Abstract

Hipertensi renovaskuler adalah salah penyebab paling sering timbulnya hipertensi sekunder dengan proses perkembangan penyakit yang gradual dan berhubungan erat dengan sistem renin-angiotensin-aldosteron. Hipertensi renovaskuler yang telat terdeteksi dan tertangani dengan tepat berpotensi menyebabkan hipoksia dan hilangnya fungsi ginjal. Oleh karena itu, identifikasi individu dengan penurunan perfusi ginjal adalah langkah awal yang penting. Tinjauan pustaka ini akan membahas secara menyeluruh mengenai epidemiologi, etiologi, patofisiologi, manifestasi klinis, hingga pemeriksaan penunjang dan manajemen terapi untuk hipertensi renovaskuler.

Abstract

Abstrak: Metode Identifikasi Penanda Biologis Feses Pada Kanker Kolorektal. Kanker kolorektal adalah salah satu penyebab kematian utama akibat kanker di seluruh dunia. Kanker kolorektal adalah jenis keganasan dengan tingkat kelangsungan hidup yang berkaitan erat dengan momentum penegakan diagnosis. Temuan klinis yang ditunjang dengan hasil pemeriksaan laboratorium dapat mengawali dugaan akan kanker kolorektal dan mencetus saran untuk pemeriksaan lanjutan, sehingga identifikasi penanda biologis untuk deteksi awal yang tepat menjadi penting. Tinjauan pustaka ini akan membahas mengenai berbagai metode dan tahapan penelitian untuk mengindentifikasi kandidat penanda biologis feses untuk kanker kolorektal.

Abstract

ABSTRACT The incidence of obesity worldwide is rapidly increasing, both in developed and developing countries. Obesity, particularly central obesity, is an inflammatory condition occurring within the body. The neutrophil-to-lymphocyte ratio (NLR) is a simple biomarker used to indicate the inflammatory response. This study aims to determine the profile of the neutrophil-to-lymphocyte ratio in students with obesity at the Faculty of Medicine, Universitas Wijaya Kusuma Surabaya.  This study employs a descriptive-analytic method with a cross-sectional approach. The study population comprises students from the Faculty of Medicine, Universitas Wijaya Kusuma Surabaya; 47 individuals, with 30 obese individuals and 17 individuals with a normal BMI. The results showed that the average neutrophil-to-lymphocyte ratio (NLR) for individuals with a normal BMI is 1,95. In contrast, the average NLR for obese individuals is 2,09. There is an increase in the average neutrophil-to-lymphocyte ratio (NLR) among obese students at the Faculty of Medicine, Universitas Wijaya Kusuma Surabaya. Keywords: Obesity, Neutrophil, Lymphocyte, Neutrophil-To-Lymphocyte Ratio

Abstract

Background: Breast milk contains compounds that strengthen and stimulate the immune system. Previous research has shown a correlation between exclusive breastfeeding, complete immunization, and the incidence of acute respiratory infections in children. Purpose: To analyze the relationship between exclusive breastfeeding and immunization on the incidence of acute respiratory infections in toddlers aged 12-48 months. Method: This quantitative study used a cross-sectional approach with a sample of 40 infants who met the inclusion criteria. Univariate and bivariate data were analyzed using the chi-square test. Results: The analysis showed a p-value of 0.540 (α<0.05) between exclusive breastfeeding and the incidence of ARI, and a p-value of 0.006 (α<0.05) between complete immunization and the incidence of acute respiratory infections. Conclusion: There was no statistically significant relationship between exclusive breastfeeding and the incidence of acute respiratory infections. However, a significant relationship was found between the level of complete immunization and the incidence of acute respiratory infections. Suggestion: Future research should use a larger sample size to increase the power of the study and expand the study by including other risk factors to more accurately determine their influence.   Keywords: Acute Respiratory Tract Infection; Breastfeeding; Exclusive breastfeeding; Toddlers; Vaccination.   Pendahuluan: Air susu ibu (ASI) mengandung komposisi yang memperkuat dan merangsang sistem kekebalan tubuh. Penelitian sebelumnya telah menunjukkan adanya korelasi antara pemberian ASI eksklusif, vaksinasi lengkap, dan kejadian infeksi saluran pernapasan akut (ISPA) pada anak. Tujuan: Untuk menganalisis keterkaitan ASI eksklusif dan vaksinasi terhadap kejadian infeksi saluran pernapasan akut pada balita usia 12-48 bulan. Metode: Penelitian kuantitatif dengan pendekatan cross-sectional menggunakan sampel sebanyak 40 bayi yang memenuhi kriteria inklusi. Analisis data univariat dan bivariat menggunakan uji chi-square. Hasil: Analisis menunjukan nilai p=0.540 (α<0.05) antara variabel pembeian ASI eksklusif dengan kejadian ISPA, dan nilai p=0.006 (α<0.05) antara variabel kelengkapan vaksinasi dengan kejadian ISPA. Simpulan: Tidak adanya hubungan yang signifikan secara statistik antara pemberian ASI eksklusif dengan kejadian ISPA. Namun, ditemukan hubungan yang signifikan antara tingkat kelengkapan vaksinasi dengan kejadian ISPA. Saran: Penelitian selanjutnya dapat menggunakan ukuran sampel yang lebih besar untuk meningkatkan kekuatan studi dan memperluas penelitian dengan memasukkan faktor risiko lain untuk menentukan pengaruhnya secara lebih akurat.   Kata Kunci: ASI Eksklusif; Balita; Infeksi Saluran Pernapasan Akut (ISPA); Menyusui; Vaksinasi.

Abstract

Introduction: DDX41 gene mutations can occur in hematopoietic malignancies, especially acute myeloid leukemia (AML) and myelodysplastic syndrome. The average incidence of myelodysplastic syndrome (MDS) in the general population is 4.5 out of 100,000 people per year. Myelodysplastic syndrome (MDS) can occur with different genetic mutations and will provide different prognoses and therapy results. Hypoplastic myelodysplastic syndrome (MDS) is often found in 10-15% of MDS patients. There have been no case reports or research studies in Indonesia that have reported a case of hypoplastic myelodysplastic syndrome associated with a mutation in the Dead Box Helicase 41 (DDX41) gene.Case Report: A 66-year-old man presented with complaints of weakness. The patient had a history of myelodysplastic syndrome diagnosed two years earlier. Bone marrow aspiration revealed cellularity with decreased granulopoiesis and thrombopoiesis, and myeloblasts accounted for 2.0%. Genetic testing using a Next Generation Sequencing (NGS) panel identified DDX41 p.(Pro258Leu) at 47.9%, DDX41 p.(Arg525His) at 1.7%, and ASXL1 p.(Gln977Ter) at 1.6%. The patient was treated with a combination therapy of venetoclax and azacitidine. Follow-up bone marrow aspiration showed improved cellularity, increased erythropoiesis activity, and a reduction in the number of dysplastic cells. Case Discussion: Mutations in the DDX41 gene can mostly be found in cases of hypoplastic myelodysplastic syndrome, while in normocellular or hypercellular myelodysplastic syndrome, genetic mutations such as SF3B1, TET2, STAG2, ASXL1, and BCOR are mostly found. The DDX41 gene is related to cellular molecules and innate immunity. The DDX41 gene mutation is a determining factor in the administration of chemotherapy. The DDX41 gene mutation is a determining factor in the selection of chemotherapy drugs.Conclusion: DDX41 genetic testing with NGS method is essential for determining the prognosis and appropriate therapy in the management of hypoplastic myelodysplastic syndrome cases.
