Abstract

Anemia in pregnancy remains a public health problem in Indonesia, with a prevalence of 48.9% (Riskesdas, 2018). Low adherence to iron and folic acid tablet (TTD) consumption is one of the main causes of high anemia rates among pregnant women. This community service activity aimed to improve TTD consumption compliance among pregnant women through interactive educational media at Sarfina Sembiring Clinic in 2025. The implementation method applied a participatory learning approach, consisting of preparation, education delivery, evaluation, and follow-up. A total of 30 pregnant women participated in the activity. Pre-test and post-test evaluations showed an increase in knowledge scores from 62.5 to 85.3 (+36.4%), while compliance with TTD consumption increased from 46.7% to 83.3% (+36.6%). Additionally, 90% of participants stated that interactive media was easier to understand compared to conventional lecture methods. These findings indicate that interactive educational media effectively improves knowledge and compliance with TTD consumption among pregnant women, making it a viable alternative for health education to support anemia prevention programs in Indonesia.

Abstract

Abstract

Pregnancy-related anemia remains a major public health problem in Indonesia, largely attributable to low iron supplementation adherence among pregnant women. Evidence regarding digital interventions based on the family centered empowerment model (FCEM) remains limited. This study aimed to evaluate the effectiveness of an FCEM-based digital E-Pocket Book in improving self-efficacy and adherence to iron tablet consumption among pregnant women. A quasi-experimental pretest–posttest control group study was conducted at the Sentosa Baru Community Health Center in Medan, Indonesia, from July to August 2025. A total of 50 pregnant women (gestational age, 14–36 weeks) were allocated to an intervention group (n = 25) and a control group (n = 25). The intervention group received the FCEM-based E-Pocket Book for four weeks, while the control group received standard antenatal education. Data were analyzed using the Wilcoxon signed-rank and Mann–Whitney U tests. The intervention group demonstrated significantly greater improvements in self-efficacy (p = 0.027; d = 0.78) and adherence to iron supplementation (p = 0.001; d = 0.91) than the control group. In conclusion, the family-centered empowerment–based digital pocketbook is effective in improving self-efficacy and adherence to iron supplementation among pregnant women.

Abstract

The first 1,000 days of life are a critical window for long-term health, yet many urban parents still prioritize curative care over preventive practices that support optimal child growth and development. Objective: This study aims to evaluate the impact of Clean and Healthy Living Behavior (CHLB) education on parental knowledge and independence in maintaining child growth and development at Sarfina Johor Clinic, Medan. Methodology: A quasi-experimental one-group pre-test and post-test design was applied. A total of 50 parents were recruited using purposive sampling. Data were collected using validated questionnaires and analyzed with the Paired Sample T-test. Findings: CHLB knowledge increased significantly from a mean of 61.44 to 87.82 (mean difference = 26.38; p = 0.000). Parental independence also improved from 56.52 to 83.24 (mean difference = 26.72; p = 0.000). Implications: Structured clinic-based education can strengthen parental self-efficacy and shift family practices toward preventive care, positioning parents as proactive managers of child health at home. This approach can be adopted by primary clinics as an operational strategy to support stunting prevention programs. Originality: This study offers a clinical-based intervention model that connects routine medical consultation with domestic health maintenance, providing a practical framework for strengthening preventive parenting practices.

Abstract

The Maternal Mortality Rate (MMR) is primarily caused by pregnancy complications that are actually preventable through early detection; however, maternal understanding of the Maternal and Child Health (MCH) Handbook remains very low. This community service aims to empower pregnant women at Sarfina Primary Clinic, Medan, by enhancing their MCH Handbook literacy. The implementation utilized a Community-Based Participatory Action (CBPA) approach conducted over four weeks, involving 30 pregnant women. The activity stages included a pre-test, interactive education, self-assessment simulations, and a post-test. Evaluation results indicated a significant surge in health literacy; pregnant women's knowledge in the "Good" category drastically increased from 13.3% to 86.7%, while the "Poor" knowledge category was successfully eliminated from 50% to 0%. In conclusion, this educational and simulation intervention proved highly effective in transforming pregnant women's behavior, making them proactive individuals in the early detection of pregnancy danger signs which is expected to reduce the risk of pregnancy complications and reduce maternal mortality rates.
