Abstract

The global increase in the older adult population necessitates the development of practical, evidence-based nursing interventions to address critical challenges to Health-Related Quality of Life (HRQoL), notably loneliness and a diminished sense of purpose. The Intergenerational Reminiscence Model (IRM), involving older adults sharing life experiences with younger generations, offers a highly promising psychosocial approach. This paper provides a conceptual analysis to philosophically ground the IRM within a pragmatist framework, arguing that this lens substantially enhances its theoretical rigor and effectively guides its future application in nursing science. Pragmatism is specifically chosen because its emphasis on action, experience, and practical consequences aligns perfectly with the IRM's primary objective of achieving measurable improvements in HRQoL. From an ontological perspective, pragmatism defines reality as dynamic, constructed actively through meaningful interactions older adults experience within their environments during intergenerational exchanges. Epistemologically, the core argument asserts that the true effectiveness of the IRM is strictly validated by its practical outcomes, specifically, its success in enhancing HRQoL. Axiologically, this framework prioritizes human flourishing, positioning observable improvements in HRQoL as the ultimate value claim. Methodologically, while this paper is philosophical, pragmatism provides robust theoretical justification for future empirical research utilizing mixed methods approaches. This is necessary to successfully capture quantitative results and the rich narratives of intergenerational experiences. Ultimately, pragmatism offers an ideal justification for a context-sensitive nursing intervention. Future research must utilize mixed methods to fully illustrate these benefits. In conclusion, pragmatism remains essential for advancing the IRM into a robust, evidence-based geriatric nursing model. Keyword: HRQoL, Intergenerational reminiscence model, Older adults, Pragmatism

Abstract

Pendahuluan: Posyandu Keluarga adalah bentuk pelayanan kesehatan berbasis masyarakat yang dirancang untuk meningkatkan derajat kesehatan pada seluruh tahapan kehidupan, termasuk kelompok lansia. Keberhasilan kegiatan Posyandu Keluarga sangat ditentukan oleh tingkat keaktifan dan kemandirian lansia dalam berpartisipasi pada berbagai aktivitas yang disediakan, seperti pemeriksaan kesehatan berkala, senam lansia, dan kegiatan penyuluhan kesehatan. Lansia yang mampu berperan aktif dan mandiri umumnya memiliki kondisi kesehatan yang lebih optimal serta dapat mempertahankan peran sosialnya di lingkungan masyarakat. Tujuan: Tujuan penelitian ini adalah untuk mengetahui hubungan dukungan keluarga dengan keaktifan dan kemandirian lansia posyandu keluarga di Desa Bentek. Metode: Metode penelitian kuantitatif dengan metode cross sectional, jumlah sampel dalam penelitian ini sebanyak 84 responden didapatkan dengan teknik Proportionate Stratified Random Sampling. Instrumen yang digunakan adalah kuesioner dan wawancara. Hasil dari kuesioner dianalisis menggunakan uji Chi Square. Hasil: Hasil penelitian menunjukkan Dukungan keluarga kategori kurang sebanyak 61 responden (72,6%) ditunjukkan dengan kurangnya penghargaan yang diberikan kepada lansia, keaktifan kategori tidak aktif sebanyak 40 responden (47,6%) ditunjukkan dengan lansia tidak mengikuti olahraga seperti senam lansia, tidak mengikuti pendidikan kesehatan, pemeriksaan secara berkala, kemandirian kategori tidak mandiri sebanyak 67 responden (79,8%) ditunjukkan dengan tidak dapat melakukan aktivitas berat seperti naik turun tangga. Uji analisa data menunjukkan p value 0,003< 0,05. Kesimpulan: Ada hubungan dukungan keluarga dengan keaktifan lansia datang ke posyandu keluarga dengan kemandirian lansia di Desa Bentek.

Abstract

Early Initiation of Breastfeeding (IMD) is the process of placing the baby on the mother's chest immediately after birth and allowing the baby to self-feed for at least one hour. IMD is beneficial for providing colostrum, increasing immunity, and supporting the success of exclusive breastfeeding. In Mataram City, exclusive breastfeeding coverage is still below the national target, with Pagesangan Health Center recording the lowest achievement in 2023 at 42.01%. This study aims to analyze the relationship between IMD and the success of exclusive breastfeeding at Pagesangan Health Center, Mataram City. This study used an observational analytic design with a cross-sectional approach. The population was 398 mothers with infants aged 7-12 months. A sample of 80 respondents was selected using purposive sampling. Data was collected through questionnaires and analyzed using the Chi-Square test with a 95% confidence level (α=0.05). Of the 80 respondents, 55 (68.8%) performed IMD and 50 (62.5%) successfully gave exclusive breastfeeding for up to six months. Statistical analysis showed a significant relationship between IMD and the success of exclusive breastfeeding (p=0.005; OR=7.855). IMD is significantly associated with the success of exclusive breastfeeding. Health workers need to strengthen education and support for postpartum mothers to improve IMD practices and exclusive breastfeeding coverage.
