Abstract

Diabetes mellitus (DM) is a growing global health problem, including in Indonesia where the prevalence continues to rise annually. The management of type 2 diabetes mellitus (T2DM) has largely been health officer-centered, with limited involvement of patients and families, which leads to suboptimal outcomes. This community service program (PkM) aimed to empower communities in Plalangan Subdistrict, Gunungpati District, Semarang City, through the training of health cadres in T2DM management. The program was conducted in two phases: pre-activity coordination with local stakeholders and the implementation of training activities. A total of 40 health cadres participated in training that included education on diabetes management, child diabetes, and simple blood glucose monitoring techniques using glucometers. Results showed that cadres successfully improved their knowledge and practical skills, demonstrated the ability to conduct capillary blood glucose tests, and acted as community educators in DM prevention and management. The involvement of trained cadres strengthened primary healthcare services by supporting early detection, encouraging healthy behaviors, and enhancing patient adherence. This program highlights the critical role of community empowerment and cadre training in improving diabetes management and quality of life at the community level.

Abstract

Stunting is a condition of growth failure in children, characterized by impaired physical growth and cognitive development due to long-term chronic malnutrition. This condition occurs from the fetal period through early childhood, commonly referred to as the first 1,000 days of life. Preventing stunting requires the involvement of multiple stakeholders, including the active role of adolescents as agents of change within the community. This community service program aimed to enhance adolescents’ capacity for early detection of stunting and to strengthen collaboration with primary health centers in addressing stunting. The methods included health education, capacity building for adolescent cadres, and cross-sector collaboration with community health centers. The results showed that adolescent cadres, in collaboration with partners, were able to implement iron supplementation (iron tablets) programs for elementary, junior high, and senior high school students. In addition, adolescent cadres actively provided education to parents and adolescents on adequate nutrition and early stunting prevention. This program is expected to serve as a sustainable strategy to support the acceleration of stunting reduction in the community.

Abstract

Exclusive breastfeeding period is a vulnerable period for breast milk (ASI) sufficiency to meet infant nutritional needs. Red spinach (Amaranthus tricolor L.) contains galactagogue compounds that have the potential to increase breast milk production by increasing prolactin levels. Objective: To determine the effectiveness of red spinach (Amaranthus tricolor L.) nanoparticles on prolactin levels and breast milk production in breastfeeding mothers. Method: This study employed a quasi-experimental design with a pretest–posttest control group design. The sample consisted of 32 breastfeeding mothers divided into two groups, with 16 participants in the intervention group and 16 in the control group, recruited from Mangunsari Primary Health Center and a private midwifery clinic (BPM Ibu Alam), as well as Sidorejo Kidul Primary Health Center, Salatiga. The intervention group received red spinach nanoparticles at a dose of 250 mg for 14 days, while the control group received breastfeeding education. Breast milk production was assessed indirectly using infant weight gain, whereas prolactin hormone levels were measured using the ELISA method. Data were analyzed using the Wilcoxon test, Paired Sample T Test, Mann–Whitney test, Repeated Measures ANOVA, and Independent t-test. Results: Administration of red spinach nanoparticles significantly increased prolactin levels in the intervention group (p = 0.013), whereas no significant change was observed in the control group (p = 0.955). Infant weight gain occurred in both the intervention and control groups (p<0.000); however, no statistically significant difference was found between the two groups. Conclusion: Red spinach nanoparticles at a dose of 250 mg administered for 14 days were effective in increasing prolactin levels in breastfeeding mothers. This intervention has the potential to support breast milk production, as indicated by infant weight gain, although no significant difference was observed compared to the control group

Abstract

Type 2 diabetes mellitus is a major public health problem with a continuously increasing prevalence, requiring accurate and continuous blood glucose monitoring. Venous blood glucose measurement using enzymatic methods remains the clinical gold standard; however, it is invasive and less practical for routine monitoring. Recent advances in sensor technology provide opportunities to develop more practical and user-friendly glucose monitoring devices. This preliminary pilot study aimed to develop and validate a needle-free sensor-based device for blood glucose monitoring and to evaluate the agreement of its measurement results with venous blood glucose examination using enzymatic methods as the reference standard. This study employed a research and development approach. The initial stage involved designing and developing a prototype sensor that monitors blood glucose without a needle. Device validation was conducted by experts to assess technical performance, safety, and usability. Subsequently, a pilot measurement of random blood glucose levels was performed among 25 patients with type 2 diabetes mellitus in a primary healthcare setting, and the measurement results were compared with venous blood glucose values obtained using enzymatic methods. The validation results indicated that the device was technically feasible, safe to use, and easy to operate. Pilot testing demonstrated that the sensor-based device produced blood glucose measurements comparable to those from the reference method (Wilcoxon Signed Ranks Test: Z = 0.000, p = 1.000), providing preliminary evidence of statistical similarity. However, further refinement is required, particularly in the interpretation and display of measurement results. These findings represent initial proof-of-concept for the device as an alternative tool for blood glucose monitoring, especially in primary healthcare settings and for patient self-monitoring. Comprehensive studies with larger, more diverse samples and advanced clinical agreement analyses (Bland-Altman plots, and MARD) are needed to confirm clinical accuracy and reliability.

Abstract

Stunting is chronic malnutrition which is based on an index of body length compared to age (PB/U) or height compared to age (TB/U) with a limit (z-score) of less than -2 SD (Standard Deviation). Stunting problems need to be detected early, so that the interventions given can reduce long-term effects. Detection and management of stunting is carried out through empowerment efforts. Family empowerment as an effective step to reduce stunting cases through prevention and management efforts. The role of parents is very important in preventing stunting through detection and management. Anthropometric-based instruments that are easy to use are needed for the detection and management of stunting. Objective to analyze the variety of health promotion strategies to prevent stunting. The research design is a Systematic Literature Review of articles or journals published in 2018-2023, accessed on Google Scholar, PubMed and Scopus with the Prisma Flow technique. Research with different designs ranging from descriptive studies to quasi-experimental and participatory studies shows that an integrated and family-based approach can significantly improve knowledge, skills and awareness about stunting prevention. Programs involving education on exclusive breastfeeding, healthy food processing for toddlers, and maternal reproductive health have proven effective in reducing the prevalence of stunting. Health promotion strategies found in the literature review were mostly conceptual rather than operational and did not show when these strategies successfully reduced stunting.
