Abstract

Bone, as an important organ in the body, has a fundamental role in supporting biomechanical functions, carrying out the hematopoiesis process, and maintaining calcium homeostasis. The cellular components of bone involve various types of cells, including osteogenic precursor cells, osteoblasts, osteoclasts, osteocytes, and hematopoietic elements from the bone marrow. Vitamin C, or ascorbic acid, is a water-soluble vitamin that plays an important role in the formation of collagen, skin, tendons, ligaments, blood vessels, as well as in wound healing and maintenance of cartilage, bones, and teeth. This study aims to analysed mean value and difference in the number of osteoblast cells in the femur of rats exposed to alcohol between groups receiving vitamin C. Based on the results of the One Way ANOVA test, the p value = 0.018 <0.05 was obtained, which indicated a significant difference in the average osteoblasts between groups. Furthermore, in the Post Hoc test, a difference in the mean osteoblasts was found between groups T3 and T5 with a p value = 0.029 < 0.05 and between groups T4 and T5 with a p value = 0.044 < 0.05. Conclusion: Administration of vitamin C can affect the increase in the number of osteoblast cells in the bone healing process in mice exposed to alcohol.

Abstract

The prevalence of Chronic Kidney Disease sufferers globally, including Indonesia, has increased every year. According to the 2018 Basic Health Research data, the prevalence of Chronic Kidney Disease in Indonesia is 0.38% of the population of Indonesia or around 713,783 people. The Indonesian Nephrology Association said that the use of reuse dialyzers is permitted with a maximum limit of 7 times of use, Indonesian 2 Renal Registry data shows that reuse dialyzers are mostly used with a frequency of 1-5 times. Manual cleaning of single-use and reuse dialyzers can have an impact on several aspects, including: dialysis adequacy and risk of infection. Putri Bidadari Hospital, Stabat has a problem with changing the reuse dialyzer to single use because if you use reuse, the used equipment is washed again. So by switching to single use, it is hoped that the output of hemodialysis will be better. However, by switching to single use, there is a possibility of additional/reduced costs per patient for each hemodialysis. This study aims to analyze the Cost Effectiveness of Using Single Use and Reuse Dialyzers for Hemodialysis Services for Kidney Failure Patients at Putri Bidadari General Hospital, a cross-sectional quantitative research design with a prospective and retrospective approach, involving 160 patients in 2023. Data were analyzed using Cost Effectiveness Analysis (CEA). The results of the analysis show that the cost efficiency of the Reuse dialyzer is proven to be lower than the Single Use dialyzer. While in producing changes in the outcome of creatinine, urea, and hemoglobin, the single use dialyzer is more effective than the reuse dialyzer. Hospitals also need to conduct an evaluation of the environmental impact of using both types of dialyzers, including medical waste and energy consumption.

Abstract

Quality of health care refers to service excellence in meeting patient expectations and providing satisfaction. Quality service is essential because it is every customer's right and allows healthcare providers to compete. The quality of service and the value provided directly impact customer satisfaction. The quality of service they receive influences customer satisfaction, which affects the hospital's reputation and competitiveness. This study aims to analyze the quality of health services and their effect on patient satisfaction at RSU Vina Estetica. This study was conducted at RSU Vina Estetica, Medan City. The study population was all patients at RSU Vina Estetica, Medan City. The sampling technique was accidental sampling with a sample size of 306 respondents. The research method used was quantitative. This study is based on primary data obtained through questionnaires. The data analysis techniques used in this study are univariate, bivariate, and multivariate analysis using SPSS. Based on the study's results, it is known that reliability, responsiveness, and empathy influence patient satisfaction. The variables of physical evidence and assurance do not affect patient satisfaction. The most dominant factor influencing patient satisfaction is the variable of reliability.

Abstract

In implementing quality improvement, there are things that need to be considered so that it can run according to the desired results, such as leadership, work motivation, and work environment which will have an impact on employee performance and will be closely related to the quality of service provided to the public. The purpose of this study is to analyze the influence of leadership, motivation, and work environment on employee performance and service quality at RSU Royal Prima Medan in 2024. This type of research is quantitative and analytical using a cross-sectional research design. The population in this study were employees at RSU Royal Prima Medan consisting of 762 medical and non-medical employees. The sampling technique in this study used Proportional Random Sampling with a sample size of 248. Data analysis used SEM with univariate, bivariate and multivariate analysis. The results showed that the variables of leadership, motivation and work environment influenced employee performance at RSU Royal Prima Medan in 2024 with the most influential variable being the leadership variable (X1). Leadership, motivation and work environment variables influence the quality of service at Royal Prima Medan Hospital in 2024 with the most influential variable being the leadership variable (X1).

Abstract

Fraktur femur merupakan cedera traumatis serius yang memerlukan proses penyembuhan optimal. Penggunaan bahan-bahan alami dapat digunakan sebagai terapi komplementer dalam penyembuhan tulang. Salah satunya adalah daun bayam merah (Amaranthus tricolor) yang diketahui memiliki kandungan sejumlah senyawa bioaktif yang dapat mendukung regenerasi tulang. Penelitian ini bertujuan mengevaluasi pengaruh pemberian ekstrak daun bayam merah terhadap jumlah osteoblast pada proses penyembuhan fraktur femur tikus galur Wistar. Penelitian eksperimental dengan desain post-test only control group ini menggunakan 12 tikus Wistar yang dibagi menjadi empat kelompok secara acak, yaitu kontrol negatif, kontrol positif, kelompok perlakuan dosis rendah (35,5 mg/150g BB), dan kelompok dosis tinggi (70,8 mg/150g BB). Analisis data menggunakan One-way ANOVA dan uji Post-Hoc Tukey. Kelompok dosis tinggi ekstrak bayam merah (P2) menunjukkan jumlah osteoblast tertinggi (41,0 ± 6,24) dibandingkan kelompok lainnya, dengan perbedaan signifikan (p<0,05). Ekstrak etanol daun bayam merah terbukti signifikan meningkatkan jumlah osteoblast pada proses penyembuhan fraktur femur tikus, dengan dosis 70,8 mg/150g BB sebagai dosis paling efektif.

Abstract

Kepuasan pasien merupakan indikator penting dalam mengevaluasi mutu pelayanan kesehatan di rumah sakit. Penelitian ini bertujuan untuk mengetahui hubungan antara kualitas pelayanan kesehatan dan tingkat kepuasan pasien di Rumah Sakit Mata Prima. Penelitian ini menggunakan metode kuantitatif dengan pendekatan cross-sectional. Data dikumpulkan dari 50 pasien menggunakan kuesioner yang mengukur kualitas pelayanan berdasarkan lima dimensi SERVQUAL (responsiveness, assurance, tangibles, empathy, dan reliability) serta tingkat kepuasan pasien. Analisis dilakukan dengan uji korelasi Spearman untuk mengetahui hubungan antara pelayanan kesehatan dan kepuasan pasien. Hasil penelitian menunjukkan adanya hubungan signifikan antara kualitas pelayanan kesehatan dan kepuasan pasien dengan nilai korelasi sebesar 0.357 dan signifikansi 0.011, yang menunjukkan bahwa semakin baik pelayanan yang diberikan, semakin tinggi tingkat kepuasan pasien. Meskipun kekuatan hubungan tergolong sedang, aspek-aspek seperti kecepatan respons tenaga medis, keramahan, kebersihan fasilitas, dan jaminan rasa aman berperan penting dalam membentuk kepuasan pasien. Kesimpulannya, kualitas pelayanan kesehatan di Rumah Sakit Mata Prima berkontribusi positif terhadap kepuasan pasien.

Abstract

Background: The pain experienced by Nucleus Pulposus Herniation (NPH) patients can have an impact on productivity and may lead to disability. Therefore, non-pharmacological therapies can serve as alternatives in reducing pain levels for Grade I and Grade II NPH patients. Purpose: To compare the difference in effectiveness between Infrared-Transcutaneous Electrical Nerve Stimulation (IR-TENS) combination and Proprioceptive Neuromuscular Facilitation (PNF) therapies in reducing pain among NPH patients. Method: This research employed a quasi-experimental design with a two group before and after intervention conducted at the Physical Medicine and Rehabilitation Unit of Royal Prima Hospital in Medan. A total of 20 NPH patients were provided with interventions, consisting of a combination of IR and TENS therapies for 10 patients, and PNF therapy for the other 10 patients, administered eight times for one month. Pain measurement was conducted using a Visual Analogue Scale (VAS) questionnaire before and after the interventions. The statistical analysis employed an Independent Sample T test, with a significance level (α) set at 0.05. Result: There was a reduction in moderate and severe pain scales in both groups, and no subjects were experiencing severe pain anymore. However, the reduction in moderate pain scale was higher in the group receiving the combination of infrared and TENS therapies (80%) compared to the PNF therapy (70%). The Independent T test showed that there was no significant difference between the effectiveness of the combination of IR-TENS therapies and PNF therapy on pain in NPH patients (p-value = 0.075). Conclusion: Both groups of patients experienced a reduction in pain scale after the interventions, although there was no significant difference in the outcome between the two treatment groups. This implies that both therapies are equally effective in suppressing pain in NPH patients.

Abstract

Pharmaceutical supplies are one of the resource elements that must be controlled in the operational management of a hospital to support hospital services in order to provide maximum health services. The study aims to analyze the Implementation of Drug Inventory Control with the ABC-EOQ-ROP method at Sundari Hospital, Medan. The research method used was descriptive qualitative which was carried out at Sundari Hospital, Medan in October-November 2024. Informants were the head of the pharmacy installation, pharmacy staff, and head of the room. Data were analyzed using interactive analysis. The results of the study showed that the availability of drugs before the implementation of the EOQ and ROP methods based on the type and quantity of drugs was not sufficient to meet the needs where there were still some drugs that could not be met in a timely manner. The availability of drugs after the implementation of the EOQ and ROP methods in the pharmacy warehouse when viewed based on the type and quantity of drugs was sufficient to meet the needs of the pharmacy installation in a timely manner. It is recommended that drug inventory control in this study only emphasizes the calculation of drug inventory control that requires greater management (category I) quantitatively.

Abstract

Medical records are important records that contain all information related to services that have been provided to patients. The less than optimal process of implementing medical records can be due to a lack of human resources, training, or supporting facilities and infrastructure. This study aims to analyze the need for workers in the medical records department of the hospital with a theoretical approach (queueing theory) at Esthomihi Hospital Medan. This type of research is quantitative with an analytical observational approach. The research design used is cross sectional. This study was conducted at Esthomihi Hospital in December-January 2025. In this study were all health workers related to the implementation of medical records consisting of doctors, nurses, medical records officers and pharmacists at Esthomihi Hospital Medan as many as 252 people. The sample in this study were all doctors, nurses, medical records officers and pharmacists related to patient medical records at Esthomihi Hospital Medan as many as 252 people. Data analysis was carried out univariately, bivariate and multivariate. The results of the study indicate that there is an influence of the level of patient arrivals, the level of patient service, the number of patient servers, and the system capacity. The most influential variable on the need for manpower in hospital medical records affairs with a theoretical approach (queueing theory) at Esthomihi Hospital Medan is the variable number of patient servers with ap value of 0.000, OR = 125.571 (95% CI = 25.114-627.866) meaning that respondents who stated that the number of patient servers was lacking a 125.571 times less chance compared to respondents who stated that the number of patient servers was good n It is expected to be able to improve, expand, develop, experience and knowledge directly regarding the need for manpower in hospital medical records affairs with a theoretical approach (queueing theory) at Esthomihi Hospital Medan.

Abstract

The Industrial Revolution 4.0 era demands digital transformation in various sectors, including the health sector. One form of innovation in health services is the digitalization of the BPJS Kesehatan program, which aims to improve the efficiency and quality of services in health facilities. This study aims to assess the effectiveness of the use of BPJS program digitalization to improve the quality of services in facing the industrial revolution 4.0 era at Royal Prima Medan Hospital, as well as to identify obstacles faced in its implementation. The research method used is a qualitative study, where data was obtained through interviews with the hospital director, the head of the BPJS primary benefit guarantee division, and six patients using BPJS services. The results of this paper indicate that the use of BPJS digitalization at Royal Prima Medan Hospital has been running in accordance with the Standard Operating Procedure (SOP) and has had a positive impact on the acceleration of the administrative process, queue efficiency, and patient data integration. However, there are still several obstacles, such as technical disruptions in the system, limited patient understanding of technology, and instability of the internet network, which can affect the effectiveness of services. To overcome these obstacles, the hospital has made various efforts, such as providing a help desk, improving digital infrastructure, and education for patients and medical personnel. Overall, the digitalization of the BPJS Kesehatan program at RSU Royal Prima Medan has proven effective in improving the quality of service, although further system optimization and innovation are still needed so that the implementation of this digitalization can run optimally in accordance with the demands of the Industrial Revolution 4.0.
