Abstract

Latar Belakang: leukemia mieloblastik akut merupakan salah satu keganasan darah yang terjadi akibat gangguan proses pertumbuhan sel mieloid, yang menyebabkan terjadinya penumpukan sel blast di sumsum tulang, dan menekan pembentukan sel darah lain yang menyebabkan anemia. Pengobatan yang diberikan pada penderita LMA adalah kemoterapi. Kondisi anemia dapat menyebabkan hipoksia di jaringan, yang dapat mengakibatkan berkurang nya efektivitas kemoterapi. Objektif: Tujuan dari penelitian ini adalah untuk menganalisis hubungan antara derajat anemia dengan tercapainya remisi komplit pada pasien LMA yang di kemoterapi induksi. Metode: Penelitian ini dilakukan dengan menggunakan desain cross sectional dengan pendekatan observasional analitik retrospektif. Sampel pada penelitian ini berjumlah 15 sampel yang diambil dari data rekam medik. Analisis univariate dilakukan untuk mendeskripsi frekuensi variable dan analisis bivariate dengan uji statistic fisher exact test. Hasil: Hasil yang didapat dari penelitian ini adalah pasien LMA banyak yang berusia >40 tahun, berjenis kelamin perempuan, dengan klasifikasi LMA M4, keluhan utama lemah letih, derajat anemia ringan dan berhasil remisi komplit yang lebih banyak. Hasil uji statistik memperlihatkan tidak adanya hubungan derajat anemia dengan tercapainya remisi komplit pada pasien. Hasil analisi LMA dengan nilai p=1,000. Kesimpulan: kesimpulan yang didapatkan adalah tidak adanya hubungan antara derajat anemia dengan tercapainya remisi komplit pada pasien leukemia mieloblastik akut dewasa yang menjalani kemoterapi induksi di RSUP Dr. M Djamil Padang.

Abstract

Latar belakang. Probiotik adalah mikroorganisme yang menguntungkan dalam pengobatan diare akut pada anak. Probiotik mengurangi frekuensi dan durasi diare dengan meningkatkan respon imun, produksi substansi antimikroba, menurunkan proses inflamasi, dan menghambat pertumbuhan kuman patogen penyebab diare. Pengukuran calprotectin feses sebagai penanda penyakit inflamasi pada diare akut merupakan metode noninvasif, cepat dan mudah.Tujuan. Penelitian ini bertujuan untuk mengetahui pengaruh probiotik terhadap kadar calprotectin feses dan durasi diare akut pada anak.Metode. Penelitian eksperimental dengan pendekatan pretest-posttest control group yang dilaksanakan di Puskesmas dan Rumah Sakit di kota Padang. Penelitian dimulai dari bulan Januari sampai Juni 2023. Populasi penelitian adalah pasien anak usia 2-60 bulan dengan diare akut yang memenuhi kriteria inklusi dan ekslusi. Terdapat 31 sampel kelompok kontrol yang mendapatkan terapi standar WHO dan 30 sampel kelompok kasus yang mendapatkan terapi standar WHO ditambah probiotik. Dilakukan pengamatan terhadap durasi diare akut dan kadar calprotectin feses.Hasil. Anak dengan usia >24 bulan lebih banyak pada kedua kelompok dengan sebagian besar kelompok dengan gizi baik. Rerata berat badan dan tinggi badan subjek pada kelompok kasus adalah 10,76 kg dan 82,6 cm, sedangkan kelompok kontrol adalah 10,15 kg dan 81 cm. Terdapat pemendekan durasi diare yang signifikan pada kelompok kasus dibandingkan kelompok kontrol adalah 10,32 (+6,35)jam (p-value =0,049). Terdapat perbedaan kadar calprotectin feses yang signifikan antara sebelum dan sesudah pemberian probiotik pada kelompok kasus (p-value =0,038).Kesimpulan. Pemberian probiotik dapat memperpendek rerata durasi diare akut dan menurunkan rerata kadar calprotectin feses secara bermakna. Pemberian probiotik ini dapat disarankan sebagai terapi adjuvan dalam tata laksana diare akut pada anak.

Abstract

Latar belakang. Kemoterapi merupakan pengobatan utama pada anak leukemia limfoblastik akut (LLA). Kemoterapi fase induksi adalah kemoterapi pertama yang membunuh 95-98% sel leukemik. Pemberian kemoterapi menyebabkan kerusakan hati ditandai dengan peningkatan kadar enzim transaminase dan dapat disertai peningkatan kadar bilirubin. Faktor yang mempengaruhi pengobatan LLA, di antaranya usia dan status gizi.Tujuan. Melihat gambaran kadar enzim transaminase dan bilirubin pada anak LLA berdasarkan usia dan status gizi pada kemoterapi fase induksi.Metode. Penelitian deskriptif retrospektif dengan pendekatan cross-sectional. Pengambilan sampel menggunakan teknik total sampling pasien anak LLA di Rumah Sakit Umum Pusat Dr. M. Djamil, Padang, periode September 2022 sampai Agustus 2023. Data demografis, SGOT, SGPT, dan bilirubin total pasien didapatkan melalui rekam medis.Hasil. Penelitian ini mendapatkan sebanyak 49 sampel, dengan mayoritas responden adalah perempuan (53,1%), usia <10 tahun (65,3%) dan status gizi normal (77,6%). Cenderung terjadi peningkatan ringan SGOT (65,3%) dan SGPT (49%) serta kadar normal bilirubin (49%) pada kemoterapi fase induksi. Kadar SGOT dan SGPT cenderung mengalami peningkatan ringan pada semua kelompok status gizi. Pada 4 anak overweight ditemukan peningkatan berat SGOT (25%) dan SGPT (50%). Kadar bilirubin cenderung normal pada setiap status gizi, tetapi meningkat sedang pada overweight (75%). Peningkatan kadar SGOT dan SGPT cenderung ringan pada setiap kelompok usia. Bilirubin meningkat sedang pada anak ?10 tahun (53%).Kesimpulan. Pada umumnya SGOT dan SGPT mengalami peningkatan kadar ringan serta bilirubin dalam kadar normal pada anak LLA selama kemoterapi fase induksi. Peningkatan berat SGOT,SGPT dan bilirubin ditemukan pada overweight dan usia ?10 tahun.

Abstract

Urinary Tract Infection (UTI) is the most common bacterial infection in children which is characterized by an increase in bacteria in the urine. The purpose of this study was to determine the clinical and bacteriological profiles of urinary tract infection patients in children at Dr. M. Djamil Padang in 2017-2021. This type of research is descriptive with a total sample method of 52 children with a retrospective approach on patient medical record data and urine culture results. The variables studied included age, gender, nutritional status, urinary tract abnormalities, clinical symptoms, physical examination, supporting examinations, and types of bacteria in pediatric urinary tract infection patients. The results of this study showed that most cases of UTI occurred in children aged 0-1 years (59,2%) with more male cases than females, good nutritional status (56,8%), with the most congenital abnormality in males is phimosis (37,9%). Based on physical examination, it was found that most of the patients did not have a fever (50%). The results of investigations showed that the patient had leukocyturia (61,5%) on urinalysis, leukocytosis (61,5%) on blood scan, cystitis (47.8%) on ultrasound, and nephrolithiasis (33%) on CT scan. Bacterial culture results show that the most common cause of UTIs in boys is Klebsiella pneumoniae (37.9%) and in girls Escherichia coli (87.0%). Based on this study, most results were obtained in the form of uncomplicated UTI. Gram-negative bacteria are the most common type of bacteria that cause UTIs.

Abstract

Background: COVID-19 clinical signs can be different for each pediatric patient. This study aims to systematically evaluate the clinical, laboratory examination, severity of COVID-19 infection of pediatric patients in RSUP Dr. M. Djamil General Hospital. Methods: This is a cohort retrospective study. Data were extracted from the medical records of pediatric patient who received COVID-19 medical treatment between September 2020 to June 2021. The data were presented as frequency, percentage and adjusted odd ratio (aOR). Bivariate and multivariate analysis was done to identify the association. Results: The results showed that from a total sample of 93 patients, the majority of these patients were male (60,2%) and fell within the age groups of 1-5 years (30,1%) and 11-15 years (31,2%). Most pediatric patients presented with a mild severity level of COVID-19. Clinical characteristics, such as consciousness (p=0.014), neurologic deficits (p=0.035), and thoracic abnormalities (p=0.040), showed a significant association with the severity level of COVID-19. There are no significant association between laboratory results and the severity level of COVID-19. Admission to PICU was identified as a protective factor against the mortality of COVID-19 (aOR=0.02) and abdominal abnormalities were identified as a contributing factor to mortality among pediatric COVID-19 cases (aOR=14.44). Conclusions: Clinical characteristics including consciousness, neurologic deficits, and thoracic abnormalities were associated with the severity level of COVID-19. PICU admission and abdominal abnormalities were associated with mortality among pediatric COVID-19 patients.

Abstract

Dengue hemorrhagic fever (DHF) is one of the highest causes of death due to infection in Indonesia. The death rate will increase if dengue infection develops into dengue shock syndrome (DSS). Children tend to experience DHF and even DSS because their capillary permeability is higher than adults. Being overweight is a risk factor that increases a person's chances of experiencing DSS due to a strong immune response, which causes increased production of pro-inflammatory cytokines. This research aims to determine the relationship between overweight and the incidence of DSS in children. This research was an observational analytical study with the cross-sectional design. Sampling was done by a total sampling technique in pediatric patients treated for dengue hemorrhagic fever at Dr. M. Djamil Padang Hospital for the period 1 January 2018 – 31 December 2021. The data was obtained from the medical records of Dr. M. Djamil Padang Hospital between November 2023 to January 2024 with a total of 179 sample of DHF children aged 0 – 18 years. Chi-square test was used as statistical test. The results of this research were that DSS cases occurred more frequently in the 5 - 10 year age group (33,5%) with female gender (33%). Most of the sampel had good nutritional status, the most common degree of infection was grade III, duration of fever for 4 – 6 days, and good outcome. Bivariate analysis with the chi-square test obtained a p value of 0.007 (p<0.05). This research is concluded that there is a statistically significant relationship between overweight and the incidence of DSS in children treated for DHF at Dr. M. Djamil Padang Hospital.

Long-term antiseizure-medication (ASM) therapy can accelerate vitamin D catabolism via hepatic cytochrome P450 induction, predisposing children with epilepsy to hypovitaminosis D and its skeletal consequences; Indonesian tertiary-centre data remain scarce. Methods. This cross-sectional study examined the association between ASM class, number and duration and serum 25-hydroxyvitamin D [25(OH)D] in children aged 1–18 years at Dr. M. Djamil General Hospital, Padang, West Sumatera, between April and October 2025. Of 82 records screened, 77 were eligible; 25(OH)D was measured by enzyme-linked fluorescent assay, with hypovitaminosis D defined as <30 ng/mL. Associations were tested with Fisher–Freeman–Halton exact and chi-square tests, odds ratios, ANOVA, multivariable logistic regression and ROC analysis. Results. Hypovitaminosis D affected 48 children (62.3%; 95% CI 51.2–72.3), with mean 25(OH)D of 18.3±6.7 versus 41.6±11.2 ng/mL in deficient versus replete children. ASM class was significantly associated with vitamin D status (exact p=0.037; Cramér's V=0.283): all nine enzyme-inducing users were deficient, versus 56.0% non-enzyme-inducing and 58.1% combination (ANOVA p=0.045, η²=0.080). Neither ASM number (p=0.642) nor duration (p=0.348) was associated. Enzyme-inducing exposure carried the largest adjusted odds (adjusted OR 5.66, 95% CI 0.62–52.06), and the model discriminated moderately (AUC 0.685). Conclusion. Hypovitaminosis D is prevalent in Indonesian children with epilepsy and is most strongly linked to enzyme-inducing ASMs, supporting early routine 25(OH)D monitoring and supplementation from treatment initiation.

Abstract

The increased of Maternal Mortality Rate (MMR) can be lowered through by various efforts, one of them with quality Antenatal Care (ANC). Quality of ANC services can be given by several health workers who work together in giving services to patients through the Interprofessional Collaboration (IPC). IPC or interprofessional collaboration can improve health service quality. The purpose of this study is to find out an overview of the level of perception of health workers about IPC on ANC services in Lubuk Buaya Health Center. This study is descriptive research using crosssectional design. The study sample is 30 health workers who are determined by the total sampling method. Data collection use the Collaborative Practice Assessment Tools (CPAT) questionnaire. The results of the study can be found that all health workers (100%) have a high perception of IPC in ANC services with components of connection among members, coordination and division of roles, and missions, goals, and objectives have the highest percentage (100%) and decision-making and conflict management components have the lowest percentage (10%). All health professions consist of GPs, midwives, nurses, dentists, pharmacists, nutritionists, and ATLM have a high perception (100%) of IPC on ANC services. The dentist professions have a high perception of all eight components, meanwhile other professions have a high perception of the seven components of perception and have a moderate perception in one component of perception, that is the components of decision making and conflict management. In general, health workers have a high perception of IPC in ANC services, but there are still a moderate perception in one components of perception so efforts are needed to improve IPC competence in that component.
