Abstract

Sindrom koroner akut (SKA) merupakan salah satu penyebab utama kematian kardiovaskular dengan manifestasi khas berupa nyeri dada yang disertai kecemasan. Penatalaksanaan gejala tidak hanya mengandalkan terapi farmakologis, tetapi juga memerlukan pendekatan nonfarmakologis yang aman dan mudah diterapkan. Salah satu intervensi yang mulai banyak diteliti adalah foot massage, teknik pijatan refleksi kaki yang diyakini mampu meningkatkan sirkulasi darah, merangsang pelepasan endorfin, dan menurunkan aktivitas sistem saraf simpatis. Penelitian quasi-eksperimen dengan rancangan pre–posttest control group melibatkan 57 pasien SKA yang dirawat di ICCU RS PKU Muhammadiyah Gamping. Intervensi pijat kaki diberikan empat kali selama dua hari dengan durasi 20–30 menit setiap sesi. Intensitas nyeri diukur menggunakan Numeric Rating Scale, sedangkan tingkat kecemasan dinilai melalui Zung Self-Rating Anxiety Scale. Analisis data menggunakan uji Wilcoxon, Kruskal–Wallis, dan post-hoc dengan tingkat signifikansi p<0,05. Hasil menunjukkan penurunan bermakna pada skor nyeri dan kecemasan (p=0,000), menegaskan efektivitas pijat kaki sebagai intervensi keperawatan komplementer. Temuan ini mendukung penerapan pijat kaki sebagai tindakan keperawatan mandiri di ICCU maupun IGD untuk meningkatkan kenyamanan fisik dan psikologis pasien SKA.

Abstract

Interprofessional collaboration (IPC) is a strategy to address the need for integrated community mental health services. In community mental health services, effective interprofessional collaboration practices can increase community participation in carrying out early detection and prevention of mental health problems. In order to create effective IPC practices, it is necessary to identify factors that hinder interprofessional collaboration in community mental health programs. The purpose of this study was to analyze the level of Interprofessional Collaboration (IPC) of community mental health programs in Jember Regency. Methods: This study is a quantitative and qualitative research (mix-method). The level of interprofessional collaboration was measured using the PINCOM (The Perception of Interprofessional Collaboration Model) questionnaire consisting of 32 questions, which included individual factors and team factors. Questionnaire scores were calculated using a 5-point Likert scale. And further descriptive analysis using logistic regression. Then individual factors and team factors were further assessed using the in-depth interview method. The results of quantitative research show that the characteristics of respondents that affect the implementation of interprofessional collaboration are age, education, position, while employment status and length of service have no impact on the implementation of interprofessional collaboration in community mental health programs. The conclusion of this study is that the level of interprofessional collaboration in community mental health programs is running well, government policies are needed so that the handling of community mental health programs can be more comprehensive.

Abstract

Self-care plays a critical role in the prevention and management of hypertension. This study aimed to evaluate the impact of Orem’s Self-Care Theory on blood pressure reduction through resistance band exercise and almond chocolate consumption in hypertension patients. A quasi-experimental pretest-posttest control group design was used. The study involved 76 respondents with hypertension grade 1, divided into intervention and control groups. The intervention group participated in resistance band exercises three times a week for 28 days and consumed almond chocolate, while the control group received standard antihypertensive treatment. Data analysis was conducted using the Wilcoxon and Mann-Whitney tests with α < 0.05. The results showed a significant reduction in both systolic (p = 0.000) and diastolic (p = 0.002) blood pressure in the intervention group. Resistance band exercises enhanced vascular endothelial function, while almond chocolate stimulated nitric oxide synthesis, leading to vasodilation. These intervention aligned with Orem’s Self-Care Theory, highlighting the importance of individual commitment to health. The innovative aspect of this study lies in its integrated approach, as it is among the first to combine Orem’s Self-Care Theory with both physical (resistance band exercise) and nutritional (almond chocolate consumption) interventions for hypertensive patients. The study concludes that applying Orem’s Self-Care Theory through exercise and dietary changes effectively reduces blood pressure in hypertensive patients. It is recommended that health practitioners incorporate these self-care practices alongside the Patient-Centered Care (PCC) concept to address individual needs. This integrated approach can enhance hypertension management, improve blood pressure control, and boost patient satisfaction, ultimately leading to better health outcomes.

Abstract

Background: Hypertension is a common non-communicable disease among the elderly and can lead to various complications. One of the frequent consequences is impaired body balance, which increases the risk of falls and reduces quality of life. Age-related physiological changes, such as decreased muscle strength and reduced vascular elasticity, contribute to balance instability. The combination of balance exercise and tandem walking exercise may be an effective intervention to improve balance in elderly individuals with hypertension. This study aimed to determine the effectiveness of a combination of balance exercise and tandem walking exercise in improving body balance among elderly patients with hypertension. This pre-experimental study used a one-group pre-test and post-test design and was conducted in the Bandongan Primary Health Care Center area, Magelang, from January to June 2025. The population consisted of 527 elderly individuals with hypertension aged ≥ 60 years, and 45 participants were selected using purposive sampling. Body balance was assessed using the Timed Up and Go Test (TUGT) before and after the intervention. Data were analyzed using the Shapiro–Wilk normality test and the Wilcoxon test. The Wilcoxon test showed a p-value of 0.000 (p < 0.05), indicating a significant improvement in body balance after the intervention. The combination of balance exercise and tandem walking exercise is effective in improving body balance among elderly patients with hypertension.
