Abstract

Human resources played an important role in the management of an organization in achieving its goals, as an organization required human resources to manage the systems within it. The role of human resources in an organization was very important in determining whether the organization was effective or not. The effectiveness of human resources could be demonstrated by the processes that took place within the organization. One of these was through employee performance. Therefore, the sustainability of the organization depended on employee performance. This employee performance could be influenced by organizational support, workload, and work motivation. This research aimed to determine the relationship between organizational support, workload, and work motivation with the performance of employees at the Dinas Kesehatan Kota Samarinda. The research design was cross-sectional with a population of employees from the Dinas Kesehatan Kota Samarinda. The sample for this study consisted of 50 employees from the Dinas Kesehatan Kota Samarinda (proportional stratified random sampling). The instrument used in this research was a questionnaire. The statistical test employed was the Fisher exact test. The results of the bivariate analysis in this study indicated that there was a relationship between work motivation and employee performance (p=0.016). However, no relationship was found between organizational support and employee performance (p=0.423). Similarly, no relationship was found between workload and employee performance (p=0.216). The conclusion of this study was that there was a relationship between work motivation and the performance of employees at Dinas Kesehatan Kota Samarinda, while there was no relationship between organizational support and workload with employee performance. Recommendations for the agency included continuously improving employee performance, especially in aspects of good motivation, and for future researchers to further investigate with other variables or in different locations.

Abstract

Dengue Hemorrhagic Fever (DHF) remains a pressing health concern in Balikpapan, with 1,081 reported cases in 2022, despite the enforcement of Mayor Regulation No. 9/2020 aimed at prevention. This study analyzed the influence of communication, resources, and disposition on the regulation’s implementation in South Balikpapan through an analytical cross-sectional survey of 189 jumantik cadres, using chi-square tests and multivariate logistic regression. Findings revealed significant relationships between implementation and communication (p = 0.008), resources (p = 0.000; OR = 8.272), and disposition (p = 0.001), with resources identified as the most influential factor. The results highlight the importance of improving resource allocation, establishing structured communication, and motivating cadres to enhance dengue prevention efforts, while recommending future studies to incorporate technological tools like GIS and assess long-term policy outcomes.

Abstract

The utilization of healthcare services is a key indicator of a region’s health system effectiveness. Puskesmas, as a primary healthcare facility, plays a vital role in ensuring access to medical services. However, outpatient service utilization in Indonesia remains low, at 35.16% in 2023, and only 33.39% in East Kalimantan. Puskesmas Bontang Selatan 1 has seen an increase in outpatient visits, yet the Community Satisfaction Survey (SKM) indicates fluctuations, particularly in requirements, procedures, and service convenience. These issues may affect future service utilization. This study analyzes factors influencing healthcare service utilization among outpatients at Puskesmas Bontang Selatan 1 in 2024. The research employs a quantitative analytical survey with a cross-sectional approach. The sample consists of 100 outpatients, selected through Proportional Stratified Random Sampling. Data analysis was conducted using univariate and bivariate methods with Fisher’s Exact Test. Findings show significant associations between healthcare availability (p = 0.018), socio-psychological factors (p = 0.000), and information accessibility (p = 0.002) with healthcare utilization, while doctor services (p = 0.140) showed no significant relationship. It is recommended that Puskesmas improve service information clarity, utilize waiting time for health education, and enhance service quality to increase healthcare utilization.

Abstract

A long with technological advancements and societal demands for services, public service providers are required to meet the expectations of the community in delivering services. One of the efforts that must be made in improving public services is conducting community satisfaction surveys. The purpose of this research is to provide an overview of public satisfaction with the services received. The survey is conducted periodically using a descriptive method approach, where the measurement employs the Likert Scale. The respondents in this study were determined based on the Krejcie and Morgan table, consisting of 70 men and 105 women. This survey was conducted at the Sungai Kapih Health Center in Samarinda City from July to October 2024. Data collection was conducted using questionnaires and interviews with respondents who came to the health center. The results of this survey indicate that the complaint and facility elements have a lower satisfaction index compared to other elements. This is caused by the Sungai Kapih Health Center, which is currently providing services at a temporary location in a rented shop,where one of the components that can influence customer satisfaction is direct evidence or physical evidence (tangibles), namely facilities and infrastructure that can be felt directly or physically by customers. It is hoped that after the renovation process is complete, services can run smoothly, with better and more adequate facilities and infrastructure so that patient expectations and comfort are met. Patient satisfaction is a benchmark in the performance of public services, so it is necessary to immediately meet the expectations of the community regarding the provision of facilities and infrastructure in services at the health center.

Abstract

Purpose: The aim of this activity was to make primary health care capable of doing management and optimum healthcare activity. Methodology/approach: This activity was held in October 2024, placed in Puskesmas Sempaja. These activities were mostly two days of training in 3P Primary Health Care Management. Results/findings: There was an increase in knowledge in the good category from 69,6% to 87%. This result shows that there was an increase in participant knowledge before and after participating in the 3P primary health care management training. The enthusiasm of the participants was also very high in this training. Conclusions : A community service activity titled “Health Center Management Training Using the 3P Method at Puskesmas Sempaja in the Humid Tropical Forest Area of Samarinda City” was conducted over two days. The program involved 23 participants, all of whom were employees of Puskesmas Sempaja. The results indicated a significant improvement in knowledge, with the percentage of participants categorized as having "good" knowledge increasing from 69.6% to 87% after the training. This demonstrates that the 3P method effectively enhanced participants' understanding of health center management. Limitations: The participants were limited to accidental workers who joined this training due to another activity. Contribution: This community service activity can have a positive impact on workers who work in Puskesmas Sempaja. These activities are used to refresh knowledge of primary healthcare management.

Abstract

Data on outpatient visits at Aisyiyah Samarinda Mother and Child Hospital in 2022-2024 where the data for 2022 is 9,839 visitors, in 2023 there has been an increase where the number of patient visits amounted to 18,689 visitors, and in 2024 until October there was a decrease where the number of patient visits amounted to 15,783 visitors. This study aims to determine the relationship between service quality and outpatient satisfaction at Aisyiyah Samarinda Maternal and Child Hospital in 2024. This type of research is quantitative research using cross sectional studies. The number of samples was 132 respondents using the purposive sampling technique. Data analysis uses univariate and bivariate analysis with chi squared test. The results showed that there was a relationship between technical competence (ρ = 0.025), access to services (ρ = 0.001), and comfort (0.008) with outpatient satisfaction at RSIA Aisyiyah Samarinda. Meanwhile, there was no relationship between human relationships (ρ = 0.916) and outpatient satisfaction at RSIA Aisyiyah Samarinda. So it can be concluded that there is a relationship between technical competence, access to services, and comfort with patient satisfaction. However, there was no relationship between human relationships and outpatient satisfaction at RSIA Aisyiyah Samarinda.

Abstract

Ventilator-associated pneumonia (VAP) occurs in patients who have been on mechanical ventilation with an endotracheal tube (ETT) for at least 48 hours. This type of pneumonia typically arises as a result of nosocomial infections or Healthcare-Associated Infections (HAIs) and is commonly linked to the use of ventilators in hospital settings. This study was to identify the factors associated with the incidence of VAP among patients admitted to the Intensive Care Unit of Abdoel Wahab Sjahranie Hospital, Samarinda. Method: A cross-sectional study was conducted. The study population consisted of all patients who had undergone mechanical ventilation in the ICU without a prior history of pneumonia, totaling 118 respondents, using the HAIs bundle observation tool and direct observation. Chi-square and multiple logistic regression were used for statistical analysis, along with a predictive model. The results showed that the prevalence of VAP among respondents in the ICU was 5.9%. Factors significantly associated with the incidence of VAP included duration of ventilator use (p = 0.000), oral hygiene procedures (p = 0.000), head-of-bed elevation to 30–45° (p = 0.007), hand hygiene compliance (p = 0.015), and aseptic suctioning procedures (p = 0.043). The multivariate analysis identified duration of ventilator use ≥96 hours as the most dominant factor, with the highest Odds Ratio (OR = 13.975; 95% CI: 0.753–227.435). Duration of ventilator use was the most significant factor associated with an increased risk of VAP, with patients ventilated for ≥96 hours being 13 times more likely to develop VAP compared to those ventilated for <96 hours. Proper oral hygiene, appropriate head-of-bed elevation (30–45°), compliance with hand hygiene, and aseptic suctioning procedures were also found to be significantly associated with VAP incidence among ICU patients at Abdoel Wahab Sjahranie Hospital

Abstract

This research was conducted to address the increasing number of NSTEMI (Non-ST Elevation Myocardial Infarction) cases, which represent a significant portion of heart attack patients (85%) according to the 2023 Ischemic Myocardial Infarction National Audit. The challenge for healthcare providers is to ensure optimal care for these patients. Clinical Pathways are structured guidelines designed to improve service quality, but despite a relatively high compliance rate (82%) for filling out Clinical Pathways at Taman Husada Bontang Hospital, there has been no clear analysis of how Clinical Pathway implementation benefits patient outcomes. Therefore, the study aims to evaluate the relationship between the implementation of Clinical Pathway and the quality of care provided to NSTEMI patients. The study aims to analyze the relationship between the implementation of the Clinical Pathway for NSTEMI patients and the quality of service at Taman Husada Bontang Hospital. Quantitative research method with a cross-sectional case study approach. The sample technique used is non-probability sampling; the number of samples is 50 Clinical Pathway forms from NSTEMI patients treated at the hospital during the 2022-2023 period. The Chi Square test on the service variance variable has a significant relationship with the implementation of Clinical Pathway, with a p value of 0.000 < 0.05, incomplete documentation resulting in a lack of information about the service variance provided with an OR value of 1.5. The conclusion is that the implementation of Clinical Pathway is significantly related to the quality of services, especially the variance of actions given to patients. To improve service quality, improvements are needed in documentation and regular evaluation of the implementation of the Clinical Pathway.

Abstract

Hemodialysis is a kidney replacement therapy where blood from outside the body is filtered by a machine. The provision of hemodialysis services currently still uses the old rates and is considered not to provide profits for hospitals, so good calculations are needed as a form of efficiency in financing hemodialysis procedures in hospitals, especially in the JKN era. Hospitals will receive comprehensive information on shopping costs, unit cost calculations and action suggestions in accordance with applicable rates. The study was carried out with the aim of finding out the unit cost rates for hemodialysis services in the Kudungga Regional Hospital Dialysis Unit using the Activity Based Costing (ABC) method and comparing them with INA-CBGs rates and hospital rates. This research uses a descriptive and quantitative approach. Unit costs for dialysis units and cost analysis using the Activity Based Costing (ABC) method approach are the main subjects of this research. This research is based on all financial balance data for hemodialysis services at the Kudungga Regional Hospital Dialysis Unit. The unit cost of hemodialysis can be determined by tracing the costs related to the hemodialysis cost center as a production center and other elements that support this action. The data will be grouped into fixed costs, semi-variable costs and variable costs, then included in direct costs, indirect costs, overhead and unit cost calculations using the Activity Based Costing (ABC) method. The research results show that the unit cost calculation for hemodialysis based on the Activity Based Costing (ABC) method is IDR 1,397,627.79. Meanwhile, the Kudungga Regional Hospital rate is only IDR 1,258,300, the hospital rate only meets 90.03% of the unit cost using the ABC method. Meanwhile, when compared with the INA-CBGs tariff, which is IDR 921,300, the INA-CBGs tariff only meets 65.92% of the unit cost using the ABC method. Things that influence are direct costs, indirect costs and the number of hemodialysis procedures.

Abstract

Hospitals that have carried out unit cost calculations can find out how much they are spending, so that the hospital can know if waste occurs. This is because the costs incurred by one hospital will be different from those incurred by another hospital, even though the hospital is in the same type of hospital. Apart from that, the results of unit cost calculations can also be used as a basis for decision making to improve hospital quality and quality. The aim of this research is to calculate rational rates for types of surgery using the multiple distribution method, then compare them with hospital rates for general surgery rates at the Central Surgical Installation of Kudungga Sangatta Regional Hospital. This study is a comparative descriptive quantitative analytical study to compare the rational rates of types of operations (special, special, major, major, and medium cito operations) in the general surgery group during the period of 2023 with the rates of general surgery operations at the Central Surgery Installation of Kudungga Sangatta Regional Hospital with a sample population of all 340 patients using BPJS insurance who received general surgery operations. The results of the research show that the rational rates obtained for special CITO operations are IDR 14,094,633, special operations IDR 12,383,431, large CITO operations IDR 5,262,904, large operations IDR 6,775,203, and medium operations IDR 3,858,857. Rational rates are higher than hospital rates, except for major CITO surgery rates. Therefore, when determining hospital rates, it is necessary to consider unit cost analysis so that hospital operational costs can be efficiently spent.
