Abstract

Kompleksitas sistem layanan kesehatan dan meningkatnya bahaya pada pasien di fasilitas layanan kesehatan menjadikan keselamatan pasien sebagai prioritas utama di pelayanan kesehatan. Pasien sebagai pengamat utama pada layanan mereka, namun tidak banyak keterlibatan pasien untuk memperbaiki keselamatan pasien di rumah sakit. Patient Measure of Safety (PMOS) merupakan alat untuk mengkaji faktor yang memiliki kontribusi terhadap keselamatan pasien. Tujuan dari systemic review ini adalah untuk mengkaji kelayakan PMOS untuk mengidentifikasi faktor yang berkontribusi pada keselamatan di rumah sakit. Pencarian literatur secara online dilakukan pada Pubmed, Proquest dan Google Scholar berdasarkan guideline PRISMA dan menghasilkan 502 artikel. Artikel yang tidak berhubungan berdasarkan kriteria eksklusi dikeluargakan dan berdasarkan teks penuh, 5 artikel dipilih untuk systemic review ini. Berdasarkan artikel yang telah dipilih, PMOS mampu menemukan kelemahan laten pada pelayanan kesehatan, identifikasi faktor yang berkontribusi terhadap keselamatan berdasarkan persepsi pasien, seperti keterlambatan atau komunikasi dan Kerjasama yang tidak baik. Sebagai bagian dari pengukur terhadap sebuah intervensi, PMOS dapat mendeteksi permasalahan yang biasanya terlewatkan di manajemen rumah sakit. Oleh karena itu, keunikan umpan balik dari persepsi pasien mampu didapatkan melalui PMOS, sebagai alat yang layak, valid dan dapat diandalkan untuk mengukur faktor keselamatan melalui persepsi pasien. Kombinasi PMOS dengan alat keselamatan dan perbaikan berkelanjutan mampu menjaga dan memperbaiki keselamatan dari pelayanan kesehatan.

Abstract

Background: Patient safety is a serious issue in a healthcare organization. Building a patient safety culture is the first step in the improvement of safety. As patient safety culture is an inherent component of organizational culture; therefore, a study of organizational culture is required to develop it. This study aims to analyze the patient safety culture and identify the profile of the organizational culture in the hospital to find strategies to improve the patient safety culture. Methods: This study was used the quantitative study using a cross-sectional research method, followed by a qualitative method to get descriptive information. This research conducted in Jambi Public Hospital, Indonesia with 128 respondents. Data were obtained from a survey by the HSOPSC (Hospital Survey on Patient Safety Culture) questionnaire and OCAI (Organizational Culture Assessment Instrument. Focus Group Discussion was conducted to identify obstacles and expectations in the implementation of patient safety culture. Results: The highest positive perceptions are organizational learning and continuous improvement (90.6%) and teamwork within units (78.91%). While "staffing" (39.25) and "non-punitive responses to errors" (38.54 %) are the weakest dimension of patient safety. The most dominant type of organizational culture, followed by a hierarchy with a minimal score difference. This cultural assessment serves as a guide for changes with a quality strategy of clan culture and hierarchy culture were decided in the development of patients safety culture in this hospital. Conclusion: The patient safety culture belongs to the moderate culture category, the reporting culture dimension is the weakest. A management’s commitment and staff empowerment, as well as system development, are all needed on the development of a patient safety culture. Further research is required to investigate various factors to develop the reporting culture. Keywords: Patient Safety Culture, Organizational Culture, Hospital

Abstract

Background: Recently medical tourism is a new form of niche tourism market, which has grown rapidly. The term of Medical Tourism describes tourists who travel to foreign countries to obtain health services and facilities such as medical, dental, surgical care, and so forth. It is necessary to hold a marketing strategy that is able to increase tourist interest in carrying out medical tourism activities in efforts to develop medical tourism. Therefore, the aim of this study was to analyze marketing strategies for medical tourism programs in hospitals. Methods: A systematic review through marketing strategy journals of medical tourism was conducted. Research articles accessed from the internet through databases namely: EBSCO, Emerald, Science direct, Neliti, and Research gate with the keywords medical tourism and medical tourism marketing strategy. The literature analysis from 2 journals with mix methods design, a journal with descriptive qualitative design, a journal with descriptive study design, a journal with SWOT analysis design, a journal with desk research methods design, and 2 journals with data analysis design. Results: The 11 articles that are considered to be in accordance with the objectives of the study. From 11 articles, then screening again based on feasibility according to inclusion and exclusion criteria obtained 8 articles for further review. This systematic review shows that the marketing concepts of segmentation, targeting, and positioning could compete competitively with other hospitals. The marketing strategy of a health service was able to increase the interest of tourists in conducting medical tourism activities in a country. Marketing efforts carried out by hospitals focus on domestic and foreign tourists. Conclusion: The existence of a marketing strategy can increase competitiveness between hospitals and increase the interest of domestic and foreign tourists to carry out medical tourism activities competitively. Abstract

Background: Throughout the world, emergency departments (ED) are characterized by overcrowding and excessive waiting times. Furthermore, the related delays significantly increase patient mortality and make inefficient use of resources to the detriment of the satisfaction of employees and patients. Lean thinking is a philosophy that focuses on eliminating waste or non-value added elements from the processes so that customers are given greater value. Therefore, many Emergency Departments have begun to apply lean as a way to fight against the problems of crowding, delays and medical incidents. Methods: Eligible studies for this systematic review constitute result summarized that lean method implementation was capable of reducing overcrowding in Emergency Departement. The journal employed in this systematic review sourced from ProQuest, Pubmed, and Google Scholar, whereas the analyzed journals were seven journals. Results: From the total of seven journals reviewed, the studies analyzed the result after implementing the lean method in the Emergency Department. In addition, all of the studies were reported successful in implementing lean intervention. Several studies explained that lean method implementation was capable of reducing waiting time, staying length, and the proportion of patients leaving without being seen in the emergency department. One study suggested that reducing radiography transportation time can reduce turnaround time and can help improve the Emergency Department flow. Conclusion: Lean method implementation can contribute to a decrease in waiting time, stay length, and the proportion of patients leaving without being seen. According to the results of the study, the lean implementation identifies that the lean is capable of decreasing waiting times, stay length, and the proportion of patients leaving without being seen. There must be a policy that governs the lean process. Lean can inspired changes to employee roles, staffing and scheduling, communication and coordination, expertise, workspace layout, and problem-solving. Abstract

Background: Patient safety is one of the most important dimensions in quality of healthcare. Nurses are the key in care delivery, their compliance toward Patient Safety policy cannot be overlooked. Studies evaluating determinants of nurse’s compliance to Patient Safety policy is still lacking. Aims: To evaluate nurse’s attitude toward Patient Safety policy using Health Belief Model (HBM). Methods: An analytical cross-sectional study was carried on in Bali Royal Hospital, Indonesia, on December 2016. An anonymous, self-administered questionnaire was used to collect the data on socio demographics, knowledge, and attitude toward Patient Safety policy. Attitudes are specifically assessed using HBM. Results: A total of 124 nurses are included in this study. Mean age of the study population was 29.7±6.7 years. Mean score for knowledge about Patient Safety policy was 12.68±3.40 from 18.0- point scale. Nurses perceived a low risk of non-compliance (mean perceived risk 8.07±1.89) and a low barrier to comply with Patient Safety policy (mean perceived barrier 8.66±1.78). Knowledge and attitudes toward Patient Safety policy did not differ by sex and educational background but it did differ according to unit of workplace. Age and length of work correlated positively to knowledge (Spearman’s r=0.439, p-value 0.000; and r=0.400, p-value <0.001, respectively). A positive and moderate correlation was observed between knowledge vs. perceived severity (r= 0.394, p-value <0.001) and knowledge vs. perceived benefit (r=0.422, p-value < 0.001). There was a strong and positive correlation between perceived severity and perceived benefit (r= 0.725, p-value <0.001). Conclusion: Nurse’s attitudes toward Patient Safety policy were generally good and correlate positively with knowledge. The use of HBM provides an increased understanding of how individual perceptions can be influenced to improve nurse’s engagement in promoting safer health care.

Abstract

Since the establishment of COVID-19 on March 11 2020 as a world pandemic, various health service efforts have continued to be made, especially in terms of implementing telemedicine which is considered to be an alternative in preventing the spread of COVID-19 during a pandemic, considering that health services are at home pain must go on. Telemedicine is a combination of medical and information communication technology, which can provide health services without being limited by space. Since the COVID-19 pandemic, telemedicine has become one of the most popular choices in remote healthcare efforts, therefore the purpose of this systematic review is to discuss the effectiveness of implementing telemedicine during the COVID-19 pandemic targeting hospitals that have not yet implemented telemedicine considering the use of telemedicine. The research is a scoping review using the framework of Arskey and O'Malley. The implementation of telemedicine in hospitals has become quite attractive during the COVID-19 pandemic, but in practice, there are still obstacles such as the platforms and networks to be used, the level of accuracy of diagnosis, and regulations between hospitals.

Abstract

Primary Health Care (PHC) is a foundation of universal health coverage for enabling equitable access. However, the access barrier is one of the challenges in implementing PHC. Home visits becomes one of the PHC innovations in several countries. This review aimed to describe home visit implementation in various countries regarding the scope, and service providers to achieve the expected impacts. This descriptive study used scoping review method referring to Arksey and O’Malley’s guidelines. Three databases, which are PubMed, ProQuest, and Embase, were searched using keywords, namely “home visit” and “primary health care” or “primary healthcare” or “PHC services”. The articles’ identification used the PRISMA method. From the search result that was limited to 2002-2022, 367 articles were retrieved and screened for titles and abstracts, resulting in 48 full-text articles for eligibility assessment. Finally, ten studies were further analyzed and synthesized according to the study’s aim. The scope of home visits is a comprehensive life cycle approach. Services providers are community health workers, nurses, midwives, doctors, and multidisciplinary teams. The impact of home visits is increasing knowledge and healthy behaviour, utilization, service quality, and health status as well as encouraging cross-sectoral collaboration and interprofessional collaboration. The scope and the impact of the home visit were then categorized based on the three PHC components according to WHO. This research result can be used as input in the development of evidence-based policies for the implementation of home visits as part of the services provided by first-level health facilities with adjustments to the context in Indonesia.

Abstract

The COVID-19 pandemic has caused an urgent need for pharmaceuticals and medical devices used for testing, treatment, and prevention. The policy of issuing an Emergency Use Authorization (EUA) for COVID-19 handling products is carried out to fulfill these needs. This study aims to describe and analyze how the EUA mechanism can be used as resilience during the pandemic. The study used Pubmed and ScienceDirect databases to search all articles using inclusion and exclusion criteria. The inclusion criteria were articles in English and Indonesian, national and international research articles covering the application of the EUA, searches limited to the last 3 years, and articles not limited to one study design. The exclusion criteria were articles not available in full text and topics unrelated to EUA in pharmaceuticals and medical devices. A total of 1,009 articles were identified using the keywords “Emergency Use Authorization”, “drug or medical device”, “COVID-19”. Finally, 11 articles that met the criteria were analyzed. The EUA approval for pharmaceuticals and medical devices is a form of resilience in the face of a pandemic and is an effective way to meet increasing needs while looking at clinical evidence and evidence of use in the field. The ability and experience in implementing the EUA can be a guide for learning in dealing with pandemics in the future.

Abstract

WHO publishes an interim guideline on the Rapid Hospital Readiness Checklist. Countries can use the guidelines to help determine hospital capacity in response to the COVID-19 pandemic and identify gaps and key areas that require development/investment actions to maintain hospital quality. This study is to analyze the readiness of hospitals in Indonesia from the results of filling out the Hospital Readiness Checklist uploaded on the mutufasyankes.kemkes.go.id application of the Indonesian Ministry of Health. This study used a quantitative approach by utilizing secondary data taken by researchers from the application. The Rapid Hospital Readiness Checklist consists of 12 components that can be measured as a percentage achievement of each component. Data collection from the mutufasyankes.kemkes.go.id application was obtained by a total of 618 hospitals from 3081 registered hospitals in Indonesia that filled and uploaded on the application until April 30, 2023. A total of 618 hospitals reported Hospital Readiness from 12 existing components, the average achievement per component of 618 hospitals was 88%. The 12th component (Infection Prevention and Control Component) has the highest percentage (94%) and the 10th component (Occupational Health, Mental Health and Psychosocial Support Component has the lowest percentage (77%). Looking at the average results of the profit web picture as a visual illustration of Hospital Readiness measurements in 618 hospitals in Indonesia, it is said that hospitals in Indonesia in terms of the Infection Prevention and Control component are said to be good and for the components of Occupational Health, Mental Health and Psychosocial Support from hospitals need to be followed up again. So that the condition of hospitals in Indonesia at the lowest component of Hospital Readiness during the pandemic can be said to be unprepared. Therefore, the results of Hospital Readiness measurements in Indonesia can be used to determine priority strategies for hospital management by maximizing the Hospital Occupational Health and Safety (K3RS) program so that healthy Indonesian health workers are realized.

Abstract

The focus of this manuscript is on developing a strategic plan for a hospital, with a case study of RSUD dr. M. Yunus Bengkulu, which is a government-owned Class B Education Hospital. This research is qualitative in nature with a descriptive design. The research findings indicate that both external and internal environmental factors position RSUD dr. M. Yunus Bengkulu in the "Growth and Build" quadrant on the IE Matrix and in the "Internal Fix-It" quadrant on the TOWS Matrix. The selected alternative strategy is Market Penetration, accompanied by improvements in the hospital's weaknesses to seize existing opportunities. An Implementation Plan is devised to align with the hospital's vision of becoming a Class A Hospital with quality, progressive, competitive services, and a focus on Education and Research. In order to achieve this vision, RSUD dr. M. Yunus Bengkulu needs to identify potential market areas for expansion and address its internal weaknesses. Strategic steps must be taken to enhance service quality, increase competitiveness, and bolster research and educational capabilities within the hospital. With the right strategic planning, RSUD dr. M. Yunus Bengkulu can effectively confront changing environmental challenges, maximize growth potential, and achieve its vision of becoming an outstanding hospital with quality services and competitiveness.
