Abstract

Indonesia is one of the countries with the highest number of type II diabetes mellitus patients in the world, reaching 10.7 million people. Diabetes mellitus requires long-term treatment to control patients' blood glucose levels. The types of therapy used vary, leading to differences in costs and treatment outcomes. The use of insulin is a significant issue in diabetes mellitus therapy, especially at RSUD Dr. R. Soedjono, concerning its cost-effectiveness and therapeutic efficacy. This study aims to analyze the cost-effectiveness of using Lantus and Apidra insulin therapies.This research is an observational analytic study with a cross-sectional design, and data collection was conducted retrospectively at RSUD Dr. R. Soedjono Selong. The sample selection was based on predetermined inclusion and exclusion criteria. Observations were made on the Length of Stay (LOS) and Direct Medical Cost (DMC) for each sample. Based on LOS and DMC data, the Average Cost-Effectiveness Ratio (ACER) was calculated. The results showed that the average LOS for Lantus insulin use was 4 days, while for Apidra insulin use, it was 5 days. The average DMC for Lantus insulin was Rp.2,395,734, while for Apidra insulin, it reached Rp. 2,590,218. Based on LOS and DMC data, the ACER value for Lantus insulin was Rp 598,934/day, while for Apidra insulin, it was Rp 518,043/day. The use of Apidra insulin is more cost-effective because it has a lower ACER value.

Abstract

In order to obtain a perfect emulsified and stable preparation, choosing the right composition emulsifier is crucial. Formula optimization is needed to determine the best combination of ingredients in formulation process based on evaluation responses. Simplex Lattice Design (SLD) is an experimental design method for mixing materials with the proportion of the total number of materials 1 or 100%. This research was aims to obtained the optimal cream formula using tween 80 and span 80 as emulsifiers and determined their effect on the physical properties of cream preparation. SLD method was used as optimization method with tween 80 and span 80 as independent variables and pH, viscosity, spreadability, and stickiness as responses. The minimum limit for tween 80 and span 80 is 1% (w/v) and the maximum is 9% (w/v) with total 10%. The optimal formula was obtained from consideration of the response by entering the target value to obtain a formula with desirability value close to 1. There are 5 formulas based on SLD recommendations, with a concentration ratio of tween 80 and span 80 9:1 (run 1); 7:3 (run 2); 1:9 (run 3); 5:5 (run 4); and 3:7 (run 5). The relationship between concentration and response value can be explained based on the significance of the model and equations obtained. The optimal formula was the concentration ratio of tween 80 and span 80 8.6:1.34 with a desirability value of 0.953. The conclusion of this research was that SLD can be used to optimize the emulsifier with response predictions close to the results obtained.

Abstract

ABSTRACT Citronella plants (Cymbopogon nardus L) contained active compounds such as citrononeal, citronellol, geraniol. Citronella has been known to have antibacterial, antioxidant and aromatherapy properties. Solid soap is used as a means to cleanse the body and skin from bacteria and protected against exposure to free radicals that have a negative effect on skin health. Solid soap has the benefit of cleaning and brightening the skin, was easy to use and has a relatively affordable price. The research aimed to make solid soap innovations using citronella ethanol extract ingredients. The extraction method used is maceration using 70% ethanol. The preparation of citronella extract solid soap consists of 3 formulas namely FI (2.5%), FII (5%), FIII (7.5%). Physical properties tests carried out on solid soap preparations are organoleptic test, pH test, irritation test, homogeneity test, foam height test, and favourability test. Data analysis in this study was descriptive. Data were obtained from the average observation results of the physical evaluation test, specifically organoleptic test, pH test, irritation test, homogeneity test, foam height test, and favourability test, test in accordance with the requirements of the SNI standards that have been set. The results showed that FI (2.5%), FII (5%) showed that they met the requirements of solid soap preparations such as organoleptic test, homogeneity test, pH test, high foam test, irritation test and favourability test. The difference in concentration in the solid soap formula of citronella ethanol extract had an effect on the physical characteristics of the product. The best formulation of some concentrations of citronella (C. nardus L) ethanol extract solid soap is in formula I with an extract concentration of 2.5%.

Abstract

Starfruit leaf extract produces antioxidant activity from the flavonoid compound content. The lotion dosage form is one of the dosage forms used as an antioxidant application to the skin. The lotion formula consists of a water phase, an oil phase, and an emulsifier. The emulsifier is the most important part of the formation of lotion because it determines the physical characteristics produced and the stability of the preparation. Carbopol is a synthetic emulsifier that can absorb liquid and form a stable mass to produce stable physical characteristics of the lotion. The purpose of this study was to influence the concentration of carbopol on the physical attributes of starfruit leaf extract lotion and to determine the concentration of carbopol with the best physical characteristics of starfruit leaf extract lotion. This study is an experimental study with different concentrations of Carbopol, namely 0.3%; 0.4%; and 0.5%. The physical characteristics measured were organoleptic, pH, homogeneity, viscosity, spreadability, adhesion, protection power, and emulsion type. Viscosity and adhesion test data were tested using Kruskal Wallis and continued with Mann Whitney, while spreadability and protection test data were tested using ANOVA followed by Pos Hoc. The results showed that increasing the concentration of Carbopol affected protection power (p<0.05). The concentration of Carbopol that produced the best characteristics was 0.3% (formula 1).

Abstract

Natural sources have complex interaction mechanisms in providing inhibitory effects against various radical reactions and multi-drug resistance (MDR). Amomum dealbatum (Renggak in Lombok) seed phytochemistry and bioactivities have the potential to be investigated. This study aims to explore the bioactivity of extracts and fractions of renggak seeds in reducing reactive oxygen species and inhibiting bacterial growth to overcome MDR. The methods used to determine the level of free radical scavenging and antibacterial activity in this study were the 2,2-diphenyl-1-picrylhydrazyl (DPPH) and agar diffusion methods. The results of the phytochemical tests of the ethanolic extract of Renggak seeds contain triterpenoids, alkaloids, saponins, and phenolics. Meanwhile, in the hexane fraction were found alkaloids, saponins, and phenolics. The results of antibacterial tests on Staphylococcus aureus using the agar diffusion method for ethanol and hexane extracts at concentrations of 20, 40 and 60% were 12.50±1.00, 19.00±0.00, and 21.8±0.76 for ethanol extracts, respectively. The hexane fraction was 5.33±0.57, 10.83±0.57, and 14.33±0.28 mm. For Salmonella sp bacteria, it was 9.00±1.00, 14.16±0.29, and 18.16±1.04 mm for ethanol extracts than was 7.50±0.50, 12.67±0.76, and 15.67±0.76 mm for hexane fractions. Moreover, the ethyl acetate fraction contains triterpenoids, alkaloids, saponins and phenolics which are responsible for giving antioxidant effects with an IC50 value of 66.515±2.37 ppm and is classified as a strong antioxidant to scavenging free radicals of DPPH. The higher the concentration of the fraction, the higher the inhibition of the growth of Salmonella sp and Staphylococcus aureus bacteria. The ability of the extract and fraction to inhibit bacterial growth and reduce free radicals makes renggak seeds very potential for further investigation.

Abstract

The pharmacological activity of Binahong has been studied for various therapeutic effects, and the plant is thought to have several bioactive compounds that contribute to its pharmacological activity. This article discusses the pharmacological profile of Anredera cordifolia as a modern medicine candidate. The use of traditional medicine utilizing this plant species has been ongoing since ancient times in Indonesia, a region blessed with abundant flora diversity. This study was examined various medical benefits of the Anredera cordifolia, particularly regarding its antioxidant, antihyperglycemic, antibacterial properties, and wound healing capabilities. The method of this systematic review used the PRISMA framework by searching scientific publications in 2020-2024 through the Crossref database. The research revealed that secondary metabolite compounds in Anredera cordifolia leaf extract include flavonoids, saponins, and phenols, which provide pharmacological effects. Antioxidant capacity measurements using the 2,2-diphenyl-1-picrylhydrazyl (DPPH) technique yielded various IC50 values, indicated its potential as an antioxidant. Additionally, Anredera cordifolia demonstrated significant antihyperglycemic effects in animal experimental models. Its antibacterial activity against various pathogens, along with its ability to accelerate wound healing, further confirms Anredera cordifolia potential as a modern herbal medicine. The conclusion is Anredera cordifolia shows great potential for development as an effective herbal medicine in modern treatment, supporting the trend toward more natural and environmentally friendly alternative medicine.

Abstract

Silver nanoparticles have demonstrated significant antibacterial efficacy against Escherichia coli and Staphylococcus aureus.  using a green synthesis method with plant extracts rich in flavonoids as bioreductor.  The pigeon orchid (Dendrobium crumenatum Sw.) is a plant with potential as a bioreductant, especially its stem, rich in flavonoid compounds. Optimizing heating temperature, extract volume, and silver nitrate concentration is essential for producing silver nanoparticles with optimal properties and enhanced antibacterial activity.  This research conducts optimization by varying the volume of pigeon orchid stem extract, heating temperature, and AgNO3 solution concentration.  The main parameter in the optimization is the Surface Plasmon Resonance (SPR) value. The characterization of silver nanoparticles includes particle size, polydispersity index, zeta potential, FTIR spectra, and antibacterial activity tests against E. coli and S. aureus. The research results show that the optimum conditions for synthesising silver nanoparticles are 1 mL of orchid stem extract, a heating temperature of 60°C, and an AgNO3 concentration of 1.629 mM..  The silver nanoparticles produced have characteristics with an SPR peak at 450 nm, a particle size of 98.267 nm, a polydispersity index of 0.2337, and a zeta potential of -21.9 mV.  Antibacterial tests showed an inhibition zone of 14,71±1,23 mm against S. aureus and 15,28±0,91 mm against E. coli.  These results prove that silver nanoparticles effectively inhibit the growth of gram-positive and gram-negative bacteria.

Abstract

Baccaurea racemosa (kepundung) is a tropical plant with potential as a source of bioactive compounds. This study aimed to identify the secondary metabolite profile of the most active subfraction of kepundung fruit pulp using High Resolution Mass Spectrometry (HRMS). MS analysis was performed with Heated Electrospray Ionization (H-ESI) positive ionization mode. Compound identification was performed using Thermo Scientific™ software. HRMS analysis revealed the presence of 1,149 compounds, with the ten most abundant being L-pyroglutamic acid, senkyunolide H, trans-acotinic acid, glutarate, zerumbone, choline, cuminaldehyde, D-(+)-camphor, N-methyl-2-pyrrolidone, and nicotinic acid. These compounds have been reported to have pharmacological activities, especially as antimicrobial and antifungal agents. These findings support the potential use of kepundung fruit pulp as a promising natural source for phytopharmaceutical development.

Abstract

Hyperlipidemia is a condition characterized by elevated levels of cholesterol and triglycerides. Hyperlipidemia can lead to vascular disorders affecting the heart and brain, potentially resulting in severe complications such as coronary heart disease and stroke. A range of treatment strategies are employed to manage this condition encompassing the use of drugs as well as traditional medicinal approaches. Among these, the use of medicinal plants presents a promising option which considering relatively safe owing to their natural origin and minimal risk of significant side effects, as well as their bioactive compounds. Catechin is one of secondary metabolites commonly found in plants which showed various biological activities that potential for medicinal use. The aim of this review was to provide a comprehensive review of existing literature regarding the catechin-containing plants in lowering cholesterol and triglyceride levels. Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) was employed to select literature obtained from reputable online databases such as Google Scholar, PubMed and Science Direct which were published from 2015 to 2025. A narrative analysis was performed to assess the selected literatures. Based on literature screening, 200 articles were identified of which 14 were selected for inclusion in the review based on predefined criteria. This study highlighted the therapeutic potential of medicinal plants containing catechins which exhibit efficacy in reducing cholesterol and triglyceride levels. Medicinal plants may offer an effective herbal remedy for reducing cholesterol and triglyceride levels, with catechins may have a contribution to this activity.

Abstract

Curcumin (diferuloylmethane), the primary bioactive compound in turmeric (Curcuma longa), is a natural polyphenol with proven pharmacological properties. Its safety and biodegradability, along with multifunctional health benefits, have been well-documented. However, curcumin exhibits poor water solubility. Consequently, curcumin shows low systemic bioavailability and rapid urinary excretion. A carrier matrix is essential to improve bioavailability and prolong systemic retention of curcumin, and one of carrier matrix that can be utilized is a bead matrix. In this study, bead matrices were prepared by combining a chitosan solution with lauric acid and carrageenan solutions and then dropping the mixture into a sodium hydroxide solution, which acts as an ionic neutralizing agent during bead formation. The encapsulation process was conducted by immersing the formed bead matrices in curcumin and glutaraldehyde solutions to facilitate crosslinking reactions with chitosan. The resulting bead matrices were analyzed using FT-IR spectroscopy to determine the interactions and bonds formed between the materials used. The results indicated that the encapsulation efficiency of curcumin was influenced by the concentrations of chitosan and lauric acid, with higher concentrations of both components leading to increased encapsulation efficiency.
