Abstract

Background: Children's physical and cognitive development are negatively impacted by stunting, a chronic nutritional problem. Stunting prevention is greatly aided by family support, especially when it comes to mothers' views as primary caregivers toward proper childcare and nutrition. One promising family-based strategy for enhancing maternal attitudes toward stunting prevention is grandmother-led health education. Objective: This study aimed to examine the effect of grandmother-led health education on mothers' attitudes toward stunting prevention. Methods: This quantitative pre-experimental study employed a one-group pretest–posttest design involving 25 grandmothers and 25 mothers living in the same household. Grandmothers participated in two 90-minute structured training sessions on stunting prevention before delivering a standardized 90-minute face-to-face health education session supported by an educational video covering stunting definition, risk factors, consequences, infant feeding, balanced nutrition, exclusive breastfeeding, and growth monitoring. Grandmothers' competency was assessed through structured questioning and direct observation using predefined competency criteria before they were allowed to conduct the educational sessions. Mothers' attitudes were measured using a validated and reliable questionnaire before and immediately after the intervention. Data were analyzed using the Wilcoxon signed-rank test. Results: The mean maternal attitude score increased from 62.30 before the intervention to 90.20 after the intervention, representing a mean improvement of 27.90 points. The Wilcoxon Signed-Rank Test indicated a statistically significant difference between pretest and posttest scores (Z = −4.235, p < 0.001). Conclusion: Grandmother-led health education using educational video media significantly improved mothers' attitudes toward stunting prevention. Integrating grandmothers into family-based health promotion programs may strengthen maternal engagement in stunting prevention and contribute to improving child nutritional outcomes.

Abstract

Background: Health cadres are strategically positioned to deliver cervical cancer prevention messages, but limited training and educational resources may reduce their motivation to provide counseling. Objective: To determine changes in cadres' motivation to provide cervical cancer prevention counseling after receiving combined video education and a guidebook. Methods: A pre-experimental one-group pretest-posttest study was conducted in Wijimulyo Village, Nanggulan, Kulon Progo, Indonesia, in March 2026. Twenty-five female health cadres were selected by simple random sampling from a population of 110. Motivation was measured using a 19-item Likert questionnaire with a raw score range of 0-57. The instrument had item-total correlations above 0.361 and a Cronbach's alpha of 0.725. Participants completed a pretest, watched a 2-minute 47-second educational video, participated in a researcher-guided review of the three guidebook modules, and joined a facilitated discussion in which key messages were clarified and linked to their counseling roles before completing an immediate posttest. The educational intervention, including the video presentation, structured guidebook review, and facilitated discussion, lasted approximately 60 minutes. Paired scores were analyzed using the Wilcoxon signed-rank test. Results: The median motivation score increased from 36 (interquartile range [IQR] 34-37) before the intervention to 37 (IQR 35-41) afterward. Thirteen cadres had higher scores, six were unchanged, and six had lower scores. The increase was statistically significant (Z = -2.28, p = 0.023) with an effect size of r = 0.52. Conclusion: Combined video education, structured guidebook review, and facilitated discussion were associated with an immediate increase in cadres' self-reported motivation to provide cervical cancer prevention counseling. Actual counseling performance and community screening participation were not measured; therefore, no conclusions can be drawn regarding these outcomes. Follow-up assessment is needed to determine whether the motivational change is sustained over time.
