Abstract

Background: HIV, syphilis, and hepatitis B can be transmitted during pregnancy, which can cause morbidity and mortality. Efforts to eliminate the transmission of IV, syphilis, and hepatitis B are contained in the Minister of Health Regulation through Antenatal Care services in community health centers by screening and case management of positive pregnant women. The coverage of triple elimination in Eastern Indonesia has not been implemented, especially in Ambon City, while the goal in 2022 is that 100% of pregnant women will be screened and managed to prevent perinatal transmission. Purpose: This study aimed to evaluate the implementation of triple elimination screening including coverage, completeness, and infection rates in Ambon City. Methods: This study is a descriptive retrospective analysis based on medical records from four Community Health Centers during the year 2022. Result: Among 1,666 pregnant women, only one of the four centers (25%) met the national screening coverage target. None achieved 100% test completeness. Syphilis showed the highest infection rate (up to 12.2%), while hepatitis B rates were below the national threshold (≤7.1%) in all centers. Conclusion: Ambon City's triple elimination screening implementation has not met national criteria. Enhancing program efficacy requires expanding access to screening services and bolstering health education.

Abstract

Molluscum contagiosum (MC) is a skin infection caused by the Molluscipox virus and is common in children and adults with active sexual activity. The disease is characterized by the appearance of small, smooth, papule-shaped lesions that have a hollow in the center (umbiliation). MC transmission takes place through direct contact with contaminated lesions or objects, such as towels and clothing, so the spread is quite wide globally. The diagnosis is established through an anamnesis, clinical examination, and physical evaluation. MC therapy is generally supportive, but in certain cases medical measures such as excision and curettage are required to remove the lesion effectively. The use of topical creams containing Fusidic acid, such as Fucilex cream, is often recommended to prevent secondary infections and speed up the postoperative healing process. Proper and prompt treatment is essential to prevent further complications and speed up the patient's recovery. MC has a psychosocial impact, especially on children, so education to parents and health workers is an important part of the treatment strategy. Prevention can be done through increased personal hygiene and avoidance of contact with contaminated objects. The study emphasizes the importance of a holistic approach in diagnosis, therapy, and education to reduce the prevalence of MC and its impact on public health.

Abstract

Jellyfish are one of the dangerous sea animals and often attack humans through their stings. Jellyfish contain toxins in the form of cardiotoxins, neurotoxins, and myotoxins which when exposed to humans can cause shock and even death. The purpose of this study was to determine the level of knowledge of the community in Luhu Village regarding the initial handling of jellyfish stings. This study is a descriptive quantitative study using a cross sectional design with a community sample of 200 samples. Data collection using questionnaires. The questionnaire data is then processed and analyzed using the SPSS program. The results of the research based on the characteristics of respondents, the most respondents found in this study were female as many as 118 respondents (59%), the age of respondents was adult age as many as 114 respondents (57%), many respondents' occupations were farmers namely 64 respondents (32%), while the level of education that was found was high school which was 113 respondents (52%). Of the 200 respondents, 47 respondents (23.5%) had less knowledge, 133 respondents (56.5%) had sufficient knowledge, and 40 respondents (20%) had good knowledge. Knowledge is very influential on how to handle jellyfish stings early. Therefore, there is a need for socialization and counseling efforts as well as training by health workers to the community in order to increase the level of public knowledge about early handling of jellyfish stings.

Abstract

Introduction: Psoriasis is a chronic-relapse erythrosquamous dermatoses disease. Psoriasis occurs in any age with one-third of the cases start during childhood. Strategy in managing psoriasis is based on the severity and consideration towards patient’s medical condition. Case: A 12-year-old male child was admitted to dermatovenereologist clinic with reddish patch covered in thick scales that occur on almost his entire body. The condition, started when he was 3 years old, was oftentimes relapsing and worsened within the last 2 months. Patient frequently went to doctor and prescribed with ointment. The condition keeps relapsing, though there was improvement after treatment. Physical examination showed sharply marginated erythrosquamous macules are over 30% of body surface area, Kaarvetsvlek phenomenon, Auzspit sign, and PASI score 30.8. Histopathological examination present elongation of rete ridges and microabscess Munro. Patient were given 1 cycle of Methotrexate (MTX) and showed significant improvement in PASI score. Discussion: Types of psoriasis treatment are topical, systemic, phototherapy and biologic agent. MTX still remain one of the systemic treatment especially for moderate to severe psoriasis in children. Oral MTX is recommended because it is more convenient than parenteral administration. Conclusion: Pediatric psoriasis occurs in one-third of the cases and may affect patient’s physical and psychological health. MTX acts as antiproliferation and antiinflammation treatment for moderate-severe psoriasis and recalcitrance lesion to topical steroid. Education about the disease is important for treatment success and risk of relapse reduction.
