Abstract

Outdoor air pollution contributed harmful impact to public health. There are several respiratory disorders related to outdoor air pollution such as acute respiratory infection, lung cancer, asthma, chronic obstructive lung disease (COPD) and lung function disorder. Respirator is a personnel protective device which has role in the primary intervention step. Currently exist many types of respirators in industrial setting that have specific function to certain hazard exposure in work process. It is difficult to choose one type of respirator that can be implemented in population setting to protect against all air pollutant content. Therefore, it is relevant choosing one respirator type which has the ability to effectively filtrate one of air pollutant content that is the particulate matter. One respirator type, N95 mask has superiority in term of cost and technical use aspects for protecting particulate matter pollutant. Respirator usage effectivity in population setting is an important subject to find out more.

Abstract

Organic dust toxic syndrome (ODTS) was manifestation of some acute symptoms, (same with acute hypersensitivity pneumonitis or extrinsic allergic alveolitis) with initially by flu like syndrome that were fever, malaise, myalgia, dry cough, dyspnea, and headache after exposure of organic dust in several hours. Etiology of ODTS usually from agricultural environment like hay, grain, straw, moldy, pollen, mycotoxins, bacteria and endotoxin. Organic dust toxic syndrome (ODTS) spontaneous recovery without sequele if patients avoid organic dust exposure immediately and avoid recurrent exposure. The best treatment were supportive therapy and preventive therapy from that etiology.

Abstract

Background: Studies evaluating endoscopic results in patients with bronchial asthma and Gastroesophageal Reflux Disease (GERD) in Gatot Soebroto hospital have never been undertaken. It leads an idea to further study, in order to find a more accurate and rapid management as solution for anticipating asthma attack and complications of bronchial asthma to upper gastrointestinal tract. Methods: A retrospective study was aimed to evaluate endoscopic result of upper gastrointestinal tract in bronchial asthma patients who had GERD symptoms as appropriate to the criteria of 4 major GERD symptoms of Talley 2002. Data was collected in one year period starting from November 2004 to October 2005. Results: Subject characteristics in this study indicated that there were more female patients compared to male with a ratio approaching 3:1 who had such symptoms. Mean age was 38.5 years and ratio of body weight to body height indicated normal weight result and the mean value for duration of asthma was 27 years. Clinical symptoms of GERD found in the present study was in accordance with four majors symptoms of GERD i.e. 32 (100.00%) cases of regurgitation, 29 (90.63%) cases for each of heartburn and non-cardiac chest pain symptom, and 7 (21.88%) cases of difficult / painful swallowing or dys/odinophagia. The endoscopic result of upper gastrointestinal tract had figured of: (1) 4 (12.50%) cases of normal esophagus, (2) 11 (34.40%) cases of non-erosive esophagitis known as Non Erosive Reflux Disease (NERD), and (3) erosive esophagitis which regarding to Los Angeles classification: 15 (46.90%) cases of grade A and 2 (6.20%) cases of grade B. Conclusion: The incidence of esophagitis in accordance with LA classification is extremely high although no severe damage (grade C and grade D) was found. Early anticipation of reflux associated respiratory symptoms and anti-reflux treatment should be considered in order to shorten or to discontinue the asthma attack cycle. Abstract

Background: : Factors related to the severity and the outcome of coronavirus disease 2019 (COVID19) are emerging subjects of interest to be studied. Smoking has long been known to have a negative impact on the response to infection. The purpose of this study was to determine the association between smoking habits with the severity, the length of hospitalization and the outcome of COVID19 patients.Methods: This study was a prospective cohort study of COVID-19 patients admitted at Persahabatan Hospital, Jakarta, Indonesia. Subjects were included by consecutive sampling from August to October 2021. Subjects were assigned into smokers and non-smokers group. All subjects were assessed for their severity, length of hospitalization and outcome; in smoking patients, the Brinkman Index (IB) and the Fagerström Test for Nicotine Dependent (FTND) score were also recorded. Statistical tests were then carried out to assess the significant association between smoking history and the degree of severity, length of hospitalization and outcome of COVID-19 in all patients and the FTND and IB scores with the degree of severity, length of hospitalization and outcome of COVID-19 in smokers.Results: Authors found that 39/100 (39%) of participants were smokers. Smoking history had a correlation with the outcome (p=0.001) but not with the degree of severity (p=0.410) and length of hospitalization (p=0.780). Among subjects with smoking history, there were correlations found between FTND and the degree of severity and the outcome (p=0.022 and p=0.012), but not with the length of hospitalization (p=0.716). The Brinkman Index was correlated with only the degree of severity (p=0.020).Conclusion: Our study found that smoking habits were correlated with the degree of severity and outcomes in COVID-19 patients.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) patients are at high risk for COVID-19 infection and severe pulmonary complications. Exercise-based pulmonary rehabilitation (PR) in outpatient settings is essential for COVID-19 survivors with COPD comorbidities, providing the most critical patient benefits, but it is challenging during the pandemic.Case: A sixty-four-year-old man with COPD since 2 years ago, a history of COVID-19 one month ago, and Pulmonary Tuberculosis since 1 year ago presented with chief complaints of tiredness, cough, and breathlessness after walking for more than 100 meters. We performed PR, including breathing retraining exercise, chest mobility exercise, active cycle breathing technique (ACBT), posture correction, and aerobic exercise with static ergo-cycle for 8 weeks.Discussion: After 8 weeks of PR, there was a 3% increase in the O2 saturation level from 94-95% room air to 98%, an increase of single breath counting test (SBCT) from 20 to 38 counts, improvement of peak cough flow from 100-110-100 to 420-435-425 L/minute, and peak flow meter from 140-150-145 to 380-400-400 L/minute. Before PR the patient could not perform the sit-to-stand test (STS) and a 6-minute walking test (6MWT), but after 8 weeks of PR, STS was 5 times in 30 seconds, and 6MWT maximum distance was 248 meters. COPD assessment test (CAT) score improved from 23 to 9, and the Modified Medical Research Council (mMRC) dyspnea scale improved from 3 to 2.Conclusion: Eight weeks of pulmonary rehabilitation showed benefits for the patient in reducing dyspnea and improving exercise tolerance and quality of life, especially in hospital-based settings.

Abstract

Background: Smoking increases the risk of tuberculosis infection and affects its treatment success rate and mortality. Most TB patients who smoke quit at the initial diagnosis, but may continue to smoke if the clinical symptoms improve. Studies show that the 4T approach (Tanya, Telaah, Tolong nasehati, and Tindak lanjut) helps smokers quit. The 4T approach was applied in Indonesia as a smoking cessation program for TB patients.Methods: We conducted a randomized controlled trial on 43 male TB patients who smoke. The trial group received a 4T approach consisting of education, counseling, and motivation to quit smoking for three months. The control group received a self-help leaflet. Smoking status, Fagerström nicotine dependence scale, exhaled carbon monoxide level, and peak expiratory flow rate were collected. We observed the subjects at months 1, 2, and 3 after quitting smoking and reported on the Motivation and Minnesota Withdrawal Scale.Results: Smoking cessation levels during months I, II, and III (Continuous Abstinence Rate I, II, and III) were higher in the trial group than in the control group. The trial group had a higher percentage of smoking cessation than the control group: until 4 weeks (66.7% vs. 54.5%), until 8 weeks (57.1% vs. 45.5%), and until 12 weeks (52.4% vs. 45.5%). The control group had higher numbers of smoking relapses (18.2% vs. 14.3%) and still smokers (18.2% vs. 9.5%) after the study. Withdrawal symptoms included an increase in appetite (44.1%), cigarette cravings (6.9%), agitation (2.3%), insomnia (2.3%), and irritability (2.3%). There were no significant differences in the withdrawal scale between groups (P=0.788), but the trial group showed better motivation to stop smoking during CAR II (P=0.043).Conclusion: The 4T approach is effective in maintaining abstinence from smoking among lung tuberculosis patients until months 1, 2, and 3 after quitting. Smoking cessation programs during tuberculosis treatment can help patients quit smoking and reduce relapse.

Abstract

COVID-19 has affected the older population disproportionately. These patients are prone to have more severe COVID-19. Clinical manifestation of COVID-19 in the geriatric can be atypical and differs from the younger population. We report an 80-year-old male with atypical presentation of COVID-19. In this case report, we describe a COVID-19 patient with anorexia and gastrointestinal symptoms. We also describe the multidisciplinary aspect of physical medicine and rehabilitation management of the patient. Clinical manifestations in geriatric with COVID-19 can be atypical. Symptoms experienced may include anorexia and gastrointestinal tract symptoms. A multidisciplinary approach is needed to manage more seniors with COVID-19 and maximize their functional abilities.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) development involves a complex pathway of host and environmental factors. Besides cigarette smoking, previous studies showed exposure to air pollution, such as particulate matter sized 2.5 μm or less (PM2.5), might also have an important role in COPD development because it might lead to airway remodeling and chronic lung inflammation. However, the cause-and-effect relationship between PM2.5 and COPD in non-smoking patients is still unclear.Methods: Literature searches were performed in five online medical databases (PubMed, EMBASE, ScienceDirect, EBSCOhost, and Cochrane Library) and hand-searching in Google Scholar. Filtering literature with the inclusion and exclusion criteria resulted in three relevant articles (1 case-control and 2 cohort studies). Critical appraisal was conducted using the Center of Evidence-Based Medicine (CEBM) worksheet from the University of Oxford for etiologic studies.Results: All three articles were considered valid. The prospective cohort was decided unimportant because of the non-significant adjusted hazard ratio (HR 1.23; 95% confidence interval [CI]=0.50-3.06). The case-control and retrospective studies had important results with adjusted odds ratio of 1.29 (95% CI=1.01-1.65) and 1.69 (95% CI=1.11-2.58), respectively. The relatively low number needed to harm (NNH) of 10-23 indicated that PM2.5 exposure was a meaningful factor for the risk of developing COPD in non-smoker adults. Both articles were considered applicable to our case. Conclusion: Non-smoking adults with exposure to PM2.5, compared to those without exposure, are at higher risk of developing COPD.

Abstract

Introduction: Tuberculosis (TB) continues to be a major global public health threat, affecting millions of people annually. Despite efforts to control TB, the number of cases remains high. The situation is exacerbated by the COVID-19 pandemic, which has strained healthcare systems. The increased workload of hospital employees also raises the risk of TB transmission. This study aims to develop an instrument to evaluate employee protection from TB in hospitals. Methods: This study used a qualitative design with an exploratory approach. Data were collected from existing policies and through in-depth interviews with stakeholders, including academics, practitioners, regulators, infection control officers, occupational health and safety officers, and non-government organization representatives for TB. Results: The instrument developed comprised seven components, namely management commitment; program planning; coordination, communication, and education; implementation of TB and other disease management programs in the workplace; handling TB cases in the workforce; training and competence; and monitoring and evaluation. Conclusion: Instrument development is crucial for capturing the current state of TB problems and managing prevention and control programs in hospitals. The terms of reference serve as an effort to protect hospital employees

Abstract

The possibility of accidents in a building project is one of the causes of hampering the effectiveness of project work activities. Workers are required to implement a good occupational safety and health (K3) management system to minimize the possibility of work accidents. An example of a case in the construction world occurred in the Gading Mediterania Apartment construction project located in Kelapa Gading. The accident that occurred killed four workers. Two people were killed by precast concrete and two people died from electric shock. Therefore, to prevent the recurrence of the incident, research was conducted using fishbone analysis. The purpose of this research is to analyze the importance of using personal protective equipment at PT Djabesmen. The method used helps the author in analyzing the possibility of accidents on the project. The results of the analysis explain that the percentage of PPE use at PT Djabesmen is good.
