Abstract

JKN (National Health Insurance) is a government program that aims to provide health assurance for all Indonesian citizens for a healthy, productive, and prosperous life. In the two years after JKN was implemented, various problems occurred. This research used the qualitative approach with the Edward II implementation theory. Results of the research indicated that there were problems in communication, stemming from the lack of socialization and inharmonic regulations, there was also the problem of the lack of healthcare resources. From the disposition side, the policy makers often obstructs the implementation preparation, this is evident from the information on determining the premium size. From the organization structure, all the stakeholders have been well coordinated. We conclude that we are not ready to implement the JKN. We recommend that mass and effective socialization program to be performed using various methods of communication and involve the community. To reduce the disparity of healthcare services, we recommend that the regional government to establish various healthcare facilities to accelerate health development. There should also be regulations that allocates healthcare staff in every corner of the country to achieve Universal Health Coverage in 2019, as stated in the National Health Insurance Road Map.

Abstract

The Indonesian Minister of Health 2019 issued regulations regarding the implementation of telemedicine services between health service facilities. Telemedicine is aimed primarily at rural areas. This research aims to look at the quality of telemedicine-based services, which are documented in an electronic health record (EHR) with complete information. This research uses the narrative literature review method: Garuda journal channels, Google Scholar, IEEE Explorer, ProQuest, PubMed, Science Direct, and Scopus. With the input-output process approach, eight scientific articles were published on countries with telemedicine/telehealth policies in rural areas. The implementation of telemedicine has advantages and disadvantages depending on the things supported and the target users. It must have policies, infrastructure, financial resources, and human resources to use, maintain and develop telemedicine. Telemedicine will help the health service process by increasing the degree of public health in rural areas if it is used on a large scale. The completeness of the EHR seems to be lacking in terms of informed consent. Still, a quality EHR can make it easier for health workers to enforce the history, establish the diagnosis, and provide patient healthcare.

Abstract

Hypertension is a major cardiovascular risk factor that causes disability throughout the world and among those who are not aware of their health condition, it is often referred to as the silent killer . The prevalence rate of hypertension in Indonesia is quite high. Based on Riskesdas (2018), South Kalimantan Province shows the highest prevalence of hypertension, but the percentage of hypertension sufferers who receive health services is still low. Banjarbaru City has the lowest percentage, namely 11.3% of the target of 100%. Health services for hypertension sufferers are included in the Minimum Service Standards (SPM) for the Health Sector which are regulated in statutory regulations and government policies. The aim of this research is to analyze the implementation of the SPM policy in the health sector for hypertension sufferers in Banjarbaru City. Qualitative research with a Rapid Assessment Procedure (RAP) research design was conducted from May to June 2023 in Banjarbaru City, South Kalimantan Province. Data collection was carried out through in-depth interviews, observation and document review. The results of the research show that the implementation of the hypertension SPM policy has not been optimal, as can be seen from the SPM achievement report in the health sector in Banjarbaru City which is in the lowest position among 12 types of basic health services. In conclusion, several obstacles in the implementation of SPM for hypertension are recording and reporting hypertension sufferers, low public awareness of carrying out examinations and treatment at community health centers so that the data is not recorded, there is no special budget allocation for SPM for hypertension at community health centers, and the dual duties of health workers so that they are not optimal in achieve a program. Cross-sector collaborative efforts are needed for integrated recording and reporting of hypertensive patient data, submission of hypertension SPM budgets, efforts to invite the community, and fair distribution of tasks for community health center officers.

Abstract

One of the challenges in improving health services in Indonesia is the uneven number and distribution of health workers in some areas, especially doctors and specialist doctors. Therefore, the aim of this study was to review the conduciveness and support of the policy environment towards meeting the needs of specialist doctors in West Nusa Tenggara (NTB) Province, Indonesia. This study was the narrative review method. Keywords used are environmental policy, specialist doctor, and remote area. Critical appraisal was performed by utilizing the PRISMA method. This study showed that there is political support from the Central and Regional Governments. However, several aspects are still recognized negatively, aspects of national policies related to incentives and facilities. Regional Government Policies are considered not sufficiently significant to improve the availability of doctors and specialist doctors, especially in terms of the advocacy mechanisms. This matter impacts the lack of doctors and specialist doctors’ interest to be placed in remote areas. The policy environment towards meeting the needs of a specialist doctors in West Nusantara Tenggara (NTB) was enough conducive and supportive. It still needs improving coordination mechanisms among institutions and providing advocacy channels, which are crucial to support the effectiveness of meeting the needs of specialist doctors’ policies.

Abstract

Background and purpose: The government has adopted policies regarding food labelling to ensure food quality and safety, but the implementation has not yet optimal. This study identifies resources, knowledge, and facilities that affect food labelling implementation in small, medium, and micro food-businesses (SMMF-B) in West Nusa Tenggara, Indonesia. Methods: We conducted a mixed method study. The qualitative data was collected through in-depth interviews with informants from the West Nusa Tenggara Provincial Office of National Agency of Drug and Food Control and the Mataram City. Health Office, and a focused-group discussion involving SMMF-B owners. The quantitative data was collected with a survey to 30 respondents using questionnaire and observation of the label of 36 SMMF-B products to determine the presence of product’s label, composition, net-weight, producer’s name and address, production code, expiry date and distribution license. Results: We found SMMF-B owners’ knowledge on food labelling was low, however, they expressed supports to the implementation of food label controlling. Of the 36 food labels, the two highest number of violations were the absence of production code (88.9%) and net weight (86.1%). Meanwhile, of the 30 surveyed respondents, the majority (96.6%) stated the importance of labels on SMMF-B products and almost two third (63.4%) read food labels before purchasing the product. Conclusion: This study reveals that SMMF-B in West Nusa Tenggara has not implemented food labelling policies optimally. The lack of implementation was influenced by the government’s lack of funds and minimal information and monitoring efforts. Therefore, government should increase budget allocation for food label controlling, number and capacity of food label controllers and strengthen the enforcement.

A survey study was carried out quantitatively using a 2022 database of client admission in February 2023. Descriptive analysis was performed to present the data as appropriate. The overall total of drug users was 592 clients (569 males, 23 females). The demographic characteristics were dominated by young adults (79.90%), voluntary (74.70%), Muslims (86.80%), high-school-to-higher graduates (74.70%), and employers (61.80%). The drug mostly used was methamphetamines (69.80%) with duration being less than 10 years (75.50%) and addiction level being mild-to-moderate (92.80%). The main reasons for starting drug use were peer/partner pressure (37.18%) and curiosity (29.92%).
