Abstract

Each year 3 million newborns die during the first seven days of life, accounting for 75% of all neonatal deaths. At least 1 million babies die during their first 24 hours of life. Three major causes of neonatal death are infections (pneumonia, diarrhea, and tetanus) 36%, preterm birth 28% and asphyxia 23%. Causes of death vary between the early and late neonatal periods, with deaths caused by preterm birth, asphyxia and congenital defects occur predominantly during the frst week of life and infection is the major cause of neonatal deaths thereafter. In Indonesia based on 2001 Household Health Survey (SKRT 2001) neonatal death caused by asphyxia was 27% which was the second cause of neonatal death after low birth weight. The objective of this study was to learn the role of midwives in handling and referring birth asphyxia in Cirebon District by using qualitative method. The result showed that many people living in villages in Cirebon district did not understand the causes of asphyxia. They thought that babies could not cry because of bad spirit or devil. The midewives were capable of handling the asphyxia babies according to their knowledges obtained from training about asphyxia management. Sometimes however, the midwives encountered some constrains in practice such as low birth weight babies (only 1,4 Kg) and oversize concave cover for resuscitation. Other limiting factors occurred when the midwives would like to refer the babies to the hospital. The families might refuse with some reasons such as, no trust in the medical staff, shortage of money or difficulty in finding transportation.  Lack of incubator in the hospital was also another obstacle.

Abstract

Pelayanan kesehatan yang bermutu dari Puskesmas dapat diukur salah satunya dengan mengetahui kepuasan pasien terhadap layanan. Bagi Puskesmas Kecamatan Tanjung Priok kepuasan pasien merupakan hal yang sangat penting, mengingat visi Puskesmas ini adalah menjadikan Puskesmas Kecamatan Tanjung Priok sebagai unit pelayan kesehatan prima, merata dan terjangkau oleh masyarakat demi mendukung pencapaian derajat kesehatan yang optimal. Tujuan penelitian ini untuk mengetahui kinerja pelayanan kesehatan terhadap masyarakat khususnya di balai pengobatan dengan mengukur tingkat kepuasaan pasien. Penelitian ini merupakan penelitian survei, untuk melihat gambaran kepuasan pasien di Puskesmas Kecamatan Tanjung Priok yang dilaksanakan pada Mei ? Juli 2009. Populasi pada penelitian adalah pasien rawat jalan di Balai pengobatan Puskesmas Kecamatan Tanjung Priok Jakarta Utara. Tingkat kepuasan pasien diukur berdasarkan metode servqual dan diagram kartesius dengan melakukan wawancara pada 63 pasien di balai pengobatan umum. Masing-masing pernyataan diberi skor dengan menggunakan skala likert 1-5. Pada penelitian ini dihasilkan sebagian besar responden merasa puas terhadap pelayanan tenaga kesehatan di balai pengobatan umum Puskesmas Tanjung Priok (71,4%). Dimensi bukti fisik (tangible) memiliki persentasi kepuasan yang tertinggi sebesar 84,12% sedangkan persentase kepuasan terendah yaitu pada dimensi jaminan (assurance) sebesar 65,07%. Kepuasan pasien terhadap pelayanan dokter lebih tinggi dari pada kepuasaan terhadap perawat 76,19% vs 65,07%. Puskesmas Tanjung Priok perlu mempertahankan dan meningkatkan prestasi yang sudah ada. Beberapa hal yang perlu menjadi bahan pertimbangan untuk lebih ditingkatkan misalnya saja kehadiran petugas yang tepat waktu, mempunyai waktu yang cukup untuk berkomunikasi dengan pasien, menanggapi keluhan dengan baik, serta memberikan informasi dengan jelas.

Abstract

The benefits of breastfeeding are well established, so that in 2001 WHO has revised global recommendation on exclusive breastfeeding that mothers should exclusively breastfeed their children until 6 months old (the old recommendation was previously 4 months). After that, to fulfill the babies need on nutrition, babies are given complementary feeding while breastfeed should be continued until 2 years. Six months exclusive breastfeeding will reduce infant morbidity and mortality in developing countries. In Indonesia, although exclusive breastfeeding for 6 month has been implemented by Ministry of Health, it is only 1% of the mothers exclusively breastfeed their babies for 6 months. In Vietnam, children who were exclusively breastfed for less than 3 months got diarrhoea earlier and the prevalence of diarrhoea was greater than babies who were breastfed for more than 3 months. Complementary feeding should be given when babies are 6 months old. In Indonesia, however complementary feeding have already been given when the babies are just 7 days old or less. The babies got formula, honey, water, sweet water, fruits, etc. Proportion of babies aged less than 1 month receiving commercial complementary feeding was 4 ? 5 %, increasing with the increase of age. Early complementary feeding will affect the baby's health.
