Abstract

Embryonal rhabdomiosarcoma (ERMS) adalah suatu keganasan yang berasal dari jaringan lunak dan sering di jumpai pada anak-anak. Insidensinya mendekati 4,5 kasus tiap 1 juta anak-anak dibawah usia 20 tahun. Insidensi tertinggi pada umur rata-rata 6 tahun. Dilaporkan sebuah kasus ERMS regio femur sinistra pada seorang anak laki-laki usia 1 tahun, dengan benjolan pada paha kiri yang dikeluhkan sejak 6 bulan sebelum masuk rumah sakit. Benjolan semakin membesar, tidak disertai nyeri, dan tidak ada riwayat trauma sebelumnya. Pada pemeriksaan fisik ditemukan benjolan 4 x 5 cm di daerah femur kiri. Pada pemeriksaan histopatologi tampak tumor berbatas tidak tegas dengan didapatkan jaringan ikat disekitar tumor. Tumor seluler terdiri dari sel-sel kecil dengan inti bulat ovoid, kromatin inti hiperkromatik, beberapa sel tumor sedang bermitosis. Tampak daerah yang lebih hiposeluler dengan gambaran pembuluh-pembuluh darah yang membentuk pola hemangio perisitik. Pasien didiagnosis suatu small round cell tumor dengan kecenderungan suatu ERMS. Diagnosis histopatologi dikonfirmasi dengan pulasan imunohistokimia. Pulasan desmin positif pada sebagian sel, MyoD1 positif pada hampir seluruh inti, SMA negatif. CD99 Non spesifik, LCA dan CD20 negatif. Diagnosis ERMS ditegakkan berdasarkan data klinis, radiologis, histopatologis dan ditunjang dengan imunohistokimia.

Abstract

Tumor phyllodes adalah tumor bifasik yang memiliki karakteristik adanya dua lapis komponen sel epitel yang tersusun di dalam celah celah yang dikelilingi oleh komponen mesenkim hiperseluler yang tumbuh berlebihan yang selanjutnya akan membentuk struktur seperti daun. Tumor phyllodes dapat diklasifikasikan menjadi tiga varian yaitu tumor benign, borderline, dan malignant phyllodes. Penelitian ini dilakukan untuk mengetahui jumlah kasus dan karakteristik pasien varian tumor phyllodes payudara berdasarkan usia, manifestasi klinis dan ukuran tumor di RSUP Sanglah Denpasar pada periode tahun 2014-2018. Penelitian ini adalah penelitian deskriptif dengan desain cross-sectional. Data diambil dari arsip pemeriksaan histopatologi di Instalasi Laboratorium Patologi Anatomi RSUP Sanglah, Denpasar tahun 2014-2018. Terdapat 89 kasus tumor phyllodes yang ditemukan di RSUP Sanglah pada periode tahun 2014-2019 dengan proporsi varian yaitu 31 kasus (34,8%) tumor benign phyllodes, 34 kasus (38,2%) tumor borderline phyllodes, dan 24 kasus (27%) tumor malignant phyllodes. Rerata usia pasien pada varian tumor benign phyllodes, tumor borderline phyllodes, dan tumor malignant phyllodes adalah 41,3 tahun, 45,8 tahun dan 42,5 tahun. Manifestasi klinis yang paling sering ditemukan pada tumor phyllodes payudara di penelitian ini adalah adanya massa pada payudara pada semua varian. Rerata ukuran tumor pasien pada varian tumor benign phyllodes, tumor borderline phyllodes, dan tumor malignant phyllodes adalah 12 cm, 16,9 cm dan 21 cm.

Abstract

Klasifikasi tumor sistem saraf pusat WHO terakhir didasarkan atas histogenesis tumor dan karakteristik molekuler genetik. Perubahan klasifikasi yang paling signifikan terjadi pada tumor glioma difus yang paling signifikan, yaitu katagori tumor secara garis besar berdasarkan status mutasi isocitrate dehydrogenase (IDH) 1 dan 2. Tulisan ini akan membahas tentang IDH1 dan IDH baik normal maupun mutan, hubungan mutasi IDH1 dan IDH2 dengan prognosis pasien, serta bagaimana penentuan status mutasi IDH1 dan IDH2 dalam diagnosis glioma.

Abstract

Introduction: Lung cancer found in young age is relatively rare with a percentage of cases only 1.4% of overall lung cancer cases in the world. Case: We reported two cases of lung adenocarcinoma in young women aged less than 30. The first case was lung adenosquamous carcinoma in a 28-year-old woman, a passive smoker, who had a family history of cancer. She had the mutation of epidermal growth factor receptor (EGFR) exon 19 deletion and received 1st generation of tyrosine kinase inhibitor. Since the patient was diagnosed at advanced stage with poor performance status, she died within 2 months after being diagnosed. The second case was lung adenocarcinoma in a 23-year-old woman. The patient was a passive smoker and had a history of cancer in the family. She was diagnosed at an advanced stage but with good performance status. The patient underwent chemotherapy with Pemetrexed-Platinum in 6 cycles and 15 cycles of radiotherapy. Conclusion: Lung cancer at a young age is more common in non-smoking female with the histopathology type adenocarcinoma. Non-specific early symptoms become a problem in the early enforcement of lung cancer at a young age. Many cancer-related issues occur in young cancer patient such as premature death, increased dependence on parents, difficulty in school and work due to medication, and reproductive disturbance.

Abstract

Background: Excessive exercise cause a deterioration of organ function and structure. Overtraining will cause reduce antioxidant reserve and excess free radical production. In this condition, there will be a decrease in the number of Leydig cells in the testis and followed by a decrease in testosterone level. Bali robusta coffee extract contains active compounds such as alkaloids, saponins, flavonoids, tannins and polyphenols. This study aims to prove whether Bali robusta coffee extract (Coffea canephora) can prevent a decrease in the number of Leydig cells and testosterone levels in male Wistar rats (Rattus norvegicus) with excessive physical training. Methods: Thirty-six rats were randomly divided into 2 groups: the control group (placebo aquabidest 2 ml) and the treatment group (Bali robusta coffee extract of 1 gram/kg BW). Both groups received excessive physical training in the form of swimming until the subjects experienced fatigue for 21-days. Results: The results showed that the mean of Leydig cell number in the treatment group was significantly higher, which was 3.70 ± 0.89 cell/field of view, compared to the control group, which was 2.92 ± 0.65 cell/field of view (p = 0.005). The mean testosterone level in the treatment group was also significantly higher, which was 6.32 ± 0.21 nmol/ml, compared to the control group, which was 1.95 ± 0.32 nmol/ml (p < 0.001). Conclusion: The administration of Bali robusta coffee extract (Coffea canephora) prevented a decrease in the number of Leydig cells and testosterone levels in male Wistar rats (Rattus norvegicus) with excessive physical training.

Abstract

Background: The main factor of skin aging is exposure to sunlight, especially UV B rays. Tamarind leaves extract known containing tannins, flavonoids, and polyphenols as anti-aging agents. This study was aimed to prove the protective effect of 3% Tamarind leaves (Tamarindus indica) ethanol extract cream in inhibiting the skin aging due to exposure to UV B by evaluate the tyrosinase expression and the amount of melanin. Methods: This was a randomized post-test only control group design involving 36 subjects of male Guinea pigs, that divided into 2 groups. The control group received placebo cream and the treatment group received 3% Tamarind leaves ethanol extract cream. Cream application was given twice a day for 2 weeks. UV B were given 3 times a week for 2 weeks with a total dose of 390 mJ/cm2. Tyrosinase expression was examined by calculating the cell percentage that express the enzyme using the immunohistochemical method. The amount of melanin was examined digitally by calculating the pixel percentage of melanin in Masson Fontana staining slides. Statistical analysis by the Independent T-test to compare the mean between two groups with p=0.05 significance. Results: The mean tyrosinase expression and the mean amount of melanin in the control group was higher than treatment group. 20.650±4.911% vs 16.067±3.451%, p=0.003 and 10.461±4.916 pixel% vs 7.300±3.111 pixel%, p=0.029, respectively. Conclusion: Administration of 3% Tamarind leaves (Tamarindus indica) etanol extract cream inhibited the increase in tyrosinase expression and the amount of skin melanin in male guinea pigs (Cavia porcellus) exposed to UV B.

Abstract

Background: Prostate cancer is the second most common malignancy in men and the fifth most common cause of death worldwide. In Asia, 59.3% of patients come for the advanced stage treatment. PTEN inactivation is identified in about 20% of primary prostate tumors in radical prostatectomy and the loss of PTEN is associated with poor clinical and pathological outcomes. The purpose of this study is to prove that there is an association between PTEN expression and risk grade group Gleason prostate adenocarcinoma in Sanglah Public Hospital, Denpasar.Methods: This is a cross-sectional study. The sample size of this study was 35 paraffin blocks. These samples were selected by proportional stratified random sampling from hematoxylin-eosin preparation. Prostate adenocarcinoma was regrouped into 3 categories based on NCCN risk stratification: low risk grade group Gleason (Gleason score ≤ 6), intermediate risk grade group Gleason (Gleason score 7), and high-risk grade group Gleason (Gleason score 8–10). Immunohistochemistry examination of PTEN was performed and the expression was evaluated by scoring method. The data were analyzed by Chi-square and logistic regression.Results: The analysis result showed that there is an association between PTEN expression and risk grade group Gleason that is statistically significant. The loss of PTEN expression associated with higher risk grade group Gleason is of the higher proportion with p=0.001; PR 3.339; 95% CI: 1.296–8.599, but there is no association between the proportion loss of PTEN expression heterogeneously or homogeneously and the risk grade group Gleason with p=0.742; PR 0.663; 95% CI: 0.179–2.457.Conclusions: This study has proved that PTEN expression is associated with higher risk grade group Gleason prostate adenocarcinoma.

Abstract

Introduction: Tethered Cord Syndrome (TCS) is a condition that occurs due to spinal cord traction caused by the attachment of the spinal cord to the surrounding tissues. The incidence of TCS is quite rare, approximately 0.25 per 1,000 live births Case Presentation: We reported a 4-year-old female patient with a lump on her sacral region since birth which enlarged over time. The patient experienced intermittent fever for 1 month before hospital admission and constipation since infancy. Physical examination revealed a lump with a diameter of 6 cm on her sacral region with no neurological abnormality. Ultrasonography examination showed an anechoic lesion with multiple linear structures on supragluteal sinistra with a size of 2.2 x 2.5 x 2.1 cm. MSCT findings showed spina bifida at L5 until S2 level with features of lipomyelomeningocele and tethered cord started from L2 level. Bilateral hydronephrosis, hydroureter, and distended bladder (pine tree appearance) with the possibility of the neurogenic bladder were also found on MSCT examination. The patient underwent surgery to excise the lump and to release the tethered cord. Microscopic findings from excised mass showed a group of neuronal and glial cells, the syncytial sheet pattern of meningothelial cells, and foci of calcification which supported lipomyelomeningocele diagnosis. Conclusions: : Intensive follow-up care and observation must be carried out in children with spinal dysraphism because it may be associated with TCS which may be asymptomatic at an early stage.

Abstract

Introduction: Kimura’s disease (KD) in Indonesia is quite rare. It is a distinct benign reactive process; however, it is usually mimicking malignancy. It usually affects young adults with a peak incidence in three decades, and men are mostly affected compared to women with a 3:1 ratio. KD is a chronic inflammatory disorder characterized by lymphoid hyperplasia, eosinophilia, and associated with soft tissue swelling that usually arises in head and neck regions with regional lymphadenopathy.Case Presentation: Herein, we report a case in a 55-year-old man from East Indonesia who presented a huge lump on his left head and neck regions. The lump started about 8 years ago. Physical examination found a tumor mass of 20 x 10 cm with indistinct border and multiple lymphadenopathies left coli with the biggest size of 1.3 x 0.7 x 0.5 cm. Peripheral blood examination shows marked eosinophilia. Nasopharyngeal CT scan revealed soft tissue masses and multiple lymphadenopathies suspected of malignancy; however, thorax plain radiology did not show any sign of metastatic process. Histopathological examination shows reactive follicular proliferation, extensive eosinophilia, focal area Folliculolysis, and polykaryocytes of the WarthinFinkeldey type.Conclusion: Based on a complete history, laboratory findings, and histopathological examination, this case is concluded as Kimura’s disease. Although clinical findings and radiological examination suspected it as malignancy, those discrepancies were confirmed as a benign lesion by specific histopathological and laboratory findings. Commonly, most patients have a favorable prognosis and good response to therapy.

Abstract

Background: Osteochondroma is a benign cartilage tumor which is quite a number of events with a clinical picture and quite varied pathology. The cause of osteochondroma is still not known with certainty. Data from osteocondroma cases in Indonesia especially in Bali are still very rarely found, even histopathological data registered at hospitals in Bali are still very rare. Aim: The study aims to determine the clinicopathological characteristics of osteochondroma patients in the Sanglah Hospital Denpasar for 5 years.Method: This research is a cross-sectional descriptive study conducted at the Sanglah Hospital using the histological data archive for the period 2013 - 2018. The sampling technique used total sampling, with a total sample of 32 people.Results: Most of the osteochondroma sufferers in this study were 59.4% male, while female sufferers were around 40.6%, with the most age range being 10 - 20 years (62.5%) and at least those aged > 20 years (15.6%). The most tumor location is the distal femur bone (37.5%). Most tumor size is at the size of 1-5 cm (78.1%). Most cases of osteochondroma based on the thickness of the hyaline cartilage cap are 1-3 mm (59.4%).Conclusion: The clinicopathological characteristics of osteochondroma patients by using histopathological data in the Sanglah Hospital shows that osteochondroma is more common in men. Osteokondroma merupakan salah satu tumor tulang rawan jinak yang angka kejadiannya cukup banyak dengan gambaran klinis dan gambaran patologinya cukup bervariasi. Penyebab osteokondroma masih belum dapat diketahui dengan pasti. Data dari kasus osteokondroma di Indonesia khususnya di Bali masih sangat jarang ditemukan, bahkan data histopatologi yang teregristasi di rumah sakit di Bali masih sangat jarang terjadi. Tujuan: Untuk mengetahui karakteristik klinikopatologi pasien osteokondroma di Rumah Sakit Umum Pusat (RSUP) Sanglah Denpasar selama 5 tahun.Metode: Penelitian ini merupakan penelitian deskriptif cross-sectional yang dilaksanakan di RSUP Sanglah menggunakan arsip data histologi periode tahun 2013 – 2018. Teknik pengambilan sampel menggunakan total sampling, dengan total jumlah sampel sebanyak 32 orang.Hasil: Sebagian besar penderita osteokondroma pada penelitian ini adalah laki – laki selitar 59,4%, sedangkan penderita perempuan sekitar 40,6%, dengan rentang usia terbanyak adalah 10 – 20 tahun (62,5%) dan paling sedikit pada usia > 20 tahun (15,6%). Lokasi tumor paling banyak adalah tulang distal femur (37,5%). Ukuran tumor paling banyak adalah pada ukuran 1 – 5 cm (78,1%). Kasus osteokondroma berdasarkan ketebalan tutup tulang rawan hialin terbanyak adalah pada ukuran 1 – 3 mm (59,4%).Simpulan: Karakteristik klinikopatologi pasien osteokondroma dengan menggunakan data histopatologi di RSUP Sanglah menunjukkan osteokondroma lebih banyak diderita oleh laki – laki.
