Abstract

Instalasi Gawat Darurat (IGD) memiliki peran krusial dalam memberikan pelayanan medis segera bagi pasien dalam kondisi kritis. Kepuasan pasien telah menjadi indikator utama mutu layanan kesehatan dan sangat dipengaruhi oleh waktu tunggu serta waktu tanggap petugas. Penelitian ini bertujuan untuk meninjau secara sistematis artikel penelitian asli yang mengkaji pengaruh waktu tanggap dan waktu tunggu terhadap kepuasan pasien di IGD. Pencarian literatur dilakukan melalui basis data Google Scholar, PubMed, dan SagePub dengan kata kunci terkait layanan darurat, waktu tanggap, waktu tunggu, dan kepuasan pasien. Artikel yang disertakan merupakan studi asli yang dipublikasikan sejak tahun 2015, dilakukan di lingkungan IGD, serta menggunakan desain penelitian seperti potong lintang, kohort, atau uji klinis acak. Sebanyak 12 studi memenuhi kriteria inklusi dan dianalisis lebih lanjut. Sembilan studi menunjukkan adanya hubungan signifikan antara waktu pelayanan yang lebih cepat dengan tingkat kepuasan pasien yang lebih tinggi, meskipun beberapa studi melaporkan hasil yang tidak signifikan. Perbedaan ini kemungkinan disebabkan oleh variasi metodologi dan kapasitas sistem kesehatan lokal. Simpulan dari tinjauan ini menegaskan bahwa waktu tanggap dan waktu tunggu merupakan determinan penting dalam membentuk kepuasan pasien dan perlu menjadi fokus utama dalam peningkatan mutu layanan IGD.

Abstract

Background: Hypertensive crisis poses a high risk for long-term cardiovascular complications, and management focuses on controlled blood pressure (BP) reduction rather than immediate normalization. Objective: To compare the effectiveness of sublingual and oral captopril in reducing BP among patients with hypertensive urgency. Methods: A literature review was conducted using English-language studies published after 2012 that evaluated BP reduction outcomes of sublingual versus oral captopril. Results: Three relevant studies met the inclusion criteria, comparing both administration routes in hypertensive urgency management. All studies demonstrated that sublingual and oral captopril were similarly effective in lowering systolic and diastolic BP within the first few hours after administration. Sublingual captopril consistently produced a faster onset of action, with BP reduction observed within 15–30 minutes, whereas oral administration required 30–60 minutes to achieve comparable effects. None of the studies reported significant differences in adverse effects between the two routes. These findings suggest that while overall efficacy is comparable, the sublingual route provides more rapid BP control, which may be crucial in acute care settings. Conclusion: Both sublingual and oral captopril are effective options for hypertensive urgency, with the sublingual form offering a faster therapeutic response suitable for rapid BP reduction in emergency situations.

Abstract

Stroke patients are vulnerable to malnutrition, which is associated with poor outcomes, both in the acute and rehabilitation phases. Malnutrition at admission and one week after admission is a predictor of poor outcomes within or after three months of stroke. Adequate nutritional assessment and management are necessary to improve the outcomes of stroke patients. The CONUT score can be used for nutrition assessment in stroke patients.

Abstract

Introduction: Postpartum psychiatric disorders, particularly postpartum depression, are one of the most common psychological complications affecting maternal and neonatal well-being. These conditions are often underdiagnosed and undertreated, particularly in developing countries. Identifying risk factors is crucial for prevention and early detection. Objective: This systematic review aims to identify and analyze risk factors contributing to postpartum psychiatric disorders based on scientific evidence from 2015 to 2025. Methods: A literature search was conducted through PubMed, Scopus, ScienceDirect, and Web of Science databases using the keywords “postpartum depression,” “psychiatric disorder,” and “risk factors.” Included articles were observational human studies, written in English or Indonesian, reporting risk factors and postpartum psychiatric outcomes. Of the 1,026 articles screened, nine studies met the inclusion criteria. Results: The most consistent risk factors found included a history of previous psychiatric disorders, low social support, perinatal stress, low economic status, obstetric complications, and postpartum immunological dysregulation. Eight of the nine studies showed a significant association (p < 0.05). Conclusion: Postpartum psychiatric disorders are multifactorial, influenced by biological, psychological, and social factors. Preventive efforts should focus on antenatal screening, strengthening social support, and monitoring the mother's mental state after delivery.

Abstract

Wernicke-Korsakoff syndrome is a common complication of thiamine (vitamin B1) deficiency. The prevalence of Wernicke-Korsakoff syndrome (WKS) worldwide is 0-2%, especially in the homeless, elderly, and psychiatric patients. Individuals with malnutrition due to any cause are at risk for developing WKS. The most common social factor associated with WKS is chronic alcohol abuse. Ethanol has been shown to interfere with thiamine absorption in the gastrointestinal tract. The syndrome is classically described as a clinical triad consisting of altered mental status (confusion or dementia), nystagmus (or ophthalmoplegia), and ataxia. The diagnosis of WKS is based on the history and clinical findings supplemented by laboratory and radiological examinations. Treatment of thiamine deficiency in patients suspected of having Wernicke’s is intravenous injection of thiamine 500 mg infused over 30 minutes. The drug is given three times daily for two days. Thereafter, transition to 250 mg either intravenously or intramuscularly once daily for 5 days.
