Abstract

HIGHLIGHTS Postpartum maternal mortality demonstrates associations with determinants including parity, place of residence, proficiency in the Indonesian language, socioeconomic status or wealth index, type of delivery attendant, and place of childbirth. Proficiency in the Indonesian language constitutes the most dominant factor, plausibly indicating inequalities in healthcare access, levels of health literacy, and conditions of geographic or social marginalization.   ABSTRACT Objective: This study utilized data from the Indonesia Demographic Health Survey (IDHS) 2020 to examine the relationship between postpartum maternal mortality and several determinants, including maternal, socioeconomic, and healthcare-related factors in Papua Province. Materials and Methods: A cross-sectional design with a quantitative analytical framework was applied, drawing on secondary data from IDHS 2020. The sample comprised 14,802 women aged 15–49 years. Data were analyzed through a simple logistic regression test for bivariate analysis with a 95% confidence level (a = 0.05). Subsequently, multivariate multiple logistic regression analysis was conducted on variables with a p-value of less than 0.25. Results: Findings demonstrated that all variables (parity p<0.001 AOR=0.533 95% CI 0.375–0.758; place of residence p=0.001 AOR=2.526 95% CI 1.438–4.436; proficiency in Indonesian language p<0.001 AOR=0.039 95% CI 0.031–0.050; delivery attendant and place of birth p<0.001 AOR=0.118 95% CI 0.142–0.249, whereas wealth index status p=0.972 AOR=1.010 95% CI 0.565–1.806) showed a significant negative association with postpartum maternal mortality (p<0.05), except for the wealth index. The strongest association, with an odds ratio (OR) of 0.039, was identified in Indonesian language proficiency, indicating a 25.64-fold increase. Conclusion: The findings provide a crucial evidence base for the development of policies and interventions aimed at lowering postpartum maternal mortality in Papua Province. Proficiency in the Indonesian language emerged as the most influential factor, underscoring the potential for targeted interventions and improved healthcare outcomes.

Abstract

The 48-hour postpartum period is the ideal time to choose contraception because ovulation can occur 34 days postpartum for mothers who are not breastfeeding optimally. Audio-visual contraceptive education is provided during pregnancy to facilitate contraceptive use during the 48-hour postpartum period. This study aims to determine the relationship between knowledge levels after audio-visual education and contraceptive use during the 48-hour postpartum period. It also aims to identify contraceptive method use during the 48-hour postpartum period. This study is a pre-experimental study with a one-group post-test only design involving 30 pregnant women in the third trimester with a gestational age of ≥ 36 weeks, selected through consecutive sampling. The intervention was provided in the form of audio-visual education. Knowledge levels were measured using a questionnaire after the intervention. Postpartum contraceptive use was identified using medical records. The relationship between knowledge levels after the intervention and contraceptive use during the 48-hour postpartum period was measured using a chi-square test, yielding a p-value of 0.002, indicating a significant association between knowledge after the audio-visual education intervention and contraceptive use during the 48-hour postpartum perio. One of the key components of the Childbirth Planning and Complication Prevention Programme (P4K) is education about contraception during pregnancy. Family planning counselling and contraceptive methods during pregnancy can increase mothers' awareness of contraceptive use.

Abstract

Partus prematurus imminens (PPI) was a condition that threatened preterm labor, marked by labor signs before term (20 to <37 weeks) and birth weight under 2500 grams. Management included rest, hydration, and pharmacological measures. One approach used to delay labor was tocolytic therapy, such as ketoprofen suppositories, to inhibit uterine contractions. This study aimed to analyze the difference in uterine contractions before and after the administration of ketoprofen suppositories in women at risk of preterm labor. This study used a quasi-experimental design with a pre-test and post-test without a control group. The sample included 30 pregnant women with PPI who met the inclusion criteria. Consecutive sampling was used. The intervention administered was ketoprofen suppository therapy, 200 mg (2 tablets) given rectally. Data were collected using an observation sheet and analyzed using the Wilcoxon signed-rank test. The results showed that the mean uterine contraction score before therapy was 2,17, which decreased to 1,20 after therapy. There was a statistically significant difference in uterine contractions before and after the intervention. The Wilcoxon signed-rank test resulted in a p-value of 0,001 (p ≤ 0,05). The findings indicated a significant difference in uterine contractions before and after the administration of ketoprofen suppositories in pregnant women at risk of preterm labor. Ketoprofen suppository therapy was effective in reducing uterine contractions in pregnant women at risk of preterm labor, with routine fetal monitoring ensuring the safety of the intervention.

Abstract

The choice of contraceptive methods among women of reproductive age is influenced by various factors, including patriarchal culture and demographic characteristics such as age, education, and occupation. In urban areas like Surabaya, shifting social values and the accessibility of information and health services may affect reproductive decision-making patterns. This study aimed to analyze the relationship between patriarchal culture and demographic factors with the choice of contraceptive methods among women of reproductive age at Bulak Banteng and Pacarkeling Public Health Centers in Surabaya. This research employed an analytical observational design with a cross-sectional approach. A total of 96 respondents were selected using consecutive sampling. Data were collected through questionnaires and analyzed using the chi-square test with a significance level of p < 0.05. The results showed no significant relationship between patriarchal culture (p = 0.235), age (p = 0.156), education (p = 0.109), and occupation (p = 0.076) with contraceptive method choice. Most respondents had a low level of patriarchal culture, moderate education, were over 35 years old, and unemployed. In conclusion, patriarchal culture and demographic factors did not directly influence contraceptive choice. The decision to use contraception may be more affected by other factors beyond socio-demographic characteristics, such as service availability and access to reproductive health information.

Abstract

HIGHLIGHTS The study examines healthcare providers’ roles in delivering holistic physical, emotional, and psychosocial support to women after pregnancy loss. The findings identify major provider barriers—limited training, institutional constraints, and emotional burden—and recommend improved training, clearer guidelines, and stronger support systems.   ABSTRACT Objective: Pregnancy loss is a complex emotional experience with a significant impact on women's physical and psychosocial health. This study aimed to explore the role of health workers in supporting women after pregnancy loss, focusing on their physical, emotional, and psychosocial needs, as well as the barriers faced by health workers. Materials and Methods: A systematic search was conducted on six electronic databases, namely PubMed, Science Direct, Springer, ProQuest, Sage Journals, and EBSCOhost, using keywords related to pregnancy support, pregnancy loss, and challenges faced by health workers. Articles published within the last 10 years (2014-2024) were selected for this review. The selection process followed PRISMA guidelines, starting with the identification of 383 articles, followed by the removal of duplicates and screening of titles and abstracts. After the eligibility assessment stage, five articles were selected that met the inclusion criteria. Data extraction and synthesis: Data were systematically extracted using tables that included study purpose, design, participants, type of pregnancy loss, and key findings. The results of the analysis mapped the physical, emotional, and psychosocial support provided by health workers and the barriers affecting the quality of that support. Results: Integrating physical, emotional, and psychosocial care is crucial to improve outcomes for women experiencing pregnancy loss. Addressing challenges faced by healthcare providers will enhance service quality and patient support. Conclusions: Holistic support that includes physical, emotional and psychosocial aspects is essential in helping women to cope with pregnancy loss. However, barriers such as lack of training and institutional support affect the effectiveness of care. This review recommends strengthening formal training, developing structured guidelines, and improving support systems for health workers to enhance the quality of care and professional well-being.
