Abstract

Latar Belakang: Akne vulgaris (AV) adalah penyakit inflamasi kronik yang menyerang unit pilosebasea yang paling banyak menyerang pasien dewasa muda di hampir seluruh negara. Pasien dengan akne vulgaris dapat mengalami tekanan psikologis sehingga memengaruhi kualitas hidup pasien. Tujuan: Mengevaluasi kualitas hidup pasien akne vulgaris derajat sedang. Metode: Merupakan penelitian potong lintang observasional dengan menggunakan kuesioner yang melibatkan pasien AV derajat sedang yang mendapatkan terapi AV standar dan penambahan fototerapi sinar biru pada periode Desember 2017 hingga Februari 2018 di Divisi Kosmetik Medik Unit Rawat Jalan Kesehatan Kulit dan Kelamin RSUD Dr.Soetomo Surabaya. Hasil: Sejumlah 40 pasien berusia 16 – 25 tahun dengan rata-rata 19,22 ± 2,76 tahun. Subjek merasakan AV berefek berat terhadap kualitas hidupnya (65%), diikuti dengan yang merasakan berefek sedang (20%), berefek ringan (10%), berefek sangat berat (2,5%), dan tidak ada efek terhadap kualitas hidup (2,5%). Skor total kuesioner Dermatology Life Quality Index (DLQI) dianalisis korelasi dengan variabel lama sakit dan usia subjek menggunakan uji Spearman’s rho. Analisis tersebut menunjukkan adanya korelasi yang bermakna antara skor DLQI total dengan lamanya subjek menderita AV (p = 0,037; CI 95%). Kekuatan korelasi antar kedua variabel (r) negatif lemah. Hal tersebut berarti semakin lama durasi menderita AV maka semakin kecil skor DLQI total. Skor DLQI total dikatakan tidak berkorelasi dengan usia subjek (p = 0,318; CI 95%). Simpulan: Penelitian ini menunjukkan penurunan kualitas hidup pada pasien akne derajat sedang, dan kualitas hidup berkorelasi dengan lama durasi menderita AV.

Abstract

Background: Bacterial vaginosis (BV) is a lower genital tract infection, which occur as a result of vaginal microbiome dysbiosis. This vaginal infection can lead to symptoms such as vaginal discharge, malodor and irritation. It is the most common condition occurred in reproductive age but may give 50–75% of asymptomatic signs. Purpose: To overview new BV patients at the Sexually Transmitted Infection Division of Dermatology and Venereology Outpatient Clinic Dr. Soetomo General Hospital Surabaya in 2015–2017. Methods: A retrospective study was performed by evaluating medical records of new BV patients, including identity, history taking, examination, diagnosis, treatment, follow up and counselling. Result: The number of new BV cases was 35 (1,38%). The most infected age group was 25–44 years old (51%), and 65.7% of them were married. The most common chief complaint was vaginal discharge without subjective complaints (74.3%), the most common type of complaint was recurrence (48.6%), the most partner roles which affected by BV patients were husbands (51.4%), history of vaginal douching (34.2%), and the most vaginal discharges were severe (75.8%). The whiff test showed positive results and pH > 4.5 in 48.6% of patients. All microscopical preparations showed clue cells. The most prescribed drug was metronidazole. Conclusion: The overview new BV cases showed a small incidence with commonly occurred among women in childbearing age and were asymptomatic. A routine high vaginal swab helps the proper diagnosis and management to prevent the recurrence and complication of BV.

Abstract

Background: Psoriasis is an inflammatory, chronic, and recurring skin disease that affects patient’s quality of life. Environment, drugs, and infections in the form of antigen and toxin such as Candida spp. are assumed responsible for triggering and exacerbating psoriasis. Purpose: To compare the prevalence of skin colonization of Candida spp. on psoriasis vulgaris patient and control. Methods: This study was observational analytic research involving 33 psoriasis vulgaris patients and 33 healthy individuals. They were matched up based on sex, age, and lesion location. Swab of skin lesions on the Koebner area of both groups was cultured on CHROMagar media. Candida spp. was later identified and the colonization was calculated. Result: Candida spp. was found on 51.5% of lesions in psoriasis vulgaris patients and 21.2% in the control group with a significant difference of p < 0.05. The average colony number of Candida spp. in psoriasis vulgaris group was lower than the control group with no significant difference of p > 0.05. The predominant Candida spp. in the psoriasis vulgaris and control groups was Candida krusei, which were 35.5% and 85.6%, respectively. No significant difference between sex, age, period of complaints, age onset, degree of severity (Psoriasis Area and Severity Index), and therapy history, with positive culture of Candida spp. Conclusion: There was no difference in the number of Candida spp. colonization in the psoriasis vulgaris group and the control group. However, the culture positivity of Candida spp. in the psoriasis vulgaris patients was higher than in the control group.

Abstract

Pendahuluan: Kandidiasis vulvovaginalis (KVV) merupakan suatu infeksi pada area vagina dan vulva yang disebabkan oleh jamur Candida baik spesies Candida albicans maupun Candida non-albicans. Diabetes melitus (DM) merupakan salah satu faktor predisposisi KVV yang dapat memicu dan memperburuk KVV. Metode: Dilaporkan satu kasus KVV pada anak dengan DM tipe 1 dengan kadar gula darah tidak terkendali Kasus: Seorang anak perempuan berusia 11 tahun, berat badan 20 kg dengan keluhan utama keputihan berwarna putih susu dari kemaluan sejak 2 hari, tidak berbau, gatal, tanpa rasa terbakar, disuria minimal dan edema pada kemaluan. Keluhan ini baru pertama kali dirasakan oleh pasien. Riwayat DM sejak 3 tahun lalu. Pemeriksaan fisik terdapat makula eritematus, berbatas tidak jelas, duh tubuh dan edema pada regio vulva, sedangkan regio vagina ditemukan duh tubuh berwarna putih seperti susu pecah, tanpa edema, erosi dan fisura. Hasil laboratorium gula darah acak (GDA) 396 mg/dL, HbA1C 9,1%. Pemeriksaan Gram dan sediaan basah ditemukan blastospora dan pseudohifa. Kultur dari media CHROMagar Candida menunjukan species C. albicans. Diagnosis KVV ditegakkan berdasarkan anamnesis, pemeriksan fisik dan penunjang. Hasil: Pada pasien diberikan terapi ketokonazol 2 x 100 mg per hari, nistatin 100.000 IU salep dua kali sehari, insulin prandial maupun basal, serta edukasi untuk menjaga higenitas personal. Pemberin terapi selama 2 minggy memberikan kesembuhan pada pasien. Kesimpulan: Pemberian obat anti jamur dengan dosis dan durasi yang tepat disertai GDA <200 mg/dL disertai edukasi yang sesuai akan memberikan perbaikan klinis pada pasien KVV dengan diabetes melitus tipe 1 serta mencegah rekurensi.

Abstract

Pendahuluan: Trikomoniasis merupakan vaginitis yang disebabkan oleh protozoa dan dapat ditransmisikan secara seksual dan non-seksual serta sering muncul astimptomatik. Metode: Dilaporkan satu kasus trikomoniasis asimptomatik pada seorang remaja putri Kasus: Seorang remaja putri berusia 15 tahun diperiksa untuk mencari fokal infeksi pencetus psoriasis vulgaris. Ditemukan keputihan putih hijau, berbau, namun tidak gatal, tanpa rasa nyeri saat berkemih. Pasien belum pernah berhubungan seksual, namun sering bertukar handuk dengan anggota keluarga lain. Pemeriksaan fisik tampak dinding vulvovaginal eritem tanpa udem, duh tubuh berwarna kuning kehijauan. Pemeriksaan sedian basah ditemukan protozoa Trichomonas vaginalis berflagel sedangkan pemeriksan Gram tidak ditemukan leukosit 10-20/LPB, epitel 5-10/ LPB. Tidak ditemukan blastospora, hifa, diplokokus Gram negatif intraseluler dan ektraseluler juga bakteri Gram positif. Diagnosis Trikomoniasis ditegakan dari manifestasi klinis dan pemeriksaan penunjang Hasil: Pada pasien diberikan terapi metronidazole 2 gram dosis tunggal serta edukasi untuk menjaga higenitas personal. Evaluasi manifestasi klinis setelah terapi menunjukan hasil kesembuhan pasien. Kesimpulan: Penegakan diagnosa cepat dan tepat serta pemberian obat sesuai rekomendasi akan memberikan perbaikan klinis dan menghindarkan pasien pada morbiditas komplikasi yang berat. Kata kunci: Trikomoniasis, remaja, asimptomatik

Abstract

Background: Communication skills play an important role in clinical practice. Attitudes toward medical communication skills   that are built since a stage of medical student, would affect practice as a doctor later.  Good communication skills based on positive attitude will improve other clinical skills that are needed as a doctor, also will help a doctor to consider the importance of the improvement of the skills quality related to it. A number of studies have explored this context widely using CSAS as an instrument that consists of 2 subscales, a positive (PAS) and a negative (NAS) subscale. This study aimed to determine the difference in attitudes among students in Faculty of Medicine Pattimura University Ambon toward medical communication skills. Method: This was a cross-sectional analytical study that used primary data, obtained using an Indonesian version CSAS questionnaire that is first validated, thus, the scale consists of 24 items within 13 items as PAS and 11 items as NAS. Subjects were 124 active students of the academic year 2014/2015, woh undergo pre-clinical (semesters 2, 4, 6 and 8) and clinical study (Co Ass), obtained through stratified random sampling and complied the restriction criteria. Data were analysed using Mann-Whitney, Kruskall-Wallis, Unpaired t-test, One-Way ANOVA, and Post-Hoc Bonferroni tests with significance level of p<0,05.  Results: There were no difference in attitude toward communication skills between students across most variables; however, there were differences in negative attitudes (NAS) on medical communication skills between each batch of students (p=0.02).Conclusion: The difference in attitude toward communication skills can be seen between students across study batch in Pattimura University Faculty of Medicine.

Abstract

Latar Belakang : Pandemi COVID-19 menyebabkan perubahan yang menimbulkan kekhawatiran. Tanggung jawab dan tuntutan kehidupan akademik pada mahasiswa dapat menjadi bagian stres yang dialami oleh mahasiswa. Tujuan : Penelitian ini bertujuan untuk melihat pengaruh social support terhadap psychological distress mahasiswa Fakultas Kedokteran, Universitas Pattimura di masa penerapan new normal. Metode : Penelitian menggunakan pendekatan kuantitatif dengan alat ukur socialsupport sebanyak 10 pertanyaan dan psychologicaldistress sebanyak 12 pertanyaan. Partisipan dalam penelitian ini adalah 610 mahasiswa yang masih terdaftar aktif di Fakultas Kedokteran Universitas Pattimura (FK UNPATTI). Teknik statistik yang digunakan adalah total sampling, dimana semua populasi dijadikan sebagai sampel penelitian. Hasil : Hasil analisis menunjukkan bahwa terdapat pengaruh social support terhadap psycological distress mahasiswa fakultas kedokteran di masa penerapan new normal dengannilai Fhitung= 165.292 (p < 0.05). Kesimpulan : Penelitian ini menunjukan bahwa dalam menghadapi perkuliahan mahasiswa membutuhkan dukungan dari orang-orang terdekatnya, agar tercapai prestasi yang baik.

Abstract

The rates of cases and deaths from COVID-19 started to decline following the increased coverage of vaccination. In Indonesia, the coverage of COVID-19 vaccination has not met the national target, including in Ambon City. By conducting a webinar, this activity aimed at improving the knowledge of the development and the importance of the COVID-19 vaccination program amongst the academic community of Pattimura University and health workers at Dr. M Haulussy Hospital, the main COVID-19 referral hospital in Ambon City.This webinar was conducted by the Faculty of Medicine of Pattimura University (FK UNPATTI) on 10 December 2021 as part of the efforts to accelerate the COVID-19 vaccination coverage in Ambon. There were 275 participants (including students, lecturers and health workers from Dr. M. Haulussy Hospital, the main referral hospital in Ambon City) filled out the evaluation questionnaires (pre- and post-test). Resource persons were from the Health Office of Ambon City, FK UNPATTI’s research team members for COVID-19 acceptance, and leaders of FK UNPATTI. We found an increased percentage of correct answers in the results of the post-test evaluation. At the pre-test, the average percentage of correct answers was 87.4%, and in the post-test the percentage increased to 92.8%. In the pre-test, 89% of participants were categorized as having a high level of knowledge, while in the post-test evaluation, the percentage increased to 93.4%. This webinar could improve participants’ knowledge and therefore could be conducted in the future as part of FK UNPATTI contributions in preventing COVID-19 transmission in Ambon City. ABSTRAK Jumlah kasus dan kematian akibat COVID-19 terus mengalami penurunan seiring meningkatnya cakupan vaksinasi.  Di Indonesia, cakupan vaksinasi COVID-19 sayangnya masih belum memenuhi target nasional, termasuk di wilayah Kota Ambon. Kegiatan pengabdian masyarakat ini bertujuan untuk meningkatkan pengetahuan civitas akademika Universitas Pattimura serta tenaga Kesehatan di RSUD Dr. M Haulussy sebagai RS rujukan utama COVID-19 di Kota Ambon tentang perkembangan dan pentingnya program vaksinasi COVID-19. Kegiatan dilakukan oleh Fakultas Kedokteran Universitas Pattimura (FK UNPATTI) pada 10 Desember 2021 dalam bentuk webinar edukasi. Sebanyak 275 peserta (termasuk mahasiswa, staf pengajar, serta tenaga kesehatan RSUD Dr. M. Haulussy) mengisi secara lengkap kuesioner evaluasi (pre- dan post-test). Narasumbe dalam webinar ini berasal dari Dinas Kesehatan Kota Ambon, tim peneliti studi penerimaan vaksinasi COVID-19 dari FK UNPATTI, serta pimpinan FK UNPATTI. Pengisian kuesioner pre dan post-test menunjukkan peningkatan persentase jawaban benar peseta webinar pada evaluasi post-test. Pada pre-test, rerata persentase jawaban benar peserta adalah 87,4% sedangkan pada post-test meningkat menjadi 92,8%.  Pada evaluasi pre-test, 89% peserta dikategorikan memiliki pengetahuan tinggi, sedangkan pada post-test, persentase ini meningkat menjadi 93,4%. Kegiatan webinar ini diharpakan dapat dilakukan untuk mendukung peningkatan pengetahuan peserta dan sekaligus sebagai kontribusi FK UNPATTI dalam pengendalian transmisi COVID-19 di Kota Ambon.

Abstract

Introduction: Norwegian (crusted) scabies is one of the classic infectious variants of scabies. This variant is common in immunocompromised patients including autoimmune diseases who are taking long-term steroids. Method:We report a case of Norwegian scabies in a child with lupus nephritis.Case:A 15-year-old boy, weighing 40 kg with the chief complaint of black-red spots and thick scales on both fingers, pubic, armpit and body accompanied by itching especially at night. Complaints of night itching were also felt by the patient's mother. The patient had a history of lupus nephritis who was treated with long-term steroids. The results of microscopic examination of skin scrapings found adult mites, eggs and Sarcoptes scabiei schibala. The diagnosis of Norwegian scabies is based on anamnesis, physical and microscopic examination. Results: he patient was treated with 5% permethrin cream, 2-4 ointment 2x a day, cetirizine 1 x 10 mg per day for 2 weeks and education on maintaining personal and environmental hygiene gave healing to the patient. Conclusion: Norwegian scabies therapy is a combination of topical scabidal agents, keratolytic agents to facilitate drug penetration and antihistamines to reduce nocturnal pruritus and education provides clinical improvement and prevents recurrence.

Abstract

Acne vulgaris is a chronic inflammation of the pilosebaceous unit with multifactorial causes and clinical manifestations in the form of comedones, papules, pustules, nodes, and cysts. According to the Global Burden of Disease (GBD) study, acne affects 85% of adolescents to young adults aged 12-25 years. Acne vulgaris has an impact both physically and psychologically, including a person's self-confidence. The purpose of this study was to determine the relationship between the severity of acne vulgaris with the level of self-confidence of the students of the Faculty of Medicine, University of Pattimura. This type of research is quantitative analytic with a cross-sectional study approach. Research respondents were students of the Faculty of Medicine, Pattimura University, Ambon with acne with a total of 83 respondents who were selected by simple random sampling technique. Characteristics of respondents based on age and gender were dominated by respondents aged 19 years (31.3%) and females (86.7%). The results showed that most of the respondents experienced mild acne as many as 54 respondents (65.1%), mostly dominated by moderate-high self-confidence as many as 55 respondents (66.3%). The results showed that there was a significant relationship between the severity of acne vulgaris and the level of confidence in the students of the Faculty of Medicine, Pattimura University, Ambon with p = 0.001. The result of this study can be used as reference material for further research on other causal factors besides acne vulgaris that affect self-confidence.
