Abstract

Rheumatic fever and rheumatic heart disease remains a significant cause of cardiovascular disease in the world, especially in industrial countries and developing countries. Rheumatic heart disease is the most serious complication of rheumatic fever which is characterized by the occurrence of heart valve defects are most of the mitral valve, aortic and tricuspid followed. Valvulitis or inflammatory process in the tissue of the mitral valve causing edema of the valve leaflets and chordae tendineae, causing disruption valve closure is causing mitral regurgitation. Eventually fibrosis and calcification of the valve that causes stiffness and the valve leaflets into mitral valve stenosis. The earliest possible introduction of cardiac involvement in rheumatic heart disease is an important part in the prevention of further heart damage.

Abstract

Cardiovascular risk factors have been known to play a role in vascular endothelial damage. Endothelial damage can cause a condition called endothelial dysfunction. Endothelial dysfunction  is the initial process of various cardiovascular problems such as coronary heart disease, stroke, peripheral artery disease, and  others. Early stage endothelial damage can actually be detected, but often patients present at the stage of cardiovascular problems. Various examinations of endothelial function start from invasive, non invasive, until biomarkers marking the occurrence of dysfunction can be done. So that we can prevent or reduce the occurrence of further damage

Abstract

Background. Running is increasingly popular in Indonesia as a recreational and competitive sport. Training volume per week is known to be associated with the cardiorespiratory fitness of runners. A straightforward method of assessing fitness is by measuring the resting heart rate (RHR). Objective. This study aimed to analyze the RHR and its runners, recreational runners, and non-runners in the city of Padang, West Sumatera. Methods. This cross-sectional study recruited 51 subjects by purposive sampling (elite runners n= 16, recreational runners n=15, and non-runners n=20). RHR (beat per minute [bpm]) was measured using electrocardiography, while exercise volume perweek (km) was obtained using an electronic questionnaire (Google form). Data were analyzed using the Spearman correlation test, Mann-Whitney U test, and One-way ANOVA/Kruskal-Wallis. Results. The results of this study indicated that the RHR of elite runners was lower than recreational runners and non-runners (55.0; 65.2; 73.4 bpm, respectively; p = 0.001). The correlation between training volume per week and RHR was not statistically significant in elite runners (Spearman r=-0.31; p=0.25), but statistically significant in recreational runners (Spearman r=-0.73; p=0.002). Conclusion. It can be concluded that elite runners have the best cardiorespiratory fitness, followed by recreational runners and non-runners. There is a correlation between weekly training volume and RHR in recreational runners, but not in elite runners.

Abstract

Latar Belakang: Pasien IMA-EST akan mendapat manfaat dari tindakan IKP apabila dilakukan dalam waktu <12 jam setelah onset. Kejadian kardiovaskular mayor (KKM) merupakan gabungan dari hasil akhir gejala klinis yang sering terjadi pada pasien IMA-EST. Terdapat berbagai faktor yang dapat memengaruhi KKM pada pasien IMA-EST. Objektif: Mengetahui faktor-faktor yang memengaruhi KKM pada pasien IMA-EST yang menjalani late IKP di RSUP Dr. M. Djamil Padang. Metode: Penelitian ini merupakan penelitian kohort retrospektif. Data diperoleh dari rekam medik RSUP Dr. M. Djamil Padang dan wawancara via telpon. Analisis data dilakukan dengan analisis kesintasan dengan metode Kaplan-Meier. Hasil: Jenis KKM terbanyak pada pasien IMA-EST yang menjalani late IKP di RSUP Dr. M. Djamil Padang adalah kematian dan tindakan revaskularisasi berulang. Dari delapan faktor yang diteliti, hanya lokasi infark yang memiliki hubungan bermakna dengan KKM adalah (p=0.006). Kesimpulan: Faktor yang memengaruhi KKM pada pasien IMA-EST yang menjalani late IKP di RSUP Dr. M. Djamil Padang adalah lokasi infark.

Abstract

Latar Belakang: Penggunaan tembakau sampai saat ini masih menjadi ancaman masalah kesehatan paling besar di dunia. Perilaku merokok terdapat pada beberapa kalangan tidak terkecuali tenaga kesehatan dan mahasiswa kedokteran. Tenaga kesehatan pada pelayanan kesehatan sehari-hari diharapkan dapat memberikan konseling kepada pasien untuk berhenti merokok. Objektif: Penelitian ini bertujuan untuk mengetahui perilaku merokok dan sikap mahasiswa tahun 3 pendidikan dokter Universitas Andalas terkait program berhenti merokok berdasarkan Global Health Professions Student Survey (GHPSS). Metode: Penelitian ini merupakan penelitian deskriptif dengan rancangan cross sectional. Penilaian perilaku merokok dan sikap terkait program berhenti merokok dinilai melalui kuesioner GHPSS. Sampel penelitian ini adalah seluruh mahasiswa tahun 3 pendidikan dokter Universitas Andalas. Penelitian ini dilakukan pada Februari 2021 – April 2021. Analisis data pada penelitian ini menggunakan analisis univariat. Hasil: Penelitian ini mendapatkan hanya 4,5% mahasiswa yang merokok, 99,1% berfikir tenaga kesehatan harus memberikan nasihat atau informasi tentang berhenti merokok, dan 61,7% pernah mendapatkan pelatihan formal mengenai pendekatan berhenti merokok untuk digunakan pada pasien. Kesimpulan: Mahasiswa tahun 3 pendidikan dokter Universitas Andalas sebagian besar tidak merokok, seluruh perokok pertama kali merokok di atas 60 menit setelah bangun tidur di pagi hari, dan sebagian besar mendukung kebijakan terkait rokok dan peran tenaga kesehatan pada program berhenti merokok. Mayoritas di antaranya juga menyatakan telah mendapatkan pendidikan/pelatihan mengenai program berhenti merokok.

Abstract

Latar Belakang: Hipertrofi ventrikel kiri (HVK) pada penderita hipertensi dapat dideteksi dengan pemeriksaan elektrokardiogram dan ekokardiografi. Objektif: Penelitian ini bertujuan untuk mengetahui hubungan hipertrofi ventrikel kiri pada elektrokardiogram berdasarkan kriteria Peguero – Lo Presti dengan ekokardiografi pada pasien hipertensi di RSUP Dr. M. Djamil Padang. Metode: Penelitian ini merupakan penelitian analitik observasional dengan disain potong lintang. Sampel sebanyak 98 pasien yang menderita hipertensi menjalani pemeriksaan elektrokardiogram dan ekokardiografi. Pengambilan sampel dilakukan dengan teknik consecutive sampling. Data dianalisis menggunakan uji statistik Chi-Square. Hasil: Hasil penelitian mendapatkan sebagian besar pasien yang menderita hipertensi adalah berada pada usia >65 tahun, jenis kelamin laki-laki, dengan indeks massa tubuh normal (IMT 18,5-25 kg/m2), dan lebih banyak menderita dislipidemia. Pasien hipertensi yang didiagnosis HVK berdasarkan ekokardiografi 72 orang (73,5%), sedangkan dengan EKG kriteria PLP 35 orang (35,7%). Analisis bivariat menunjukkan tidak terdapat hubungan yang signifikan antara hipertrofi ventrikel kiri berdasarkan elektrokardiogram kriteri Peguero-Lo Presti dengan ekokardiografi pada pasien hipertensi di RSUP Dr. M. Djamil Padang dengan nilai signifikan p value = 0.275 (p>0,05). Kesimpulan: Kesimpulan penelitian ini adalah tidak terdapat hubungan antara hipertrofi ventrikel kiri berdasarkan elektrokardiogram kriteria Peguero-Lo Presti dengan ekokardiografi pada pasien hipertensi dan dibutuhkan penelitian lebih lanjut.

Abstract

Background: Morbidity and mortality of patients after cardiac surgery can be influenced in the preoperative, intraoperative, and postoperative periods. This study aimed to explore factors that affect the mortality of patients undergoing open heart surgery with cardiopulmonary bypass at Dr. M. Djamil General Hospital, Padang, Indonesia. Methods: This study is an analytical observational research with a cross-sectional approach and uses secondary data obtained from the medical record installation of Dr. M. Djamil General Hospital, Padang, Indonesia. A total of 57 research subjects participated in this study. Univariate, bivariate, and multivariate analyses were carried out using SPSS software. Results: The final multivariate modeling results show that the variables that influence patient mortality are EuroSCORE II (p-value=0.003) and duration of CPB (p-value=0.003). With an odds ratio of 37.21 groups, EuroSCORE II high risk had a risk of death 37.21 times higher than the low-risk group (CI 3.325 – 416.324). Meanwhile, increasing the duration of CPB by one minute increased the risk of mortality 1.02 times (CI 1.006 – 1.030). Meanwhile, there is no influence of variables (NYHA, LV function, ACC duration) on the incidence of mortality in patients undergoing open heart surgery with a machine cardiopulmonary bypass at Dr. M. Djamil General Hospital, Padang. Conclusion: EuroSCORE II affects the mortality of patients undergoing open heart surgery with CPB. The duration of CPB affects the mortality of patients undergoing open heart surgery with CPB.

Abstract

Coronary heart disease (CHD) is a coronary atherosclerotic disease which refers to a pathological process that affects the coronary arteries. In general, there are two types of risk factors for coronary heart disease, namely risk factors that cannot be modified and risk factors that can be modified. Non-modifiable risk factors are age, gender, and genetics. Modifiable risk factors are hypertension, hypercholesterolemia, hypertriglyceridemia, diabetes mellitus, smoking, obesity, lack of activity, and stress. The purpose of this study was to describe the risk factors for coronary heart disease at a young age. This type of research is a descriptive study using a cross-sectional study design from secondary data of medical records of patients with coronary heart disease at Dr. RSUP. M. Djamil Padang in 2017-2019 with a total sample of 61 people. The results showed that there were 47 male patients (77.05%) and 14 female patients (22.95%). For patients who have a family history of risk factors as many as 3 people (4.9%), hypertension as many as 30 people (49.18%), hypercholesterolemia as many as 12 people (19.67%), hypertriglycerides as many as 10 people (16.39%) , diabetes mellitus as many as 10 people (16.39%), obesity as many as 5 people (8.2%), and smoking as many as 16 people (26.23%). The conclusion of this study is there were 61 patients with CHD at a young age in 2017-2019, the risk factors for CHD patients at a young age were mostly at the age of 26-45 years, followed by male gender, risk factors for hypertension, risk factors for smoking, risk factors for hypercholesterolemia, risk factors for hypertriglycerides, risk factors for diabetes mellitus, risk factors for obesity, and risk factors for family history.

Abstract

Latar Belakang: Koping merupakan suatu upaya kognitif atau perilaku untuk menghadapi tuntutan dari luar maupun dari dalam diri. Mahasiswa perantau sebagai individu yang pergi ke daerah lain dan dituntut untuk mampu menyesuaikan diri pada lingkungan baru, bila upaya penyesuaian tidak berjalan dengan baik dapat memunculkan stres atau bahkan beban mental yang dapat berdampak pada kemampuan mencapai prestasi akademik yang optimal. Dibutuhkan suatu mekanisme untuk menghadapi masalah tersebut, salah satunya adalah koping. Objektif: Tujuan dari penelitian ini adalah untuk mengetahui hubungan antara kemampuan koping dengan prestasi akademik pada mahasiswa perantau Program Studi Kedokteran Fakultas Kedokteran Universitas Andalas angkatan 2019. Metode: Penelitian ini merupakan penelitian analitik dengan pendekatan cross sectional yang menggunakan total sampling dengan didapatkan jumlah sampel sebanyak 96 orang yang merupakan mahasiswa perantau Program Studi Kedokteran Fakultas Kedokteran Universitas Andalas. Pengumpulan data responden diperoleh dari kuesioner dan data dianalisis menggunakan uji Fisher’s Exact Test. Hasil: Hasil analisis univariat menunjukkan mayoritas responden memiliki kemampuan koping adaptif (93,8%) dan minoritas responden memiliki kemampuan koping maladaptive. Kategori cukup memuaskan, memuaskan, dan sangat memuaskan (53,1%) merupakan kategori indeks prestasi yang terbanyak diperoleh responden dan minoritas responden memiliki kategori dengan pujian (46,9%). Kesimpulan: Tidak terdapat hubungan yang signifikan antara kemampuan koping dengan prestasi akademik (p=1).

Abstract

Background Pulmonary arterial hypertension (PAH) and pulmonary venous hypertension (PVH) are both associated with increased right ventricular (RV) afterload, potentially leading to RV systolic dysfunction. Despite its prognostic value in predicting right heart failure (RHF), echocardiographic evaluation of RV function remains underutilized in the routine assessment of patients with PAH and PVH. Methods This study aims to evaluate differences in RV systolic function between PAH and PVH patients. A total of 116 patients (58 PAH and 58 PVH) were selected through consecutive sampling from medical records at RSUP Dr. M. Djamil Padang. Echocardiographic parameters were analyzed using appropriate statistical tests based on data distribution and type, including Chi-square, Fisher’s exact, and Mann-Whitney U tests. Results Significant differences were observed between PAH and PVH groups in terms of tricuspid regurgitation (TR) severity (p = 0.003), tricuspid annular plane systolic excursion (TAPSE) (p = 0.013), and tissue Doppler-derived systolic velocity of the tricuspid annulus (S′) (p = 0.001). No significant differences were found in ejection fraction (EF), systolic pulmonary artery pressure (sPAP), mean pulmonary artery pressure (mPAP), or TR velocity (TR Vmax). Overall, RV systolic function differed significantly between the PAH and PVH groups (p = 0.006). Conclusions The findings demonstrate a statistically significant difference in RV systolic function between patients with PAH and PVH. These results underscore the need for more comprehensive and routine echocardiographic evaluation of RV function in these populations to enhance risk stratification and management.
