Abstract

Background: Retropharyngeal abscess (RPA) is a serious deep neck space infection that can lead to life-threatening complications such as airway obstruction, mediastinitis, and sepsis. The Trendelenburg position, where the patient's head is lower than their feet, has been traditionally used in the management of RPA to prevent aspiration of pus in case of abscess rupture. However, the optimal positioning for patients with RPA remains a topic of debate. Case presentation: A 51-year-old male patient presented to the emergency room with a 5-day history of progressive dysphagia and neck swelling. He was diagnosed with a ruptured retropharyngeal abscess extending to the mediastinum (descending necrotizing mediastinitis). The patient underwent surgical drainage and was subsequently admitted to the intensive care unit (ICU), where he was intubated and mechanically ventilated in the Trendelenburg position. Five days later, the patient developed pneumonia, suspected to be ventilator-associated pneumonia (VAP). Conclusion: The Trendelenburg position may be considered in the management of RPA to minimize the risk of aspiration. However, it is essential to weigh the potential benefits against the risks, including VAP, and to consider alternative positions such as the lateral Trendelenburg position, which may offer similar benefits with reduced risk. Further research is needed to determine the optimal positioning strategy for patients with RPA.

Abstract

This study aims to analyze the implementation of Tahfidzul Qur'an learning at the Adlaniyah Modern Islamic Boarding School in West Pasaman, with a focus on the methods used, supporting and inhibiting factors, and the results achieved by students in the memorization process. The type of research used is qualitative research with a descriptive approach. Data were obtained through observation, interviews with the head of the foundation, tahfidz teachers, and students, and supported by documentation. Data analysis was carried out through the stages of data reduction, data presentation, and drawing conclusions. The results of the study indicate that the implementation of the tahfidz program was carried out systematically through the talaqqi and muraja'ah methods which have proven effective in guiding students. Supporting factors include a conducive Islamic boarding school environment, teacher and parental support, and the availability of adequate facilities and infrastructure. Meanwhile, inhibiting factors include limited time, lack of student motivation, difficulties in tajwid, and the emergence of boredom in memorizing. Overall, the implementation of tahfidz learning at the Adlaniyah Modern Islamic Boarding School in West Pasaman has gone well, but there is still a need for innovation in methods, increased motivation, and the use of more varied learning strategies to improve the quality of students' memorization.

Abstract

This research purpose to examine the impact of audit findings and follow-up on audit opinions on local government financial statements, as well as to analyze the role of local government size as a moderating variable. The research subjects include local governments headed by Acting Regional Heads in 2023 in Indonesia. The method used is quantitative with a comparative causal approach, with a sample of 255 local governments selected using purposive sampling. Data were obtained from the audit reports of the Supreme Audit Agency and local government financial statements. Logistic regression analysis shows that follow-up on audit recommendations has a partial positive effect on audit opinions, while audit findings and follow-up on recommendations together influence audit opinions. Furthermore, the size of the local government strengthens the relationship between audit findings and follow-up on recommendations on audit opinions, indicating that the capacity and operational scale of local governments have an impact on the effectiveness of improvements and auditors' assessments of public financial management.

Abstract

Background: Procalcitonin (PCT) is widely used as an inflammatory marker in sepsis, but still controversial in deep neck abscesses. Purpose: To evaluate the usefulness of PCT as outcome predictor in deep neck abscess. Method: A prospective observational cohort study was conducted at Dr. M. Djamil Hospital, Padang, Indonesia (March 2022–May 2023). Patients with confirmed deep neck abscess by pus aspiration in the Emergency Department were included. Baseline laboratory and serum PCT tests were performed before surgical drainage. All patients received broad-spectrum antibiotics and standard postoperative care. Assessed outcomes were mortality, septic shock, organ failure, mediastinitis, empyema/ pleural effusion, necrotizing fasciitis, tracheostomy, pharyngocutaneous fistula, Intensive Care Unit >24 hours, and hospital stay. Result: Thirty-five patients met inclusion criteria. Multiple neck space involvement was found in 74.3%, and 93.4% underwent surgery. Mortality occurred in 28.6%, septic shock in 20%, mediastinitis in 14.3%, empyema/pleural effusion in 11.4%, necrotizing fasciitis in 8.6%, tracheostomy in 5.7%, fistula in 8.6%, and ICU stay >24 hours in 20%. Mean hospital stay was 9.03±7.90 days. Mean admission PCT was 0.313±0.677 ng/mL, with 17.1% having elevated PCT>0.5 ng/mL. No significant correlation was found between PCT and overall outcome. However, Receiver Operating Characteristic (ROC) analysis showed PCT≥0.14 ng/mL, predicted <5-day mortality (sensitivity 85.7%, specificity 78.4%), Area Under Curve (AUC) 0.773, p=0.027). Conclusion: Procalcitonin showed potential as an adjunctive marker in deep neck abscess. Although low initial levels are frequent, they do not exclude severe complications, highlighting its role in early risk stratification and clinical decisions.

Abstract

Jurnal ini bertujuan untuk mendeskripsikan zakat dalam perspektif ekonomi Islam. Zakat dalam ekonomi Islam mempunyai potensi yang signifikan, maka dari itu Pemerintah harus benar-benar memperhatikan bagaimana kondisi ekonomi masyarakat guna mencapai kesejahteraan hidup. Zakat dapat menjadi solusi utama yang dapat diterapkan untuk mencapai kesejahteraan masyarakat dalam memenuhi kebutuhan dan memeratakan kesenjangan yang terjadi dalam masyarakat. Sehingga zakat tidak hanya dikatakan sebagai nilai agama yang wajib bagi umat Islam, akan tetapi zakat juga dapat dikatakan sebagai nilai yang mempunyai manfaat pada perekonomian dengan sangat signifikan.

Abstract

This study examines the various character types of millennials currently found in high schools. Millennials, born between the early 1980s and the mid-1990s and early 2000s, are known for their unique characteristics influenced by technological advancements and social change. This study identified several dominant character types among millennial high school students, namely: Digital Natives: Students who are highly skilled in technology and social media, tend to be constantly connected to the virtual world, and have strong multitasking skills. Socially Sensitive: Students who show a strong concern for social and environmental issues, are active in social and community activities, and are often involved in campaigns or non-profit organizations. Ambitious and Competitive: Students who have clear academic and career goals, are highly achievement-oriented, and are frequently involved in various extracurricular activities to enhance their skills and experience. Creative and Innovative: Students who have a strong interest in art, design, or creative technology, often generate new ideas and engage in creative projects both inside and outside of school. This study also explores how these characteristics affect students' social interactions, academic achievement, and emotional well-being1 Quantitative research is a type of research that produces findings that can be achieved (obtained) through the use of statistical procedures or other forms of quantification. The informants of this study were 65 people. The results of this study show that Islamic Religious Education has a positive and significant influence on the character of students. This can be proven from the results of the Islamic Religious Education hypothesis test which shows a t-value of 6.171 > 2.000 or positive with a significance level of 0.000 < 0.05. This means that the hypothesis in this study rejects Ho and accepts Ha. And obtained the linear regression value Y = 14.688 + 0.661X, sig. Level 0.000 < 0.05 (alpha value), based on the output obtained of 0.661, meaning that if Islamic Religious Education is better then the character will experience an increase of 66.1%. The influence of Islamic Religious Education on the character of students based on the results of the determination coefficient analysis obtained R2 (R Square) of 0.377 (0.614 x 0.614) or 37.7%. This problem shows that the independent variable (Islamic Religious Education) used is able to explain 37.7% of the dependent variable (character). While 62.7% is influenced or explained by other variables not included in this study.
