improve cardiopulmonary resuscitation abilities in ordinary people, which include education, simulation, team, and comprehensive programs. Conclusion:Appropriate media can have a major impact on improving knowledge, attitudes and skills to performing CPR

Abstract

Backgrounds: Satisfaction patient is indicator quality service nursing and success in fulfillment service patient. Standardized service​ already should runs on all units, fine room take care regular, and maintenance units intensive. Purpose: Study this aim for analyze influence caring training towards ability nurse manage complain. Method: Study this using a quantitative design with approach quasy experiment design with group control. Sample size are 21 nurses for every group. Group intervention given based caring training theory swanson. Before and after giving training group intervention will assessed ability nurse manage complain then compared to with group control. Results: Research result show there is significant improvement in implementation​ caring behavior of nurses in manage complain, after get training caring behavior with a mean of 11.38. behavior nurse in give service nursing this is form quality service nursing at home sick that can be influence satisfaction patient. Behavior nurse in give service expected and possible care​ satisfying patient is caring behavior, attention and treatment nurse to client with good. Conclusions: This result expected become reference strategy development for overcome problem related enhancement ability nurse manage complain with consider all over components that influence it in the field nursing.

Abstract

Introduction: Patient density in the emergency room (ER) is a worldwide phenomenon and a major health-care issue. Because of the high degree of patient severity and the dynamic workflow, treating patients in the ER is frequently delayed. But, determining the need for nurses in the ER is difficult since patients arrive at random, including several complex situations, the quantity of patients, and the length of stay shifting greatly. This study aims to assess the emergency nursing staff need by determining the nurses' workload and patient dependency level in emergency room of the hospital using the baseline emergency staffing tool (BEST). Methods: A prospective observational research design was used in this study. A consecutive sample of 101 nurses selected in seven-day period in emergency department. The research was carried out in two government general hospitals situated in the provinces of Central Java and East Java. The BEST was used to estimate nursing staff requirement in emergency department and patient dependency was assessed using the Jones Dependency Tool (JDT). Results: Calculation of staff requirements in the ER of the first hospital has an average of 16 nurses, while the second hospital has an average of 30 people. The staffing challenges faced by these hospitals are significant, with a discrepancy of 3 nurses and 14 individuals in the ER. Based on the official hours, the highest number of staff needed in the evening and night while the relatively low number is in the morning. Conclusions: This gap in staffing might have an effect on how patient's outcomes and poses potential risks. To maintain the greatest standard of service, it is critical to account for changes in patient volume and alter staffing levels accordingly

Abstract

The Covid-19 pandemic has left healthcare workers working in uncertain and stressful situations to care for all patients with disabilities. The second wave of the Covid-19 pandemic that swept across the world caused many nurses in health facilities to experience fatigue in carrying out their duties. This study aims to determine the cause of burnout symptoms in isolation room nurses in the Covid-19 pandemic. The study was conducted at X Hospital in September-October 2021. The design of this study was descriptive analysis. Observations were made on the service system, data, and environment of the Covid-19 isolation room at X Hospital. Interviews and discussions were conducted with the head nurse, the head of the occupational safety and health committee, and the head of nursing medical services related to nurse burnout in the isolation room. 67.74% experienced emotional exhaustion in the medium and high categories in isolation room nurses in the second wave of the Covid-19 pandemic caused by a lack of staffing. The solution to detect and overcome burnout is to conduct mental health checks and regular medical check-up programs for further treatment by considering the comfort factor and high functional value.

Abstract

The hospital provides health services such as inpatient, outpatient, and emergency departments. The quality of nursing care is crucial as patients rely on nurses to address their complaints. This study aims to determine the relationship between nurse burnout and patient safety culture with the quality of nursing care. An analytical observational design with a cross-sectional study approach was conducted in two Type B Hospitals in Central Sulawesi Province with 235 nurse respondents in the inpatient room, using purposive sampling. Data analysis included the Pearson product-moment test and multivariate modeling. Results indicated a significant relationship between nurse burnout and nursing care quality (p-value <0.05), as well as between patient safety culture and nursing care quality. The dominant variable related to the quality of nursing care was patient safety culture (β = 0.723). The study highlights that nurse burnout negatively impacts the quality of nursing care, whereas a strong patient safety culture improves it. Theoretically, this research expands the understanding of the interplay between nurse burnout, patient safety culture, and nursing care quality, emphasizing the need for organizational support and effective communication. Practically, it suggests that hospital administrators should implement strategies to reduce nurse burnout and foster a positive patient safety culture, such as stress management programs and a supportive work environment, to enhance nursing care quality and improve patient outcomes.
