Abstract

Young doctors often have different perceptions of the effectiveness of clinical learning in medical professional education. Clinical learning is a crucial stage in shaping the competence of young doctors, in terms of knowledge, skills, and professionalism. This study aims to explore young doctors' perceptions of the effectiveness of the clinical learning system at Royal Prima General Hospital in Medan. This qualitative study with a phenomenological approach involved six informants (five young doctors and one clinic coordinator) who were selected purposively. Data were collected through in- depth interviews, focus group discussions, and observations. The results of the study revealed four main themes. First, the quality of clinical learning is influenced by the clarity of instructions, tutor involvement, patient availability, and rotation structure. Second, the learning environment is influenced by the relationship with the supervisor and the clinical work atmosphere. Third, the young doctors' self-competence includes self-confidence, readiness to face patients, and stress management strategies. Fourth, supporting factors (senior support, hospital facilities) and inhibiting factors (patient case variations, high workload, differences in tutor supervision styles) also influence the effectiveness of clinical learning. In conclusion, the effectiveness of clinical learning is influenced by the interaction of instructional, personal, environmental, and structural factors. Therefore, improvement efforts are needed in the form of strengthening tutor supervision skills, equalizing practice opportunities, and providing psychosocial support for young doctors in order to optimize the clinical learning system.

Abstract

Mangkokan leaves (Nothopanax scutellarium) contain various phytochemicals such as flavonoids and saponins, where these compounds have antioxidant activity that can improve oxidative stress in the body. Treatment of Diabetes Mellitus disease can be done with drugs and herbs, optimal blood glucose control alone cannot prevent complications. The available therapies for DM include insulin therapy and other hypoglycemic agents that can not be separated from various side effects and failure to significantly prevent complications, including the Mangkokan leaf herbal treatment (Nothopanax scutellarin) which has an antidiabetic effect. To systematically review the effect of the ethyl acetate fraction of the extract of the Mangkokan (Nothopanax scutellarium) leaf extract on blood sugar levels, malondialdehyde, insulin expression in pancreatic cells and pancreatic histopathology in hyperglycemic rats. A systematic search was carried out in the PubMed database, Medline, Web of Science, Scopus, EMBASE from 2017 to 2021. The reported results stated that Alloxan is a compound that has diabetogenic properties and is toxic especially to pancreatic beta cells and when administered to animals will cause the rat to be diabetic. Alloxan causes damage to pancreatic beta cells by activating reactive oxygen (ROS) which is initiated by the reduction reaction of alloxan.Daun mangkokan (Nothopanax scutellarium) mengandung berbagai fitokimia seperti flavonoid dan saponin, dimana senyawa tersebut memiliki aktivitas antioksidan yang dapat meningkatkan stres oksidatif dalam tubuh. Pengobatan penyakit Diabetes Mellitus dapat dilakukan dengan obat-obatan dan herbal, pengendalian glukosa darah yang optimal saja tidak dapat mencegah terjadinya komplikasi. Terapi yang tersedia untuk DM antara lain terapi insulin dan agen hipoglikemik lainnya yang tidak lepas dari berbagai efek samping dan kegagalan untuk mencegah komplikasi secara nyata, termasuk pengobatan herbal daun mangkokan (Nothopanax scutellarin) yang memiliki efek antidiabetik. Mengkaji secara sistematis pengaruh fraksi etil asetat ekstrak ekstrak daun mangkokan (Nothopanax scutellarium) terhadap kadar gula darah, malondialdehid, ekspresi insulin pada sel pankreas dan histopatologi pankreas pada tikus hiperglikemik. Penelusuran sistematis dilakukan di database PubMed, Medline, Web of Science, Scopus, EMBASE dari tahun 2017 hingga 2021. Hasil yang dilaporkan menyatakan bahwa Alloxan merupakan senyawa yang memiliki sifat diabetogenik dan bersifat toksik terutama terhadap sel beta pankreas dan bila diberikan pada hewan akan menyebabkan tikus menjadi diabetes. Aloksan menyebabkan kerusakan sel beta pankreas dengan mengaktifkan oksigen reaktif (ROS) yang diawali dengan reaksi reduksi aloksan.

Abstract

Improving self-care in T2DM patients is critical so that patients can achieve targeted blood glucose levels to prevent complications and independently perform daily tasks to improve their quality of life and life satisfaction. Psychological intervention is believed to be able to change a person's behavior towards a better direction in disease control and prevention. This study measures the impact of Cognitive-Behavioural Therapy (CBT) interventions on the self-care management of T2DM patients. This quasi-experimental study used a pretest-posttest with a control group design. The research subjects were divided into two groups, namely the intervention group (35 people) and the control group (35 people). The study was conducted at the Tello Primary Health Center in April 2021. The data were analyzed using the t-dependent test (?=0.05). The results of the study showed that in the intervention group, there were differences in the self-care management of T2DM patients before and after being given CBT, which included eating patterns (p less than 0.001; 95%CI -6.663-(-4.796)), physical activity (p less than 0.001; 95% CI -5.344-(-4.370)), and attitude (p= less than 0.001; 95%CI -13.086-(-10.114)). In the control group, there was no difference in the self-care management of T2DM patients, which included eating patterns (p= 0.083; 95%CI -0.183-0.012), physical activity (p= 0.058; 95%CI -0.291-0.005), and T2DM patients attitudes (p= 0.094; 95%CI -0.225-0.003). This means that CBT effectively improves the self-care management of T2DM patients.Abstrak: Peningkatan perawatan diri pada pasien DM tipe 2 sangat penting dilakukan agar pasien dapat mencapai kadar glukosa darah yang ditargetkan untuk mencegah komplikasi dan secara mandiri melakukan tugas sehari-hari sehingga meningkatkan kualitas hidup dan kepuasan hidup mereka. Intervensi psikologis dipercaya mampu mengubah perilaku seseorang ke arah yang lebih baik dalam pengendalian dan pencegahan penyakit. Studi ini bertujuan mengukur dampak intervensi Cognitive–behavioural therapy (CBT) terhadap manajemen perawatan diri pasien DM tipe 2. Quasi-experimental study ini menggunakan desain pretest-posttest with control group design. Subjek penelitian dibagi ke dalam dua kelompok yaitu kelompok intervensi (35 orang) dan kelompok kontrol (35 orang). Penelitian dilakukan di Puskesmas Tello pada bulan April tahun 2021. Artinya CBT efektif dalam meningkatkan manajemen perawatan diri pasien DM tipe 2.

Abstract

Compliance with type 2 patients with DM in taking medication is an important key in maintaining blood glycemic levels and preventing complications. This study was conducted to determine the prevalence and to investigate the determinants of medication adherence in type 2 patients with DM. A total of 250 types 2 patients with DM who visited Pulau Tello Health Center, South Nias Regency, were involved in this cross-sectional study. Collecting data using questionnaires and patient medical records. The results showed that the motivational variables (p= less than 0.001; PR= 2; 95%CI 1.560-2.668), family support (p= less than 0.001; PR = 4.2; 95%CI 2.827-6.496), individual coping (p = less than 0.001; PR=1.9; 95%CI 1.409-2.267), and income (p= less than 0.001; PR=1.7; 95%CI 1.346-2.258) were associated with medication adherence in type 2 patients with DM. family is the dominant variable that influences medication adherence in type 2 patients with DM. Patients with DM who does not adhere to medication are 7.8 times more likely to have poor support than patients who adhere to medication.Abstrak: Kepatuhan pasien DM tipe 2 dalam mengonsumsi obat menjadi kunci penting dalam menjaga glikemik darah dan pencegahan komplikasi. Studi ini dilakukan untuk mengetahui prevalensi dan menyelidiki determinan kepatuhan minum obat pada pasien DM tipe 2. Sebanyak 250 orang pasien DM tipe 2 yang berkunjung di Puskesmas Pulau Tello, Kabupaten Nias Selatan dilibatkan dalam studi cross-sectional ini. Pengumpulan data menggunakan kuesioner dan rekam medis pasien. Hasil penelitian menunjukkan bahwa variable motivasi (p= kurang dari 0,001; PR= 2; 95%CI 1,560-2,668), dukungan keluarga (p= kurang dari 0,001;PR = 4,2; 95%CI 2,827-6,496), koping individu (p= kurang dari 0,001; PR=1,9; 95%CI 1,409-2,267), dan pendapatan (p= kurang dari 0,001; PR= 1,7; 95%CI 1,346-2,258) berhubungan dengan kepatuhan minum obat pasien DM tipe 2. Dukungan keluarga merupakan variabel yang dominan berpengaruh terhadap kepatuhan minum obat pada pasien DM tipe 2. Pasien DM yang tidak patuh minum obat 7,8 kali kecenderungannya memiliki dukungan yang kurang baik dibanding dengan pasien yang patuh minum obat.

Abstract

This research investigates the pivotal role of hospitals in healthcare, categorizing them into general and specialized facilities based on resources and capabilities. Focusing on North Sumatra, the study explores the significance of hospital conditions in influencing patients' choices, particularly in a competitive healthcare landscape driven by responsiveness to quality information. Employing a quantitative approach with non-probability sampling, the research follows a positivist philosophy to examine the connection and influence between independent and dependent variables. Conducted at the General Government Hospital in North Sumatra, the study spans from a preliminary survey in September 2023 to November 2023, with data collection ongoing and results to be presented in a seminar. The analysis reveals a substantial impact of Patient Satisfaction (PS) on Patient Loyalty (PL) (coefficient = 0.581), supported by a t-statistic value of 5.572 (>1.96) and a p-value of 0.000 (<0.05). PS also significantly influences Hospital Staff Behavior (HSB) (coefficient = 0.280, t-statistic = 2.630, p-value = 0.009 < 0.05) and Hospital Reputation (HR) (coefficient = 0.450, t-statistic = 4.789, p-value = 0.000 < 0.05). Hospital Staff (HS) demonstrates a notable influence (coefficient = 0.101) on PL, though the relationship is not statistically significant (t-statistic = 1.520, p-value = 0.129 > 0.05). Similarly, HR influences PL (coefficient = 0.124), but the relationship is not statistically significant (t-statistic = 1.402, p-value = 0.161 > 0.05). Furthermore, PS exhibits a mediated influence on PL through HSB (coefficient = 0.028, t-statistic = 1.221, p-value = 0.223 > 0.05) and through HR (coefficient = 0.056, t-statistic = 1.269, p-value = 0.205 > 0.05). These findings underscore the critical role of patient satisfaction in shaping loyalty, highlighting the mediating effects of Hospital Staff Behavior and Hospital Reputation. Understanding these dynamics is crucial for promoting healthcare competition and enhancing care quality in the North Sumatra region.

Abstract

The National Health Insurance (JKN) program is a government initiative aimed at supporting comprehensive health development to ensure that all Indonesians have the right to good health. According to data obtained from BPJS Kesehatan and RS Murni Teguh Medan, the average waiting time for polyclinic services exceeds the national quality indicators, which requires outpatient waiting times should not exceed 60 minutes. This study aims to analyze the challenges in implementing the JKN program, specifically related to patient admission, medical data processing, reporting, and funding at RS Murni Teguh Medan. This is a qualitative descriptive research involving three key informants (Persons-in-Charge or PICs) and six supporting staff members from the Registration, Medical Records, and JKN Billing units. The research identified several challenges: Man: The registration unit's human resources do not meet the required qualifications, as staff should ideally have a medical records background, resulting in reporting issues. Materials: Patient files are incomplete for administrative processes. Machine: The applications used, such as MTHIS (Hospital Management Information System), Mobile JKN, V-Claim, DIVA, and Online Pharmacy, face operational issues, leading to inefficient medical data processing and reporting. Methods: Although procedures are aligned with the SOPs, issues remain include patient overcrowding at registration, outpatient service waiting times exceeding 60 minutes, issues with registration in executive polyclinics, self-registration issues at APM, service penalties, and semi-manual medical data processing and reporting. Markets: Patients have limited knowledge of using Mobile JKN application, and pending claims are still prevalent. Money: Prolonged use of outdated electronic meda reduces systems performance.

Abstract

Objective: To determine the unit cost of lithotripsy for treatment of urinary bladder calculus at Vita Insani Hospital Pematangsiantar. Material & Methods: This study is a descriptive quantitative study, in which the data was obtained from the finance division of Vita Insani Hospital. The research object is the unit cost of accommodation that occurs in patients with urinary bladder calculus with lithotripsy cases from January 2020 to December 2020. Results: The unit cost of lithotripsy for urinary bladder calculus services at Vita Insani Hospital Pematangsiantar which is calculated using the activity-based costing (ABC) method is Rp 4.167.009. There is a difference in cost among the ABC method, hospital calculations, and INA CBG’s rates with the following details: the amount calculated for the hospital is Rp 4.996.718, while the calculation using ABC method is Rp 4.167.009; the difference in costs is Rp 829.709. INA CBG’s rates Rp 6.137.500, and compared to calculation of hospital, the difference cost is Rp 1.140.782. Compared to the INA CBG’s rates, to the unit cost of the ABC method, there is a difference, amounting Rp 1.970.491. Conclusion: The results of unit cost lithotripsy for treatment of urinary bladder calculus through the ABC method are lower compared to hospital calculation and INA CBG’s rates.

Abstract

An important indicator to determine the quality of service in a hospital is about the completeness of the hospital's medical record files. There are many problems arise due to incomplete medical record files and do not recorded neatly and regularly. The incident is due to lack of medical team awareness in recording all information or actions that would be given to the patient. The aim of this study is to analyse the effect of Job Demand-Control-Support and Protection Motivation on compliance in filling out Medical Record files at RSU ESHMUN Medan 2023. This study uses quantitative research method which is analytical in nature. The total sampling of this study is 115 active nurses at RSU ESHMUN Medan. The result of the study informs that there are very strong and significant effect among Job Demand-Control-Support and Protection Motivation on compliance in filling out Medical Record files at RSU ESHMUN Medan 2023. The variable that has the most influence on Obedience with filling in inpatient medical record files at RSU ESHMUN in 2023 is the Protection Motivation variable. It is recommended to RSU ESHMUN Medan to improve the administration pattern in order to all of nurses have the same job demands and receive the same support

Abstract

The impact of errors in storing medical records causes delays for officers in searching for medical record documents so that services at the polyclinic are hampered. It is necessary to control the incidence of incorrect insertion of medical record documents so that the incidence of incorrect insertion of medical record documents can be reduced. Electronic Medical Record (RME) is a form of health information service that is computerized and recorded. Health service facilities that implement RME in an effort to improve the quality and quality of services. This research is a qualitative research, case study design which aims to evaluate the timing of providing electronic medical record documents for outpatients at Murni Teguh Hospital. Determination of research subjects using purposive sampling technique. The research subjects consisted of the Director of Murni Teguh Hospital, the head of the Medical Record Installation as the main informant, the Patient Registration Officer, and 3 medical record staff as triangulation informants. In collecting data, researchers conducted in-depth interviews, direct observation and document studies. Data analysis was carried out using interactive models, data reduction processes, verification and drawing conclusions. In this study, it was found that the time required to provide electronic medical record documents for outpatients at Murni Teguh Hospital was a minimum of 4 minutes, a maximum time of 9 minutes with an average time needed to provide electronic medical record documents was 6.6 minutes. Obstacles encountered are errors during use, as well as HR who still do not understand the use of electronic medical record systems. By providing electronic medical record documents, the work of the patient medical record department becomes more efficient, makes it easier for officers to search for patient medical records and the benefits that patients get are shorter waiting times.

Abstract

Health service is a concrete form of public service. Mobile JKN is an application made by BPJS Health to make it easier for participants to obtain services. The aim of the research is to evaluate health insurance services through mobile JKN at the Dental Clinic of Haj Hospital Medan in 2022. The research method is qualitative and quantitative design (Mix Methode). The research was conducted at the Dental Polyclinic at Haji Hospital Medan in May 2023. A qualitative research sample of 10 participants came to dental polo, a quantitative sample of 100 respondents came to dental polo. Collecting data with questionnaires and in-depth interviews. The results of the study, the majority of respondents who came to the dental polyclinic were aged 36-45 years (late adulthood) totaling 26 respondents (26%), health insurance services through mobile JKN are not good enough (20-55) as many as 55 respondents (55%). Participants who haven't used mobile JKN don't even know about mobile, don't know the convenience with mobile JKN, don't know the benefits of mobile JKN, participants are not interested in using mobile JKN because they haven't received education about mobile JKN. Participants who use mobile JKN assume that the JKN car program is an application that is used to register for health services without having to come to the hospital, the JKN mobile application simplifies and reduces queues at the hospital so that participants also assume that mobile JKN helps in health services, using mobile JKN makes it easier and saves time as well as efficiently and effectively.
