Abstract

Abstract

Abstract

Introduction: Critically-ill patients experience catabolic stress and inflammatory responses resulting in increased catabolism to meet energy demands. Neutrophil lymphocyte ratio (NLR) is used to assess immune and stress responses. Method: This study was a cross-sectional study of subjects aged ≥18 years admitted to the intensive care unit of Dr. Cipto Mangunkusumo National Central General Hospital and University of Indonesia Hospital. The characteristics of the study subjects included age, gender, nutritional status, diagnosis of ICU admission, protein intake, fat intake, and carbohydrate intake. Bivariate analysis was performed to evaluate the correlation between energy intake and neutrophil-lymphocyte ratio.Result: There were 44 subjects with a median age of 55 years (19-86). Nutritional status based on body mass index (BMI) obtained 31.8% normal nutritional status, 20.5% severe malnutrition, 25% obesity grade 1. The most common diagnoses were surgery and trauma 20.5% of subjects and sepsis 20.5% of subjects. Median RNL was 10.39 (2.51-59.5), energy intake was 10.5 (3-29) kcal/kg BW/day, protein intake was 0.4 (0-1.2) g/kg BW/day, fat intake was 18 (0-29)% and carbohydrate intake was 92 (23-184) g/day.Conclusion: There was no correlation between energy intake and NLR.

Abstract

BACKGROUND: Indonesia has the second highest burden of tuberculosis (TB) worldwide, with an incidence of 354 per 100,000 population and approximately 969,000 cases reported in 2021. Micronutrients such as vitamin A and zinc play important roles in immune function and may influence inflammatory responses in TB. The neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) are potential markers of systemic inflammation; however, evidence regarding their association with dietary micronutrient intake in pulmonary TB patients remains limited, particularly in Indonesia. AIMS: To examine the association between vitamin A and zinc intake and inflammatory markers in pulmonary TB patients. METHODS: A cross-sectional study was conducted among 133 pulmonary TB patients recruited consecutively from March to April 2024 at Persahabatan Hospital. Vitamin A and zinc intake were assessed using a semi-quantitative food frequency questionnaire (SQ-FFQ), while NLR and PLR values were obtained from medical records. Data normality was tested using the Kolmogorov–Smirnov test, and correlations were analyzed using Spearman’s test. The median vitamin A intake was 105.47 RE/day and zinc intake was 7.38 mg/day, with median NLR and PLR of 2.91 and 202.08, respectively. RESULTS: No significant correlations were found between vitamin A or zinc intake and NLR or PLR (p > 0.05). CONCLUSION: In conclusion, vitamin A and zinc intake were not associated with inflammatory markers in pulmonary TB patients, although host-related factors may contribute to the inflammatory response.

Abstract

Burn injuries result in tissue damage due to exposure to thermal sources or extreme cold. Following burn trauma, systemic inflammatory responses can impair gastrointestinal (GI) function, often leading to constipation. While dietary fiber supplementation is commonly recommended to prevent constipation in the general population due to its ability to enhance intestinal motility, there is limited literature regarding its role in managing constipation among patients with major burn injuries. This case series presents two patients admitted with 34% and 35% total body surface area (TBSA) burns, respectively. Both patients experienced delayed defecation during the first week of hospitalization, without experiencing hypovolemic shock, and had not been administered any sedative agents or vasopressors. Their average dietary fiber intake were 11.1 g and 6.5 g, respectively. One male patient, who was diagnosed with diabetes mellitus (DM), continued to experience constipation until the second week, which improved following dietary modification to a DM-specific regimen. Meanwhile, the female patient showed an improvement in bowel movements in the second week, despite her low fiber intake. Keywords: fiber intake, defecation, major burns

Abstract

Background: An autoimmune condition known as myasthenia gravis (MG) targets the receptors for neurotransmitter acetylcholine at the neuromuscular junction, resulting in inhibition of muscle contraction. This results in muscle weakness resulting in a decrease in quality of life. Immunoregulation and muscle contractility are known to play a role in vitamin D. Literature on how vitamin D affects myasthenia gravis risk has not been widely carried out and the results are still controversial. Objective: To evaluate the association between vitamin D and MG risk. Methods: The search of the literature was conducted from PubMed, Cochrane Library, Embase, and EBSCOhost with the eligibility criteria determined by the authors. The literature search was using MeSH Term, text word, and title/abstract.Results: Two articles were selected and critically appraised. The first article shows an odds ratio of 3.96 (CI95 1.26 to 12.52), which means that myasthenia gravis has vitamin D levels almost 4 times lower than healthy population. A case-control study that followed described a comparison of mean levels of vitamin D (25(OH)D) in myasthenia gravis (mean, 18.8±8.4 ng/mL) compared to healthy controls (26.3±6.1). ng/mL) (p <0.05). Both studies revealed a strong interaction between MG and vitamin D inadequacy. Conclusion: Both studies above support the theory that vitamin D deficiency is associated with the risk of developing MG.

Abstract

Abstract: Interstitial lung disease (ILD) is a group of lung disorders characterized by chronic inflammation and fibrosis, with a global prevalence ranging from 6.3 to 71 cases per 100,000 population. One subtype, connective tissue disease–associated ILD (CTD-ILD), is closely related to immune system dysregulation and progressive fibrosis. We reported a 45-year-old woman with CTD-ILD prone to malnutrition due to increased metabolic demands and reduced intake caused by dyspnea, nausea, and vomiting. This condition may impair muscle function, reduce lung capacity, and worsen clinical outcomes. Therefore, nutritional assessment plays a crucial role in patient management. Interventions include a balanced diet and nutritional support to meet energy and protein requirements, targeting 1.2–1.5 g/kgBW/day. Supplementation with vitamin D at a dose of 5,000 IU once daily and micronutrients were provided to support metabolism and immune function. During treatment, the patient showed improvement in energy intake, reaching up to 95% of daily requirements and enhanced clinical response through continuous nutrition education. In patients with ILD, especially CTD-ILD, vitamin D plays a supportive therapeutic role by modulating immune responses, reducing fibrotic processes, preserving respiratory muscle function, and supporting bone health. In conclusion, vitamin D deficiency is prevalent in CTD-ILD patients and is linked to disease progression, making vitamin D supplementation an important component of maintenance therapy.
