Abstract

Outpatient services at community health centers experience a high and dynamic service burden influenced by evolving disease patterns in the community. However, the use of disease pattern data to support outpatient service management remains limited and is often restricted to administrative reporting purposes. This community service activity aimed to enhance the capacity of health workers and managers at Darussalam Community Health Center to utilize disease pattern data as a basis for managing outpatient service burden. A community-based participatory approach was applied through mentoring, training, and hands-on practice in data analysis using routine data from the Integrated Recording and Reporting System of Community Health Centers (SP2TP). The subjects of the activity were health workers and managerial staff at Darussalam Community Health Center. The results demonstrated an improvement in participants’ understanding and ability to analyze disease pattern data, reflected by an increase in average knowledge scores from a moderate to a high category. Behavioral changes were also observed, including increased data-based discussions, the emergence of internal change agents (local leaders), and the development of collective awareness regarding the importance of data-driven outpatient service management. This community service activity contributed to strengthening outpatient service management and fostering a data-driven work culture at Darussalam Community Health Center.

Abstract

Young age sex education is often considered a taboo topic in Indonesian society, leading to a lack of understanding among adolescents. This can result in negative behaviors and risks such as sexual violence and unplanned pregnancy. This community service activity (PKM) aimed to provide accurate information and shape positive attitudes towards reproductive health among junior high school students. This service employed an educational, participatory, and applicative approach, consisting of three stages: preparation, implementation, and evaluation. The target participants were 30 students of class IX-6 at SMP Negeri 16 Medan, selected through purposive sampling. The intervention was conducted in two structured sessions using PowerPoint presentations, leaflets, and audiovisual media, covering the importance of sex education, reproductive organ function, ethics, norms, and how to avoid sexual deviations and violence. Evaluation was carried out using a pre-test and post-test knowledge questionnaire (10 items, KR-20=0.85) and observation. Data were analyzed using descriptive statistics and a Paired Sample T-Test. The activity successfully achieved its participant target. The results showed a significant increase in knowledge, with the average pre-test score of 5.2 (Sufficient category) rising to 8.1 (Good category) in the post-test (p-value=0.001). Observation also indicated a positive shift in participants' attitudes and understanding, as they were able to explain the importance of sex education and ways to avoid sexual violence. Counseling on sex education for young adolescents is effective in increasing knowledge and shaping positive attitudes towards reproductive health. Sustainable, school-based educational interventions are recommended as a routine part of adolescent health promotion programs.

Abstract

Background: Nurse burnout is a persistent challenge in hospital settings and has been associated with compromised patient safety performance. Although previous studies have examined burnout and general safety outcomes, empirical evidence directly linking nurse burnout to the implementation of standardized Patient Safety Goals remains limited, particularly in low- and middle-income country contexts. Therefore, this study aimed to examine the relationship between nurse burnout levels and the implementation of patient safety goals among hospital nurses in Indonesia. Methods: A quantitative cross-sectional correlational study was conducted in October 2025. The study involved 97 registered nurses providing direct patient care, selected using purposive sampling. Inclusion criteria were nurses with a minimum of six months of work experience, while nurses in managerial positions or on leave were excluded. Nurse burnout (independent variable) was measured using the Maslach Burnout Inventory, and patient safety goal implementation (dependent variable) was assessed using a structured observation checklist based on World Health Organization and Indonesian Ministry of Health indicators. Data were collected anonymously following ethical approval and informed consent. Statistical analysis was performed using Spearman’s rank correlation test. This study followed the STROBE guidelines for reporting cross-sectional studies. Result: Most participants were female (86.6%) and aged 25–35 years (75.3%). Moderate burnout levels were reported by 58.8% of nurses, while 61.9% demonstrated good implementation of patient safety goals. A statistically significant correlation was identified between nurse burnout and patient safety goal implementation (r = 0.456; p < 0.001), indicating that higher burnout levels were associated with lower adherence to patient safety practices. Conclusion: Nurse burnout is significantly associated with the implementation of patient safety goals, indicating that nurses’ psychological well-being plays a critical role in ensuring safe care delivery. These findings highlight the need for organizational and psychosocial interventions targeting burnout reduction as an integral component of patient safety improvement strategies.

Abstract

Hypertension, often called the "silent killer," is a leading non-communicable disease contributing to high mortality and morbidity, particularly in the elderly. As individuals age, peripheral blood vessel elasticity decreases, increasing vascular resistance and leading to systolic hypertension, commonly seen in older adults. Hypertension requires long-term treatment, which can result in non-compliance among elderly patients. Family support plays a crucial role in assisting and encouraging adherence to treatment regimens. This quantitative correlation study, with a cross-sectional approach, aimed to examine the relationship between emotional family support and treatment compliance in elderly hypertensive patients. The study was conducted at the Sei Agul Health Center, with a sample size of 65 elderly individuals selected using the Slovin formula and purposive sampling technique in October 2023. The Morisky Medication Adherence Scale (MMAS-8) was used to measure treatment compliance, while the emotional support instrument was developed by the researcher. Data were analyzed using the chi-square test, showing a significant relationship between emotional family support and treatment compliance (p-value < 0.05). The findings conclude that emotional support from family positively influences the compliance of elderly patients undergoing hypertension treatment at the Sei Agul Health Center. Health workers are encouraged to involve families in supporting hypertensive patients to enhance treatment outcomes.

Abstract

This study aimed to analyze the effect of cancer stage on hemoglobin levels and to examine the moderating role of chemotherapy cycles in this relationship using Orem’s Self-Care Deficit Theory as a conceptual framework. This analytical cross-sectional study involved 54 cancer patients who had undergone at least one cycle of chemotherapy at H. Adam Malik General Hospital, Medan, Indonesia. Secondary data were obtained from medical records, including cancer stage, number of chemotherapy cycles, and hemoglobin levels. Data were analyzed using a Generalized Linear Model (GLM) with an interaction term to assess moderation effects. The results revealed that cancer stage had a significant negative effect on hemoglobin levels (β = -128.226; p < 0.001), indicating that more advanced cancer stages were associated with lower hemoglobin levels. In contrast, the number of chemotherapy cycles showed no significant effect (β = -0.004; p = 0.609) and did not moderate the relationship between cancer stage and hemoglobin levels (β = 0.025; p = 0.379). These findings suggest that cancer stage is a more dominant factor influencing hemoglobin levels than chemotherapy exposure. From a nursing perspective, the findings highlight the importance of early anemia screening, hematological monitoring, patient education, and self-care support, particularly for patients with advanced-stage cancer. This study was limited by the use of secondary data and a single-center setting, which may restrict the generalizability of the findings. Future studies are recommended to include larger samples, longitudinal designs, and additional clinical variables that may influence hemoglobin levels in cancer patients
