Abstract

Pada kanker payudara operabel dengan metastasis regional, pemberian kemoterapi ajuvan berbasis antrasiklin dapat menyebabkan terjadinya anemia ringan sampai berat pada sekitar 4% - 63% kasus. Telah dilakukan berbagai penelitian tentang peran epoetin alfa sebagai alternatif pengganti transfusi, tetapi hasilnya masih belum memuaskan. Dilakukan penelitian uji klinik desain paralel dengan randomisasi blok terhadap 64 sampel penelitian yang dibagi atas 2 kelompok, yaitu kelompok perlakuan yang mendapatkan epoetin alfa dan kelompok kontrol. Kadar Hb pasca-operasi yang termasuk dalam kriteria inklusi adalah > 10 gr/dL - 12 gr/dL. Epoetin alfa diberikan 1 minggu pasca-mastektomi diteruskan sampai 6 kali pemberian dengan dosis 40.000 IU/ minggu dan kadar Hb dinilai mulai dari pemberian kemoterapi siklus pertama sampai 3 minggu setelah kemoterapi siklus keenam. Hasil penelitian menunjukkan pemberian epoetin alfa dengan strategi di atas mampu mempertahankan kadar hemoglobin di atas 10 gr/dL dan tidak dibutuhkan transfusi selama kemoterapi. Sedangkan pada kelompok kontrol membutuhkan transfusi sebanyak 28,1% kasus dan kebutuhan transfusi paling banyak terjadi pada kemoterapi siklus keempat. Kesimpulan: strategi pemakaian epoetin alfa yang dapat dipilih adalah mulai 1 minggu pasca-mastektomi yang diteruskan sampai kemoterapi siklus kedua atau 6 kali pemberian dengan dosis 40.000 IU/minggu dan kadar Hb pasca-mastektomi harus > 10 gr/dL - 12 gr/dL.Kata kunci: kanker payudara, anemia, epoetin alfa.

Abstract

ABSTRACT: Arabic is indispensably necessary, especially in the Muslim-majority in Indonesia. However, it takes a lot of time to acquire Arabic, since there are various linguistic branches to learn for anyone who wishes to fluently understand it. On the top of that, Arabic has different characteristics from those of the Indonesian language. The present study was based on the results of a preliminary study that show a significant lack of Arabic teaching innovation, when compared with the teaching of other foreign languages, especially English. The purpose was to figure out the concept of Arabic teaching in the “Ma’had Jami’i” in terms of its objective, process, evaluation, issues, and challenges. To this end, an embedded case study was carried out. The data sources included informants, teaching activities, and documents. The data were analyzed using data source triangulation and an interactive model of method triangulation. The results show that the objective of Arabic teaching was to equip students with Arabic knowledge and skills in order for them to be able to keep up with the lesson and practice Arabic in university classrooms; the teaching process was theory and practice-oriented to create a conducive language learning environment; the evaluation was conducted through written and performance tests during the placement test, midterm test, and final test; and the teaching challenges lay in the psychological, educational, and social aspects. Abstract

ABSTRACT: Practical Teaching Program (PTP) is a program which must be taken by all university students in the education program. The purpose of PTP is, for the students (practitioners), to gain educational experience factually in the field, both with the school administration and its teaching-learning process. But in reality, there are numerous of students, who have finished their teaching exam in school who said that there are still confuse if they are going to be a professional teacher in the future. This research tries to explore the implementation of PTP based on Lesson Study (LS) principal and procedure. The method use is qualitative descriptive. This study conducted by Class Action Research: planning, action, observation, and reflection. The subjects of this research are 5 Arabic language students who are undergone PTP in the MAN (Madrasah Aliyah Negeri or State Senior High School for Islamic Education) 1 in Bandung, West Java, Indonesia. The instrument used to measures the professionalism of future Arabic language teacher are test, interview, observation, and direct supervision during the lesson study activity. The result shows that the PTP model of guidance and evaluation process with LS based can increase the professionalism of future Arabic language teacher which related to the pedagogic, professional, social, and personality of the teacher competencies. Abstract

Seroma adalah pengumpulan cairan serous di subkutis dan merupakan komplikasi pascaoperasi kanker payudara yang paling sering dijumpai. Insidensinya mencapai lebih dari 60%. Walaupun tidak mengancam jiwa, namun dapat menimbulkan morbiditas yang serius. Berbagai penelitian dilakukan untuk menentukan faktor risiko untuk mencegah timbulnya seroma. Saat ini, seroma terjadi karena proses inflamasi pascaoperasi. Teori inilah yang mendasari peranan antiinflamasi terhadap pembentukan seroma. Diketahui obat golongan glukokortikoid, memiliki peranan menghambat respons inflamasi. Tujuan penelitian ini untuk menilai efek profilaksis pemberian metilprednisolon perioperatif mastektomi terhadap pembentukan seroma. Penelitian ini merupakan uji klinis secara acak tersamar ganda terhadap 2 kelompok, yaitu antara kelompok karsinoma payudara stadium lokal lanjut yang diberikan metilprednisolon perioperatif mastektomi sebagai kelompok perlakuan dan kontrol. Penelitian ini dilakukan di Rumah Sakit Dr. Hasan Sadikin Bandung, periode April–Juli 2013. Subjek penelitian meliputi 30 wanita yang memenuhi kriteria inklusi dan dibagi menjadi dua kelompok, 15 pasien diberikan metilprednisolon dan 15 pasien sebagai kontrol. Penelitian ini menggunakan uji-t tidak berpasangan dan Mann Whitney. Didapatkan perbedaan rata-rata volume drainase hari ke-1, pada kelompok metilprednisolon sebanyak 104,7 mL dan pada kontrol sebanyak 158 mL, namun secara statistik tidak bermakna (p=0,053). Demikian pula dengan hari-hari selanjutnya dan total seroma, tidak didapatkan perbedaan yang bermakna. Simpulan, pemberian metilprednisolon tidak efektif menurunkan volume drainase seroma pascaoperasi mastektomi. The incidence is more than 60%. Although it is not life-threatening but it can cause serious morbidity. Various studies were conducted to determine the risk factors to prevent seroma formation. Currently, seroma occurrs due to postoperative inflammatory processes. This theory underlies the antiinflammatory role for seroma formation. Glucocorticoid drugs is known as playing a role in inhibiting the inflammatory response. The purpose of this study was to assess the prophylactic effect of perioperative administration of methylprednisolone on seroma formation. This study was a double-blind randomized control trial of 2 groups, i.e. a group of locally advanced breast carcinoma which were given methylprednisolone perioperatively as a treatment group and a control group. The research was conducted in Dr. Hasan Sadikin General Hospital Bandung from April to July 2013. Thirty women who met the inclusion criteria were included as subjects who were then divided into two groups: 15 women received methylprednisolone and 15 women serve as controls. This study used independent t and Mann-Whitney tests. There was a difference in the mean volume of drainage on day 1 between the methylprednisolone and control group as much as 104.7 mL and 158 mL, respectively, but not statistically significant (p=0.053). The same situation was also seen for the following days and total seroma, which was no significant difference was found. In conclusion, the use of methylprednisolone is not effective for reducing postoperative seroma drainage volume after mastectomy. Abstract

Tamoksifen dan inhibitor aromatase sebagai terapi adjuvan memiliki peranan penting pada karsinoma payudara (KPD) pascamenopause dengan reseptor estrogen positif. Saat ini inhibitor aromatase dapat menggantikan tamoksifen karena memiliki keuntungan yang lebih baik dibanding dengan  tamoksifen. Inhibitor aromatase juga memiliki efek samping meningkatkan penyerapan tulang yang memicu osteoporosis dan fraktur dibanding dengan tamoksifen. Saat terjadi proses penyerapan tulang, matriks tulang yang 90% terdiri atas kolagen tipe I akan terurai sehingga salah satu hasil penguraiannya berupa ?-CrossLaps terdapat dalam darah dan dapat diukur kadarnya. Penelitian ini mengukur perbedaan kadar ?-CrossLaps serum baik pada pasien KPD pascamenopause yang diberikan anastrozol dan tamoksifen di Rumah Sakit Dr. Hasan Sadikin Bandung periode Januari?Juli 2013. Penelitian ini merupakan penelitian potong lintang. Terdapat 32 pasien, yaitu 16 pasien tiap kelompok. Hasil pengolahan data statistik dengan derajat kepercayaan 95% menunjukkan perbedaan yang bermakna kadar ?-CrossLaps serum KPD yang diberikan anastrozol dibanding dengan yang diberikan tamoksifen (p<0,05). Simpulan, kadar ?-CrossLaps rata-rata serum KPD yang diberikan anastrozol lebih tinggi dibanding dengan tamoksifen, artinya  pemberian obat anastrozol pada pasien KPD pascamenopause dalam jangka waktu >6 bulan meningkatan proses penyerapan tulang dibanding dengan yang diberikan tamoksifen. [MKB. 2015;47(4):207?11] DOI: 10.15395/mkb.v47n4.615Kata kunci: Anastrozol, ?-CrossLaps serum, tamoksifenComparison of ?-crosslaps Serum Levels between Postmenopausal Breast Carcinoma Patients Receiving Anastrozol and TamoxifenAbstractTamoxifen and inhibitor aromatases as adjuvant therapy plays an important role in postmenopausal breast cancer patients with positive estrogen receptor. Currently, inhibitor aromatases can replace tamoxifen because it has more advantages than tamoxifen. Inhibitor aromatases also have side effects which is increased bone resorption that triggers osteoporosis and fractures when compared to tamoxifen. During the process of bone resorption, bone matrix, composed of 90% type I collagen, were broken down resulting in ?-crosslaps content in the blood, which is measurable. This study measured the levels of serum ?-crosslaps in postmenopausal breast cancer patients who received anastrozol and tamoxifen in Dr. Hasan Sadikin General Hospital Bandung between January and July 2013. This was a cross-sectional study with comparative analytics to compare the mean levels of serum ?-crosslaps in postmenopausal breast cancer patients between the two groups. There were 32 patients in this study, 16 patients in each group. The results of data processing using statistical tests at 95% confidence level revealed that there was significant differences in serum levels of ?-crosslaps between the group who received anastrozol and those who received tamoxifen (p<0.05). In conclusion, the mean value of ?-crosslaps serum in anastrozol group is higher than in tamoxifen group. This means that postmenopausal breast cancer patients receiving anastrozol for more than 6 months could may experienced a higher degree of bone resorption compared to those receiving tamoxifen. Abstract

Karsinoma payudara merupakan kanker pada wanita dengan frekuensi tertinggi di dunia dan merupakan penyebab kematian utama. Pasien karsinoma payudara mengalami periode stres fisik dan emosional yang mendorong diproduksinya protein seperti interleukin-6 (IL-6). Interleukin-6 berperan penting dalam tumorigenesis karsinoma payudara melalui aktivasi jalur janus kinase (JAK). Penelitian ini mengukur kadar IL-6 serum yang berperan penting pada patogenesis molekuler proses metastasis pada karsinoma payudara lanjut lokal (KPLL) dibandingkan dengan karsinoma payudara bermetastasis (KPM). Rancangan penelitian adalah potong lintang, analitik komparatif dengan membandingkan kadar IL-6 serum rata-rata pada kedua kelompok KPLL dan KPM. Penelitian dilakukan di Rumah Sakit Dr. Hasan Sadikin Bandung (RSHS) dari Juni–November 2012. Subjek penelitian adalah 35 pasien yang terdiri atas 18 KPLL dan 17 KPM. Kadar IL-6 diukur dengan teknik pemeriksaan enzyme-linked immunosorbent assay (ELISA). Hasil yang diperoleh dari penelitian ini kadar IL-6 serum rata-rata pada KPLL 4,99 pg/mL dan kadar IL-6 serum pada KPM 32,73 pg/mL. Terdapat perbedaan bermakna kadar IL-6 serum antara KPLL dan KPM, p<0,01. Hasil analisis dengan uji chi-kuadrat pada derajat kepercayaan 95%, bahwa terdapat hubungan kadar IL-6 serum dengan kejadian metastasis. Subjek dengan kadar IL-6 serum >9,85 pg/mL memiliki risiko kejadian metastasis 37,5 kali dibandingkan dengan kadar IL-6 serum ≤9,85 pg/mL. Simpulan, kadar IL-6 serum pada KPM lebih tinggi dibandingkan dengan KPLL. Patients with breast carcinoma experience periods of stress both physical and emotional which encourages them to produce proteins such as interleukin-6 (IL-6). Interleukin-6 plays an important role in tumorigenesis via activation of janus activated kinase (JAK). This study measured the levels of serum IL-6 which plays an important role in the molecular pathogenesis of metastasis in locally advanced breast cancer (LABC) compared to metastatic breast cancer (MBC). The study design was cross-sectional, comparative analytics by comparing the mean levels of IL-6 in both groups: LABC and MBC. The study was conducted at the Dr. Hasan Sadikin Hospital Bandung (RSHS) from June–November 2012. Subjects were 35 women consisting of 18 LABC and 17 MBC. Levels of serum IL-6 were measured by enzyme-linked immunosorbent assay (ELISA) technique. The results of this study show that the mean levels of serum IL-6 on LABC 4.99 pg/mL and the mean levels of serum IL-6 on MBC 32.73 pg/mL. There was a significant difference between the levels of serum IL-6 on LABC and MBC, p<0.01. Results of the analysis with chi-square test shows there is a correlation between level of IL-6 with metastasis incidence. Subjects with levels of serum IL-6>9.85 pg/mL had 37,5 times higher risk for incidence of metastasis. In conclusion, the levels of serum IL-6 on MBCare higher than LABC. Abstract

Penelitian ini bertujuan untuk mengetahui penggunaan media I-CHAT sebagai media pembelajaran bahasa Indonesia dalam meningkatkan kemampuan anak tunarungu menyusun kalimat secara terstruktur. Penelitian dilakukan di SLB BC Budi Bakti I Kawali dengan subjek penelitian siswa tunarungu kelas satu SMALB sebanyak dua orang. Metode penelitian yang digunakan adalah Single Subjek Research dengan desain A-B-A. Hasil penelitian menunjukkan bahwa penggunaan media I-CHATdapat meningkatkan kemampuan penyusunan struktur kalimat pada anak tunarungu sehingga dapat dijajdikan alternatif bagi guru dalam pengembangan kemampuan bahasa pada anak tunarungu, khususnya pada penyusunan kalimat berstruktur.Kata kunci: Menyusun kalimat, media I-CHAT, anak tunarungu

Abstract

Hambatan dalam perkembangan bahasa dapat berpengaruh terhadap proses belajar, sehingga mereka memerlukan pengalaman belajar yang kongkret. Keterampilan berbahasa lisan (berbicara) merupakan modal utama bagi setiap orang, tidak terkecuali siswa tunagrahita dalam mengadakan interaksi dan komunikasi dengan lingkungan sekitar. Komunikasi akan bermakna apabila antara pembicara dengan lawan bicaranya tahu apa yang dibicarakan. Untuk itu perlu adanya kejelasan dalam setiap ucapannya. Alat komunikasi yang paling dominan dipergunakan dalam aktivitas sehari-hari adalah dalam bentuk wicara atau bahasa lisan. Dengan berkomunikasi memungkinkan individu membangun suatu kerangka rujukan dan menggunakannya sebagai panduan untuk menafsirkan situasi apapun dan permasalahan yang dihadapi. Tanpa melibatkan diri dalam komunikasi, seseorang tidak akan tahu bagaimana makan, minum, berbicara sebagai manusia, dan memperlakukan manusia lain secara beradab. Cara-caraberperilaku seperti itu harus dipelajari lewat pengasuhan keluarga dan pergaulan dengan orang lain, semuanya berpangkal dari adanya komunikasi.Kata kunci: Teknik pembelajaran, bahasa lisan, siswa tunagrahita

Abstract

Penelitian ini bertujuan untuk mengetahui pengaruh teknik over control terhadap prilaku agresif, khususnya meremas pundak orang lain secara tiba-tiba pada orang lain serta perubahan-perubahan prilaku agresif pada anak autistik sebelum, selama, dan setelah intervensi. Penelitian ini menggunakan metode campuran dengan desain concurrent embedded. Dijadikan subjek penelitian adalah HK, siswa SLB Al-Hikmah Bandung. Dalam pendekatan kuantitatif peneliti menggunakan metode Single Subject Research (SSR). Secara kuantitatif, hasil penelitian menunjukkan bahwa intervensi yang dilakukan mampu menurunkan prilaku agresif HK. Secara kualitatif terdapat perubahan-perubahan prilaku yang menguatkan pengaruh penggunaan teknik over control yang ditandai dengan terjadinya perubahan-perubahan dari prilaku yang cenderung dari negatif ke arah yang lebih positif, sehingga teknik tersebut dapat dijadikan pilihan bagi lingkungan dalam mengatasi hambatan prilaku agresif anak autistik.Kata kunci: over control, agresif, autistik

Abstract

This research based on the presence of mentally retarded students who feel afraid of water. The purpose of this study was to determine learning swimming sports to reduce student fear. This research is a descriptive study using a survey method. The research data was obtained through a questionnaire using saturated sampling technique because the number of respondents was less than 30 people. The validity test in this study used the product moment formula with r hit r tab (0.352), there were 13 valid items. Reliability test using Cronbach's alpha formula with reliability results of (0.793). Data analysis was performed using descriptive statistics. The results obtained indicate that the response of parents to the research questions is acceptable (positive) with a percentage of (68%). There are obstacles that occur, namely the fearful behavior of students shown during the learning process. However, due to the efforts of students and the solution from the teacher, namely by implementing the introduction of water, students' courage can be formed. It can be concluded that learning swimming sports which is implemented goes well.
