Study of Contraceptive Devices Management in The National Health Insurance Maulana, Afif; Saimi, Saimi; Khalik, Lalu Abdul

Abstract

The National Health Insurance or JKN, which was launched on January 1 2014, is intended to increase public access to comprehensive and quality health services, so as to achieve universal health coverage (UHC). Effective promotive-preventive efforts will reduce the incidence of disease and have an impact on reducing the number of sick people and the number of people seeking treatment so that health financing is more efficient. Family planning (KB) services are part of promotive-preventive efforts. In the management of program contraceptive devices provided by the government in the last 3 years, various problems were still encountered, including shortages of contraceptive devices (stock out), accumulation of Alkon in storage warehouses, and Alkon distribution mechanisms that were not smooth. The research was conducted using a descriptive quantitative approach. Research location in East Lombok Regency taking into account the high vacancy of Alkon (Data F/II/KB online). Then, 31 First Level Health Facilities (FKTP) were taken, namely Community Health Centers, Doctor's Practices, Pratama Clinics and Type D Pratama Hospitals. The research was carried out for 6 months, namely May-October 2017. The results of the research show that in general the management of Alkon in the OPD-KB of East Lombok Regency has complied with technical instructions, but there are several mechanisms for providing, storing, distributing, recording and reporting as well as Alkon monitoring and evaluation requires clear and written Standard Operating Procedures (SOP) that refer to technical manuals, and are spread throughout the OPD-KB, UPT-KB, Health Faskes and Network areas, so that the management of the mechanism runs uniformly.

Abstract

The focus of the global problem that is of concern both in the world and in Indonesia is the HIV/AIDS case, that reported HIV/AIDS cases in districts/cities in Indonesia were 22,331 HIV cases and 4,534 AIDS cases. Meanwhile in NTB in 2022, there were 1471 accumulated AIDS cases and the addition of 192 new cases, in Central Lombok Regency there were 378 HIV cases and 220 AIDS cases as of September 2022, as well as 211 HIV cases in the MSM group. The strategy determined was to carry out in-depth interviews using the Snow Ball method with 4 informants, namely the VCT/KTH Mandalika Team at Praya Regional Hospital, Management at Praya Regional Hospital, MSM Clients at KTH at Praya Regional Hospital and the HIV Program Holder of the Central Lombok District Health Service. Test the validity of the data with Credibility, Continuous Observation, Triangulation, Transferability, Depreciability, Confirmability. This research has received ethical approval from the Ethics Committee of Qamarul Huda Badaruddin Bagu University with Number: 022/EC/FKES-UNIQHBA/YPPQH/VIII/2023. Management of HIV/AIDS Prevention Services in the MSM Group at KTH Mandalika RSUD Praya still needs to improve the effectiveness of HIV/AIDS prevention, more intensive strategic planning is needed to overcome the increase in HIV prevalence, involving an approach through a TEAM of Field Officers in providing more effective education and social support through VCT services and hotline counseling. Coordination and a stable organizational structure in KTH Mandalika RSUD Praya cannot be ignored, so that response efforts can run more focused. Staff training and development, especially those related to handling PLWHA, needs to be based on objective selection and unit needs, while the use of information technology, such as the SIHA 2.1 application and RS SIM, can be improved to ensure accurate recording and reporting. Management of BHP and ARV availability needs to be optimized through efficient coordination with the District and Provincial Health Services, and regular evaluations need to be carried out to increase distribution efficiency. Implementing the recommendations from 4 informants can have a significant positive impact in managing the management of HIV/AIDS prevention services at KTH Mandalika RSUD Praya, so as to reduce HIV transmission, and provide better support to PLWHA in the MSM community..

Abstract

In West Nusa Tenggara the prevalence of stunting is placed seventh with a frequency of 33.49%, whereas the prevalence of stunting in Indonesia is 30.8%, according to Riskesdas 2018. A useful strategy for raising awareness and preventing stunting problems is to use KIA book media to facilitate brainstorming sessions for 1000 HPK. To ascertain the impact of counseling on 1000 HPK (First Day of Life) participants' knowledge of reducing stunting at the Narmada Community Health Center Working Area through the use of the KIA book media and the brainstorming approach. Nonequivalent Control Group Design is the research method utilized in quasi-experimental design studies. There were 112 research targets as the research subjects. A questionnaire was the tool utilized. The independent sample t-test and the paired sample t-test were used to test the data. Findings from Research. There were four people (7.14%), eighteen people (32.14%), and thirty-four people (60.71%) who knew about pregnant women before treatment for the poor category. Following the intervention, it rose to fall into one of three categories: less than none, moderate (consisting of 11 individuals/19.64%), and good (consisting of 45 individuals (80.36%). The results of statistical tests utilizing the paired sample t-test show that there is a significant influence between the usage of KIA book media at 1000 HPK and brainstorming approaches, with a p value of 0.000 < 0.05. Pregnant women's knowledge increased after counseling with the brainstorming method using KIA book media, according to the analysis's results, which also indicate a substantial difference in the women's knowledge before and after counseling.
