Abstract

Providing information is not enough to improve diabetic patient’s compliance and achieve goals of therapy. Patient’s good awareness as well as emotional and social supports from family and community may play an important role to improve their compliance and clinical outcomes. Therefore, diabetes support groups were developed and each support group consisted of two pharmacists, two nurses, diabeticpatients and their family members. A total of 70 type 2 diabetic patient’s were enrolled and randomized into support group 1 and support group 2. Patients in the group 1 received information leaflets only, while patients in the group 2 received pharmacist counselling and information leaflets at each meeting. Patient’s awareness of diabetes and compliance with medications were assessed by a short questionnaire at baseline and final follow-up. Blood glucose and cholesterol levels were also evaluated in both groups.At the end of study, the overall patient’s awareness and compliance improved by 61.5%. The random and fasting blood glucose levels decreased over than 30% in the group 2 and around 14% in the group 1. This study reveals that collaboration between health care professionals and community in the diabetes support group might help diabetic patients to increase their knowledge and compliance with the diabetes therapy as well as glycaemic control.Key words: Diabetes, group awareness program, pharmacist, patient counselling Kelompok Dukungan terhadap Diabetes Meningkatkan Kepatuhan dan Kontrol Kadar Glukosa Darah PasienMenyediakan informasi tidak cukup untuk meningkatkan kepatuhan pasien diabetes dan mencapai tujuan terapi. Kesadaran pasien serta dukungan emosional dan sosial dari keluarga dan masyarakat dapat memainkan peran penting untuk meningkatkan kepatuhan dan hasil klinis. Oleh karena itu, kelompok pendukung diabetes dikembangkan dan masing-masing kelompok pendukung terdiri atas dua apoteker, dua perawat, pasien diabetes dan anggota keluarga mereka. Sebanyak 70 pasien diabetes tipe 2 yang terdaftar dan acak ke dalam kelompok dukungan 1 dan kelompok dukungan 2. Pasien dalam kelompok 1 menerima selebaran informasi saja, sedangkan pasien di kelompok 2 menerima konseling dari apoteker dan informasi diabetes pada setiap pertemuan. Kesadaran Pasien diabetes dan kepatuhan dengan obat dinilai oleh kuesioner singkat pada awal dan akhir. Kadar glukosa darah dan koleste-rol juga dievaluasipada kedua kelompok. Pada akhir penelitian, kesadaran dan kepatuhan pasien secara keseluruhan meningkat 61,5%. Kadar glukosa darah acak dan puasa menunjukkan penurunan 30% pada kelompok 2 dansekitar 14 % pada kelompok 1. Penelitian ini mengungkapkan bahwa kolaborasi antara profesional perawatan kesehatan dan masyarakat dalam kelompok pendukung diabetes dapat membantu pasien diabetesuntuk meningkatkan pengetahuan dan kepatuhan mereka dengan terapi diabetes serta kontrol glikemik. Abstract

The purposes of this study were to review utilization of insulin therapy in type 2 diabetes mellitus out patients and identify its drug related problems. The data were collected cross-sectionally with purposive sampling method in the period March 2016 until May 2016 in Outpatient Clinic Universitas Airlangga Teaching Hospital Surabaya. The results of 240 patients showed that insulin was used as monotherapy insulin in 2,9% patients; combination 1 insulin & 1-4 OAD in 31,3%; basal bolus therapy 27,9%; combination basal−bolus therapy & 1-3 OAD 43,9%. Based on blood glucose target achievement, only 20,8% of patients achieve the target, 75,1% failed to achieve the target and 4,1%   suffered from hypoglycemia. Drug related problems identified adverse drug reaction of antidiabetic therapy such as hypoglycemia (6.7%), nausea (3.8%), bloating (1.3%), increase of flatulency (2.9%) and inappropriate combination (0,4%). In conclusion insulin therapy was complicated and individually, most of the patients still did not reach the target and there was potential drug related problem in this patients group. So that caring from solid inter-professional health collaboration is needed

Abstract

Tuberculosis is an acute infectious disease that primarily affects the lungs. Probiotics supplementation can increase the number and activity of NK cell in peripheral blood by modulation of IL-12, thus increasing IFN-γ production by Th1 response. Vitamin B1 acts on macrophages and affects neutrophil motility. Vitamin B6 is associated with the release of cytokines and the responsiveness of NK cells, while vitamin B12 affects to lymphocytes, Tcell proliferation, CD4+ ratios, and NK cell activity. To analyze the effects of probiotics and vitamin B1, B6, B12 supplementation on IFN-γ and IL-12 levels during intensive phase of antituberculosis treatment. The study was pre-post test randomised control by time series. The control group was TB patients with standard therapy of antituberculosis and vitamin B6, while the intervention group was TB patients receiving therapy plus once daily probiotics and vitamin B1, B6, B12supplementation during the intensive phase. Blood samples were withdrawn at baseline, one month, and two months after therapy to measure plasma IFN-γ and IL-12 levels using the ELISA method. Twenty two patients were divided equally into two groups. There was a tendency to greater increase of IFN-γ in the first month of the intervention group, followed by a significant decline after two-month therapy (p < 0.05). In both groups there was a rise in IL-12 levels after one month followed by a decrease in the second month (p > 0.05). However, the percentage was higher in the supplementation group. Adding probiotics and vitamins B1, B6, B12 could improve immune response through IL-12 and IFN-γ modulation during intensive phase therapy.

Abstract

Diabetes mellitus (DM) is a leading caused morbidity in geriatric patients. The prevalence of type-2 DM is more than 90% of DM population and increase with age, and half of those patients were geriatric. Blood glucose (BG) control is important for prevention diabetes complications, but attention must be given in geriatric patients due to the increasing susceptibility to risk of hypoglycemia. The aimed of this study was to identify BG achievement in diabetic geriatric patients and its therapeutic management. This study was done in Outpatient Geriatric Clinic, Dr. Soetomo General Hospital Surabaya Indonesia in the period of March to June, 2012. The inclusion criteria were type-2 diabetic geriatric patients with/without diabetic complication that received antidiabetic therapy and had BG data. The results from 165 patients showed that BG target achieved by 53% patients, 41% patients not achieved the target, while 6% patients in risk ofhypoglycemia. Management therapy for patients with achieved BG target was done by (1) continued therapy as before, (2) increasing dosage regimen for patients with BG already in the target but still within the upper limit target or decrease dosage regimen for patients with BG in lower limit target to avoid hypoglycemia, (3) change type of medication for patients who experienced side effects. Meanwhile, from all patients that failed to achieve BG target there were some patients received additional medications and regimen changes, but the rest of those didn’t receive any additional medication or regimen changes, which were many of them eventually became one of the drug-related problems in this patient group. In conclusion, there were still quite large number patients that did not achieve BG target, therapy management changes were made based on BG profile and there were drug related problems related to dosage regimen that needs pharmaceutical care intervention

Abstract

Reuse of dialyzers for hemodialysis can help in bringing down the cost of hemodialysis. On the other hand reuse of dialyzer may change dialyzer integrity. This study was undertaken to determine dialyzer reuse effect on Urea Reduction Ratio (URR), Kt/V urea and serum albumin. This was prospective study in Sanglah Public General Hospital Denpasar. Inclusion criteria for this study were patients who receive hemodialysis more than 3 months on twice weekly hemodialysis. In the study we used hollow fiber or dialyzer Elisio type H-130H reprocessed with Renaline automatically by machine renatron.  After each session blood urea, post dialysis weight and serum albumin were measured. Measurements was performed on the use new dialyzer, 1st reuse, 4th reuse and 7th reuse. The dialyzer was discarded, if TCV fell below 80% of baseline value. Kt/V and urea reduction ratio (URR) were calculated as measure of dialysis adequacy. A total of 23 people completed the study. There was a lack of uniformity duration of hemodialysis, so that we also performed an analysis using a uniform length hemodialysis duration (4.5 hours) with 15 samples. There were no significant difference between URR and Kt/V urea of new dialyzer and dialyzer reprocessed by renaline respectfully with p=0.131 and p=0.373. If we analyzed only using uniform time of dialysis (4.5 h) the value of URR and Kt/V urea between new and reused dialyzer not significantly different with p=0.520 and p=0.784. There was also not found significant differences between serum albumin of the use new dialyzer and reused dialyzer by the time of uniform or non-uniform, respectfully with p=0.271 and 0.073. Reuse dialyzer does not alter efficacy of hemodialysis.Key words : Dialyzer, Urea Reduction Ratio (URR), Kt/V Urea, Albumin, Total Cell Volume (TCV)

Abstract

Background: Patients with cardiovascular diseases (CVD) are vulnerable to experiencing drug-drug interactions (DDIs). DDIs are a concern among patients receiving multiple drug regimens but they are also an avoidable cause of adverse drug reactions. The study of potential DDIs (pDDIs) would make it easier for the healthcare provider to deliver better patient care and mitigate pDDIs incidence. Objective: This study aimed to assess the frequency, severity level and risk factors associated with pDDIs among medications used to treat hypertensive ambulatory patients in Universitas Airlangga hospital. Methods: A retrospective observational study was carried out from electronic prescriptions received by hypertensive patients in March 2021. Data collection includes demographic data, the profile of antihypertensive drug use, and pDDIs. pDDIs were identified by severity using Lexicomp Drug Interaction Checker (Application). Univariate logistic regression analysis was used to find associated factors of major pDDIs. A p-value less than 0.05 (≤ 0.05) was considered statistically significant. Results: From 704 patients, 53.98% women and 46.02% men, 89.06% (n = 627) patients had minor to major pDDIs; 1354 pDDIs were identified, 89.4% (n = 1,210) were moderate and 9.8% (n = 133) were major class. Multiple antihypertensive drug regimens had significance associated with the major pDDIs occurrence. Conclusion: We found a high prevalence of pDDIs among hypertensive patients. The majority of pDDIs were of moderate severity. Multiple antihypertensive drug regimens were associated factors in the presence of major pDDIs.

Abstract

EMPOWERMENT OF PATIENTS AND PATIENT FAMILIES IN PREVENTION OF AMPUTATION OF DIABETES IN MULYOREJO DISTRICT, SURABAYA CITY, EAST JAVA PROVINCE. The purpose of implementing this community service program is to empower the community, especially patients and families of diabetes patients, to be able to take steps to prevent disability in diabetes due to amputation. The number of people with diabetes who have to undergo amputation is often influenced by the poor knowledge of diabetes, the low adherence to taking medication, and the poor knowledge and ability of patients and their families in performing wound care for diabetics. In addition, the lack of understanding of diabetes drugs causes the patient to experience side effects that can lead to withdrawal or the patient to experience side effects of hypoglycemia which can put the patient in critical condition. The solution to this problem is to empower diabetes patients and their families. They were given education about diabetes, the importance of taking the medication regularly according to doctor's recommendations, and wound care. In addition, they will be trained on how to do proper wound care for diabetes patients. A medication control post-program will be implemented to ensure that patients will take their medication according to the prescribed guidelines. It is hoped that with this program, the community will understand more about diabetes and its complications to avoiding the disabilities due to amputation by diabetics and patients can avoid unwanted side effects.

Abstract

Problems: COVID-19 is a respiratory infection disease caused by SARS-CoV-2. This disease can be transmitted to humans and has spread globally. One of the complication that accompanies COVID-19 is the occurrence of coagulopathy, therefore anticoagulants are used in the treatment of COVID-19. However, studies or reports on the use of anticoagulants in COVID-19 patients have never been carried out by the Research and Development Agency of the Ministry Health Republic Indonesia. The aim of the research: To determine the profile of the use of anticoagulants in COVID-19 patients being treated at IRNA Fatmawati RSUD Dr. M Yunus Bengkulu. Research method: This study is a descriptive observational study with retrospective data collection using a time limited sampling technique, where the data is taken from the medical records of COVID-19 patients being treated at IRNA Fatmawati RSUD Dr M Yunus Bengkulu and met the inclusion criteria in the April 2020–December 2021 period. The results: In this study, 258 medical records of COVID-19 patients met the inclusion criteria, with a greater proportion of male (58.1%) than female (41.9%). The greatest use of anticoagulants was at the age of 56-65 years reaching 37.6%. Fondaparinux was the most widely used anticoagulant (51.2%) followed by heparin (36.8%) and enoxaparin (12.1%). Conclusions: Men aged 56-65 years are more at risk of infection with COVID-19 and coagulopathy complications that require the use of anticoagulants. Abstract

Background: Isoprinosine is an immunomodulator that is now being used to treat Covid-19 patients. Objectives: To evaluate Isoprinosine with Favipiravir or Oseltamivir in moderate Covid-19. Methods: In a retrospective observational analysis, in-hospital moderate Covid-19 patients treated between June 2020 and June 2021 were included. Results: Inclusion criteria for 364 patients were met, with 135 receiving Favipiravir-Isoprinosine (Group 1) and 229 receiving Oseltamivir-Isoprinosine (Group 2). In group 1, the majority of patients (58.50%) were female (35.60%), had no comorbidities (71.60%), were discharged with a positive PCR (74.80%), did not require a breathing apparatus (99.26%), had leukocyte levels between 4,5-11,0 (82.22%), lymphocyte levels between 25-33 (34.07%), and were discharged with no ground-glass opacity (34.07%) (54.10%), LOS was 9-13 days (50.37%), while the mortality rate was 0.70%. In group 2, the majority of patients were male (54.10%), with the highest age range being 42-56 years (35.80%), without comorbidities (69.0%), discharged with a positive PCR (72.50 %), and without the need for a breathing apparatus (99.13%), with leukocyte levels ranging from 4.5 – 11.0 (81.22 %), with lymphocyte levels ranging from 25.0 – 33.0 (26.20 %), and were discharged with no ground-glass opacity (49.34 %), LOS was 9 - 13 days (34.06 %), and the mortality rate was 0.87%.Conclusion: In this trial, it was determined that combining isoprinosine with antivirals favipiravir or Oseltamivir could produce significant clinical improvement.

Abstract

Background: Cardiovascular disease is one of the chronic diseases with an increasing prevalence from year to year. The results of Indonesia's basic health research (RISKESDAS 2018) showed that it was experienced by 2.9% of the population. Ensuring the continuity of drug availability is very important in pharmacy services for this patient group. The history of drug use is considered in the planning dan procurement of drug products. Objective: This study aims to analyze the drug use pattern and cost of drugs for cardiology outpatients in the National Health Insurance scheme (JKN). Methods: A retrospective observational study was conducted using the prescription of cardiology outpatients JKN for March-May 2021. The ABC method carried out the analysis of drug use patterns and cost. Results: From 2,986 prescriptions for cardiovascular disease, there were 94 types of drugs from 37 therapeutic classes. In the ABC analysis of drug use for class A, 68.82% contained seven types of drugs, class B, 20.55% contained nine types of drugs, and class C, 10.63% with 78 types of drugs. The results of ABC analysis for investment value (cost) of drugs class A 66.96% contained two types of drugs, class B 19.81% with four types of drugs, and class C 14.23% with 88 types of drugs. Conclusion: ABC analysis of the drug use pattern and cost/investment showed different patterns which had value to consider in procurement planning to maintain service continuity.
