Abstract

Background Diagnosing hypertension in children and adolescents is not always straightforward. The blood pressure-to-height ratio (BPHR) has been reported as a screening tool for diagnosing hypertension.Objective To evaluate the diagnostic value of blood pressure-to-height ratio for evaluating hypertension in adolescents.Methods A cross-sectional study was conducted among 432 healthy adolescents aged 12-17 years in Singkuang, North Sumatera from April to May 2016. Blood pressure tables from the National High Blood Pressure Education Program (NHBPEP) Working Group on High Blood Pressure in Children and Adolescents were used as our standard of comparison. Sex-specific systolic and diastolic blood pressure-to-height ratios (SBPHR and DBPHR) were calculated. ROC curve analyses were performed to assess the accuracy of BPHR for discriminating between hypertensive and non-hypertensive adolescents. Optimal thresholds of BPHR were determined and validated using 2x2 table analyses.Results The accuracies of BPHR for diagnosing hypertension were > 90% (P<0.001), for both males and females. Optimal SBPHR and DBPHR thresholds for defining hypertension were 0.787 and 0.507 in boys, respectively, and 0.836 and 0.541 in girls, respectively. The sensitivities of SBPHR and DBPHR in both sexes were all >93%, and specificities in both sexes were all >81%. Positive predictive values for SBPHR and DBPHR were 38.7% and 45.2% in boys, respectively; and 55.9% and 42.4% in girls, respectively; negative predictive values in both sexes were all >97%, positive likelihood ratios in both sexes were all >5, and negative likelihood ratios in both sexes were all <1.Conclusion Blood pressure-to-height ratio is a simple screening tool with high sensitivity and specificity for diagnosing hypertension in adolescents.

Abstract

Abstrak: Dukungan dan komitmen yang tinggi dari Pusat Kesehatan Masyarakat diperlukan untuk menciptakan layanan primer yang berkualitas bagi peserta Badan Penyelenggara Jaminan Sosial (BPJS) Kesehatan. Salah satu cara dalam menilai keberhasilan Puskesmas adalah tingkat kepuasan pasien. Penelitian ini bertujuan untuk mengetahui tingkat kepuasan peserta BPJS terhadap pelayanan kesehatan di puskesmas Medan Selatan. Penelitian ini menggunakan desain cross sectional yang menggunakan kuesioner terhadap 85 responden yaitu peserta BPJS Kesehatan di Puskesmas Medan Selatan. Teknik analisis data menggunakan statistik deskriptif dan Customer Satisfaction Index (CSI). Berdasarkan metode CSI ditemukan bahwa tingkat kepuasan peserta BPJS dalam setiap dimensi keandalan 70,4%, daya tanggap 71,6%, jaminan 73,6%, tangible 69,4%, dan empati 73,8%. Simpulan, kepuasan pasien dalam pelayanan kesehatan di Puskesmas Medan Selatan (71,8%), skor ini dengan metode CSI adalah kategori puas. Namun, berdasarkan standar BPJS termasuk dalam kategori tidak puas.Kata kunci: BPJS kesehatan, kepuasan pasien, perawatan kesehatan, puskesmas. Customer Satisfaction Index of Badan Penyelenggara Jaminan Sosial on Health Service at Puskesmas Medan Area Selatan in 2017 Abstract: The support and high commitment from Public Health Center are necessary needed to create quality primary service for Badan Penyelenggara Jaminan Sosial (BPJS) Health participants. One that can assess the success of Public Health Center is the level of patient satisfaction. This research aimed to determine the level of BPJS participant satisfaction toward health services in South Medan Area health centers. This study used cross-sectional design with a questionnaire with 85 respondents that are BPJS Health participants in South Medan Area health centers. Data analysis technique using statistic descriptive and Customer satisfaction index (CSI).  Based on the method of CSI found that the level of satisfaction of BPJS participants in each dimension reliability 70.4%, responsiveness 71.6%, assurance 73.6%, tangible 69.4%, and empathy 73.8%.  Conclusion, Patient satisfaction in health care in South Medan Area health centers (71.8%), this score with CSI method are the satisfied category. Abstract

The coronavirus pandemic (COVID-19) is designated as a Public Health Emergency that is troubling the world, much remains unknown about the virus that causes COVID-19, but in general, this virus is transmitted through direct contact with droplets from the respiratory tract of an infected person (which exits through coughs). And sneezes). People can also become infected by touching a surface contaminated with this virus and then touching their face (eg, eyes, nose, mouth). The school health program (SHP) is a health program implemented at the school level. The main activities of SHP are carried out through health education, health services, and fostering a healthy school environment with the primary target of students, educators, educators, and the school community. Efforts to maintain health can be started from the school environment, which emphasizes changing behavior. Therefore this SHP program is essential because it includes early health education. This community service aims to provide education and socialization to prevent the spread of the COVID-19 virus to teachers and students based on UKS at Amanah SMPS, Binjai District, Langkat Regency. Before being given education, assessing the COVID-19 prevention behavior that they have implemented is carried out using a valid questionnaire. Then proceed with educational activities. The educational method carried out was by lecturing and discussing the prevention of COVID-19 and simulating the proper use of masks, after which several participants were asked to practice it and witnessed by other participants. It is hoped that with this community, serving students and teachers will increase their understanding and be able to prevent COVID-19 both for themselves and prevention in the community.

Abstract

This study aims to look at the analysis of the level of patient satisfaction with nurse services at VIP (Very Important Person) inpatient care and Wards at Padangsidimpuan City General Hospital. This research method is analytic with a cross-sectional approach. This research began to be carried out in September 2022 - March 2023 in the VIP Inpatient Ward at Padangsidimpuan City General Hospital. The study sample of VIP inpatients was 96 people and Ward was 96 people with sideways purposive technique. Univariate and bivariate data analysis with Chi-square test. The results showed that in the VIP room, the most dissatisfied were 50 people (52.1%) and the wards were the most dissatisfied with 76 people (79.2%). The results of the chi-square test obtained a p-value ≤ 0.05, so there was a significant relationship between VIP and Ward patient satisfaction with the dimensions of Patient Satisfaction at Padangsidimpuan City General Hospital. In conclusion, there is a significant relationship between VIP and Ward patient satisfaction with the dimensions of Patient Satisfaction at Padangsidimpuan City General Hospital. Abstract

Until now, there has been a tendency for dengue fever patients to increase, and its transmission has become one of Indonesia's most common public health problems. Getting rid of mosquito nest eradication (MNE) with the 3M Plus Movement regularly, which includes draining water reservoirs, burying used items, and closing water reservoirs, which make it harder for mosquitoes to breed, can stop the spread of dengue. Even though this disease can attack all groups, several studies have shown that children and teenagers are more susceptible to dengue fever, so it is essential to make efforts to increase understanding and education for school-aged children, including high school students and teachers, who must work together to prevent dengue fever. As a result, the community service team from the Medical Faculty of Universitas Sumatera Utara carried out this activity using an outreach strategy that included questions and answers with students about MNE, Clean and Healthy Living Behavior, and other topics as well as the actual implementation of 3M Plus. A good understanding of MNE among students and teachers allows for the formation of awareness of MNE's significance in the school environment, which is the outcome of this activity.

Abstract

This study aims to analyze nurses' knowledge about supervision and their ability to carry out documentation of nursing care at Murni Teguh Hospital, Medan. This type of research uses a cross sectional approach design. With this type of observational research, quantitative approach. The instruments used in this research were questionnaires and patient care documents from medical records. The research results showed that there was a relationship between nurses' knowledge about supervision and nurses' ability to carry out nursing care documentation, with a value of p = 0.004. In conclusion, nurses' knowledge about supervision at Murni Teguh Hospital in Medan is still lacking, so based on the results of statistical tests it was found that there was a relationship between knowledge about documentation and the ability to carry out documentation. Abstract

Background. Diabetes is a major health problem worldwide including in Indonesia. There is a close relationship between diabetes and cardiovascular disease, with cardiovascular events being the most prevalent cause of morbidity and mortality in diabetic patients. Therefore, it is important to assess cardiovascular risk in patients with diabetes in order to conduct appropriate preventive strategies. Various methods have been developed to assess the degree of cardiovascular risk, one of which is the Jakarta Cardiovascular Score, a scoring method that has been adapted for the Indonesian community. This study aims to describe the profile of cardiovascular risk in diabetes patients at Universitas Sumatera Utara Hospital. Methods. A descriptive, cross-sectional study was conducted in the internal medicine outpatient clinic at Universitas Sumatera Utara Hospital. All subjects were diabetic patients who had not developed any cardiovascular complications. The risk of cardiovascular disorders was assessed using the Jakarta Cardiovascular Score. Data were collected through medical records, interviews, and physical examinations. Results. There were 64 subjects in this study. The majority of subjects (81.3%) were classified as high risk for cardiovascular disease. Conclusion. Most patients with diabetes have a high degree of risk of cardiovascular disease.

Abstract

Background: Primary Health Center Laubaleng (PHCLB) first used the System Information Tuberculosis (SITB) at the start of the COVID-19 pandemic. SITB is more effective and efficient than manual, contains TB patient data, and is easy to view again. This research aims to describe the characteristics of Drug Sensitive Pulmonary TB (DSPTB) patients at the PHCLB during COVID-19 from SITB which is useful for detecting TB risk group cases. Method: The research type was descriptive quantitative with a cross-sectional approach by comparing each age group, productivity, and gender. All DSPTB patients were recorded and reported in the SITB PHCLB database during the COVID-19 pandemic from January to December 2020, 2021, and 2022. SITB database access permitted by PHCLB, Jl. Renun, No. 597, Laubaleng District, Karo Regency, North Sumatra Province, Indonesia Results: The largest percentage of DSPTB patients were aged 45-54 years in year 2022 (41.4%). The productive age category (15-64 years) dominated not (≥65 years) and not yet productive (0-14 years) in 2020, 2021, and 2022, respectively 90.9%, 85.4%, and 86.2%. The total number of DSPTB decreased in 2020, 2021, and 2022, respectively 55, 48, and 29 people. Conclusion: Characteristics of DSPTB from SITB PHCLB during the Covid-19 pandemic, were mostly in the adult age category 45-54 years, productive (15-64 years), male and total number of patients decreased by 7 and 19 people respectively.

Abstract

Patient non-adherence to antihypertensive medication is a significant risk factor for increased morbidity and mortality in individuals with hypertension. Hypertension, a prevalent cardiovascular condition, is classified as a degenerative disease. Consistent antihypertensive treatment is crucial for improving patient quality of life. This study employed a cross-sectional survey design. The sample consisted of hypertensive patients who presented at the Belawan Health Center between July 22 and 26, 2024. Consecutive sampling was used to recruit 75 participants. Data were collected via interviews regarding participant characteristics and the administration of the Morisky Medication Adherence Scale-8 (MMAS-8), Hypertension Fact Questionnaire (HFQ), and World Health Organization Quality of Life-BREF (WHOQOL-BREF) questionnaires to assess medication adherence, hypertension knowledge, and quality of life, respectively. Analysis revealed strong positive correlations between medication adherence and quality of life (ρ = 0.677, p < .001), and between hypertension knowledge and quality of life (ρ = 0.700, p < .001). Furthermore, both medication adherence and hypertension knowledge significantly predicted quality of life (F = 53.704, p < .001). Multiple linear regression analysis (Y = 30.488) indicated that a one-unit increase in adherence was associated with a 2.680-unit increase in quality of life (p < .001), and a one-unit increase in knowledge was associated with a 2.704-unit increase in quality of life (p < .001). In conclusion, there are strong positive correlations between both medication adherence and hypertension knowledge with the quality of life of hypertensive patients at the Belawan Health Center.

Abstract

Nephrotic syndrome (NS) is one of the most common chronic kidney diseases in children, characterized by massive proteinuria, hypoalbuminemia, edema, and dyslipidemia. Relapse occurs in most cases and contributes to worsening the disease course. Dyslipidemia is suspected to influence relapse risk, but the specific relationship between lipid profile and relapsing NS remains unclear. Objective to analyze the association between total cholesterol, triglycerides, LDL, and HDL levels with relapse incidence in children with nephrotic syndrome. A retrospective study was conducted on 66 children aged 2–18 years with NS treated at H. Adam Malik General Hospital, Medan, in 2024. Subjects were divided into relapsing (n=54) and non-relapsing (n=12) groups. Data were obtained from medical records and analyzed using the Mann-Whitney test and Spearman correlation. Triglyceride (p=0.007), LDL (p=0.013), and HDL (p<0.001) levels differed significantly between the relapsing and non-relapsing groups. Total cholesterol showed no significant difference (p=0.164). The mean albumin level was significantly lower in the relapsing group (2.9 g/dL vs. 4.1 g/dL; p<0.001). A significant negative correlation was found between albumin and triglyceride levels (r=-0.326; p=0.008). Triglyceride, LDL, and HDL levels are significantly associated with relapse in pediatric nephrotic syndrome. Hypoalbuminemia is also an important risk factor. Lipid profile evaluation can serve as a useful indicator in monitoring and managing relapses.
