Abstract

Shivering after cesarean section is one of the common side effects of spinal anesthesia that is often overlooked but can cause significant discomfort, even increasing the risk of postoperative complications such as increased oxygen consumption and impaired recovery. This community service program aims to improve understanding in both patients and health workers regarding the importance of effective shivering management through health education and practical training. The methods used include socialization, interactive counseling, action simulation, and evaluation of the level of understanding before and after educational activities. The results showed an increase in patient knowledge from 50% to 85% after education, and a decrease in the incidence of shivering from 70% to 30% after implementation of the intervention. These findings indicate that educational interventions and promotive-preventive approaches play an important role in improving the quality of postoperative care, especially in patients undergoing cesarean section. Therefore, the active involvement of health workers in providing health information and non-pharmacological pain management is essential to prevent and overcome shivering optimally.

Abstract

Preoperative anxiety is a common issue that can affect a patient's physiological and psychological condition before undergoing surgery. One effective method to reduce anxiety is through proper education, including the use of audiovisual media such as educational videos. This study aims to evaluate the effectiveness of educational videos in reducing preoperative anxiety levels in patients. The method used involves a pre-test and post-test design with an intervention consisting of an educational video about the surgical procedure, patient preparation, and relaxation techniques. The results indicate a significant reduction in patients’ anxiety levels after receiving education through video. Therefore, the use of educational videos can be an effective strategy to enhance patients' mental readiness before surgery and should be widely implemented in healthcare facilities.

Abstract

Patient safety during intraoperative procedures is a top priority in nursing services in the operating room. However, patient and family understanding of intraoperative nursing management is still limited, which can have an impact on patient readiness before undergoing surgery. This community service activity aims to improve patient and family knowledge about the important role of intraoperative nursing in maintaining patient safety. The method used is an educational approach through interactive counseling delivered directly to patients and families in the pre-operative room. The material includes patient safety standards, the role of nurses during surgical procedures, and the importance of effective communication between the medical team, patients, and families. Evaluation was carried out by comparing the level of understanding before and after education using a simple questionnaire. The results showed a significant increase in understanding, seen from changes in patient attitudes and readiness to face surgical procedures. Appropriate and easy-to-understand education has been shown to be effective in supporting active participation of patients and families, as well as encouraging the creation of a safer surgical environment.

Abstract

Background: Endometrioid endometrial carcinoma (EEC) is a prevalent gynecological malignancy whose prognosis is influenced by factors including histopathological grade and lymphovascular invasion (LVI). Angiogenesis, crucial for tumor growth and metastasis, is significantly mediated by vascular endothelial growth factor (VEGF). This study aimed to investigate the expression of VEGF in EEC and its correlation with histopathological differentiation and LVI. Methods: This observational analytical study employed a cross-sectional design using 36 archival paraffin block samples of EEC diagnosed between January 2022 and December 2024 at the Anatomical Pathology Laboratory of Dr. M. Djamil General Hospital Padang. Cases were selected via simple random sampling from a population of 59. Histopathological grade (Grade 1, 2, or 3 based on FIGO architectural and nuclear criteria) and LVI (negative, focal, or substantial) were re-evaluated from Hematoxylin-Eosin (H&E) stained slides. VEGF expression was assessed by immunohistochemistry, scored semiquantitatively based on the percentage of positive tumor cells and staining intensity, and categorized as low or high. Data were analyzed using Chi-square tests, with p<0.05 considered statistically significant. Results: The mean age of patients was 54.36 years, with the highest prevalence in the 51-60 age group (41.7%). Grade 3 tumors were most common (38.9%), followed by Grade 2 (33.3%) and Grade 1 (27.8%). LVI was present in 47.2% of cases, predominantly focal (38.9%). High VEGF expression was observed in 58.3% of EEC cases. A statistically significant association was found between high VEGF expression and higher histopathological grade (p=0.000), with 66.7% of Grade 3 tumors showing high VEGF expression. No significant association was found between VEGF expression and LVI (p=0.080). Conclusion: High VEGF expression significantly correlated with higher histopathological grades in EEC, suggesting its role in tumor aggressiveness and dedifferentiation. However, a significant association with LVI was not established in this cohort. VEGF expression warrants further investigation as a potential prognostic biomarker and therapeutic target in EEC.

Abstract

RSD Dr.A. Dadi Tjokrodipo Bandar Lampung, sectio caesaria patients in the last 3 months of 2021 totaled 40 patients. Hypotension or decrease in Mean Arterial Pressure (MAP) in caesarean section often occur consequence spinal anesthesia due to overall blockade effect on the sympathetic nervous system . Study this aim for This study aims to determine the effect of loading colloid fluid in sectio caesarea patients with spinal anesthesia. Method study qualitative with a descriptive research design is a research that tends to use analysis . Population study 40 patients sectio caesarea with spinal anesthesia at Dr A. Dadi Tjokrodipo Hospital Bandar Lampung with amount sample of 8 patients. From result study "Effectiveness of Loading Colloidal Fluids Against Hypotension in Sectio Caesarea Patients With Spinal Anesthesia at Dr A. Dadi Tjokrodipo Hospital Bandar Lampung" could obtained results that Patient Sectio Caesarea performed Partial spinal anesthesia big experience hypotension (87.5%) due to effect of spinal anesthesia , so that required gift fluid colloid for raise pressure blood . Effect gift fluid colloid on patient cesarean section hypotension really mean for normalize return pressure blood patients (75%). Based on results Paired Samples T-Test statistical test obtained value of Sig.(2-tailed) = 0.001 or < 0.05 which means there is significant difference on the Mean Arterial Pressure (MAP) before and after gift fluid colloid on patient sectio caesarea with spinal anesthesia at Dr A. Abstract

Background: Adenoid cystic carcinoma (ACC) is a rare malignant tumor of the salivary glands known for its slow growth, invasive behavior, and propensity for perineural invasion. The diagnosis of ACC can be challenging, especially during intraoperative frozen section consultation. This case report highlights the crucial role of frozen section diagnosis in guiding surgical management and ensuring tumor-free margins, which are critical for patient prognosis. Case presentation: A 44-year-old male presented with a slow-growing, occasionally painful nodule on his upper lip, without facial numbness or other systemic symptoms. A frozen section examination was performed intraoperatively to assess the lesion and determine the margin status. The frozen section revealed malignancy, characterized by tumor cell proliferation in tubular, cribriform, and solid patterns, all hallmarks of ACC, along with perineural invasion. A wide excision was performed, and subsequent frozen sections of the surgical margins confirmed they were free of tumor. The diagnosis of ACC was confirmed on histopathological examination of the paraffin block. Conclusion: This case underscores the importance of frozen section diagnosis in the management of ACC of the minor salivary glands. The accurate assessment of the lesion and margin status during surgery allows for appropriate surgical decision-making, ensuring complete tumor removal and minimizing the risk of recurrence.
