Abstract

Spinal anesthesia or Block (SAB) is one of the regional anesthesia techniques, which is one technique to obtain analgesia as high as certain dermatons and relax skeletal muscles by injecting spinal anesthesia drugs into the subarachnoid space of the block. One of the complications that can occur is Postoperative nausea and vomiting (PONV). PONV can be caused by many factors, one of which is the length of fasting. Preanesthesia fasting about 6-8 hours before surgery is performed as an effort to minimize the incidence of aspiration of stomach contents or nausea and vomiting. Objective:Determine the relationship between the length of fasting and the incidence of nausea and vomiting in post-spinal anesthesia. This type of research is an observational study with a Cross-sectional research design. The population in this study is all patients who will undergo spinal anesthesia in the IBS room of RSUD Kab. Batang. Sampling in this study was using the Purposive Sampling method of 49 samples and using the Spearman Rank analysis test. The research instrument uses observation sheets. The age of most respondents in this study was 36-40 years as many as 26 respondents (53.1%), the most gender is women 32 respondents (65.3%), and those who experienced the most fasting length of 2-6 hours with 30 respondents (61,3%), while PONV as many as 29 respondents (59.2%). Based on the results of the Spearman Rank test to determine the relationship between the length of fasting and the incidence of Post Operative Nausea and Vomiting (PONV) obtained a result of 0.007 (P0.5). The majority of respondents who did not experience PONV were 14 respondents (28.6%). There is a significant relationship between the length of fasting and the incidence of Post Operative Nausea and Vomiting (PONV).

Abstract

Non-pharmacological pain management is an important approach to improving the quality of life of patients with chronic pain. This technique includes various methods, such as relaxation, heat and cold therapy, distraction, and deep breathing techniques, which aim to reduce the perception of pain without the need for medication. This approach is especially useful for patients who cannot or do not want to rely on pharmacological treatment. This training aims to improve the understanding and skills of health cadres in implementing non-pharmacological techniques in pain management in the community. This community service uses a quantitative method with a pre-experimental one-group pre-test and post-test design to measure changes in the understanding and skills of health cadres. Respondents consisted of 20 health cadres who lived in Jajar Village, Surakarta. The training materials included basic theories of pain management, as well as direct practice of non-pharmacological techniques. The results of the community service showed a significant increase in the understanding and skills of health cadres after being given training. This can be seen from the significant difference in pre-test and post-test scores. This increase indicates that non-pharmacological training is effective in improving the competence of health cadres, which is expected to be applied in helping patients manage pain in a safer and more efficient way.

Abstract

This study explored the challenges of emotion recognition in Indonesian speech using deep learning techniques, addressing the complex nuances of emotional expression in spoken language that posed significant difficulties for automatic recognition systems. The research focused on the application of feature extraction methods and the implementation of convolutional neural networks (CNN) and a hybrid convolutional neural networks-long short-term memory (CNN-LSTM) model to identify emotional states from speech data. By analyzing key features of speech signals, including mel frequency cepstral coefficient (MFCC), zero crossing rate (ZCR), root mean square energy (RMSE), pitch, and spectral centroid, the study evaluated the models’ ability to capture both spatial and temporal patterns in the data. Testing was conducted using an Indonesian dataset comprising 200 samples. The CNN model, utilizing four features (MFCC, ZCR, RMSE, and pitch), and the CNN-LSTM model, which used three features (MFCC, ZCR, and RMSE), both achieved an emotion classification accuracy of approximately 88%. The result showed that the CNN-LSTM model achieved comparable performance with a simpler feature set compared to the CNN model. This highlighted the significance of choosing the appropriate techniques in feature extraction and classification to enhance the accuracy of identifying emotions from speech data while also managing computational complexity.

Abstract

Background: Parenting styles influence sleep routines and problems in children with Attention-Deficit/Hyperactivity Disorder (ADHD); sleep quality includes efficiency, latency, duration, and wakefulness after onset. This study explores the experience of implementing bedtime routines based on caregiver parenting styles and their implications for the sleep quality of children with ADHD. Methods: Qualitative study with a phenomenological approach; non-probability sampling (purposive sampling). Participants consisted of 8 caregivers (mothers, aunts, grandmothers) of children with ADHD at YPAC Surakarta; semi-structured in-depth interviews were conducted 2–3 times per participant and analyzed thematically with credibility and confirmability tests. Results: Three main themes emerged: (1) Caregiver parenting styles (implementation and influence on sleep), (2) Types of parenting styles based on Baumrind's aspects, (3) Sleep quality of children with ADHD (sleep quality, sleep problems, supporting factors). Authoritarian parenting was linked to a stable sleep schedule but had no impact on efficiency; democratic parenting influenced emotional regulation; permissive parenting had no apparent effect on sleep quality; neglectful parenting was associated with sleep-onset insomnia without a clear cause. Conclusion: Variations in parenting styles result in different profiles of impact on sleep quality indicators. Education and harmonization of parenting styles are recommended in pediatric occupational therapy services.
