Abstract

General Background Preeclampsia remains a major contributor to maternal morbidity and mortality worldwide. Specific Background Early detection strategies using clinical indicators such as Body Mass Index (BMI), Mean Arterial Pressure (MAP), and Roll Over Test (ROT) have been widely discussed in maternal health research. Knowledge Gap However, integrated assessment of these indicators in identifying preeclampsia risk is still limited in specific clinical settings. Aims This study aims to examine the role of BMI, MAP, and ROT in early screening of preeclampsia among pregnant women. Results The findings show that variations in BMI, MAP, and ROT measurements are associated with differences in preeclampsia risk classification. Novelty This study provides a combined perspective of three clinical indicators in preeclampsia screening. Implications The results highlight the importance of utilizing simple clinical measurements as part of early detection strategies in maternal healthcare services. Abstract

ABSTRACT Breech presentation is one of the leading causes of maternal mortality, although it occurs in only 2-3% of pregnancies, it carries significant risks of complications, including an increased risk of maternal death by 20-30%. The purpose of this study is to analyze the factors associated with breech presentation. This study uses an analytical design with a Cross-Sectional approach to analyze the relationship between several factors such as age, parity, polyhydramnios, maternal height, and placenta previa with the incidence of breech presentation. The study population consists of third-trimester pregnant women with a gestational age of 36-42 weeks who meet the inclusion criteria. The sample size of this study is 114 pregnant women in each group, calculated using the Lemeshow formula. Data was collected secondarily from medical records and analyzed using univariate analysis through frequency distribution tables, bivariate analysis using the chi-square test with a significance level of 0.05, and the prevalence of breech delivery was calculated using Prevalence Ratio (PR).  Keywords: Breech Presentation, Pregnancy, Risk Factors.  ABSTRAK Sungsang menjadi salah satu penyebab utama angka kematian ibu, yang walaupun hanya terjadi pada 2-3% kehamilan, tetapi memiliki risiko komplikasi yang signifikan, termasuk peningkatan risiko kematian ibu hingga 20-30%. Tujuan penelitian ini adalah menganalisis faktor yang berhubungan dengan janin letak sungsang. Penelitian ini menggunakan desain analitik dengan pendekatan Cross Sectional untuk menganalisis hubungan antara beberapa faktor seperti usia, paritas, hidramnion, tinggi badan ibu, dan placenta previa dengan kejadian letak sungsang. Populasi penelitian adalah ibu hamil trimester III dengan usia kehamilan 36-42 minggu yang memenuhi kriteria inklusi. Biesar sampiel pienielitian ini adalah 114 ibu hamil tiap kielompok yang di hitung mienggunakan rumus Liemieshow. Data dikumpulkan secara sekunder dari rekam medis dan dianalisis menggunakan analisis univariat dengan menggunakan tabel distribusi frekuensi, analisis bivariat dengan menggunakan uji chi – square dengan tingkat kemaknaan 0,05 serta besar prevalensi persalinan sungsang dihitung menggunakan RP (Rasio Prevalensi). Kata Kunci: Letak Sungsang, Kehamilan, Faktor Resiko.

Abstract

Maternal-neonatal emergencies require rapid decision-making and timely referral to prevent poor outcomes for mothers and newborns. However, referral decisions are often influenced not only by health care providers but also by family members who hold authority, provide resources, and give permission during emergencies. This study aimed to explore family roles in referral decision-making for maternal-neonatal emergency cases. This study used a qualitative design with a phenomenological approach. Ten family members who had been directly involved in referral decision-making during maternal or neonatal emergency situations were selected through purposive sampling. Data were collected using in-depth, semi-structured interviews supported by field notes. Interview recordings were transcribed verbatim and analyzed using thematic analysis. Trustworthiness was maintained through credibility, transferability, dependability, and confirmability. Four main themes emerged from the analysis: family authority in referral decision-making, recognition of emergency conditions, barriers to timely referral, and trust in health care providers. Husbands, parents, and in-laws often influenced approval or delay of referral. Families who recognized danger signs tended to accept referral more quickly, while limited knowledge, financial concerns, transportation difficulties, distance, and the need for family discussion contributed to delays. Clear communication and reassurance from health care providers strengthened family trust and supported faster referral decisions. Family roles in maternal-neonatal emergency referral are complex and multidimensional. Strengthening family-centered education, effective provider communication, and referral preparedness is essential to improve timely decision-making and support better maternal-neonatal outcomes.

Abstract

Background: Postpartum hemorrhage (PPH) remains one of the leading causes of maternal mortality worldwide. The management of PPH requires rapid and accurate clinical decision-making by healthcare professionals, particularly midwives who serve as frontline providers in maternal care. In addition to clinical competence, emotional responses, teamwork, and personal values may influence midwives’ decision-making processes during obstetric emergencies. Objectives: This study aimed to explore midwives’ experiences of clinical decision-making in managing postpartum hemorrhage, with consideration of the role of spirituality in clinical practice. Methods: This qualitative phenomenological study involved 14 midwives working in delivery or postpartum units who had direct experience managing at least two PPH cases within the previous year. Participants were selected using purposive sampling until data saturation was achieved at the 14th interview. Data were collected through in-depth semi-structured interviews and field notes and analyzed using the Colaizzi phenomenological method. Trustworthiness was strengthened through member checking, source triangulation across participants and hospital contexts, peer debriefing, and an audit trail. Results: The analysis revealed four major themes describing midwives’ experiences in managing PPH: (1) emotional responses in life-threatening situations, (2) rapid clinical decision-making based on standard operating procedures (SOP) in critical conditions, (3) midwives’ leadership and teamwork within health system limitations, and (4) the integration of spiritual values as a source of strength and professional meaning in clinical practice. Conclusions: Midwives’ experiences in managing postpartum hemorrhage represent a dynamic process involving emotional regulation, adherence to clinical protocols, teamwork, and spiritual coping. The integration of clinical competence and psychosocial support plays an important role in supporting effective clinical decision-making during obstetric emergencies.

Abstract

The high incidence of postpartum fatigue and anxiety in Indonesia hindering mothers’ recovery and breastfeeding success. Non-pharmacological and safe interventions aligned with local cultural values are needed. Islamic guided imagery provides relaxation that integrates positive visualization with the spiritual value of zikr. This study aims to examine the effectiveness of Islamic guided imagery in reducing fatigue and anxiety scores in postpartum mothers. The study used a quasi-experimental design with a pre-posttest and a control group. A total of 45 postpartum mothers (0–6 weeks) were recruited using consecutive sampling techniques and divided into intervention (n = 23) and control (n = 22) groups. The Islamic guided imagery intervention was delivered in audio format for 15–20 minutes per day for 3 days, demonstrating its simplicity and feasibility for busy postpartum care routines. Fatigue levels were measured using the Postpartum Fatigue Scale (PFS), and anxiety was measured using the State Anxiety Inventory (STAI-S). Data were analyzed using an independent t-test. The intervention group showed a significant and progressive reduction in fatigue and anxiety scores from day 1 to day 3 compared with the control group (p 0.001). The quick improvements of -15.8 and -17.7 points in fatigue and anxiety scores, respectively, can inspire healthcare providers and researchers to consider this intervention as an effective, fast-acting option. The Islamic guided imagery effectively reduces postpartum fatigue and anxiety by strengthening emotional regulation and spiritual coping. Recommendations for future research include conducting randomized controlled trials with longer follow-up and including stress biomarker measurements to strengthen the generalizability of findings.

Abstract

Perkembangan Penggunaan smartphone pada remaja berdampak signifikan terhadap kesehatan mental dan nasionalisme remaja. Rendahnya literasi digital serta tingginya paparan budaya global menjadi tantangan utama di lingkungan sekolah. Kegiatan pengabdian ini bertujuan meningkatkan digital wellbeing melalui penguatan kesehatan mental dan nasionalisme pada siswa SMA Muhammadiyah 2 Sidoarjo. Metode menggunakan pendekatan partisipatif berbasis intervensi edukasi melalui workshop literasi digital kesehatan mental remaja, intervensi mindfulness dan manajemen stres, serta digital patriotism campaign. Evaluasi dilakukan menggunakan desain pre-post test dengan instrumen kuesioner (IDASSY) literasi  digital modul kesehatan mental dan nasionalisme. Hasil menunjukkan peningkatan literasi digital sebesar 25.9%, penurunan stres 30,8% dan kecemasan 28.7%, peningkatan regulasi emosi 24.3%, serta peningkatan nasionalisme 23.9%. Selain itu terjadi peningkatan partisipasi kegiatan kebangsaan. Program ini terbukti efektif dalam membentuk perilaku digital sehat, meningkatkan kesejahteraan psikologis, serta memperkuat karakter nasionalisme remaja.
