Abstract

ABSTRAKLatar belakang: Masalah infertilitas dihadapi 15-20% pasangan usia subur yang membawa dampak masalah pada sosial dan psikologis kepada pasangan, keluarga maupun masyarakat. Secara garis besar, pasangan yang mengalami infertilitas akan menjalani proses panjang, dimana proses ini dapat menjadi beban fisik dan psikologis bagi pasangan infertilitas. Saat ini terdapat 12% pasangan infertilitas yang tersebar di seluruh Indonesia baik di desa maupun di kota. Mengingat pelayanan infertilitas yang masih belum merata dan hanya terpusat pada kota-kota yang besar. Pelayanan kesuburan belum merambah ke seluruh lapisan masyarakat.Metode: Penelitian ini bertujuan untuk melihat perbedaan hambatan terhadap akses pelayanan infertilitas pada masyarakat urban dan rural yang berobat di Klinik Bayi Tabung Halim Fertility Center RSIA Stella Maris Medan. Jenis penelitian yang digunakan dalam penelitian ini adalah kuantitatif dengan teknik wawancara menggunakan kuesioner. Data yang diperoleh dilakukan analisis deskriptif dan analitik. Sebanyak 130 responden terbagi dalam dua kelompok masing masing 65 responden kelompok rural dan 65 responden kelompok urban.Hasil: Pada penelitian ini ditemukan tiga variabel yang signifikan (p=0,014, p=0,023 dan p=0,005) dalam analisis multivariat yaitu tingkat ekonomi dengan nilai OR 2,606 (95% IK 1,210-5,611), letak geografi dengan nilai OR 3,905 (95% IK 1,203-12,677), dan sosial budaya dengan nilai OR 5,299 (95% IK 1,659- 16,929).Simpulan: Terdapat perbedaan bermakna tingkat ekonomi, letak geografi dan sosial budaya pada pasien kawasan perkotaan dan perdesaan dalam akses pelayanan infertilitas di Klinik Bayi Tabung Halim Fertility Center RSIA Stella Maris Medan. Kata kunci: hambatan akses, infertilitas, urban, ruralABSTRACTTitle: Infertility Care Access Barriers in Patient of Urban and Rural Areas of the Treatment Clinic IVF Fertility Center RSIA Halim Stella Maris  Background: The problem of infertility is faced by 15-20% of couples of childbearing age that have an impact on social and psychological problems for couples, families and communities. Broadly speaking, infertile couples will undergo a long process, where this process can be physical and psychological burden for the couple infertility. Currently, there are 12% of couples infertility scattered throughout Indonesia, both in villages and cities. Given the infertility services that are still not evenly distributed and only focused on large cities. Fertility services have not penetrated to the whole society.Method: The research looked at how different barriers to access to infertility services in urban and rural communities who seek treatment at the Fertility Clinic Halim tube baby center RSIA Stella Maris Medan. This type of research used in this study is quantitative with interview techniques using a questionnaire. The data obtained were analyzed descriptively and analytically. A total of 130 respondents were divided into two groups each 65 rural respondents and 65 urban respondents.Result: This study found three significant variables (p = 0.014, p = 0.023 and p = 0.005) in multivariate analysis that economic level with OR 2.606 (95% CI 1.210 to 5.611), geography with OR 3.905 (95% CI 1.203 to 12.677), and socio-cultural with OR 5.299 (95% CI 1,659- 16.929).Conclusion: There are significant differences in economic levels, geographical and socio-cultural patient urban and rural areas in access to infertility services at the Clinic IVF Fertility Center RSIA Halim Stella Maris Medan. Keywords: barriers to access, infertility, urban, rural

Abstract

The beginning of 2020 was shocked by the WHO's statement which mentioned Coronavirus-19 (COVID-19) as a pandemic. As many as 37420 confirmed cases of COVID-19 were reported by the Ministry of Health of the Republic of Indonesia until June 2020 and are growing every day. One of the supporting examinations to establish a diagnosis of COVID-19 is a radiological examination. It is undeniable that the increasing incidence of COVID-19 has caused the workload of health workers in hospitals is increase, including in radiology installations in hospital. The purpose of this study was to analyze the need of workforce according to the workload using the Workload indicators of staffing need (WISN) method in the radiology installation of the RSU X. This research uses quantitative and qualitative methods. The results showed that the workload of the workforce in the radiology installation at RSU X was in the optimum category. Based on calculations using the WISN method, it was found that the total number of workers needed at the radiology installation at the RSU X was 15 people and the workforce gap ratio was 1.00.

Abstract

Hipertensi ialah ketika tekanan darah sistolik > 140 mmHg dan tekanan darah diastolik mencapai > 90 mmHg.Gejala penyakit hipertensi dapat mempengaruhi gangguan kualitas tidur penderita dan sebagian penderitakadang tidak merasakan gejala sehingga hipertensi disebut dengan Silent Killer. Pada usia lanjut dibutuhkanwaktu tidur 5-8 jam untuk memelihara keadaan fisik karena semakin bertambah usia fungsi organ tubuh semakinberkurang. Penelitian berikut bertujuan untuk menilai hubungan kualitas tidur dengan pasien hipertensi diRumah Sakit Umum Royal Prima Medan. Jenis penelitian ini adalah penelitian observational analitik sertamenggunakan desain cross- sectional. Sampel yang diperlukan pada penelitian ini berupa 38 responden yangmemenuhi kriteria inklusi dan eksklusi. Sampel pada penelitian ini diambi dengan teknik purposive sampling.Instrumen penelitian kuesioner PSQI dan tensimeter digital dengan teknik analisis data ialah analisis univariatuntuk menjabarkan penyebaran angka karakteristik responden (umur dan jenis kelamin), kualitas tidur danhipertensi. Analisis bivariat memakai uji statistik chi square dan korelasi spearman melalui derajat kemaknaan5%. Hasil penelitian berdasarkan karakteristik usia diperoleh mayoritas responden lansia akhir (56-65 tahun)sebanyak 13 orang (34,2%). Karakteristik jenis kelamin diperoleh mayoritas responden ialah perempuansebanyak 20 orang (52,6%). Distribusi frekuensi tekanan darah responden didapatkan mayoritas respondenhipertensi stadium 1 dan stadium 2 sebanyak 14 orang (36,8%) dan kualitas tidur responden ditemukankebanyakan kejadian berupa kualitas buruk dengan total 23 orang (60,5%). Berdasarkan hasil uji korelasiSpearman diperoleh p value sebesar 0,029 (p<0,05) dimana terdapat hubungan kualitas tidur dengan pasienhipertensi di RSU. Royal Prima Medan dan didapatkan hasil correlation coefficient (r) sebesar 0,852 yang dapatdinyatakan bahwa memiliki kekuatan hubungan yang sangat kuat.Kata Kunci : Kualitas Tidur, Hipertensi

Abstract

Indicators showing how needs of steam in health facilities based on workload , so that the location / relocation would be easier and rational known as the workload indicators of staffing need ( wisn ) .Meeting the needs of of human resources in hospital was a the tone for hospital management .The management of the hospital have to are able to count needs human resources precisely on demand in each service unitMeeting the needs of excess of human resources in the hospital give a consequence labor costs high and gives rise to one of inefficiency of in financial management at the hospital .Lack of meeting the needs of human resources hospital give the effect of the workloads high to health workers and led the decline in of satisfaction with patient care , increase desire move work and diminishes the quality of patient safety. The number of human resources varied at every hospital in the city of medan.To achieve, vision and mission hospital skill and the ability of human resources needed to solve all problems is.From observation carried out in several hospitals in the city of medan, had shown an imperfection in planning the health workers and have not managed in a professional based on the workloads to the needs of human resources in every hospital. As a result , the available health numbers of workers have not been in accordance with a load of work and the real needs of in the field .The purpose of this research to analyze the needs of paramedics by counting discrepancy between the number of ideal paramedics ( skilled manpower including nurses and energy pharmacy ) by the fact that is said wisn difference and counting the workloads .The results of the study stated that the needs of labor is in accordance with the workloads is in some hospitals.

Abstract

This study aims to determine the factors associated with the use of the JKN program, namely individual characteristics (age, education, and employment), capability characteristics (availability of health workers and location accessibility), and characteristics of needs (illness perception). The populations in this study are all people who underwent health services at Batang Toru Health Center in South Tapanuli Regency with a total sample of 98 people. Data were collected using a questionnaire and analyzed using the Pearson Correlation test and multiple logistic regression tests. The results showed that there was a significant relationship with age (p = 0,000); education (p = 0.001); occupation (p = 0,000); availability of health workers (p = 0,000); location accessibility (p = 0,000) and pain perception (p = 0, 000) with the use of the JKN program at Batang Toru Health Center. The results of paper multivariate analysis and modeling showed that the factors that were significantly and dominantly related to the use of the JKN program at Batang Toru Health Center were the availability of health workers, location accessibility factors, and illness perception factors. Thus it was concluded that the availability of health workers, accessibility of location, and perception of pain were significantly and dominantly related to the use of the National Health Insurance (JKN) program at Batang Toru Health Center in South Tapanuli Regency in 2019 of health workers is the most dominant factor and has the biggest influence on the use of the JKN program location accessibility factors and illness perception factors.

Abstract

This research aims to determine the effectiveness of the antibacterial soursop leaf extract (Annona muricata L.). The method used is the agar diffusion method. The leaves were extracted by maceration method, using ethanol as a solvent. The soursop leaf extract obtained was then diluted using ethanol 96% at a concentration of 20%, 40%,60%, 80%. Positive control using clindamycin and negative controls using sterile aquadest. The research model used in this study is an experimental research model In Vitro of type. The results showed that soursop leaf extract had antibacterial effectiveness against Propionibacterium acnes with an average inhibition zone diameter of 9,7 mm; 13,7 mm; 15,7 mm; 16,3 mm; positive control (clindamysin) 21,0 mm; and negative control (aquadest) have no value tobe able to inhibit bacterial growth.

Abstract

Type 2 diabetes is a metabolic disorder caused by insulin resistance and is associated with oxidative stress. In Indonesia, the mean prevalence of diabetes ranges from 1.4% to 1.6%; however, some areas have a much higher diabetes prevalence such as Pekajangan (2.3%) and in Manado (6%). The 2013 Indonesian Basic Health Research (IBHR) stated that the number of people with DM in Indonesia has reached an alarming rate. This study aimed to determine the antidiabetic effect of Polyscias scutellaria on alloxan-induced male Wistar. This was an experimental study conducted in July 2020 in the Faculty of Medicine, Prima Indonesia University. This study used 25 rats that were grouped into 5 treatment groups: control group (Na-CMC), standard (Metformin), and 3 extract groups with different doses (125 mg/kg BW, 250 mg/kg BW, and 500 mg/kg BW). Mangkokan leaf extract was obtained through the maceration method. All ratswere induced intraperitoneally using alloxan monohydrate 10% at a dose of 175 mg/kg BW. The parameters used in this study were fasting blood glucose level before induction, after induction, and after treatment and body weight before treatment. It was observed that there was a significant change in blood glucose level between the extract groups. The blood sugar level in the 125 mg/kg BW group was 495.00 mg/dl while in the 250 mg/kg BW and 500 mg/kg BW, the blood glucose levels were 317.00 mg/dl and 126.00 mg/dl, respectively, with the 500 mg/Kg BW dose as the most effective dose (P-value = 0.001). Thus, mangkokan leaves have the potential to reduce blood glucose level but are not as good as the standard group.

Abstract

ABSTRAKDaun salam (Syzygium polyanthum) banyak digunakan sebagai obat tradisional untuk mengobati berbagai macam penyakit, salah satunya sebagai antimikroba. Penelitian ini bertujuan untuk mengetahui aktivitas antimikroba dan konsentrasi efektif dari ekstrak daun salam (S. polyanthum) terhadap Pityrosporum ovale. Pengujian menggunakan rancangan acak lengkap faktorial dan metode pengujian antijamur dengan metode disc diffusion. Data diolah dengan SPPS versi 23. Konsentrasi ekstrak daun salam (S. polyanthum) yang digunakan yakni 25%, 50%, 75%, dan 100%. Hasil penelitian menunjukkan adanya potensi antimikroba pada semua konsentrasi ekstrak daun salam (S. polyanthum) yang ditunjukkan dengan zona bening di sekitar paper disk. Zona bening terbesar adalah 18,2 mm dan 15,03 mm diperoleh dari ekstrak daun salam (S. polyanthum) dengan konsentrasi 100%. Kata kunci: antifungi; disc diffusion; Pityrosporum ovale; Syzygium polyanthum ABSTRACTEffectiveness of salam leaves (Syzygium polyanthum) extract against Pityrosporum ovaleBayleaf (Syzygium polyanthum) is a plant that is used as a traditional medicine to treat various diseases. One of them is as an antimicrobial. This study aims to determine the antimicrobial activity and effective concentration of bayleaf extract against P. ovale. This research use a factorial completely randomized design and the antifungal disc diffusion method. The data were processed using SPPS version 23. The bayleaf (S. polyanthum) extract concentrations are 25%, 50%, 75%, and 100%. The results showed the presence of antimicrobial potential in all concentration of bayleaf (S. polyanthum) extracts indicated by the clear zone around the paper disk. The largest clear zones were 18.2 mm and 15.03 mm obtained 100% bayleaf (S. polyanthum) extracts. Keywords: antifungus; disc diffusion; Pityrosporum ovale; Syzygium polyanthum

Abstract

Dermatitis kontak merupakan dermatitis yang disebabkan oleh bahan/substansi yang menempel pada kulit. Dermatitis terbagi menjadi dua yaitu : Dermatitis Kontak Iritan (DKI) dan Dermatitis Kontak Alergi (DKA). Menurut Data yang dipublikasikan oleh Departemen Kesehatan Tahun 2008 mengenai insiden dan prevalensi dermatitis di Provinsi Sumatera Utara Sebesar 26,3 %. Tujuan dari penelitian ini adalah untuk mengetahui bagaimana karakteristik dermatitis kontak di Poli Klinik Kulit Kelamin RS. Putri Hijau Medan Tahun 2016. Karakteristik dalam penelitian ini meliputi usia, jenis kelamin dan pekerjaan. Dari hasil penelitian ini didapati bahwa Dermatitis Kontak Iritan (DKI) merupakan jenis dermatitis dengan kasus tertinggi (77%) dibanding dengan dermatitis kontak alergi (23%). Dengan jenis kelamin perempuan paling banyak dijumpai (62%). Dan Kelompok Umur 25-44 tahun (40%) paling banyak dijumpai menderita dermatitis Kontak.

Abstract

Chronic kidney disease (CKD) is a condition in which there is a gradual decline in kidney function. This disease is progressive and usually irreversible. Indications for hospitalized CKD patients with severe complications and do not allow inpatient therapy. This study was aimed to describe the overview and management of chronic kidney disease inpatient in Royal Prima General Hospital Medan. The type of research used is research with a descriptive and retrospective case study design. This study is based on medical record data related to inpatient CKD patients' diagnostic and management features at the Royal Prima General Hospital Medan in 2020. The research subjects were all inpatients diagnosed with CKD, whereas 100 research subjects with complete medical records were included in this study. It was found that the highest age group was 57-65 years, as much as 37.0%, and men as much as 27%. The chief complaint was shortness of breath at 43% and additional complaints, the most of which were fever + low back pain + edema at 38%. On physical examination, most of the inspections were weak, 55%. Most palpation was sociable 57%. There was auscultation of four abdominal regions and normal positive bowel sounds (93%). Complete blood count + blood sugar level + electrolytes + urea + creatinine 70%, the most combination medication is NaCl 0.9% + Furosemide injection 31%. Based on the length of stay, the longest was 13-14 days (20%).
