Abstract

Coronavirus disease 2019 (Covid-19) pandemic and Large-Scale Social Restriction (PSBB) policy in Indonesia caused health problems for athletes, such as mental, psychological, nutritional, decreased maximum oxygen consumption (VO2Max), loss of strength and muscle mass. Athletes are also individuals who are at risk of being infected Covid-19 so they need health care services. This effort is being realized by the National Sports Hospital (RSON). This study aims to explain the athlete's health maintenance activities by RSON during the Covid-19 pandemic, to analyze the possible gaps, and how to overcome them. This research uses descriptive method and case stnudy approach based on the experience of researchers as RSON medical staff and secondary data from March 1, 2020 to July 31, 2020 starting from the preparation stage with system approach (man, money, material, method, machine), implementation stage (promotive, preventive, curative, and rehabilitative), and output. The entire data was then analyzed based on literature. The results showed that RSON was able to make preparations that support athlete health care, such as socialization and screening for covid-19, treatment and rehabilitation of athletes both directly and online media. Some conditions that still have gaps such as the lack of athletes being screened, and not yet doing routine monitoring of athlete's health during the self training period can be overcome by resocialization and starting routine monitoring activities. Activities such as monitoring the athlete's diet, increasing the role in the prevention of covid-19 in disabled athletes, and collaboration with trainers and team management in the "return to play" program after the PSBB period can be started by RSON to optimize athlete health care services. Manajemen Pemeliharaan Kesehatan Atlet selama Pandemi Covid-19: Studi Kasus di Rumah Sakit Olahraga Nasional AbstrakPandemi coronavirus disease 2019 (Covid-19) dan kebijakan Pembatasan Sosial Berskala Besar (PSBB) di Indonesia menimbulkan masalah kesehatan pada atlet, seperti mental, psikologis, gizi, penurunan konsumsi oksigen maksimal (VO2Max), dan hilangnya kekuatan dan massa otot. Atlet juga individu yang beresiko terinfeksi covid-19 sehingga perlu upaya pemeliharaan kesehatan. Upaya ini sedang berusaha diwujudkan oleh Rumah Sakit Olahraga Nasional (RSON). Penelitian ini bertujuan untuk menjelaskan kegiatan pemeliharaan kesehatan atlet yang dilakukan oleh RSON selama masa pandemi covid-19, menganalisis kemungkinan gap yang terjadi, dan cara mengatasinya. Penelitian ini menggunakan metode deskriptif dan pendekatan studi kasus berdasarkan pengalaman peneliti sebagai staf medis RSON dan data sekunder sejak 1 Maret 2020 sampai dengan 31 Juli 2020 mulai dari tahap persiapan melalui pendekatan sistem (man, money, material, method, machine), tahap pelaksanaan (promotif, preventif, kuratif, dan rehabilitatif), dan output. Keseluruhan data kemudian dianalisis berdasarkan studi literatur. Pada hasil didapatkan RSON mampu melakukan persiapan yang mendukung terlaksananya pemeliharaan kesehatan atlet yaitu berupa sosialisasi dan screening covid-19, pengobatan, dan rehabilitasi atlet baik secara langsung maupun media daring. Beberapa kondisi yang masih terjadi gap seperti kurangnya jumlah atlet yang terjaring di dalam screening, dan belum dilakukannya monitoring rutin kesehatan atlet selama masa latihan mandiri dapat diatasi dengan resosialisasi dan memulai kegiatan monitoring rutin. Kegiatan seperti pengawasan terhadap pola makan atlet, peningkatan peran RSON dalam pencegahan infeksi covid-19 pada atlet difabel, dan kerjasama RSON dengan pelatih dan manajemen tim dalam program “return to play” setelah masa PSBB juga dapat mulai dilakukan oleh RSON guna mengoptimalkan pelayanan pemeliharaan kesehatan atlet.

Abstract

Latar Belakang: Kesehatan mental merupakan sektor penting dalam mewujudkan kesehatan secara menyeluruh. Terdapat sekitar 450 juta orang menderita gangguan mental dan perilaku di seluruh dunia, terbanyak di India (4,5%). Satu dari empat orang menderita satu atau lebih gangguan mental selama masa hidup mereka. Gangguan mental jika tidak ditangani dengan tepat, akan bertambah parah, dan akhirnya dapat membebani keluarga, masyarakat, serta pemerintah. Studi ini bertujuan mengetahui situasi kesehatan mental pada masyarakat Indonesia dan strategi penanggulangannya.Metode: Tulisan ini menggunakan analisis deskriptif eksploratif, melalui tinjauan literatur dan kajian data sekunder. Unit analisis yaitu situasi kesehatan mental di Indonesia.Hasil Penelitian: Berdasarkan kajian data Riskesdas 2013 diketahui prevalensi gangguan mental berat pada penduduk Indonesia 1,7%, terbanyak di Yogyakarta, Aceh, Sulawesi Selatan. Adapun gangguan mental emosional dengan gejala-gejala depresi dan kecemasan sekitar 6%. Hingga saat ini, masih terdapat stigmadan diskriminasi terhadap orang dengan gangguan mental di Indonesia, sehingga mengalami penangananserta perlakuan salah seperti pemasungan. Oleh karena itu strategi yang optimal perlu dilakukan bagi setiapindividu, keluarga dan masyarakat dengan pendekatan promotif, preventif, kuratif, dan rehabilitatif secaramenyeluruh, terpadu dan berkesinambungan. Kesehatan mental dapat ditingkatkan dengan intervensikesehatan masyarakat yang efektif. Paradigma dalam gerakan kesehatan mental yang lebih mengedepankanpada aspek pencegahan serta peran komunitas untuk membantu optimalisasi fungsi mental individu.Kesimpulan: Masih banyaknya kasus gangguan kesehatan mental pada masyarakat, dan penanganannyayang salah di Indonesia. Pemerintah perlu melakukan upaya penanggulangan yang menyeluruh, dimulaiadanya peraturan kebijakan yang menjadi dasar dukungan pendanaan dan akses ke pelayanan kesehatanmental serta didukung pendekatan berbasis komunitas.

Abstract

The COVID-19 pandemic has had a negative impact on hospitals. Hospitals have to survive and make various modifications in an effort to adapt to uncertain conditions, even transform the hospital's strategic plan that has been made previously. This study aims to re-evaluate various hospital strategic plans and evaluate aspects that often change during a pandemic, as well as determine the potential for strategic planning that can be used. We initially conducted an English literature search using the electronic database, PROQUEST, Scopus, Pubmed, Science Direct, 2020-2021. After that, the study quality assessment and data extraction were carried out. A total of 1951 eligible articles were identified, The remaining 18 studies were screened. Subsequently, 10 articles were removed after a full-text review. Finally, 8 articles met the inclusion criteria for this review. Most of the hospitals in the sample show increasing and decreasing returns to scale. The inefficiency stems from the hospital's suboptimal scale, not from a lack of management ability to convert inputs into outputs. Public health centers develop systems for office support, infection control, hospital coordination, and outsourced inventory control. The impact of COVID-19 that can be felt by the community in the field of health services is the lack of availability of hospitals that can accommodate COVID-19 patients and non-COVID-19 patients. There are still many hospitals that still accept non-COVID-19 patients who have comorbidities, while hospitals also accept COVID-19 patients, one of which is asymptomatic people (OTG) who are difficult to detect. Efforts to respond to the pandemic from the start and then implement strategic measures are highly dependent on the resilience of hospitals. Strategic planning transformation can be a solution for hospital organizations to remain resilient and advanced in an uncertain era.

Abstract

Kompleksitas sistem layanan kesehatan dan meningkatnya bahaya pada pasien di fasilitas layanan kesehatan menjadikan keselamatan pasien sebagai prioritas utama di pelayanan kesehatan. Pasien sebagai pengamat utama pada layanan mereka, namun tidak banyak keterlibatan pasien untuk memperbaiki keselamatan pasien di rumah sakit. Patient Measure of Safety (PMOS) merupakan alat untuk mengkaji faktor yang memiliki kontribusi terhadap keselamatan pasien. Tujuan dari systemic review ini adalah untuk mengkaji kelayakan PMOS untuk mengidentifikasi faktor yang berkontribusi pada keselamatan di rumah sakit. Pencarian literatur secara online dilakukan pada Pubmed, Proquest dan Google Scholar berdasarkan guideline PRISMA dan menghasilkan 502 artikel. Artikel yang tidak berhubungan berdasarkan kriteria eksklusi dikeluargakan dan berdasarkan teks penuh, 5 artikel dipilih untuk systemic review ini. Berdasarkan artikel yang telah dipilih, PMOS mampu menemukan kelemahan laten pada pelayanan kesehatan, identifikasi faktor yang berkontribusi terhadap keselamatan berdasarkan persepsi pasien, seperti keterlambatan atau komunikasi dan Kerjasama yang tidak baik. Sebagai bagian dari pengukur terhadap sebuah intervensi, PMOS dapat mendeteksi permasalahan yang biasanya terlewatkan di manajemen rumah sakit. Oleh karena itu, keunikan umpan balik dari persepsi pasien mampu didapatkan melalui PMOS, sebagai alat yang layak, valid dan dapat diandalkan untuk mengukur faktor keselamatan melalui persepsi pasien. Kombinasi PMOS dengan alat keselamatan dan perbaikan berkelanjutan mampu menjaga dan memperbaiki keselamatan dari pelayanan kesehatan.

Abstract

Background: Patient safety is a serious issue in a healthcare organization. Building a patient safety culture is the first step in the improvement of safety. As patient safety culture is an inherent component of organizational culture; therefore, a study of organizational culture is required to develop it. This study aims to analyze the patient safety culture and identify the profile of the organizational culture in the hospital to find strategies to improve the patient safety culture. Methods: This study was used the quantitative study using a cross-sectional research method, followed by a qualitative method to get descriptive information. This research conducted in Jambi Public Hospital, Indonesia with 128 respondents. Data were obtained from a survey by the HSOPSC (Hospital Survey on Patient Safety Culture) questionnaire and OCAI (Organizational Culture Assessment Instrument. Focus Group Discussion was conducted to identify obstacles and expectations in the implementation of patient safety culture. Results: The highest positive perceptions are organizational learning and continuous improvement (90.6%) and teamwork within units (78.91%). While "staffing" (39.25) and "non-punitive responses to errors" (38.54 %) are the weakest dimension of patient safety. The most dominant type of organizational culture, followed by a hierarchy with a minimal score difference. This cultural assessment serves as a guide for changes with a quality strategy of clan culture and hierarchy culture were decided in the development of patients safety culture in this hospital. Conclusion: The patient safety culture belongs to the moderate culture category, the reporting culture dimension is the weakest. A management’s commitment and staff empowerment, as well as system development, are all needed on the development of a patient safety culture. Abstract

Background: Recently medical tourism is a new form of niche tourism market, which has grown rapidly. The term of Medical Tourism describes tourists who travel to foreign countries to obtain health services and facilities such as medical, dental, surgical care, and so forth. It is necessary to hold a marketing strategy that is able to increase tourist interest in carrying out medical tourism activities in efforts to develop medical tourism. Therefore, the aim of this study was to analyze marketing strategies for medical tourism programs in hospitals. Methods: A systematic review through marketing strategy journals of medical tourism was conducted. Research articles accessed from the internet through databases namely: EBSCO, Emerald, Science direct, Neliti, and Research gate with the keywords medical tourism and medical tourism marketing strategy. The literature analysis from 2 journals with mix methods design, a journal with descriptive qualitative design, a journal with descriptive study design, a journal with SWOT analysis design, a journal with desk research methods design, and 2 journals with data analysis design. Results: The 11 articles that are considered to be in accordance with the objectives of the study. From 11 articles, then screening again based on feasibility according to inclusion and exclusion criteria obtained 8 articles for further review. This systematic review shows that the marketing concepts of segmentation, targeting, and positioning could compete competitively with other hospitals. The marketing strategy of a health service was able to increase the interest of tourists in conducting medical tourism activities in a country. Marketing efforts carried out by hospitals focus on domestic and foreign tourists. Conclusion: The existence of a marketing strategy can increase competitiveness between hospitals and increase the interest of domestic and foreign tourists to carry out medical tourism activities competitively. Abstract

Background: Throughout the world, emergency departments (ED) are characterized by overcrowding and excessive waiting times. Furthermore, the related delays significantly increase patient mortality and make inefficient use of resources to the detriment of the satisfaction of employees and patients. Lean thinking is a philosophy that focuses on eliminating waste or non-value added elements from the processes so that customers are given greater value. Therefore, many Emergency Departments have begun to apply lean as a way to fight against the problems of crowding, delays and medical incidents. Methods: Eligible studies for this systematic review constitute result summarized that lean method implementation was capable of reducing overcrowding in Emergency Departement. The journal employed in this systematic review sourced from ProQuest, Pubmed, and Google Scholar, whereas the analyzed journals were seven journals. Results: From the total of seven journals reviewed, the studies analyzed the result after implementing the lean method in the Emergency Department. In addition, all of the studies were reported successful in implementing lean intervention. Several studies explained that lean method implementation was capable of reducing waiting time, staying length, and the proportion of patients leaving without being seen in the emergency department. One study suggested that reducing radiography transportation time can reduce turnaround time and can help improve the Emergency Department flow. Conclusion: Lean method implementation can contribute to a decrease in waiting time, stay length, and the proportion of patients leaving without being seen. According to the results of the study, the lean implementation identifies that the lean is capable of decreasing waiting times, stay length, and the proportion of patients leaving without being seen. There must be a policy that governs the lean process. Lean can inspired changes to employee roles, staffing and scheduling, communication and coordination, expertise, workspace layout, and problem-solving. Abstract

Background: Patient safety is one of the most important dimensions in quality of healthcare. Nurses are the key in care delivery, their compliance toward Patient Safety policy cannot be overlooked. Studies evaluating determinants of nurse’s compliance to Patient Safety policy is still lacking. Aims: To evaluate nurse’s attitude toward Patient Safety policy using Health Belief Model (HBM). Methods: An analytical cross-sectional study was carried on in Bali Royal Hospital, Indonesia, on December 2016. An anonymous, self-administered questionnaire was used to collect the data on socio demographics, knowledge, and attitude toward Patient Safety policy. Attitudes are specifically assessed using HBM. Results: A total of 124 nurses are included in this study. Mean age of the study population was 29.7±6.7 years. Mean score for knowledge about Patient Safety policy was 12.68±3.40 from 18.0- point scale. Nurses perceived a low risk of non-compliance (mean perceived risk 8.07±1.89) and a low barrier to comply with Patient Safety policy (mean perceived barrier 8.66±1.78). Knowledge and attitudes toward Patient Safety policy did not differ by sex and educational background but it did differ according to unit of workplace. Age and length of work correlated positively to knowledge (Spearman’s r=0.439, p-value 0.000; and r=0.400, p-value <0.001, respectively). A positive and moderate correlation was observed between knowledge vs. perceived severity (r= 0.394, p-value <0.001) and knowledge vs. perceived benefit (r=0.422, p-value < 0.001). There was a strong and positive correlation between perceived severity and perceived benefit (r= 0.725, p-value <0.001). Conclusion: Nurse’s attitudes toward Patient Safety policy were generally good and correlate positively with knowledge. The use of HBM provides an increased understanding of how individual perceptions can be influenced to improve nurse’s engagement in promoting safer health care.

Abstract

Since the establishment of COVID-19 on March 11 2020 as a world pandemic, various health service efforts have continued to be made, especially in terms of implementing telemedicine which is considered to be an alternative in preventing the spread of COVID-19 during a pandemic, considering that health services are at home pain must go on. Telemedicine is a combination of medical and information communication technology, which can provide health services without being limited by space. Since the COVID-19 pandemic, telemedicine has become one of the most popular choices in remote healthcare efforts, therefore the purpose of this systematic review is to discuss the effectiveness of implementing telemedicine during the COVID-19 pandemic targeting hospitals that have not yet implemented telemedicine considering the use of telemedicine. The research is a scoping review using the framework of Arskey and O'Malley. The implementation of telemedicine in hospitals has become quite attractive during the COVID-19 pandemic, but in practice, there are still obstacles such as the platforms and networks to be used, the level of accuracy of diagnosis, and regulations between hospitals.

Abstract

Primary Health Care (PHC) is a foundation of universal health coverage for enabling equitable access. However, the access barrier is one of the challenges in implementing PHC. Home visits becomes one of the PHC innovations in several countries. This review aimed to describe home visit implementation in various countries regarding the scope, and service providers to achieve the expected impacts. This descriptive study used scoping review method referring to Arksey and O’Malley’s guidelines. Three databases, which are PubMed, ProQuest, and Embase, were searched using keywords, namely “home visit” and “primary health care” or “primary healthcare” or “PHC services”. The articles’ identification used the PRISMA method. From the search result that was limited to 2002-2022, 367 articles were retrieved and screened for titles and abstracts, resulting in 48 full-text articles for eligibility assessment. Finally, ten studies were further analyzed and synthesized according to the study’s aim. The scope of home visits is a comprehensive life cycle approach. Services providers are community health workers, nurses, midwives, doctors, and multidisciplinary teams. The impact of home visits is increasing knowledge and healthy behaviour, utilization, service quality, and health status as well as encouraging cross-sectoral collaboration and interprofessional collaboration. The scope and the impact of the home visit were then categorized based on the three PHC components according to WHO. This research result can be used as input in the development of evidence-based policies for the implementation of home visits as part of the services provided by first-level health facilities with adjustments to the context in Indonesia.
