Abstract

Background; There is no specific drug for COVID-19 yet, therefore it is important to focus on preventing its spread in the community. COVID-19 prevention strategies in each country are certainly different. This review will compare strategies to prevent COVID-19 in Indonesia, Turkey, and China by the number of COVID-19 new cases. Reviews; The number of new cases in Turkey showed a decline after implementing social distancing policies, calling for activities from home, and implementing policies to limit the mobility of people, especially vulnerable groups. In China, the number of new cases shows a decline and tends to ramp up until there are no new cases for some time with additional preventive measures (different from others) shifting offline consultations and treatment to online. In Indonesia, the prevention strategy to stop the spread of COVID-19 which is almost the same as in other countries, but the number of new positive cases of COVID-19 continues to increase. Conclusion; China and Turkey can be said to have succeeded in preventing the spread of COVID-19 by restricting people's entry and exit policies in an area, working from home, and having a good health system, meanwhile Indonesia's COVID-19 new cases tend to continue to increase because of lack of hygiene awareness. Abstract

Background: Healthcare workers in Indonesia have been prioritized for vaccination. Nevertheless, fully vaccinated healthcare workers are still at risk of being infected with COVID-19, but will be less likely to develop severe symptoms, be hospitalized or be at risk for death as compared to those who have not been vaccinated. Objectives: This study aims to analyze the incidence of COVID-19 in fully vaccinated healthcare workers. Methods: This cross-sectional study was conducted in 2021. All healthcare workers who have been fully vaccinated, have recovered from COVID-19 (2-4 weeks after vaccination) and able to complete a questionnaire were the participants. The collected data was then analyzed using the cascade method. Results: Based on the 529 collected questionnaires, by using the cascade analysis conclude that the percentage of healthcare workers who have been fully vaccinated was 99%, healthcare workers who have been fully vaccinated and then infected with COVID-19 was 14%, healthcare workers who have been fully vaccinated, infected with COVID-19 and hospitalized was 4%, healthcare workers who have been fully vaccinated, exposed to COVID-19, hospitalized and experienced the long-haul effect of COVID-19 was 0%. Discussion: Health workers are still at risk of being confirmed by COVID-19, because have high risk of being exposed in the workplace. The risk of being confirmed and severity are also influenced by age, gender and comorbidities. Conclusions: Complete vaccinations of healthcare workers did not reduce their risk of being infected with COVID-19, however, it can reduce the severity and the risk of the long-haul effects.

Abstract

Background: Pneumonia is an infection of the lung tissue which is mainly caused by bacteria. High utilization and inappropriate use of antibiotics increase resistant bacteria, morbidity, mortality, and treatment cost. Quantitative evaluation becomes one of the indicators to assess the use of antibiotics which is one indicator of the quality of the antimicrobial resistance control program. Objective: This study aimed to evaluate the use of antibiotics an effort to increase the rationality of the use of antibiotics. Methods: This study was a retrospective observational study with a sampling method of time-limited sampling in Universitas Airlangga hospital from January until December 2019. Quantitatively using Anatomical Therapeutic Chemical (ATC)/Defined Daily Dose (DDD). Result: The samples obtained were 68 severe pneumonia patients who met the inclusion criteria. From the result of the study, there were 13 types of antibiotics used for pneumonia therapy, and the three most used were Levofloxacin, Ceftriaxone, and Meropenem, with a total of all antibiotics 73.64 DDD/100 patients-days. Most useful is parenteral Levofloxacin at 21.92 DDD/100 patient-days, Ceftriaxone at 20.45 DDD/100 patient-days and Meropenem at 14.29 DDD/100 patient-days. Conclusion: The DDD value indicates high antibiotic usage, but high antibiotic use does not imply unreasonable drug use, so we must undertake a qualitative review of antibiotic use.

Abstract

The world pandemic of COVID-19 which caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) is a very infectious and contagious. Lopinavir and ritonavir is recommended based on guidelines from National Health Commission of China as an antiviral therapy for COVID-19. Both lopinavir and ritonavir inhibit the 3CLpro enzyme’s action, which its role in replication also release of viruses. As protease inhibitor in Human Immunodeficiency Virus (HIV), both lopinavir and ritonavir’s target is a symmetrical bag of C2 where does not exist in 3CLpro enzyme of corona virus. This literature review is needed to determine the outcome of using lopinavir and ritonavir in order to find a conclusion that can be used as a basis for intervening drug administration in patients of COVID-19. A method of narrative review was used in this study which was carried out by searching on Pubmed and Science direct. After doing abstract screening and title based on the inclusion criteria, 13 articles were left to be analyzed. These articles were analyzed using predetermined clinical outcome parameters, namely duration of hospital stay, composite event, clinical symptom score, time to achieve negative RT-PCR, duration of mechanical ventilation, and all-cause mortality. Based on all reviewed articles, the majority of clinical outcome parameters showed that the outcome was no better than standard care or other drugs. From this review, it can be concluded that lopinavir along with ritonavir provide no better clinical outcomes than standard therapy or other drugs in COVID-19’s patients.

Abstract

Indonesia is the 3rd ranked country with the highest number of TB cases in the world, namely 845,000 cases in 2020. In the same year, the number of suspected TB in East Java was 42,922 cases and Sumenep Regency was in the 11th position with the most TB with 1,924 cases. Therefore, community service activities aim to carry out training for TB cadres at the Bluto Health Center as an effort to reduce TB cases. Community service activities are carried out from April to July 2022. Activities begin with planning, obtaining permits, coordination meetings, training preparations, conducting webinars, conducting face-to-face training, pre-test and post-test evaluations. Activities went well. The impact of implementing training activities was in the form of 93% of cadres experiencing an increase in knowledge and the other 7% experiencing a decrease in knowledge. These good results can be used as a basis for local health workers to continue activities to increase the knowledge and skills of cadres using the lecture method; question and answer; health promotion media in the form of videos; and direct practice. Cadres can also use the same method to educate the target community regarding TB, how to prevent and treat it.

Abstract

COVID-19 cases in Indonesia still remain a concern, particularly for public health. Several factors, such as gender, age, comorbidity, occupation, and vaccination status, might influence COVID-19 infection. Individuals who have many predicting factors have a higher risk of being infected by COVID-19. Other studies have not yet shown the significance of predicting factors for COVID-19 infection in Indonesia. The study explored the association between the predicting factors and COVID-19 infection in Indonesia. The study used a cross-sectional method with a population of all Indonesian communities. It was conducted in August 2021 by distributing a Google Form questionnaire in Indonesia. By a saturated sampling of the population in Jawa, Sumatera, Sulawesi, Kalimantan, and Papua, 776 Indonesians were selected; they were aged > 17 years and voluntarily completed the questionnaires. whereas respondents with incomplete data were excluded from this study. The data were analyzed using a binary logistic regression test in SPSS (version 21.0). The respondents include 134 men (17.3%) and 642 women (82.7%). The binary logistic regression analysis showed that COVID-19 infection was more common among respondents who were non-health-care workers (p 0.001) and less common among those who had been fully vaccinated (p 0.001). The COVID-19 infection was significantly associated with occupation and vaccination status. Abstract

Chronic obstructive pulmonary disease (COPD) is a progressive lung dysfunction that can be triggered by exposure to chemicals. This disease can be identified with spirometry, but the patient feels uncomfortable, affecting the diagnosis results. Other disease markers are being investigated, including exhaled breath. This method can be applied easily, is non-invasive, has minimal side effects, and provides accurate results. This study applies the electronic nose method to distinguish healthy people and COPD suspects using exhaled breath samples. Twenty semiconductor gas sensors combined with machine learning algorithms were employed as an electronic nose system. Experimental results show that the frequency feature of the sensor responses used by the principal component analysis (PCA) method combined with graph convolutional network (GCN) can provide the highest accuracy value of 97.5% in distinguishing between healthy and COPD subjects. This method can improve the detection performance of electronic nose systems, which can help diagnose COPD.

Abstract

Highlights: Budesonide-Formoterol was the most commonly prescribed medication for controlling and relieving symptoms in most individuals with bronchial asthma. Most patients reported good control of their asthma, and most also reported no subjective problems.   Abstract Background: Bronchial asthma is a common disease affecting the patient's quality of life and impacting daily life. The incidence of asthma is increasing in many countries around the world, although hospitalization and death rates due to asthma have decreased. Objective: This study aimed to explain the sociodemographic (age, sex, body mass index, smoking status) and clinical (comorbidities, subjective complaints, asthma control status, pharmacological therapy, and pharmaceutical step therapy) characteristics of bronchial asthma patients at the Pulmonary Outpatient Clinic the Universitas Airlangga Hospital, Surabaya, Indonesia. Material and Method: An analytic observational study with retrospective cross-sectional research was conducted from December 2021 to January 2023 using secondary data in medical records with total sampling as the sampling technique. Result: From 70 cases, 35 patients made the three-times visits. The majority of these patients who met the inclusion criteria were female (78.6%), 50 to 59 years old (27.1%), had class 1 obese (38.6%), non-smokers (77.1%), and had no comorbidities (28.6%). On the first, second, and third visits, the patients had no subjective complaints, had well-maintained asthma status control, used controllers and relievers in the form of Budesonide-Formoterol, and did not receive additional therapy. They had step therapy in the category of preferred controllers and relievers, steps 1–2. Conclusion: The majority of bronchial asthma patients at Universitas Airlangga Hospital, Surabaya, Indonesia, were female, aged 50–59 years, non-smokers, obese class 1, had no subjective complaints, well-controlled asthma, preferred controllers and relievers steps 1–2, using Budesonide-Formoterol as controller and reliever therapy, and did not receive additional therapy.

Abstract

Improving the quality of healthcare services in hospitals is one of the main goals of achieving a high-quality healthcare system. One of the efforts to enhance the quality of healthcare services in hospitals is implementing an integrated care plan. This study aims to determine the implementation of integrated care plans in hospitals. There were no limitations on methodologies, dates, or settings when reviewing papers describing qualitative or quantitative studies examining the implementation of integrated care plans in hospitals. The setting, design, and findings were all extracted from key study aspects. The major components of the integrated care plan's key components served as a framework for organizing the findings. There are 15 relevant papers published between 2018 and 2023. Most of the studies reported are qualitative, where healthcare workers reflect on their experiences and preferences regarding implementing an integrated care plan. These studies reported on issues underlying the need for the implementation of an integrated care plan, including poor administrative and management systems, lack of access, lack of coordination, lack of regulation, lack of knowledge, and lack of experience, with the ultimate goal of care being aware of cooperation and benefits for patients. An efficient way to raise patient satisfaction and improve the standard of healthcare services is to establish an Integrated Care Plan in hospitals. The efficacy and efficiency of patient care in hospitals can be improved by implementing a more structured and measurable care plan. Abstract

                                                ABSTRACTAsthma presents a global health challenge with high morbidity and potential mortality. Data from Manguharjo Lung Hospital in Madiun (2020-2022) show a consistent rise in asthma cases and related visits. Although asthma is incurable, its management aims to control and reduce attack frequency. Varied understandings of asthma complicate patient management and control. This community service initiative aims to enhance asthma knowledge and control among patients at Manguharjo Lung Hospital. Educational and health promotion methods were employed, assessed via pre- and post-test questionnaires. Results indicate a significant knowledge increase, with post-test scores substantially higher than pre-test scores. Initially, only 30% of participants scored high on the pre-test; this figure rose to 90% post-education. This study demonstrates that the asthma awareness program significantly improved patient knowledge. This psychosocial approach is expected to enable asthma patients to become health advocates within their community.                                          ABSTRAKAsma merupakan tantangan kesehatan global dengan tingkat morbiditas yang tinggi dan potensi mortalitas. Data dari Rumah Sakit Paru Manguharjo di Madiun (2020-2022) menunjukkan peningkatan kasus asma dan kunjungan terkait setiap tahun. Meskipun asma tidak dapat disembuhkan, pengelolaannya bertujuan untuk mengendalikan dan mengurangi frekuensi serangan. Pemahaman yang beragam tentang asma mempersulit pengelolaan dan pengendalian pasien. Studi pengabdian masyarakat ini bertujuan untuk meningkatkan pengetahuan dan pengendalian asma di kalangan pasien di Rumah Sakit Paru Manguharjo. Metode edukasi dan promosi kesehatan digunakan, dengan penilaian melalui kuesioner pra-tes dan pasca-tes. Hasil menunjukkan peningkatan pengetahuan yang signifikan, dengan skor pasca-tes jauh lebih tinggi dibandingkan skor pra-tes. Awalnya, hanya 30% peserta yang mendapatkan skor tinggi pada pra-tes; angka ini meningkat menjadi 90% setelah edukasi. Studi ini menunjukkan bahwa program kesadaran asma secara signifikan meningkatkan pengetahuan pasien. Pendekatan psikososial ini diharapkan dapat memungkinkan pasien asma menjadi penggerak kesehatan dalam komunitas mereka.
