Abstract

The rough turban snail (Turbo setosus) is a marine mollusk species often utilized as a traditional food source due to its high protein content and its empirically believed aphrodisiac properties. However, limited information is available on its bioactive compound content, so this study aims to be a preliminary study to analyze the bioactive compound content of the rough turban snail extract using qualitative and quantitative tests. Qualitative tests of bioactive compounds were carried out by adding various reagents that produce color changes or precipitate formation, while quantitative tests were carried out using UV-Vis spectrophotometry. The results of qualitative tests showed that the ethanol extract of the rough turban snail contains alkaloids, flavonoids, phenolics, steroids, and tannins. Quantitative tests gave results for phenolic content, flavonoid content, alkaloid content, and tannin content of 34.749 mg GAE/g extract, 11.6226 mg QE/g extract, 3.365 mg CE/g extract, and 332.79 mg TAE/g extract, respectively. The results of this study provide information on the bioactive compound content of T. setosus, which can be further developed as a natural product with pharmacological properties.

Abstract

Orthopedic surgery such as surgery on the hip and knee have a high risk of experiencing Deep Vein Tromoembolism (DVT), especially in patients with hypercoagulability. In patients undergoing Hip and Knee Replacement, one of the standard therapies for treating DVT is by administering combination therapy (Unfractionated Heparin + Warfarin). Rivaroxaban is another therapy for the treatment of DVT which is a new oral anticoagulant (NOAC), which was developed to address the limitations of previous standard therapy, but has a more expensive price per unit. Pharmacoeconomic studies are carried out to determine the cost-effectiveness of patient treatment, one of which is carried out Cost Effectiveness Analysis (CEA), used in making decisions about selecting the best alternative in selecting medical costs. This study aims to analyze the most effective anticoagulant prophylaxis in post-treatment patients Hip and Knee Replacement at Dr. Regional General Hospital. Moewardi Surakarta. Observational descriptive research with retrospective data collection and using the CEA method. The data taken is in the form of medical records of patients undergoing procedures Total Hip and Knee Replacement with prophylactic therapy with anticoagulants. Samples were taken from 50 patients who met the inclusion and exclusion criteria. The research results showed that the ACER value of patients who used rivaroxaban prophylaxis cost Rp 24.569/day and patients who use LMWH prophylaxis incur a cost of Rp 36.304/day, so it can be concluded that the use of the drug rivaroxaban is more cost effective at RSUD Dr. Moewardi Surakarta.

Abstract

Breast cancer is the most common cancer in the world with high complication and mortality rates. Overexpression of human epidermal growth factor receptor-2 (HER2) is one of the mechanisms of breast cancer. Kilemo plant (Litsea cubeba) contains compounds such as naringenin, ellagic acid, caffeic acid, ferulic acid, and syringic acid, which can inhibit the overexpression of HER2. This research is a preliminary study of the development of the plant as an anti-breast cancer natural drug candidate through a molecular docking study. The method used in the molecular docking of kilemo compounds with the protein kinase domain of human HER2. Firsly, the preparation of proteins and test ligands, followed by validation of the docking method with redocking. The compounds was testing the interaction of with proteins and analyzing and visualizing results using Autodock-4 and Biovia Discovery Studio, compounds was the evaluation of pharmacokinetic predictions. The results showed that the compounds contained have good affinity for the protein kinase domain of human HER2. Naringenin and ellagic acid have the best potential with the binding energy of -8.60 kcal/mol and -7.76 kcal/mol, with inhibition constants of 493.67 µM and 2.05 µM. Pharmacokinetic predictions of the six compounds fulfill the requirements of oral drug candidates. Although the binding affinity of all tested compounds were lower than lapatinib, but based on toxicity prediction, the compounds are not as hepatotoxic as Lapatinib. In conclusion, the plant has potential to be developed as a traditional medicine candidate for natural anti-breast cancer drugs that are safer than lapatinib.

Abstract

Butterfly pea flowers are proven to contain phenolics and flavonoids, which are natural antioxidants that can be utilized in skincare such as peel-off gel masks. This study aims to obtain the optimal formula of peel-off gel mask preparation with varying concentrations of Sodium Alginate (SA) as a gelling agent, Xanthan Gum (XG) as a viscosity modifier and Propyleneglycol (PG) as a plasticizer. Optimization of the base formula was carried out with Box-behnken Design using Design Expert v.13.0 software with test responses of pH value, viscosity, spreadability, drying time and thickness. The base optimization process was carried out with 15 formulas and the optimization results showed a significant research model with the optimum formula results of 1.5% SA, 1% XG and 10% PG with a desirability value of 0.933. The optimum formula was then made and used as a base for pee-off gel mask formula of butterfly pea extract (BFE). The stability test showed significant differences (p<0.05) for the pH, viscosity, spreadability, and thickness tests. The drying time test showed no significant difference (p>0.05) in the peel-off gel mask formula of BFE against the formula base. The results of all test responses during physical stability testing still meet the gel evaluation criteria.

Abstract

Pharmaceutical services are considered by some observers to be below standard. Both pharmacists and pharmaceutical technicians as health technicians are required to improve their skills, knowledge and behavior in order to provide quality direct services to patients. The purpose of this study was to determine the implementation of pharmaceutical services at Health Centers in Pandeglang Regency in 2023. This study is an observational study with a case study design. The method used is descriptive using checklist sheets and open statements. Based on the results of this study, there are significant differences in the aspects of facilities and infrastructure and clinical pharmacy services and there are differences but not significant in the aspects of managing pharmaceutical preparations and disposable medical materials. The conclusion of this study is that pharmaceutical services at Health Centers in Pandeglang Regency are in accordance with Permenkes No. 74 of 2016 by obtaining a percentage value of 79%, entering the GOOD category.

Abstract

Diabetes mellitus (DM) increases free radicals that can cause complications of several organs, including reproductive organ that cause abnormalities in sperm morphology. Damage caused by ROS can be treated with antioxidant compounds that are widely found in plants, including tamarind leaves. This study was aimed to see the improvement in sperm morphology of diabetic rats given tamarind leaf extract. This experimental study used a true experimental method with post-test only control group design. The samples were 30 male Wistar strain white rats aged 10-12 weeks, weighing 150-200 grams. Rats were selected through simple randomization method and divided into 5 groups. K1 was given standard drinking food, K2 was induced with alloxan dose of 125 mg/kgBW, P1, P2, and P3 were given alloxan and extracts of 75, 150, and 300 mg/kgBW respectively. Rats were treated for 38 days and then terminated and epididymis were dissected. The results of the study with One Way Anova showed that there is an of administering tamarind leaf extract on the sperm morphology of diabetic male rats (p=0.000). Post Hoc Bonferroni test showed that the 150 mg/kgBW dose did not significantly different with the normal control (p=0.420) and produced the highest percentage of normal sperm morphology.

Abstract

The Transition of Primary Healthcare Centers to Regional Public Service Agency (BLUD) status allows them to independently manage their finances, including medicine procurement. This change is expected to impact the effectiveness of planning and procurement. This study evaluated the effectiveness of medicine planning and procurement in Primary Healthcare Centers that had transitioned to BLUD status. The study used retrospective observations based on data from 2022 to June 2024. Samples were selected using purposive sampling based on inclusion and exclusion criteria. The independent variable in this study was the implementation of BLUD financial management, while the dependent variables included compliance with FORNAS, medicine stockouts, and excess medicines. The study involved 30 Primary Healthcare Centers and the KruskalWallis test was used for statistical analysis. The results showed that the average scores for all indicators did not meet the expected standards. After BLUD implementation, there was a decrease in medicine compliance with FORNAS, although it remained above the minimum performance standard of 80%, and a significant reduction in stockouts (p < 0.05). The percentage of excess medicines increased, but not significantly (p > 0.05). In conclusion, medicine planning and procurement were ineffective. The BLUD financial management model impacted medicine planning and procurement by reducing compliance with FORNAS, and decreasing stockouts.

Abstract

Free radicals are the cause of cell damage and death. Free radical molecules are highly unstable and reactive, and can cause cell damage and metabolic disorders. Antioxidants are compounds that can reduce the negative impact of oxidants, enzymes, and metal-binding proteins. Bundung plant (Actinoscirpus grossus) contains secondary metabolite compounds namely flavonoids, tannins, saponins, phenolics, steroids, and terpenoids. Bundung showed moderate antioxidant activity with an IC50 value of 128 ppm. Flavonoids have high polarity, but are difficult to absorb due to their large size and poor solubility in lipids. Phytosomes can overcome the absorption barriers of natural active ingredients, by converting hydrophilic phytoconstituents into hydrophobic or lipid soluble components thereby increasing bioavailability through increased ability to cross cellular membranes and enter into blood circulation. The purpose of this study was to obtain the formulation and characterization of phytosomes of bundung plant extract as an antioxidant. The method uses maceration until a thick extract is obtained. Then, phytosomes of bundung extract were made by thin layer hydration method using rotary vaccum evaporator. The results showed that phytosomes of bundung extract in formula 4 were the most optimal, having a particle size value of 282.63 nm, a polydispersity index of 0.278 nm, a zeta potential of -5.89 nm, an absorption efficiency of 100%, and the best vesicle morphology was evenly distributed, had relatively the same size between one vesicle and another, and there was no aggregation between vesicles.

Abstract

The Head of the Hospital Pharmacy Department (HPD) plays a vital role in achieving the department's performance goals. Each HPD adopts a unique leadership style that has implications for their job satisfaction. Therefore, it is necessary to examine the influence of the HPD's leadership style on performance achievement and job satisfaction. A survey was conducted using questionnaires. The questionnaire consisted of three instruments: the MLQ,  The BSC, and a job satisfaction questionnaire . Spearman correlation analysis was used to evaluate the data. The transformational leadership had a significant positive impact on all dimensions of performance, with innovation being the most influenced dimension (r = 0.720, p < 0.01), as well as on motivation (r = 0.516, p < 0.01) and hygiene factors of job satisfaction (r = 0.613, p < 0.01). Transactional leadership also showed a positive relationship with performance but was less effective in enhancing job satisfaction. Conversely, laissez-faire leadership showed no significant relationship and even negatively affected innovation and motivation. Transformational leadership emerged as the most effective style for improving the performance and job satisfaction of HPDs in East Java, particularly in fostering innovation. Transactional leadership remains relevant for operational efficiency, while laissez-faire leadership should be minimized. The practical implications for the Head of the Hospital Pharmacy Installation are that the application of transactional and reward-based leadership styles is effective in improving staff performance in the short term, while transformational leadership is also important for building an innovative work culture, with a long-term empowerment and motivation approach

Abstract

Tuberculosis (TB) is an infectious disease that ranks second as the cause of death in the infectious disease group after HIV (Human Immunodeficiency Virus) infectious disease. Pamekasan Regency is ranked fifth in East Java in terms of TB cases, after Sumenep, Lumajang, Malang City, and Jember Regencies. The Head of the Pamekasan Regency Health Office is committed to increasing the understanding of TB Cadres to help patients carry out long-term treatment. The activity method was in the form of training on TB Treatment for TB Cadres of Health Centers and TB Managers of the Health Office in Pamekasan Regency. The implementation was held on October 17, 2020 in Pamekasan. There were 58 participants consisting of 35 TB Cadres and 23 TB Managers. The results of the training showed that there was an increase in understanding, with an average score of 8.4828 after the training and 6.6379 before the training with a p pired t test = 0.000. Conclusion The training has an effect on increasing the understanding of TB Cadres and Managers of the Health Office in Pamekasan Regency regarding the treatment and prevention of TB transmission. This activity targets four categories of Sustainable Development Goals (SDGs), namely SDG 3 (Good health and well-being), SDG 4 (Quality education), SDG 16 (Peace, justice and strong institutions) and SDG 17 (Partnership for the goals).
