Abstract

Introdution: Clinical pathway (CP) is a disease management tool used to reduce unnecessary variations in services, increase clinical outcomes, and control resources. Acute appendicitis is one cause of acute abdominal pain, where cases of appendix perforation range from 20-30% increasing to 32-72% at the age of more than 60. Methods: This research was conducted using mix method with case study design. Qualitative data were taken by interview and observation. Quantitative data were taken with a simple description from the acute appendicitis CP documentation in medical records and ICPAT (January - September 2020, n = 117). Result: According to the result of the research, the level of completeness for CP was 100% and form filling was 85%. Conclusion: ICPAT dimension 1 made sure that the form was the clinical pathway. The content and quality were good, and dimension 2 assesses the CP documentation process. The content is lacking and the quality is moderate, dimension 3 assess the CP development process which content was good, and quality was moderate, and dimension 4 assesses the CP implementation process. The content was moderate, and the quality was good. Dimension 5 is to assess the maintenance of clinical pathways in which content was lacking and quality was moderate. Dimension 6 serves to assess the role of the organization that content was good and quality was moderate where the obstacles were due to lack of understanding and time constraints.

Abstract

Choosing a location for delivery by pregnant women and deliveries assisted by skilled health workers are important keys in reducing MMR and IMR. The low birth rate at health facilities in Mandrehe Utara Subdistrict, Nias Barat Regency is a serious challenge to achieve the success of the maternal and child health service program. This study analyzes the factors that influence pregnant women who do not give birth in health facilities. This study uses a quantitative method with a cross-sectional approach. The samples were 180 people. The data were collected using a questionnaire and then tested using the Chi-Square test and logistic regression test. The results showed that the reason why mothers did not choose to give birth in health facilities was influenced by knowledge (p=0.022), attitude toward birth attendants (p=0.002), perception of distance (p=0.004), husband and family support (p=0.000), and access to health information (p=0.007). The role of health workers had no significant effect in this study (p=0.756). The results of the logistic regression test showed that husband and family support was the most dominant variable affecting mothers who did not give birth in health facilities. Mothers who perceive the lack of support from their husbands and families are 5,668 times more to choose to give birth outside a health facility. Education and socialization by health workers to mothers and their husbands and families are suggested to increase knowledge, awareness and change attitudes. No less important are the government's efforts to provide health facilities and infrastructure development to reduce access difficulties.Pemilihan lokasi persalinan oleh ibu hamil dan persalinan yang ditolong tenaga kesehatan terampil menjadi kunci penting dalam menurunkan AKI dan AKB. Rendahnya angka persalinan di fasiltas kesehatan di Kecamatan Mandrehe Utara Kabupaten Nias Barat menjadi tantangan berat untuk mencapai keberhasilan program pelayanan kesehatan ibu dan anak. Studi ini bertujuan menganalisis faktor-faktor yang mempengaruhi ibu hamil tidak bersalin di fasilitas kesehatan. Studi ini menggunakan metode kuantitatif dengan pendekatan cross-sectional. Sampel yang terlibat sebanyak 180 orang. Pengumpulan data menggunakan kuesioner dan kemudian diuji dengan menggunakan uji Chi-Square dan uji regresi logistik. Hasil penelitian menujukkan penyebab ibu tidak memilih bersalin di fasilitas kesehatan dipengaruhi oleh pengetahuan (p=0,022), sikap terhadap penolong persalinan (p=0,002), persepsi tentang jarak (p=0,004), dukungan suami dan keluarga (p=0,000), dan akses terhadap informasi kesehatan (p=0,007). Peran petugas kesehatan tidak berpengaruh signifikan dalam studi ini (p=0,756). Hasil uji regresi logistik menunjukkan dukungan suami dan keluarga merupakan variabel yang paling dominan mempengaruhi ibu tidak melahirkan di fasilitas kesehatan. Ibu yang menganggap kurangnya dukungan suami dan keluarga berpeluang 5,668 kali memilih melahirkan di luar fasilitas kesehatan. Edukasi dan sosialisasi oleh tenaga kesehatan terhadap ibu beserta suami dan keluarga disarankan untuk peningkatan pengetahuan, kesadaran dan perubahan sikap. Tidak kalah penting upaya dari pemerintah untuk penyediaan fasilitas kesehatan dan pembangunan infrastruktur guna memangkas kesulitas akses.

Abstract

The incidence of stunting in children under five is still the main cause of death in children under five in the world because the impact of stunting is not only on health problems but is closely related to the level of intelligence of children. This study aims to determine the prevalence and investigate the determinants of stunting in children under five. This study is quantitative research with a cross-sectional design. The population of this study was all mothers of children under five who visited the Mandrehe Barat Non-Inpatient Puskesmas in 2021, and the research sample was 200 mothers of children under five. Data was collected directly by distributing questionnaires to mothers of children under five. Then, continued with the process of data analysis with the chi-square test and binary logistic regression. The results showed that exclusive breastfeeding (p= less than 0.001; PR= 9.5; 95% CI 6.788-22.948), feeding patterns (p= less than 0.001; PR = 7.6; 95%CI 4.833-12.266), and consumption of vitamin A and zinc during pregnancy (p= less than 0.001; PR = 4.4; 95%CI 3.172-6.043) had a significant effect on the incidence of stunting, but a history of low birth weight (p= 0.785; PR= 1.08; 95% CI 0.372-3.306) and history of infectious disease (p=0.111; PR=1.5; 95%CI 0.978-2.441) were not significant for the incidence of stunting. Furthermore, the dominant factor in the incidence of stunting shows that toddlers who are stunted are 5.7 times more likely not to be given exclusive breastfeeding compared to toddlers who are not stunted. Thus, mothers of children under five are expected to take the time to visit health services by bringing their children under five to monitor their growth and development of children such as height and weight according to the child's age. Abstrak: Kejadian stunting pada anak balita hingga saat ini masih merupakan penyebab utama kematian pada anak balita di dunia, karena dampak stunting bukan hanya pada masalah kesehatan, namun, sangat erat kaitannya pada tingkat kecerdasan anak. Penelitian ini bertujuan untuk mengetahui prevalensi dan menyelidiki determinan kejadian stunting pada balita. Studi ini merupakan penelitian kuantitatif dengan desain crossectional. Populasi penelitian ini adalah seluruh ibu balita yang berkunjung di Wilayah Puskesmas Non Rawat Inap Mandrehe Barat Tahun 2021, dan sampel penelitian sebanyak 200 orang ibu balita. Pengumpulan data dilakukan secara langsung dengan menyebarkan kuesioner kepada ibu balita. Kemudian, dilanjutkan dengan proses analisis data dengan uji Chi Square dan regresi logistik biner. Hasil penelitian menunjukkan bahwa pemberian ASI Eksklusif (p=kurang dari 0,001;PR=9,5;95%CI 6,788-22,948), pola pemberian makan (p= kurang dari 0,001; PR=7,6; 95%CI 4,833-12,266), dan konsumsi vitamin A dan Zinc pada masa kehamilan (p= kurang dari 0,001;PR=4,4; 95%CI 3,172-6,043) berpengaruh signifikan terhadap kejadian stunting, namun riwayat BBLR (p=0,785; PR=1,08;95% CI 0,372-3,306) dan riwayat penyakit infeksi (p=0,111; PR=1,5; 95%CI 0,978-2,441) tidak signifikan terhadap kejadian stunting. Selanjutnya, faktor dominan terhadap kejadian stunting menunjukkan bahwa balita yang mengalami stunting 5,7 kali kecenderungannya tidak diberi ASI Eksklusif dibanding dengan balita yang tidak mengalami stunting. Dengan demikian, kepada ibu balita diharapkan agar melungkan waktu untuk berkunjung ke tempat pelayanan kesehatan dengan membawa anak balita untuk memantau tumbuh kembang anak seperti tinggi dan berat badan sesuai usia anak.

Abstract

There are many problems arise due to incomplete medical record files and do not recorded neatly and regularly. The incident is due to lack of medical team awareness in recording all information or actions that would be given to the patient. The aim of this study is to analyse the effect of Job Demand-Control-Support and Protection Motivation on compliance in filling out Medical Record files at RSU ESHMUN Medan 2023. This study uses quantitative research method which is analytical in nature. The total sampling of this study is 115 active nurses at RSU ESHMUN Medan. The result of the study informs that there are very strong and significant effect among Job Demand-Control-Support and Protection Motivation on compliance in filling out Medical Record files at RSU ESHMUN Medan 2023. The variable that has the most influence on Obedience with filling in inpatient medical record files at RSU ESHMUN in 2023 is the Protection Motivation variable. It is recommended to RSU ESHMUN Medan to improve the administration pattern in order to all of nurses have the same job demands and receive the same support

Abstract

The task of handling and caring for people infected with COVID-19 places the position of health workers in a high-risk group. Health workers not only face physical problems when infected, the mental burden due to fears of infecting family members while doing self-isolation, and the negative stigma that develops in the community. This research is a qualitative descriptive study using a phenomenological study design. The phenomenon studied is about the experience and quality of life of health workers infected with COVID-19 in Gunungsitoli City. The study was conducted from April to May 2021. In collecting data, researchers interviewed 10 health workers consisting of 2 doctors and 8 nurses. The processed data are displayed in narrative text. The results showed that infected health workers experienced stress and physical disorders such as fever, difficulty breathing, tired easily to digestive disorders that hindered physical activity and social interaction with family members. In this study, the role of psychological support from family and co-workers is quite important in improving the health status of patients who are self-isolating. Given the negative stigma toward patients and health workers infected with COVID-19, efforts to overcome the negative stigma require collaboration from the government, mass media, and religious leaders as well as community leaders. Abstrak: Tugas penanganan dan perawatan orang yang terinfeksi COVID-19 menempatkan posisi petugas kesehatan dalam kelompok beresiko tinggi. Petugas kesehatan tidak hanya menghadapi gangguan fisik ketika terinfeksi, beban mental akibat kekhawatiran menulari anggota keluarga saat melakukan isolasi mandiri dan stigma negatif yang berkembang di masyarakat. Penelitian ini merupakan penelitian deskriptif kualitatif dengan menggunakan desain studi fenomenologi. Adapun fenomena yang diteliti adalah mengenai pengalaman dan kualitas hidup tenaga kesehatan yang terinfeksi COVID-19 di Kota Gunungsitoli. Penelitian dilaksanakan pada bulan April hingga Mei 2021. Dalam pengumpulan data, peneliti mewawancarai sebanyak 10 orang tenaga kesehatan yang terdiri dari 2 dokter dan 8 perawat. Data yang telah diolah ditampilkan dalam teks naratif. Hasil penelitian menunjukkan tenaga kesehatan yang terinfeksi mengalami stress dan gangguan fisik seperti demam, susah nafas, mudah lelah hingga gangguan pencernaan menghambat aktivitas fisik dan interaksi sosial dengan anggota keluarga. Dalam studi ini, peran dukungan psikis dari keluarga dan rekan kerja cukup penting dalam perbaikan status kesehatan pasien yang isolasi mandiri. Mengingat stigma negatif terhadap pasien dan tenaga kesehatan yang terinfeksi COVID-19, upaya untuk mengatasi hal tersebut memerlukan kolaborasi pemerintah, media massa dan tokoh agama serta tokoh masyarakat.

Abstract

Waiting time in patient care the treatment is a problem that often causes patient complaints in various health institutions. The time a patient remains at registration, examination, and drug taking reflects how the agency manages the service component tailored to the patient's situation and expectations. This research aims to see the waiting time with patient satisfaction. This study uses a research design analytic observational with a cross-sectional study approach conducted at the hospital Royal prima. The sampling technique uses purposive sampling with as many as 170 respondents. Analysis test with chi-square test with a significance level of p < 0.05. From the chi-square test results, the waiting time for registration and examination drug on patient satisfaction obtained p value = 0.000 and 0.021 <0.05 means Ho is rejected and Ha accepted, implying a relationship between the waiting time for registration and inspection with patient satisfaction. Meanwhile, the time of taking the drug on patient satisfaction is obtained p value = 0.400> 0.05 means Ho is accepted and Ha is rejected, meaning there is no relationship between the time Wait for taking medication and patient satisfaction. So, it is recommended that officers be more committed and disciplined in providing appropriate services with a predetermined schedule.

Abstract

Brand equity will make it easier for the hospital to market and determine the patient's wishes. This research is to find out how the Brand Equity Elements at RSU Wira Husada Kisaran. The sample size in this study was 20 samples. Research time was conducted since the author surveyed in January 2022 until data collection, followed by a seminar on results. The results showed that only the brand loyalty variable did not affect the interest of patients in reusing Wira Husada Kisaran Hospital, for variables brand awareness, perceived quality, association, word of mouth, and marketing mix affect the patient's interest in reusing Wira Husada Kisaran Hospital. The suggestion is that RSU Wira Husada Kisaran should optimize patient satisfaction and meet patient needs by adding health services that are to patient needs and improving service quality.is the new face of Turkey under Erdogan's political management

Abstract

Nurse performance is one of the determinants of hospital success rate in providing health services. Efforts to improve nurse performance include paying attention to nurses' work stress. The purpose of this study was to determine the effect of communication and incentives for nurses' performance in the inpatient room at Royal Prima Hospital Medan. This type of cross-sectional research with an explanatory research method approach. The total population of 41 nurses and all nurses used as respondents (total sampling). Data were analyzed using the chi-square test and logistic regression was used to obtain an OR (Odds Ratio) value. The results showed nurses with good communication would increase performance by 6.7 times (p = 0.012: OR = 6.708) and the provision of incentives would increase performance by 6.6 times (p = 0.006; OR = 6.667).

Abstract

Bidan sebagai salah satu petugas kesehatan mempunyai waktu yang banyak untuk berinteraksi dengan pasien bersalin. Dengan adanya upaya kesehatan ibu bersalin maka peran tenaga kesehatan sangatlah penting, karena bidan biasa memberikan keyakinan dan kepercayaan kepada ibu bersalin serta menuntun pelaksanaan IMD sesaat setelah bayi lahir. Kesalahan yang sering dilakukan adalah bayi yang baru lahir sudah dibungkus dengan kain sebelum diletakkan di dada ibunya dan kesalahan lain adalah bayi bukannya menyusu tetapi disusui. Penelitian ini bertujuan menganalisis pengaruh lama kerja dan pelatihan terhadap pelaksanaan inisiasi menyusui dini. Penelitian ini bersifat observasional analitik dengan sampel penelitian berjumlah 30 orang bidan. Hasil penelitian ini menunjukkan bahwa terdapat pengaruh antara lama kerja dan pelatihan dengan pelaksanaan inisiasi menyusui dini dengan p-value sebesar 0.013 dan 0,008. Hasil uji regresi memperlihatkan bahwa bidan dengan mengikuti pelatihan akan meningkatkan melaksanakan inisiasi menyusui dini sebesar 11.1 kali dibandingkan dengan bidan yang tidak mengikuti pelatihan. Sedangkan bidan dengan lama kerja yang lebih lama akan meningkatkan pelaksanaan inisiasi menyusui dini sebesar 9 kali dibandingkan dengan bidan dengan masa kerja yang singkat.
