Abstract

Hipertensi renovaskuler adalah salah penyebab paling sering timbulnya hipertensi sekunder dengan proses perkembangan penyakit yang gradual dan berhubungan erat dengan sistem renin-angiotensin-aldosteron. Hipertensi renovaskuler yang telat terdeteksi dan tertangani dengan tepat berpotensi menyebabkan hipoksia dan hilangnya fungsi ginjal. Oleh karena itu, identifikasi individu dengan penurunan perfusi ginjal adalah langkah awal yang penting. Tinjauan pustaka ini akan membahas secara menyeluruh mengenai epidemiologi, etiologi, patofisiologi, manifestasi klinis, hingga pemeriksaan penunjang dan manajemen terapi untuk hipertensi renovaskuler.

Abstract

Abstrak: Metode Identifikasi Penanda Biologis Feses Pada Kanker Kolorektal. Kanker kolorektal adalah salah satu penyebab kematian utama akibat kanker di seluruh dunia. Kanker kolorektal adalah jenis keganasan dengan tingkat kelangsungan hidup yang berkaitan erat dengan momentum penegakan diagnosis. Temuan klinis yang ditunjang dengan hasil pemeriksaan laboratorium dapat mengawali dugaan akan kanker kolorektal dan mencetus saran untuk pemeriksaan lanjutan, sehingga identifikasi penanda biologis untuk deteksi awal yang tepat menjadi penting. Tinjauan pustaka ini akan membahas mengenai berbagai metode dan tahapan penelitian untuk mengindentifikasi kandidat penanda biologis feses untuk kanker kolorektal.

Abstract

ABSTRACT The incidence of obesity worldwide is rapidly increasing, both in developed and developing countries. Obesity, particularly central obesity, is an inflammatory condition occurring within the body. The neutrophil-to-lymphocyte ratio (NLR) is a simple biomarker used to indicate the inflammatory response. This study aims to determine the profile of the neutrophil-to-lymphocyte ratio in students with obesity at the Faculty of Medicine, Universitas Wijaya Kusuma Surabaya.  This study employs a descriptive-analytic method with a cross-sectional approach. The study population comprises students from the Faculty of Medicine, Universitas Wijaya Kusuma Surabaya; 47 individuals, with 30 obese individuals and 17 individuals with a normal BMI. The results showed that the average neutrophil-to-lymphocyte ratio (NLR) for individuals with a normal BMI is 1,95. In contrast, the average NLR for obese individuals is 2,09. There is an increase in the average neutrophil-to-lymphocyte ratio (NLR) among obese students at the Faculty of Medicine, Universitas Wijaya Kusuma Surabaya. Keywords: Obesity, Neutrophil, Lymphocyte, Neutrophil-To-Lymphocyte Ratio

Abstract

Background: Breast milk contains compounds that strengthen and stimulate the immune system. Previous research has shown a correlation between exclusive breastfeeding, complete immunization, and the incidence of acute respiratory infections in children. Purpose: To analyze the relationship between exclusive breastfeeding and immunization on the incidence of acute respiratory infections in toddlers aged 12-48 months. Method: This quantitative study used a cross-sectional approach with a sample of 40 infants who met the inclusion criteria. Univariate and bivariate data were analyzed using the chi-square test. Results: The analysis showed a p-value of 0.540 (α<0.05) between exclusive breastfeeding and the incidence of ARI, and a p-value of 0.006 (α<0.05) between complete immunization and the incidence of acute respiratory infections. Conclusion: There was no statistically significant relationship between exclusive breastfeeding and the incidence of acute respiratory infections. However, a significant relationship was found between the level of complete immunization and the incidence of acute respiratory infections. Suggestion: Future research should use a larger sample size to increase the power of the study and expand the study by including other risk factors to more accurately determine their influence.   Keywords: Acute Respiratory Tract Infection; Breastfeeding; Exclusive breastfeeding; Toddlers; Vaccination.   Pendahuluan: Air susu ibu (ASI) mengandung komposisi yang memperkuat dan merangsang sistem kekebalan tubuh. Penelitian sebelumnya telah menunjukkan adanya korelasi antara pemberian ASI eksklusif, vaksinasi lengkap, dan kejadian infeksi saluran pernapasan akut (ISPA) pada anak. Tujuan: Untuk menganalisis keterkaitan ASI eksklusif dan vaksinasi terhadap kejadian infeksi saluran pernapasan akut pada balita usia 12-48 bulan. Metode: Penelitian kuantitatif dengan pendekatan cross-sectional menggunakan sampel sebanyak 40 bayi yang memenuhi kriteria inklusi. Analisis data univariat dan bivariat menggunakan uji chi-square. Hasil: Analisis menunjukan nilai p=0.540 (α<0.05) antara variabel pembeian ASI eksklusif dengan kejadian ISPA, dan nilai p=0.006 (α<0.05) antara variabel kelengkapan vaksinasi dengan kejadian ISPA. Simpulan: Tidak adanya hubungan yang signifikan secara statistik antara pemberian ASI eksklusif dengan kejadian ISPA. Namun, ditemukan hubungan yang signifikan antara tingkat kelengkapan vaksinasi dengan kejadian ISPA. Saran: Penelitian selanjutnya dapat menggunakan ukuran sampel yang lebih besar untuk meningkatkan kekuatan studi dan memperluas penelitian dengan memasukkan faktor risiko lain untuk menentukan pengaruhnya secara lebih akurat.   Kata Kunci: ASI Eksklusif; Balita; Infeksi Saluran Pernapasan Akut (ISPA); Menyusui; Vaksinasi.

Abstract

Background: Typhoid fever is an infectious disease transmitted primarily through contaminated food, caused by Salmonella typhi, and is characterized by prolonged high fever and systemic involvement. Serological tests such as the Widal and Tubex tests are widely used for diagnosis. While the Tubex test is considered more sensitive, the Widal test remains prevalent due to its lower cost. Purpose: To compare the concordance between the Widal and Tubex tests and evaluate the diagnostic agreement in patients with suspected typhoid fever. Method: This observational analytical study using a cross-sectional approach was conducted at the Clinical Pathology Laboratory of the Faculty of Medicine, Syarif Hidayatullah State Islamic University, using 70 serum samples from patients at Hermina Hospital, Ciputat, selected through consecutive sampling. Widal and Tubex tests were performed on all samples. Data analysis was performed using univariate and bivariate tests, as well as the Cohen's Kappa consistency test. Results: The level of agreement between the Widal and Tubex tests was moderate, with a Kappa value of 0.519. Of the 70 subjects, 50% had negative results on both tests, 27.14% were positive on both tests, and the remaining subjects showed discordant results between the two tests. Conclusion: The concordance between the Widal and Tubex tests in diagnosing suspected typhoid fever was moderate.   Keywords: Serology Test; Tubex; Typhoid Fever; Widal.   Pendahuluan: Demam tifoid adalah penyakit infeksi yang ditularkan terutama melalui makanan terkontaminasi, disebabkan oleh Salmonella typhi, dan ditandai dengan demam tinggi berkepanjangan serta keterlibatan sistemik. Uji serologi seperti Widal dan Tubex digunakan secara luas untuk diagnosis. Meskipun uji Tubex dianggap lebih sensitif, uji Widal tetap umum digunakan karena biayanya yang rendah. Tujuan: Untuk membandingkan kesesuaian antara uji Widal dan Tubex serta mengevaluasi kesepakatan diagnostik pada pasien dengan suspek demam tifoid. Metode: Studi analitik observasional dengan pendekatan cross-sectional dilakukan di Laboratorium Patologi Klinik FK Universitas Islam Negeri Syarif Hidayatullah, menggunakan 70 sampel serum dari pasien di Rumah Sakit Hermina Ciputat, dipilih melalui consecutive sampling. Uji Widal dan Tubex dilakukan pada semua sampel. Analisis data dilakukan menggunakan uji univariat, bivariat, serta uji konsistensi Cohen's Kappa. Hasil: Tingkat kesepakatan antara uji Widal dan Tubex adalah sedang (moderate), dengan nilai Kappa 0.519. Dari 70 subjek, 50% memiliki hasil negatif pada kedua uji, 27.14% positif pada kedua uji, dan sisanya menunjukkan hasil diskordan (tidak selaras) di antara kedua uji tersebut. Simpulan: Kesesuaian (concordance) antara uji Widal dan Tubex dalam mendiagnosis suspek demam tifoid berada pada tingkat sedang.   Kata Kunci: Demam Tifoid; Tubex; Uji Serologi; Widal.

Abstract

Background: Primary Sjögren syndrome is chronic systemic autoimmune disorder characterized by lymphocytic infiltration of the exocrine glands, most commonly presenting with dryness of the mouth and eyes. This case is very rare, which the antibody in the Sjogren's syndrome case concerns platelets. Objective: This case report emphasizes the occurrence of epistaxis in Sjögren Syndrome. Case Presentation: This case describes a 30-year-old male presenting with recurrent epistaxis as the initial clinical manifestation. Further clinical evaluation revealed hypertensive crisis and dryness in the mouth and nasal mucosa. The clinical presentation raised suspicion of an underlying autoimmune condition, prompting further testing, which confirmed the presence of anti -Sjögren’s syndrome type B (SSB) antibodies on an antinuclear antibody (ANA) profile.  Result: The criteria for Sjogren's syndrome are based on the criteria if eye and mouth symptoms, eye and mouth clinical signs, and one of autoantibodies are found Anti-Ro (SSA), Anti-La (SSB), Antinuclear antibodies (ANA), Rheumatoid factor (RF). From this case has found symptoms of mouth and antibodies SSB. Conclusion: This case highlights that epistaxis can be caused by autoimmune cases, for epistaxis it is better to do screening for autoimmune examination test, emphasizing the need for further research to elucidate the diverse clinical manifestation, progression and prognosis factor.

Abstract

Background: Acute myeloid leukemia (AML) is a heterogeneous hematologic malignancy characterized by the clonal proliferation of immature myeloid precursor cells in the bone marrow and peripheral circulation. Hematologic manifestations such as anemia, thrombocytopenia, and leukocytosis are welldocumented, its association with oral involvement, particularly tonsillitis, is a rare presenting feature. Objective: This case report emphasizes membranous tonsilitis as the primary presenting feature in acute monocytic leukemia (AML-M5). The aim of writing this case study is to provide an illustration that the causes of membranous tonsillitis, beside from the infection, can also be caused by infiltration of leukemia cells. Case Presentation: We report a case of a 27-year-old female presented with fever and was found to have a fluctuating mass on the right side of the neck along with gingival swelling and hyperemic, hypertrophic tonsils with a membranous exudate. The diagnosis from bone marrow aspiration and immunophenotyping revealed as acute monocytic leukemia (AML-M5). Bacterial and fungal infections were excluded through gram staining, potassium hydroxide (KOH) examination, and throat swab culture. The patient was initially stabilized with hydroxyurea, with subsequent clinical improvement, before being referred to a tertiary center for definitive management. Result: This diagnostic workup altogether from all the clinicians leads to the diagnosis of acute myeloid leukemia (AML-M5). Conclusion: This case highlights primary tonsilitis as the primary presenting feature of acute myeloid leukemia (AML-M5), emphasizing the need for continued monitoring and regular follow-up assessment to detect potential relapses and ensure optimal disease management
