Abstract

The SARS-CoV 2 virus pandemic has been a scourge since 2020. The number of cases that continues to increase, both mild, moderate, severe-critical confirmed cases are directly proportional to the need for treatment rooms, both ordinary wards and intensive care units (ICU), which is inversely proportional to the available capacity. room. Based on research, the high mortality and morbidity rate due to COVID 19 is due to the incidence of patient sepsis. Research abroad states that the value of D-dimer is a prognostic factor for COVID-19 patients, although there is no agreement on the best cut-off point for specificity and sensitivity. hypercoagulability conditions and increased lactic acid in patients with severe sepsis. It is necessary to evaluate the role of this laboratory result in estimating the prognosis of COVID-19 patients in Indonesia. There is no study that examines the relationship between the incidence of sepsis and the characteristics of hypercoagulability with the dominance of the thrombotic process in COVID19 patients in Indonesia. The purpose of this study was to provide information to health workers about the role of D-dimer value as a prognostic factor for severe-critical COVID-19 patients. This quantitative retrospective research data in the form of proportions were collected for 12 months from the medical records of the UGM Academic Hospital of patients with confirmed severe-critical COVID-19. Overall survival was assessed by the Kaplan-Meier curve. The cut-off determination for the D-dimer variable was carried out using the Receiver Operating Curve (ROC) followed by the calculation of the Youden index. Then the hazard ratio was determined by cox regression with a p value of <0.05 which was considered significant. Statistical analysis with SPSS version 26 software.

Abstract

Thalassemia is a disease that attacks the hemoglobin gene, causing disturbances in erythropoiesis. Anemia is a condition in which the hemoglobin (Hb) concentration and the number of erythrocytes are below normal limits. The high number of anemia and thalassemia causes the need for early detection in order to prevent adverse effects in the future. Research objectives: to determine nutritional status, anemia status, and application of the Mentzer Index for SMK Muhammadiyah II Moyudan students. Methods: This study is a descriptive study with a cross-sectional design. The research subjects were first graders at SMK Muhammadiyah 2 Moyudan. Venous blood samples were examined at the Clinical Pathology Laboratory of FK-KMK UGM using the Sysmex XN-1000 tool on November 25, 2021. Data analysis used IBM SPSS Version 26 and Microsoft Office 365. Results: A total of 53 subjects participated in this study. A total of 47 (89%) respondents had normal hemoglobin values, and 6 (11%) respondents suffered from anemia. 31 (58.5%) respondents have normal nutrition, 15 (28%) respondents have less nutrition, 7 (13%) have excess nutrition, and 29 (54.7%) respondents have normal status. A total of 2 (3.8%) respondents showed Thalassemia. Conclusion: There are two students of SMK Muhammadiyah 2 Moyudan diagnosed with thalassemia and five students diagnosed with anemia. No statistically significant difference exists between Anemia Status in Adolescents at SMK Muhammadiyah 2 Moyudan and Nutritional Status. Further study is needed in wider and more diverse populations to analyze the influence of nutritional conditions on anemia in adolescents.

Abstract

Metabolic syndrome is one of the main risk factors for various diseases such as cardiovascular disease and diabetes. The effect of physical activity has been widely studied regarding its benefits in reducing the development of metabolic syndrome. However, there are still underlying shortcomings in the results of previous research. Objective: to assess the influence of physical activity and metabolic syndrome components on the risk of metabolic syndrome in workers. Cross-sectional research was conducted on employees of PT. PLN. Variables assessed include waist circumference, arm circumference, body mass index (BMI), blood pressure, blood glucose and total cholesterol. Physical activity assessment was carried out using the Global Physical Activity Questionnaire (GPAQ). A total of 40 employees were included in this study. A total of 18 respondents (45%) were at risk of metabolic syndrome. The majority of metabolic syndrome components in employees are overweight (50%), excessive arm and waist circumference (85% and 65%, respectively). The majority of respondents had moderate physical activity (57.5%) and high activity (27.5%). Excessive arm and waist circumference had a significant relationship with the risk of metabolic syndrome in respondents (p < 0.05), however, physical activity did not affect the risk of metabolic syndrome. It is hoped that further research in broader populations will be needed to assess risk factors for metabolic syndrome in workers.

Abstract

Jaminan Kesehatan Nasional (JKN) yang diselenggarakan BPJS adalah jaminan berupa perlindungan kesehatan agar peserta memperoleh manfaat pemeriksaan kesehatan dan perlindungan dalam memenuhi kebutuhan dasar kesehatan yang diberikan kepada setiap orang. Pengukuran tingkat kepuasan pasien dibutuhkan untuk mengetahui tingkat pelayanan yang diadakan suatu penyelenggara jasa. Tujuan dari penelitian ini untuk mengidentifikasi perbedaan kepuasan pasien JKN kelas 3 PBI dan pasien JKN kelas 3 non PBI yang mendapatkan pelayanan kesehatan pada salah satu rumah sakit di Bantul. Penelitian ini menggunakan desain penelitian observasional analitik dengan metode cross sectional melalui pendekatan Kuantitatif. Data didapatkan dengan menggunakan kuesioner dari responden. Tingkat kepuasan pasien dianalisis menggunakan skor GAP. Hasil skor GAP menunjukkan bahwa pasien JKN PBI memiliki skor GAP terendah pada dimensi reliability yaitu -0,1733. Dari hasil analisis, pasien di salah satu rumah sakit di Bantul secara umum merasa puas terhadap pelayanan yang diberikan.

Abstract

Background: The gold standard therapy for acute ischemic stroke is timely reperfusion of ischemic brain tissue. Intravenous thrombolysis with tPA is the only proven medical therapy for acute ischemic stroke within 4.5 hours of symptom onset using intravenous alteplase at a dose of 0.9 mg per kilogram of body weight. Hemorrhagic transformation is one of the complications of thrombolytic therapy and East Asian population having a higher prevalence of cerebral hemorrhage. This study will examine several cases of ischemic stroke that were treated with thrombolysis using a standard dose (0.9 mg/kg) of intravenous alteplase in acute ischemic stroke with an onset of less than 6 hours in the Special Region of Yogyakarta, especially at the UGM Academic Hospital. Objective: To determine the efficacy of standard dose thrombolysis using intravenous alteplase (rTPA-recombinant tissue plasminogen activator) in acute ischemic stroke with an onset of less than 6 hours through activation of the Code Stroke. Methods: Descriptive research design using a case series, the hyperacute stroke patients with an onset of less than 6 hours who received intravenous alteplase which were then assessed by the National Institutes of Health Stroke Scale (NIHSS) score at initial admission, 24 hours post-alteplase and 30 days post -alteplase. The study took place and was conducted from May to October 2022 by administering intravenous alteplase at a dose of 0.9 mg/kg body weight in acute ischemic stroke patients at UGM RSA who are eligible for thrombolysis therapy with a maximum administration time of 6 hours after stroke onset with the maximum dose of alteplase is 50 mg. Results: The study sample was 8 patients with acute ischemic stroke who were treated between May to October 2022. There were 2 patients who died before completing the 3-month follow-up. One of the patients died within the second week of treatment from sepsis which may have occurred from a pre-existing pneumonia. Another patient died from ileus that occurred 1 month after tissue plasminogen activator (tPA). However, the patient with this ileus showed clinical improvement at the 24-hour post-tPA follow-up, i.e. the initial NIHSS score of 12 improved to an NIHSS score of 6. Conclusion: The modified Alteplase dose, which is 0.9 mg/kg body weight with a maximum dose of 50 mg, at the onset of stroke less than 6 hours can be an option to maximize thrombolytic therapy while still considering the efficiency of treatment costs.

Abstract

Suicide is an incident that is a severe problem in society which is currently widespread. Suicide is the act of ending one's own life by using several techniques, such as hanging, making an incision in a blood vessel, or crashing into a moving vehicle such as a train or motorbike. Based on data from the World Health Organization (WHO), as many as 703,000 people committed suicide, with variations in suicide rates in various countries between 2 and 80 suicide deaths per 100,000 population. Abstract

Introduction: Butterfly pea flowers (Clitoria ternatea L.) currently popular in society as high-antioxidant supplement. One of the organs affected by aging is the brain. D-galactose is a compound that is widely used for induction of aging in animal studies. The aim of this study was to assess the effect of butterfly pea flower tea on changes in brain weight in mice induced by D-galactose. Method: 24 rats were divided into four groups consisting of control, groups with 1%, 5%, and 10% butterfly pea flower tea intervention after being induced by aging for 6 weeks. The brain organs were then weighed to assess changes in each group. Result: There was an increase in body weight but no significant differences between each group either before or after the intervention. No significant differences were found in the average brain weight of each group, with the brain weight of the P3 group being lower than that of the other intervention groups. There is a positive correlation between body weight and brain weight. Conclusion: Although the butterfly pea flower has no statistically significant effect, at concentrations of 1% and 10% it can have an effect on brain weight after induction of aging by D-galactose.

Abstract

Banyak penelitian yang menunjukan bahwa kualitas layanan pada berbagai fasilitas kesehatan dipengaruhi oleh sumber pembiayaan. Hal serupa juga ditemukan di Unit Rawat Inap. Berbagai data menunjukan bahwa ada perbedaan layanan pasien antara pasien umum dan pasien JKN. Hal ini tentunya bertentangan dengan Peraturan Presiden Republik Indonesia No.82 Tahun 2018 bahwa peserta JKN berhak mendapatkan layanan kesehatan sesuai dengan kebutuhan medis yang diperlukan. Oleh karena itu, penelitian ini bertujuan untuk membahas seberapa besar perbedaan tingkat kepuasan pasien JKN dan pasien umum terhadap mutu pelayanan di Unit Rawat Inap. Metode studi dilakukan dengan pendekatan Narative review yang bersumber pada database PubMed, Google schollar dan IJPHS. Terdapat 39.970 artikel yang ditemukan dengan kata kunci kepuasan pasien dan mutu layanan kesehatan; patient satisfaction and quality of health services. Selanjutnya artikel tersebut diidentifikasi, disaring, hingga diperoleh 5 artikel yang memenuhi kriteria untuk kemudian diulas secara mendalam. Secara garis besar terdapat perbedaan tingkat kepuasan pasien JKN dan pasien umum terhadap mutu pelayanan di Unit Rawat Inap. Perbedaan terletak pada kepuasan pada dimensi kehandalan/tangible, daya tanggap/ responsiveness, jaminan/assurance, peduli/empaty, dan bukti langsung/ reliability. Sebanyak 60% penelitian menunjukan adanya perbedaan dalam tingkat kepuasan antara pasien JKN dan pasien umum di Unit Rawat Inap dapat dijadikan evaluasi bagi Pelaksana Layanan Kesehatan sehingga tidak ada lagi perbedaan tingkat kepuasan pasien. Hal ini dikarenakan setiap pasien berhak mendapatkan layanan kesehatan yang sama sesuai dengan kebutuhan medisnya.

Abstract

The transition to online learning has brought significant challenges and opportunities for higher education, emphasizing the importance of self-regulation as a critical skill for academic success. This study aims to profile students' self-regulation patterns in online learning environments using a combination of cluster and discriminant analyses. Data were collected from 577 undergraduate students using the Online Self-Regulated Learning Questionnaire (OSLQ), which measures six dimensions of self-regulation: goal setting, environment structuring, task strategies, time management, help-seeking, and self-evaluation. Cluster analysis, employing the K-Means method, identified three distinct student groups: (1) students with high self-regulation, excelling in goal setting, time management, and self-evaluation; (2) students with moderate self-regulation, showing adequate abilities with some limitations in task strategies and help-seeking; and (3) students with low self-regulation, struggling across all dimensions. Discriminant analysis revealed that self-evaluation, goal setting, and task strategies were the primary variables differentiating these clusters, with self-evaluation emerging as the most significant predictor. The findings underscore the critical role of self-regulation in online learning success and highlight the need for tailored interventions to support students with low self-regulation. These insights provide valuable implications for educators and institutions to design more adaptive and effective online learning strategies.
