Abstract

The dengue virus (DENV) is a blood-borne Flavivirus transmitted through vectors and ranks as the fastest-spreading tropical disease globally. Dengue infection could exhibit mild to severe symptoms depending on various factors. Antibody-dependent enhancement (ADE) is a cross-reactive phenomenon between different DENV strains that clinicians should be aware of, as it leads to a higher incidence of dengue with warning signs and severe dengue. On the molecular level, many genes contribute to the severity and pathophysiology of ADE. Mutations and polymorphisms in these genes could alter the course of dengue infection. This article aims to comprehensively review studies that measure the impact of dengue-related genetic mutations and polymorphisms on the severity, mortality, and ADE of DENV infection. We performed a literature review to identify relevant articles on PubMed, Science Direct, Europe PMC, and Google Scholar. The review examines polymorphisms across multiple gene families: Fcγ receptors, cytokines (TNF-α, TGF-β1, IL-10), immune regulatory genes (MICB, PLCE1, RIPK2), antigen-presenting machinery (DC-SIGN, TAP), and host defense mechanisms (VDR, CTLA-4, MBL, HPA). These genetic variations have population-specific effects and may confer both protection in some populations and increased susceptibility to severe outcomes of dengue in others. The effects of these polymorphisms are context-dependent and differ according to the phase of infection, ethnicity, and gene-gene interactions. Overall, it could be concluded that aside from external factors, polymorphisms on human genes regulating mechanisms and components related to dengue infection have been extensively discussed to play a role in the overall clinical outcome.

Abstract

Streptococcus suis (S. suis) is a significant zoonotic pathogen that can cause severe infections in humans, primarily manifesting as meningitis, septicemia, and endocarditis. This review focuses on the pathophysiology of S. suis meningitis, emphasizing the bacterium's mechanisms for crossing the blood-brain barrier (BBB) and eliciting a robust inflammatory response. The review highlights the virulence factors, including collagenase-like protease and capsular polysaccharides, that facilitate the pathogen's evasion of the immune system and contribute to the severity of the disease. Notably, the prevalence of S. suis infections remains a critical public health concern, particularly in regions with high pig consumption, where occupational exposure increases the risk of human disease. The manuscript also evaluates current diagnostic techniques, comparing traditional methods, such as cerebrospinal fluid culture, with advanced molecular diagnostics, including polymerase chain reaction (PCR) and next-generation sequencing (NGS). These newer methods demonstrate enhanced sensitivity and specificity, crucial for timely and accurate diagnosis. Furthermore, emerging innovations in diagnostic technology, including point-of-care testing and biosensors, hold promise for rapid identification of S. suis, potentially mitigating morbidity associated with the disease. This comprehensive review highlights the urgent need for enhanced diagnostic capabilities and public health strategies to reduce the burden of S. suis meningitis, promoting interdisciplinary collaboration to improve disease prevention and management.

Abstract

Background: Stroke is the leading cause of disability and the third leading cause of death worldwide. This study aims to investigate hypertension as a risk factor for non-hemorrhagic stroke (NHS). Methods: This study was designed as a retrospective, observational, analytic case-control study involving a total of 112 participants. Data analysis utilised the chi-square test, with a significance level set at p < 0.05 and a 95% confidence interval (CI). Results: The findings revealed that NHS patients were predominantly in the 45-60 year age group (44.6%), primarily male (69.6%), and mostly of Balinese ethnicity (94.6%). A significant number of NHS patients were diagnosed with hypertension (53.6%). The most frequently reported clinical symptoms were weakness or paralysis in the extremities (94.6%). An improvement in outcomes was observed in 80.4% of patients, with a median length of hospital stay of 4 days. Laboratory data were normal, and CT scans identified hypodense lesions in 60.7% of cases. The results of the chi-square statistical analysis indicated an odds ratio (OR) of 2.65, with a 95% CI of 1.22–5.74 and p = 0.013. In conclusion, this study suggests that hypertension increases the risk of non-hemorrhagic stroke by 2.65 times compared to individuals without hypertension.

Abstract

Pengabdian ini bertujuan untuk meningkatkan pengetahuan masyarakat tentang neurosistiserkosis, memberdayakan masyarakat dengan keterampilan praktis melalui praktek kebersihan dalam memasak daging babi, membuat lawar dan memelihara babi. Mitra program PKM ini adalah ibu-ibu PKK desa Blahbatuh, Gianyar, Bali sejumlah 15 orang. Berdasarkan wawancara dengan kelian dinas banjar, teridentifikasi beberapa masalah utama yang dihadapi oleh mitra yaitu rendahnya pemahaman higiene peramu makanan, belum mengetahui cara pengolahan lawar yang benar dan aman, dan teknik pemeliharaan babi dan sapi yang kurang memadai. Metode sosialisasi yang digunakan melibatkan berbagai pendekatan, termasuk ceramah dan sesi demonstrasi langsung. Hal ini bertujuan untuk memastikan bahwa informasi yang disampaikan tidak hanya dipahami, tetapi juga dapat diterapkan dengan baik. Kegiatan dilaksanakan satu hari pada 10 Desember 2024 yang  diawali dengan pre-test, dilanjutkan sosialisasi tentang pengolahan daging babi yang benar untuk mencegah neurosistiserkosis dengan memberikan uraian menggunakan poster dan booklet, kemudian demonstrasi pengolahan lawar merah bebas bakteri,  dan  diakhiri dengan evaluasi hasil melalui pelaksanaan post-test. Kegiatan  ini  menunjukan dampak  positif  dan  memperoleh  apresiasi dari  masyarakat  dan perangkat   desa. Hal ini terlihat dari persentase kehadiran 100% dan partisipasi aktif 100%, nilai pretest dan posttest dengan peningkatan >40% dan peningkatan keterampilan mitra mengenai pengolahan daging babi yang benar.

Abstract

Aging process represents the natural process which is inevitable. It is caused by a biological factor that goes naturally resulting in the anatomical, biochemical and physiological changes. The natural changes contribute to falling in elderly. The objective of this study was to assess the falling prediction in elderly by using Functional Gait Assessment. This study used a cross-sectional research design with falling prediction as the variable. Forty-three elderly in Panti Werdha Wana Seraya (aged 60-103 years, - mean of 77.48 ± 1.61), selected according to the inclusion criteria, were included in the study. The data were collected using Functional Gait Assessment and analyzed both descriptively and statistically. The results showed that the elderly had a high risk of falls. Abstract

Though COVID-19 caught healthcare authorities worldwide off guard, several countries successfully dampened the morbidity and mortality curve by imposing strict biosecurity protocols. We would like to observe the effect of healthcare policies enforced in correlation to the formation of new clusters during early phases of the pandemic, thereby providing statistical justification for laws enforced by authorities. With access to medical records from Udayana University Hospital, a tertiary COVID-19 referral health center, we then observe epidemiological data of patients admitted and their admission date in correlation to dates where major changes in national COVID-19 protocols were established. We found a double distinctive curve that is mainly made of two distinct demographic groups: migrant workers and private employees. We found that during March 2020, when strict travel protocols are yet to be imposed, the viral introduction was mostly carried by returning migrant workers. Sporadic cases emerged, though soon vanishing due to the enforcement of quarantine protocols. Subsequently, in June 2020, a work-from-office setting was adopted, where some offices perform at a capacity higher than allowed. This incites the formation of new office-linked clusters. After this period, no recognizable pattern found due to massive local transmission affecting all layers of society. However, it can be concluded that COVID-19 law does shape the curve, and strict protocols may end the pandemic sooner. Keywords: COVID-19, Migrant Workers, Health Policy, Private Employee

Abstract

Abstrak Sectio Caesarea (SC) merupakan tindakan pembedahan untuk melahirkan janin dengan cara membuat sayatan untuk membuka dinding perut dan dinding uterus atau suatu histerotomi untuk mengeluarkan janin yang berada di dalam rahim ibu. Angka persalinan SC di Indonesia terus mengalami peningkatan pada tahun 2018. Persalinan dengan metode SC menunjukan angka 17,6 % dari seluruh jumlah persalinan di Indonesia, angka ini melebihi standar WHO yaitu 15 %. Menurut data tahun 2018, Bali menempati posisi kedua terbesar di Indonesia untuk persalinan melalui metode SC dengan persentase 30.2%. Jumlah ini meningkat hampir dua kali lipat dibandingkan data 2013 sebanyak 17.3%. Tujuan dari penelitian ini untuk mengetahui proporsi indikasi tindakan SC di RSUD Sanjiwani Gianyar Tahun 2017-2019. Proporsi ini mencakup gambaran responden yang melakukan tindakan SC berdasarkan indikasi SC, usia, pendidikan, dan pekerjaan. Penelitian ini menggunakan desain deskriptif dengan pendekatan cross-sectional. Hasil penelitian ini didapatkan usia responden terbanyak pada rentang usia 30-34 tahun sebanyak 65 (26,0%) orang, pendidikan responden bersalin dengan metode SC terbanyak adalah SMA sebanyak 123 (49,2) orang, pekerjaan terbanyak responden bersalin dengan metode SC adalah tidak bekerja sebanyak 124 (49,6%) orang, dan proporsi tertinggi indikasi SC yaitu persalinan SC berulang sebanyak 79 (31,6%) orang. Saran diharapkan untuk penelitian selanjutnya agar dapat memuat lebih banyak indikasi SC khususnya yang dilakukan di RSUD Sanjiwani Gianyar. Kata kunci: Sectio Caesarea, Indikasi, RSUD Sanjiwani,Bali Abstract [Indications for Sectio Caesarea at the Sanjiwani Hospital, Gianyar in 2017-2019] Sectio Caesarea (SC) is a surgical procedure to give birth to a fetus by making an incision to open the abdominal wall and uterine wall or a hysterotomy to remove the fetus in the mother's womb. The SC delivery rate in Indonesia continues to increase in 2018, with the SC method showing the rate of SC in 2018 was 17.6% of the total number of deliveries in Indonesia, this figure exceeds the WHO standard of 15%. The data from 2018 showed that Bali occupies the second largest position in Indonesia with deliveries through the SC method with a percentage of 30.2%, this number has almost doubled compared to data from 2013, which was 17.3%. The purpose of this study was to determine the proportion of indications for SC at Sanjiwani Hospital, Gianyar in 2017-2019. This proportion includes the description of respondents who perform SC based on SC indications, age, education level, and occupation. This study was a descriptive design with a cross-sectional approach. This research sample must meet the inclusion criteria of maternity women using the SC method recorded in the medical records at Sanjiwani Hospital in 2019 and the exclusion criteria of incomplete medical records data. The results of this study found that the most respondent were in the age range of 30-34 years old as many as 65 (26.0%) people, the level education of respondents who gave birth using the SC method was high school graduates as many as 123 (49.2%) people, the occupation of most of the respondents who gave birth with the SC method were unemployed as many as 124 (49.6%) people, and the highest proportion of CS indications, namely repeated CS deliveries as many as 79 (31.6%) people. Suggestions are expected for further research so that it can contain more SC indications, especially those carried out at the Sanjiwani Hospital, Gianyar. Abstract

The working activities of carrying load workers in Komodo whole sale shop in Denpasar are considered traditional work posture with repetitive movement and some time do poor work posture. The carrying posture triggers the complaint of Musculosceletal disorders ( MSDs). This study aims to distinguish the correlation between work posture and MSDs. The method of the study was analyzed observational which cross sectional approach. The work posture was measured using REBA (Rapid Entire Body Asessment) and MSDs were measured using Nordic Body Map with Likert Scale. The samples were carried out on 41 workers and the data were analyzed using SPSS 22. The results were showed as follows: there was a significant relationship between work posture and MSDs (r=0.85) The conclusion could be drawn that the relationship between work posture and MSDs was significantly. It could be recommended that the work posture should be ergonomic approach.

Abstract

Stroke is a neurological disorder that attacks central nervous system. Around 80-90% stroke patients suffer from ischemic stroke. Ischemic stroke is known to have higher potential of causing its sufferer disability. The decline of cognitive function is a condition that is caused by ischemic stroke. If someone experiences decline of cognitive function, it is hard for the person to live independently, reduces productivity, and lowers quality of life. The purpose of this research was to examine the correlation between the cognitive decline and ischemic stroke at Mangusada Regional General Hospital, Badung. This research was an analytical-observational study with cross-sectional design. The research subject sampling technique was consecutive sampling. The research subjects comprised of 70 people with 1:1 ratio, they are 35 ischemic stroke patients and 35 non-ischemic stroke patients who visited the neuro polyclinic at Mangusada Regional General Hospital in December 2021 until February 2022. The data sampling was performed using MoCA-Ina questionnaire. The data were processed and analyzed using contingency coefficient correlation test. The test obtained p value = 0,001 which implied 95% confidence level, it showed that there was significant correlation, and the correlation level is categorized low because the correlation coefficient was 0,378. The conclusion of this research is that there is significant correlation, and the correlation level is categorized low between the cognitive decline and ischemic stroke at Mangusada Regional General Hospital.

Abstract

Epilepsy is an abnormal condition in the brain characterized by the continuous emergence of predisposing factors in the occurrence of an epileptic seizure, which is also characterized by the presence of neurobiological, cognitive, psychological, and social consequences of the condition. Epileptic seizures are signs and/or symptoms that appear briefly due to abnormal and synchronous neuronal activity in the brain. This study aims to determine the correlation between the frequency of epileptic seizures with the quality of life in epileptic patients at the Mangusada Hospital. This research is an analytic observational study with a cross sectional research design and uses a consecutive sampling technique. The subjects of this study were 52 epileptic patients who came to the neurosurgery polyclinic at RSUD Mangusada in the period of December 2021 to February 2022. The data were collected by questionnaires, collecting secondary data from patient medical records, and interviews. This research was conducted for 3 months, from December 2021 to Febuary 2022. Data were analyzed by Spearman test. The level of significance in this study obtained results (p) <0.001 which means it has a significant correlation between frequency of epileptic seizures with quality of life. The value (r) in this study is -0.705, the correlation is strong with a negative direction, meaning that the more frequent epileptic seizures occur, the lower the quality of life in epileptic patiens.
