Abstract

Left and right oblique positions are ambulation that must be done in patients who have hemorrhagic strokes to prevent decubitus. Decubitus is a complication due to pressure on a squeezed area of ​​the body resulting in pressure sores. This research method is a case study research with a medical-surgical nursing care approach. The study was conducted by two hemorrhagic stroke patients who were given left and right oblique ambulation interventions. The results of the study showed that early left and right oblique ambulation is very useful in preventing decubitus so that it must be applied by nurses to hemorrhagic stroke patients. In addition to preventing decubitus, in patients I and II there was an increase in muscle strength so that it would prevent muscle atrophy. The research recommendation is that left and right oblique ambulation can be applied to hemorrhagic stroke patients, so as to prevent complications such as pressure sores.

Abstract

Stroke non-hemoragik merupakan jenis stroke yang terjadi secara patologis dari 80% jenis stroke yang terjadi karena penyumbatan pembuluh darah yang menyebabkan aliran darah ke otak sebagian atau keseluruhan terhenti. Tujuan penelitian ini adalah melakukan asuhan keperawatan dengan implementasi 5 elemen kunci bundle care terhadap pressure ulcer pada pasien stroke non-hemoragik. Metode penelitian ini adalah desain studi kasus dengan pendekatan asuhan keperawatan medikal bedah yang terdiri dari pembagian: anamnesis, identitas klien, keluhan utama, riwayat penyakit sekarang, riwayat penyakit terdahulu, riwayat penyakit keluarga, diagnosa keperawatan, intervensi dan implementasi 5 elemen bundle care yaitu: pengkajian risiko, pengkajian kulit, permukaan pendukung, nutrisi dan reposisi/alih baring serta melakukan evaluasi yang dilakukan pada dua pasien stroke non-hemoragik. Hasil penelitian diperoleh bahwa 5 elemen kunci bundle care dapat mencegah pressure ulcer dengan hasil pada kasus 1 yaitu sebelum implementasi luas luka tekan 2 Cm dan setelah implementasi menjadi 1 Cm sedangkan pasien 2 sebelum diberikan implementasi luka tekan bernilai 3 Cm dan setelah dilakukan implementasi menjadi 1 Cm. Kesimpulan dan saran penelitian ini diharapkan pencegahan dan perawatan dengan mengimplementasikan 5 elemen kunci bundle care dapat mengurangi pressure ulcer pada klien stroke non-hemoragik.

Abstract

Rheumatoid Arthritis (RA) in the elderly is a chronic systemic autoimmune disease that causes inflammation of connective tissue, primarily in the joints. Common complaints among patients include lower back pain and mobility disorders. William's flexion exercise is a stretching technique targeting body muscles to promote relaxation, increase range of motion, provide comfort, and help prevent injuries. This study aims to describe the implementation of William's flexion exercise for reducing lower back pain in elderly patients with RA. The research method used is descriptive, presented in the form of a scientific paper with a nursing care approach, starting from assessment, diagnosis, intervention, implementation, and evaluation. The nursing care plan instrument was based on the Indonesian Nursing Intervention Standards (SIKI, 2018). This study involved two elderly RApatients who experienced mild to severe lower back pain. William's flexion exercise was implemented for 30 minutes over six days. The results showed a reduction in lower back pain, from moderate to mild pain levels. In conclusion, the implementation of William's flexion therapy demonstrated a decrease in the painscale among elderly patients with RA. It is recommended that this intervention be developed into a Standard Operating Procedure (SOP) for managing lower back pain in elderly RA patients in geriatric care units.

Abstract

Joint pain caused by osteoarthritis (OA) is a common health condition among the elderly, often resulting in decreased mobility and reduced quality of life. This condition leads to limitations in daily activities and increases the risk of complications due to prolonged immobility. One of the non-pharmacological interventions that can be applied is Elderly Fitness Exercise, which aims to improve physical fitness, reduce pain, and maintain joint flexibility. This study aims to describe the implementation of Elderly Fitness Exercise in elderly individuals with osteoarthritic joint pain. The research employed a case study design using the nursing care process approach, which includes assessment, diagnosis, planning, implementation, and evaluation. The subjects were two elderly individuals with knee osteoarthritis experiencing moderate pain. Data were collected through interviews, observation, physical examination, as well as documentation using pain assessment scales and fitness evaluation sheets. The intervention was conducted over six consecutive days in February 2025. The findings indicate a reduction in pain levels, improvement in mobility, and increased joint flexibility following the consistent implementation of the exercise. This study concludes that Elderly Fitness Exercise is an easily applicable intervention to relieve joint pain and help improve physical fitness in elderly individuals with OA

Abstract

Rheumatoid arthritis (RA)is an autoimmune disease that causes inflammation ana pain in the joints, particularly in the lower extremities. Active range of motion (ROM)is a physiotherapy intervention that can help improve mobiliry and reduce joint pain. ROM movements can enhance blood circulation to the joint capsule and increase joint flexibility, thereby reducing or even eliminating pain and improving physical mobility. This study aims to implement active ROM exercises in elderly individuals experiencing joint pain in the lower extremities. The research employed a descriptive method in the form of a scientific care approach, starting a from evaluation. The mild 10 ROM paper using nursing assessment, diagnosis,intervention .implementation,to subjects were two elderly individuals experiencing moderate joint pain in the lower extremities.Active exercises were implemented for 15 minutes, twice a day,over a period of 6 days. The results showed a reduction in pain scale from moderate to mild after administering ROM exercises to both subjects for 6 days. The implementation ofROM exercises should be carried out continuously.along with the development of standard operating procedures for ROM exercises in RA patients experiencing lower extremity joint pain.

Abstract

ABSTRACT Rheumatoid Arthritis (RA) is a chronic autoimmune disease that causes joint inflammation and is commonly experienced by the elderly. This condition affects body balance and increases the risk of falls. One non-pharmacological method to improve postural balance is the application of the 12 Balance Exercise. This exercise aims to strengthen the lower extremity muscles and enhance the function of the vestibular system. This study used a descriptive case study method with a nursing care approach. starting from assessment. nursing diagnosis, implementation, and evaluation. involving two elderly patients with RA who had postural balance disturbances. The intervention applied was the 12 Balance Exercise for two weeks with a frequency of three times per week. Each session lasted 15 minutes and was evaluated using the Berg Balance Scale (BBS). After six intervention sessions, the results showed an improvement in postural balance scores in both patients, shifting from a moderate fall-risk category to a low fall-risk category. The patients demonstrated better ability to stand and walk without assistance. This study concludes that the application of the 12 Balance Exercise is effective in improving postural balance among elderly patients with RA and reducing the risk of falls. The implementation of the 12 Balance Exercise can be used as part of nursing interventions and a standard approach for elderly individuals with balance disorders due to RA in geriatrie care units ABSTRAK Rheumatoid Arthritis (RA) adalah penyakit autoimun kronis yang menyebabkan peradangan pada sendi dan sering dialami oleh lansia. Kondisi ini dapat mengganggu keseimbangan postural dan meningkatkan resiko jatuh. Salah satu metode non-farmakologis untuk meningkatkan keseimbangan postural adalah dengan penerapan 12 Balance Exercise. Latihan ini bertujuan memperkuat otot ekstremits bawah dan sistem vestibular. Metode penelitian ini merupakan studi kasus deskriptif pada dua lansia penderita RA dengan gangguan keseimbangan postural di UPT pelayanan Sosial Lanjut Usia Binjai. Intervensi yang diberikan adalah 12 Balance Exercise selama dua minggu dengan frekuensi tiga kali perminggu. Setiap sesi latihan dilakukan selama 15 menit dan dinilai dengan menggunakan Berg Balance Scale (BBS). Hasil penelitian ini setelah dilakukan intervensi selama dua minggu, terjadi peningkatan skor keseimbangan postural pada kedua pasien, dari kategori resiko jatuh sedang menjadi resiko jatuh rendah. Pasien menunjukan kemampuan yang lebih baik dalam berdiri dan berjalan tanpa bantuan. Kesimpulan dari penerapan 12 Balance Exercise efektif dalam menngkatkan keseimbangan postural pada lansia dengan RA serta menurunkan resiko jatuh. Saran penelitian ini latihan 12 Balance Exercise dapat dijadikan sebagai bagian dari intervensi keperawatan standar untuk lansia dengan gangguan keseimbangan akibat RA di unit pelayanan lanjut usia.
