Abstract

Hypertension is the most common disease suffered by Sungai Rangas Ulu Village people. The majority of people who are living in the village are fishermen. The fish obtained are preserved in salted fish and consumed by them. The patient in this case report is a smoker and one of the residents who often consume salted fish. Local culture is one of the factors that cause hypertension. One of hypertension management is complementary therapy such as isometric exercise and yoga breathing. This study described the results of nursing care on hypertension patients through isometric exercise interventions and yoga breathing. This study used a case study method on one of the hypertension sufferers. A client in Sungai Rangas Ulu Village was chosen randomly or accidentally for the case report. A client was given isometric exercise and yoga breathing intervention and observed their blood pressure using a sphygmomanometer every day for six days. The assessment results on the client can establish a nursing diagnosis: risk of the ineffectiveness of peripheral tissue perfusion associated with hypertension. The client was given an isometric exercise and yoga breathing intervention to achieve the outcome. The intervention result was decreased blood pressure (before = 180/120 mmHg, and after = 153/100 mmHg). Isometric exercise and yoga breathing can make the client more relaxed and increase blood circulation, decreasing blood pressure. There is an effect when given isometric exercise intervention and yoga breathing on reducing blood pressure in a client with hypertension.

Abstract

Background: Intradialytic hypertension is one of the problems experienced by patient with chronic kidney failure who undergoing hemodialysis with a prevalence of 13,2% to 33,9%. This kind of hypertension is one of the biggest causes of death in patient who undergoing dialysis, with a rate of 59%. Several factor can affect intradialytic hypertension, such as interdialytic weight gain, age, gender, and the duration of hemodialysis. Objective: To determine the characteristic of patients who underwent hemodialysis and to determine the correlation between interdialytic weight gain (IDWG) and confounding factors (age, gender, and the duration of hemodialysis) with intradialytic hypertension. Methods: This research used quantitative methods with observational analytic design. The sampling used simple random sampling with a sample size of 126 patients, who met inclusion and exclusion criteria. Data were collected using secondary data and then analyzed using Fischer Test. Results: The result showed there was no correlation between Interdialytic weight gain (IDWG) and intradialytic hypertension (p value = 0,484). For confounding variable, there was also no correlation between age with intradialytic hypertension (p value= 0,584), gender with intradialytic hypertension (p value= 1,000), and the duration of hemodialysis with intradialytic hypertension (p value= 0,333).Conclusion: There was no statistically significant correlation between interdialytic weight gain (IDWG) and intradialytic hypertension. Age, gender, and duration of hemodialysis did not contribute to intradialytic hypertension.ABSTRAKLatar belakang: Hipertensi intradialisis merupakan salah satu permasalahan yang dialami pasien gagal ginjal kronik yang menjalani hemodialisis, dengan prevalensi sekitar 13,2% sampai 33,9%.

Abstract

State spending to deal with stroke is quite large every year due to the high incidence of stroke. Data from the Ministry of Health of Indonesia mentions an increasing number in health financing every year, the country financed stroke services by 2.56 trillion in 2018. The magnitude of the incidence of stroke can be lowered by controlling the risk factors of stroke. This study aimed to identify stroke risk factors, both those modifiable risk factors and irreversible risk factors. Research is conducted with a literature review using descriptive study methods or correlation analysis with cross sectional approaches. Sixteen articles identified from Google scholars and Garuda databases in the 2015-2020 range, and were analysed using JBI critical appraisal tools-checklist for analytical cross-sectional studies and JBI critical appraisal tools-checklist for prevalence studies.  Synthesis narrative was used to analyse the articles. Out of 16 articles examined the modifiable risk factors were hypertension, dyslipidemia, diabetes mellitus, lack of physical activity, blood sugar levels, cholesterol levels in blood, alcohol and drug consumption, kidney disease, tuberculosis, heart disease, heart failure, obesity, central obesity, atrial fibrillation and also smoking are declared associated with the incidence of stroke. The irreversible risk factors are age, gender, education, family history, income employment, previous stroke history, and rural and urban distribution. Risk factors divided into two categories modifiable risk factors and irreversible risk factors. It is important to know the risk factors of stroke in order prevent the occurrence of stroke as early as possible by avoiding modifiable stroke risk factors. Hypertension, smoking, diabetes mellitus obesity, dyslipidemia, and lack of activity are the most modifiable stroke risk factors identified in the articles. The most irreversible risk factors identified in the article are age, gender, and level of education
