Abstract

Background: Fractures will experience pain, which is an unpleasant multidimensional experience due to tissue damage. One of the non-pharmacological therapies that can be given to patients with Pertrochanteric Femur Fractures is the Cold Pack technique (cold compress), which effectively reduces pain in mild orthopaedic cases, while in severe orthopaedic cases, using ice water immersion is more recommended. Research Objective: To determine the effect of cold pack nursing therapy on reducing pain levels in the close-fracture Pertrochanteric Femur in the Emergency Room of Klungkung Regency Hospital. Methods: Methods using a case study approach. The sample in this study amounted to two people selected by the purposive sampling technique. Results: Providing cold compress action with cold packs to reduce pain in patients who have closed fractures obtained the results of the patient saying there is a change that is felt, namely slightly reduced pain in the fracture area in patients with Pertrochanteric femur fractures in the emergency room of Klungkung Hospital. Conclusions: Giving cold pack therapy can effectively reduce pain intensity in patients with closed fractures and decrease the pain scale after getting cold pack therapy nursing interventions.

Abstract

Introduction: Patients undergoing hospitalization are at risk of sudden cardiac arrest, necessitating swift and accurate intervention from nurses. The preparedness and confidence of nurses in such situations significantly impact the desired outcomes. Basic and advanced life support training is crucial and can be delivered through various media, including instructional videos. This study aims to evaluate the effectiveness of learning videos on the management of lethal arrhythmias in improving the knowledge and skills of nurses at the inpatient unit of Mangusada Hospital. Methods: This study utilized a quasi-experimental design with a Non-Equivalent Control Group Design. The sample comprised 60 inpatient nurses, 30 in the control and 30 in the treatment group. Data was collected using questionnaires, checklists, and instructional videos. Data analysis was conducted using univariate and bivariate analyses with the Wilcoxon Signed Rank Test and Mann-Whitney Test. Results: There were significant differences in nurses' knowledge (p-value = 0.003) and skills (p-value = 0.040) between the control and treatment groups, with learning videos being more effective in enhancing nurses' knowledge and skills in managing lethal arrhythmias at the inpatient unit of Mangusada Hospital. Conclusion: Learning videos effectively improve nurses' knowledge and skills, though they have limitations, particularly regarding skills. Continuous direct action or simulation training, combined with learning videos, is essential. It is recommended that such training be conducted twice a year.

Abstract

As of 2022, an estimated 39 million people were living with HIV (PLHIV) globally, with approximately two-thirds—or 25.6 million—residing in the African region. In the same year, HIV was responsible for approximately 630,000 deaths worldwide. Individuals living with HIV are particularly susceptible to stigma, which can adversely affect their quality of life. HIV-related stigma has been shown to significantly impact the health outcomes, daily lives, and overall well-being of PLHIV, particularly within key populations. Objective: This study aims to examine nursing students' perceptions of HIV-related stigma. Method: This type of research is a qualitative research with narrative design. In-depth interviews was data collection techniques with informants determined by purposive sampling technique with a total of 8 Nursing Students. The research instrument is the researcher himself and the collection of data through interviews and documentation. The data were analyzed using thematic analysis, which involved reviewing the transcripts and categorizing the data into themes. The next step involved coding, where labels were assigned to each relevant piece of information. Results: Through interviews, four main themes emerged, illustrating students' attitudes, challenges, and readiness in engaging with people living with HIV/AIDS (PLWHA) Conclusions: Despite possessing adequate knowledge regarding HIV/AIDS transmission, nursing students' perceptions in providing care for PLWHA remain influenced by prevailing social stigma.

Abstract

Ambulance services are becoming a trend for early response to emergencies in major cities. The performance of the ambulance service will increase in line with the population and possible problems in the area. EMS services are essential at this time, as any life-threatening condition requires immediate action to prevent disability and death. Ambulances are required to provide assurance to chronically ill patients that the service they provide is adequate and can handle them in case of deterioration. This study aims to describe the frequency of pre-hospital ambulance use in patients with chronic diseases. This research method is an observational study with a retrospective approach to find the frequency of ambulance use by chronic diseases during the period 2019-2021. The results showed the three highest cases of pre-hospital ambulance use in Denpasar City were accident, fire and evacuation services for sick patients. The use of prehospital ambulances by chronic diseases has not reached half of the ambulance services. Stroke, diabetes mellitus and heart disease are the three most common chronic diseases that use pre-hospital ambulance services.

Abstract

Introduction: Knowledge and skills of health workers regarding Advanced Life Support (ALS) are an important part that needs to be trained and improved with appropriate training modalities in handling cardiac arrest patients. This literature review aims to find out which modalities are the most effectively applied to ALS training which can increase the knowledge and skills of health workers in conducting ALS. Methods: Literature study using 11 articles that have been published and peer-reviewed from data-based PubMed, CINAHL, and ScienceDirect using the keywords effectiveness, advanced life support, modalities, nurses, knowledge, and skills. Searches were limited to full text articles, experimental designs, publications from 2017 to 2022. Selection of articles in research using flowchart Prisma.Results: Analysis of eleven articles or journals related to the research objectives found that the video training was the most effective methods in increasing the knowledge and skills of respondents in ALS training. The implementation of ALS training in a hybrid manner, namely the use of non-face-to-face learning methods using video with face-to-face training led by an instructor, showed no significant difference, but video training was preferred because it provides convenience regarding time, place of implementation, and more training participants. Conclusion: ALS training must be given to nurses at least once a year, so it can be suggested to institutions to use the learning video method as a method of providing training.

Abstract

The elderly is likely to experience chronic diseases because there is a decrease in the function of organs in the body. An increase in life expectancy will correspond to an increase in the number of chronic disease sufferers, because every one elderly person has one or more chronic diseases. It is important for the elderly to understand the emergency conditions that may be experienced, because fluctuations in the condition of the elderly can occur in a short time. This Community Service aims to increase the understanding of the elderly regarding emergency conditions in chronic diseases. The method used is through counseling and pre and post tests to determine the understanding of the elderly. After the test was carried out, it was found that there was an increase in the average value of the participants' knowledge of emergency conditions in chronic diseases after being given counseling. With an increase in knowledge, it is hoped that patients will be able to perform first aid and find the right place for further help if chronic disease patients experience emergency conditions at home.

Abstract

Prehospital care has historically provided medical care with the aim of saving lives and preventing disability. Prehospital services that are accommodated by ambulances have complex and unpredictable care arrangements, so ambulance personnel, namely nurses, must prepare themselves to deal with all situations and conditions. This study aims to describe the competencies possessed by nurses while on duty in prehospital ambulances. This research is a literature study from several databases, and we obtained 20 articles for analysis. The results of the article analysis show that nurses who work in prehospital ambulances must have the competence to be able to save patients' lives, which is formed from knowledge, attitudes, and skills. Knowledge that must be possessed by ambulance nurses includes assessment, triage, and situation analysis. The positive attitudes of ambulance nurses include decision-making, communication, collaboration, caring, ethics, and culture. The skills possessed by ambulance nurses are BHD, intubation, examination of vital signs, interpretation of supporting examinations, and drug management. Ambulance nurses can improve their emergency care competencies through continuous education and training.

Abstract

ABSTRAK Cidera akibat kecelakaan lalu lintas menyebabkan kematian 1,2 juta jiwa per tahunnya. Saat terjadi kecelakaan sering kali korban mengalami multiple trauma yang perlu dilakukan tata laksana dengan baik di prehospital. Hal yang menjadi penatalaksanaan awal saat korban mengalami kecelakaan yaitu airway, breathing dan circulation. Penatalaksanaan lanjutan setelah 3 hal penting dilakukan adalah manajemen dalam tatalaksana nyeri berupa penatalaksanaan fraktur, balut tekan, dislokasi, aputasi dan log roll sehingga perlu dilakukan tata laksana yang baik saat di pre hospital. Secara umum keterlambatan atau kurang tepatnya pemberian pertolongan pertama pada fraktur muskuluskeletal sering kali terjadi karena kurangnya pengetahuan dan kepedulian penolong yang menemukan kejadian tersebut. Remaja menjadi target pengabdian masyarakat saat ini dikarenakan remaja merupakan komunitas produktif yang aktif dalam bermasyarkat sehingga merupakan generasi penerus yang perlu di didik untuk dapat menjadi agen perubahan kesehatan. Memberikan edukasi dan pendampingan kepada remaja sekolah dalam melakukan tindakan pertolongan pertama pada trauma muksuluskeletal, kontrol pendarahan, dislokasi, aputasi dan log rolling. Masalah yang ditemukan yaitu remaja belum memahami cara melakukan penatalaksanaan manajemen nyeri pada patah tulang dengan baik, remaja belum mengetahui cara melakukan penatalaksanaan nyeri pada dislokasi sendi, remaja belum mengetahu cara melakukan transportasi pasien trauma. Kegiatan pendekatan yang dilakukan kepada sekolah dan remaja berjalan dengan baik, dimana remaja mau kooperatif dalam kegiatan yang telah dilakukan, hasil post test baik pengetahuan memiliki peningkatan sebanyak 49 responden memiliki pengetahuan yang baik, dan terdapat 49 responden memiliki kemampuan yang baik dalam praktik pertolongan pertama dan transport pasien trauma. When an accident occurs, the victim often experiences multiple trauma which needs to be managed properly at the prehospital. The initial management when a victim experiences an accident is airway, breathing and circulation. Follow-up management after 3 important things have been done is pain management in the form of fracture management, compression dressings, dislocations, aputations and log rolls so that good management needs to be carried out in the pre-hospital setting. In general, delays or inaccurate provision of first aid for musculoskeletal fractures often occur due to a lack of knowledge and concern of the rescuers who discover the incident. Teenagers are currently the target of community service because teenagers are a productive community who are active in society so that they are the next generation who need to be educated to be agents of health change. Objective: to provide education and assistance to school teenagers in carrying out first aid measures for mucosal-keletal trauma, bleeding control, dislocation, amputation and log rolling. Abstract

Background: Non-pharmacological management of hypertension through complementary medicine has now been developed in Indonesia. However, the interest of hypertensive patients in complementary medicine is not optimal because there are many factors that influence the behavior of patients in the use of complementary medicine. Methods: This study employed an analytic study design with a cross-sectional approach. This research was conducted at Puskesmas 1 East Denpasar for 1 month. This study involved 101 hypertensive patients using accidental sampling technique. Data collection was carried out using a questionnaire. Data were analyzed univariately and bivariately using SPSS version 20. Results: This study showed that most hypertensive patients had good knowledge of hypertension treatment with complementary therapies (64.4%). A total of 53.5% of hypertensive patients showed adequate behavior in the treatment of hypertension with complementary therapy. Based on the chi square test, it was found that there was a significant relationship between knowledge and behavior of hypertensive patients towards hypertension treatment with complementary therapy (p-value = 0.04). Conclusion: It is recommended for health workers to increase education and promotion about hypertension treatment with complementary therapy, especially for hypertensive patients with poor knowledge.

Abstract

Background: Rapid response to medical emergencies is a crucial factor in reducing patient mortality and morbidity. However, the ambulance calling system in Indonesia still faces various challenges, such as limited access to information, delayed responses, and lack of integration with emergency command centers. Purpose: To review various studies related to the development of mobile/web-based applications for emergency medical services, and to identify research gaps and opportunities for further development. Methods: A search was conducted using the PubMed and ScienceDirect databases, followed by a review and synthesis process using PRISMA. An initial 219 articles were found, of which 15 were included in the study. Results: The application is capable of facilitating ambulance ordering, real-time location tracking, and accelerating emergency response. However, most studies are limited to technical function tests with small sample sizes and have not measured the application's effectiveness on clinical indicators. Conclusion: Innovation in web-based and mobile emergency applications has significant potential to improve response speed, ambulance calling accuracy, and community empowerment in first aid. However, further research is still needed with a broader scope, strong quantitative methods such as quasi-experiments or RCTs, as well as multi-dimensional evaluations involving technical, clinical, and user experience aspects.   Keywords: Emergency Medical Services; First Aid; Mobile Application.   Pendahuluan: Respon cepat terhadap keadaan gawat darurat medis menjadi faktor penting dalam menurunkan mortalitas dan morbiditas pasien. Namun, sistem pemanggilan ambulans di Indonesia masih menghadapi berbagai kendala, seperti keterbatasan akses informasi, keterlambatan respon, dan kurangnya integrasi dengan pusat komando darurat. Tujuan: Untuk meninjau berbagai penelitian terkait integrasi pertolongan pertama dan aktivasi ambulans. Metode: pencarian artikel dilakukan database PubMed dan ScienceDirect kemudian proses review dan sintesis dilakukan menggunakan PRISMA. Artikel awal yang ditemukan adalah 219, kemudian 15 artikel disertakan dalam penelitian. Hasil: Menunjukkan bahwa aplikasi mampu memfasilitasi pemesanan ambulans, pelacakan lokasi real-time, dan percepatan respon darurat. Namun, sebagian besar studi masih terbatas pada uji fungsi teknis dengan sampel kecil, belum mengukur efektivitas aplikasi pada indikator klinis. Simpulan: Inovasi aplikasi darurat berbasis web maupun mobile memiliki potensi besar dalam meningkatkan kecepatan respon, akurasi pemanggilan ambulans, dan pemberdayaan masyarakat dalam pertolongan pertama. Namun, masih diperlukan penelitian lanjutan dengan cakupan yang lebih luas, metode kuantitatif yang kuat misalnya quasi-experiment atau RCT, serta evaluasi multi-dimensi yang melibatkan aspek teknis, klinis, dan pengalaman pengguna.
